CABLE AND RAN SHUTTLE CAR INTO OFFSET IN RIB, CAUSING MACHINE TO COME TO AN AB RUPT STOP. HIS RIGHT HAND WAS ON STEERING WHEEL & WAS CAUGHT IN IT WHEN HE WAS THROWN FORWARD, CAUSING A FRACTURE TO HIS RT. HAND.

OPERATOR WAS STANDING IN WATER WITH HOLES IN HIS BOOTS, WHEN HE PICKED UP A CABLE WHILE MAKING CONTACT WITH HIS SHUTTLE CAR THAT HAD A FAULTY GROUND IN A VULCANIZED SPLICE. HE RECEIVED AN ELEC TRICAL SHOCK. HE WAS SENT TO HOSPITAL FOR OBSERVATION. HE WAS RELEASED AND SENT BACK TO WORK, MISSING ONE DAY.

WE MINE 8 FT. OF COAL. THE MAN WAS HANGING A RUN THRU CHECK WHICH REQUIRES REACHING THE ROOF. HE CLIMBED ON THE END OF A SHUTTLE CAR AND SLIPPED BETWEEN THE SUPPORT ARM & FRAME, TWISTING HIS LEG.
