Employee was looking for hydraulic leak and was struck in head by eject all plate. Employee received a massive head trauma and later became a fatality.

EE got hand caught between junction box and basket railing while moving junction box. EE received contusion to left hand. EE placed on light duty on 05/09/2022 while waiting on x-ray report to come back. X-ray report was negative for fracture. EE returned to full duty on 5/12/2022.

29 MINE MAN HOIST BEGAN MAKING A NOISE ON 7/2/2020. MAINTENANCE ISSUE OF BULL GEAR KEY WELD HAD BROKEN LOOSE. MSHA: 10:15 AM - EMPLOYEES EVACUATED OUT OF THE MINE FOR REPAIRS. REPAIRS MADE AND MSHA WAS CALLED BACK TO LET THEM KNOW THAT SITUATION WAS COMPLETE. EMPLOYEES WERE RELEASED BACK TO WORK AT 12:00 PM

29 Mine Main Hoist went down with an electrical issue while skipping ore. MSHA was called and everyone evacuated out of the mine through the escape shaft. Unfound Electrical Issue - hoist back in operation at 7:51 PM.

EE cutting a cylinder stem with torch, the stored energy cause the cylinder to kick back striking EE on forehead, lacerating forehead. EE received sutures to forehead and released back to full duty following that medical treatment.

EE dismounting grader and slipped on step, falling into grader on left side torso. EE taken for medical examination on 3/4/2019 and was placed on restricted duty for pain and contusions. EE had a follow up medical exam on 3/8/19 and was returned to full duty at that time.

1:00 pm #29 Mine hoist went down. After evaluation it was determined to be a skip hung in shaft. MSHA was called and evacuation procedures were in process. Evacuation process completed at 4:45 pm. Maintenance was completed to put shaft back into service on 12/30/17.

Hoist went down at 8:25 am on Monday, March 9, 2015. Electricians were called. After repairs were made to a relay and inspection was made, the hoist was released back into service at 10:15 am.

Hoist went down at 2:55 pm. Electricians were called and a burnt relay was found and repaired. Hoist was repaired at 3:35 pm. All employees were brought to shop area while repairs were being made.

Employee was stopped to take a bathroom break, he was at 30 feet away from truck when tire blew spraying employee with rocks and dust. Employee still has piece of rock under skin that was removed on 04/18/13 required stitches. Employee returned to work same day.

On 11-23-09 the main hoist suffered a control module failure at the #29 mine. The hoist failed at 2:34pm and MSHA was notified at 2:37pm. The hoist was back in service at 5:00pm and MSHA was notified at 5:05pm.

#29 mine hoist lost power at 8:53am due to electrical failure. MSHA was notified by phone at 8:58 and MSHA was notified again at 11:20 that hoist was back in service.

EE was attempting to move a boulder. He put his hand on wire rope while pushing the down button on crane control wire rope pulled his finger into the sheeve. Smashing pinky and ring finger on right hand. EE was taken to ER and later transferred to Des Peres Hospital where he had surgery on 11-17-09 making this a lost time accident.

EE was operating a forklift on the surface, EE went to adjust the left fork and it came completey off hitting top of EE's right foot. EE was seen by ER and released to CO. Dr. after x-rays it was revealed a possible fracture/contusion, referred to Ortho Dr. EE released to sedentary work only.

Co-worker approached the injured EE from behind and struck him behind the right knee, causing him to loose his balance and right knee popped. After continuous treatment, the physician gave him restrictions that caused him not to be able to perform his normal job description. This became a lost time incident on 2/20/08.

Employee was dismounting the truck to check a tire when he lost his grip on the handrail and fell about five feet to the ground. Employee was taken to the emergency room where he received 19 sutures to his finger.

Employee was pulling a piece of loose off of a pillar. The loose gave way causing the mining bar to move and smash his left ring finger between the mining bar and multicrane basket. Employee was taken to Salem ER. Employee fractured tip of left ring finger.

Power went out causing hoist to go down at 8:40 A.M. Problem was with electric company (Amerun UE). Hoist was back in service at 11:30 A.M. MSHA was notified of power outage and when power was restored.

Boulder hung in load chute in skip pocket. Boulder was too big to go thru the load out system getting thru grizzly. While trying to move cable, hook slipped off boulder hitting the employee across the nose. Employee was taken to emergency room where he was diagnosed with a crushed bridge and would need surgery. Employee is currently off work.

EE WAS FILLING THE HYDRAULIC TANK ON #93 TRUCK. AS HE PREPARED TO DISMOUNT THE GRAB HANDLE BROKE. THIS CAUSED THE EE TO FALL INTO PILLAR NEAR THE HYDRAULIC SWITCH. EE HURT LEFT SHOULDER, NECK & BACK. ON 5/15/05 EE WAS PUT ON LIMITED DUTY OF NO LOOKING OVERHEAD & NO LIFTING OVER 30 LBS.

Motor lead wire burnt in to - repaired in approx. 1 hour. Hoist went down at 1:55 p.m. MSHA was on job site so they were notified. Hoist was back up and running at 3:00 p.m. MSHA was still there on the property and they were notified.
