4 recordedEE was fatally injured while performing the standard practice of scaling loose material using a mechanical scaler. Loose material fell from the back contacting the scaler and employee.
Employee was looking for hydraulic leak and was struck in head by eject all plate. Employee received a massive head trauma and later became a fatality.
The worker was in process of adding Material to the base of the tailings dam at Brushy Creek Mine. The moxy haul truck was going down the slope of the dam when the driver lost control. As he tried to negotiate the turn near the bottom, the truck rolled over and landed on side in a 6' deep seep pond. Diver's body was found pinned under truck and in water.
