Investigator narrative
The worker was in process of adding Material to the base of the tailings dam at Brushy Creek Mine. The moxy haul truck was going down the slope of the dam when the driver lost control. As he tried to negotiate the turn near the bottom, the truck rolled over and landed on side in a 6' deep seep pond. Diver's body was found pinned under truck and in water.
