Freedom Mine Coal

A supervisor was working on writing an employee review in the office. As another individual walked past the office, they heard a loud noise and went into the office to see what it was. They found the supervisor lying on the floor actively having a seizure. Sometime during the event the left ear and left head suffered lacerations that required stitches.

On October 9th, 2024, an employee was in the process of putting on a new poly end. During the process the employee noticed it was going on crooked. Employee reached down to adjust the end at the same time another employee was swinging a sledgehammer. The hammer hit the employee's right index finger, resulting in a fractured finger.

Employee was assisting another employee helping to put a clevis on a machine. The clevis rotated and the employee's finger was pinched between the machine and the clevis.

Employee was assisting to load a backhoe onto a low-boy trailer. After loading and chaining the backhoe was completed, employee dismounted the trailer and in doing so felt a pop in their left knee.

On March 7th, 2024, a dragline ground person was in the process of pushing in a ramp off the bench to access the pit floor for cleaning. During the process of pushing in the ramp, the dozer blade caught a rock or frost edge. The dozer turned sideways, overturned and then came to rest on its tracks. The employee was wearing the seatbelt at the time of the incident.

On October 17th, 2023, an employee was dismounting 510-190 dozer. As the employee was stepping off the draft arm, they slipped and fell backwards. As they fell the left foot became wedged between the arm and the track of the dozer causing their lower leg and ankle to twist. This resulted in a fracture to the tibia and fibula of the left lower leg.

On May 31st, 2023, an employee was working to remove the crowd brake on 935 electric shovel to replace broken bolts. When the final bolt was removed, the stored energy in the crowd brake caused the brake to rotate. When the brake rotated, the employees arm came into contact with it. This contact caused lacerations to the employees' arm, which resulted in stitches.

Employee was dismounting blade and slipped on the ice catching self with left hand, causing pain and swelling in the left wrist. Employee sought medical attention on 1/15/2022 where there was suspicion of a non-displaced possible fracture. Employee sought a second medical opinion on 1/25/2022 and the left wrist was immobilized with a cast.

Employee was maneuvering becket end of pendant lines into a spool for storage. Employee had fingers between cotter key keeper on the pin and becket end. When the becket end was positioned onto the spool, back of pin made contact with spool sliding pin away pinching finger.

Employee was performing a welding/grinding repair on a Dragline bucket. Later in the shift the employee's eye became irritated. The eye was flushed several times and nothing was identified in the eye. The next morning the eye was still irritated. An eye doctor identified a metal sliver in the eye. Proper PPE was being utilized when welding/grinding work was being performed.

Employee was replacing a fan drive on an OB truck. Employee went to reposition and EE's foot slipped off of a crossover support. This resulted in EE's shin contacting a clamp bolt causing a laceration that required stitches. Employee was utilizing fall protection at the time of this incident.

An employee was in the process of starting to line bore. During this time the employee was shaking a can of spray lubrication to prepare to lube the cut. During this action the employees hand encountered the boring head which then pinched EE's right thumb against the bearing plate. This caused a partial amputation of the right thumb, and a broken right ring finger.

Employee was lifting a 4 inch suction hose off pump truck and when turning on uneven ground felt a pain in EE's back.

Employee was trying to remove a water hose fitting with a hammer and chisel. EE ended up missing the chisel and hit left thumb with the hammer. Employee was wearing gloves.

Employee was working on rebuilding a planetary. The planetary had 3 gears that were presumed stuck, a pry bar was used to free the first 2 gears. On the 3rd gear the employee was showing the shop planner that the gear was stuck. EE gently wiggled the 3rd gear & unexpectedly it freed up causing EE's finger to pinch in the housing. EE was wearing a latex mechanic glove.

EE was performing inspection on truck to start afternoon shift. EE lost EE's footing, regained EE's balance before falling, but felt a pop/burn in EE's upper right thigh. EE finished EE's shift, but decided to get a medical opinion on Monday. EE had a hip flexor muscle strain, EE was told to rest/ice for two weeks. Freezing rain and blowing snow conditions at the time of incident.

Dragline dump rope caught in drag hitch. Groundman moved drag ropes to create slack and went to park tractor. Two maint. employees developed a plan to use the boom truck arm to release the rope. While releasing the rope they did not see groundman enter area unannounced from behind. Rope released and contacted groundmans face causing injury to nose, lip, upper maxillary and teeth.

A mechanical failure of a drive-line on a Kress coal hauler caused damage to oil lines which started a fire in the rear of the truck. The "One Call" was made for the 30 min. fire qualification. The truck operator was not exposed to any injury as EE exited the truck off the front and remained clear. Emergency procedures extinguished the fire with no injuries to any personnel.

An employee was fabricating a swing case stand while working on a welding table. During the process the employee attempted to flip the piece over to continue welding and in doing so the steel frame fell off the table. The frame landed on the employee's foot, fracturing the foot.

A shop mechanic was dismounting a track dozer in the shop. The mechanic's foot slipped off a deck step and when trying to catch self the mechanic's left arm got caught between the hood handrail and the hood. This caused a fracture of the upper arm. The dozer steps were clean, dry and in good condition, as the dozer was washed for maintenance.

Employee was performing weld repairs in the car body of a loading shovel. Employee was wearing safety glasses & face shield, but at some point during grinding/air arcing a particle contacted the employees right eye. The eye was flushed & employee finished the shift and went on days off. On 8/28/18 the eye was bothering so employee went to eye doctor who removed a small particle.

Employee was cutting tack welds loose with a grinder. The grinder kicked back and struck employee's left leg causing a laceration.

A 500 ton hydraulic ram fell onto the employee's foot causing a fracture.

Employee was kneeling down changing a tire on a pickup in the shop, while doing so employee's legs fell asleep. Employee went to pull themselves up by grabbing the side of the pickup and while doing so employee said they felt a tear in the lower abdomen.

A miner was carrying an aluminum beam assisting with a coal handling facility repair. The miners foot slipped off the bottom step of a staircase which resulted in the employee catching balance, causing mild compression fractures to vertebrae in the back. The miner returned to work with no restrictions.

A grass fire was discovered burning in a remote area next to a substation. The fire was burning away from the substation and any active work areas. No miners were endangered and emergency personnel responded and put the fire out. Extremely dry conditions in the area has the fire danger very high.

EE was washing small backhoe in wash bay. EE tried to reposition for better cleaning of engine. EE had left hand on handrail and both feet on the track. EE slipped and lost grip. Realized step was not below EE and made contact with floor.

Employee was using a pocket knife to cut a zip tie off of a wiring harness. Employee was cutting towards his hand, knife slipped cutting employee's thumb.

Employee was cutting off a tie strap with a knife and lacerated the top of his hand.

While cutting a strap on a cable spool, the torch got caught pulling the employee's hand into the spoke causing a fracture.

A welder was die grinding a weld on a dragline mast repair and a particle of metal got in his eye. The particle/irritation was not noticed until later that evening at home. Welder was examined the next day by an eye doctor who identified the particle and removed it.

Employee was stowing the safety cable for the box of an overburden truck. The employee had trouble getting the clevis onto the hook. When the cable was moved the clevis pinched the employees finger against the box. The employees finger was cut and finger tip was fractured. Stitches were required.

Debris entered employee's eye while grinding on a beam. Employee was wearing a face shield and safety glasses.

Employee was setting light plant for Reclamation operations. Employee had set all the outriggers and was moving to unhook hitch pin when his feet slid out. Snow had covered an ice patch causing the slip. Employees left leg (shin area) contacted the light plant hitch causing a cut. The cut required stitches.

Employee was pushing an oil drain cart and in doing so pinched his thumb between the cart handle and a router bit on a scraper bowl. Employee was looking in the direction of cart movement and not paying attention of the position his hand was in. The weight of the cart pinched the thumb which required stitches.

While torqueing lug nuts, the right middle finger was pinched between the tool and wheel.

Filling floor cleaning detergent and it splashed on his left foot. EE did not report it until the following day. I met him at the clinic. They sterilized his foot and recommended to not work until healed because of chance of infection.

Employees were pulling center pin on dozer frame hard bar and the pulling ram could not break the pin loose. Employee decided to hit the pin with a hammer. When he did a piece broke off the pin and hit employee in left cheek. the employee thought he was using a soft head hammer, but it was not. A physician was required to remove the steel chip and insert seven stitches.

Employee was walking into the main building from employee parking lot prior to shift. Employee had one foot slip on snow/ice and tried to catch them self. Employee went down on opposite knee and then contacted nose on the ground. The parking lost had been cleaned and sanded prior to the incident.

A diesel mechanic was preparing to install a radiator in an OB truck. Was positioned at front of engine and noticed the hood in a questionable position. Pushed on the hood to check if stable. The hood shifted and moved towards mechanic. The mechanic reacted to the movement and turned to get off truck. Mechanic went off the front bumper and contracted hands with the floor.

Employee was walking up a sloped pond embankment and felt a pop and burning sensation in the calf of his right leg. Normal work activities were ongoing and employee was not running and did not trip or slip. Employee was scheduled for a CT scan which diagnosed a tear in a gastro muscle of the calf. Physical therapy/healing time was ordered and employee could not RTW for 4-6 weeks.

Employee was grinding out a weld area on the mast of the dragline. Employee was wearing all his personal protective equipment. Grinder trigger was not engaged but disc was still spinning when grinder slipped in employee's hand, disc contacted left index finger cutting tip. Finger required 4 stitches, employee returned to work.

Employee was grinding out an air arc cut for repair on a front end loader. Employee was wearing safety glasses and a face shield. The grinding particle found its way under both and contacted his right eye. The work area was fairly open, but had some potential for particles to deflect around. The employee was taken to eye doctor who removed the particle. Employee returned to work.

EE was washing the top cab of a flatbed pickup and was stepping off the toolbox back onto the bed of the pickup. Stepped onto a crow bar and twisted his left ankle. After further examination found out ee fractured a small bone in his foot.

Mechanic was sitting on top of dozer transmission area doing wiring maintenance work. When EE was exiting area he heard a "pop" in his knee. EE experienced pain in knee & could not support weight on knee. He had been receiving personal med. attention prior to the incident. The symptoms of "popping" in rt knee were being diagnosed & treated by his attending physician.

EE was operating scraper & when looking backwards ran over a rock in the cut. Speed was slow while loading, but tire slipping off rock caused EE discomfort in tailbone area. There were no mechanical issues w/seat or scraper. EE worked next shift on 19th, but couldn't return on 22nd due to discomfort. A dr's appt recommended time to heal, but no treatment was administered.

On 4/8 an EE stepped off the personnel lift of a loading shovel and felt discomfort in their knee. Area had just been bladed & was relatively flat with no holes or chunks. The EE continued to work several days, but knee was bothering. EE saw a dr who recommended an MRI. Scheduling took time & EE continued to work. The MRI showed torn meniscus. Worked until 4/29, out for surgery.

Employee mounted the motor grader using the proper hand holds and walkway. His objective was to clean the right rear side window. While walking on walkway located on the tandems, employee slipped and struck elbow on a tire. Employee was utilizing the grab rail, but lost his grip. The walkway was clean and dry.

Employee was assisting with propel gearing work during an annual outage. When pulling an intermediate pinion the gear unexpectedly shifted and a hole on the side of the gear caught the steel toe of employee's boot bending his foot over the side of a platform he was standing on. The incident caused a fracture of employee's left foot.

Employee twisted ankle on uneven floor surface in dragline. Employee missed one day of work due to the injury.

EE was kneeling down and lifted a piece of power cable a short distance onto the basket of a cable horse. EE felt pain in his back after lifting. A lower back muscle was strained and required therapy.

EE was tightening bolts on the strut of a coal hauler. When swinging the hammer it glanced off the truck frame and hit EE's left hand. The hand was pinched between the hammer and a wrench, which lacerated the hand. The laceration required five stitches.

An office emplyee was walking to her car after completing her scheduled shift. She stepped wrong, rolling her ankle and striking the parking lot. There was no slip, trip or fall. The parking lot was free of obstructins and snow/ice. The incorrect placement of her foot caused the employee to sustain a chipped tooth and minor abrasions.

While attending a safety mtg. the EE leaned back and shifted weight in a rolling/reclining office chair. The motion caused the chair to overbalance and EE fell backwards striking head on a cabinet edge. Chair had no defects prior to or after the incident. Cut required nine staples for closure.

EE was manually moving scrap steel onto a fork lift for disposal. While positioning a second plate on the lift the EE's finger was pinched between the plates. Leather gloves were being worn. Three stitches were required.

Employee lifted hood after welding in the shoe horn. He felt slag enter left eye from another employee grinding nearby.

Particle blew into left eye, behind safety goggles, while walking around machine. Reported to employer on 6/4/11.

EE was welding/grinding overhead around a shoe horn. EE later felt something in his right eye.

Employee was turning a 3/4" plate when the plate slipped and caught his right hand on the corner. Employee was treated with first aid on site due to no pain and only a minor abrasion. Employer was unaware of fracture until 04/27/11 when employee was taken to the physician.

Employee was doing "bolt-up" work when the wind blew a foreign body into his right eye.

EE was removing bolts on the cutting edge of the skidsteer, on impact bolt caught rubber glove pulling finger into bolt cutting finger. Required stitches. EE had no restrictions from Dr and was able to come right back to work.

Conditions very cold, lost footing on ice. Fell backward, landed on right arm, breaking bone near wrist.

EE was pushing down on hoist lever to lower dump body when the lever slipped and recoiled against the EE's thumb. The EE felt pain in her thumb but had full movement of it. The discomfort did not go away so the EE chose to see a Dr. on 3/9/11. EE has a fractured thumb.

EE was grinding on 11/19, when a piece of slag flew into his left eye, while wearing proper eye and face protection. Eye was washed out on the job, but EE was taken to clinic on 11/22.

Unknown if foreign body entered right eye from grinding or from sweeper that came near employee's work area blowing dust particles.

EE was assisting a co-worker move cribbage. EE later complained of abdominal discomfort. This claim is being investigated by the carrier for possible denial.

A piece of slag fell into EE's left boot while welding. EE waited until weld was complete to remove boot and retrieve slag. Dime sized burn became infected and required debridement on 6/17/10.

EE was working in a revolving frame next to a welder and got a foreign body in right eye.

EE was walking out of a revolving frame while a co-worker was grinding nearby. EE felt something blow into right eye.

Employee was removing an equalizer seal from a dozer frame with a hammer and chisel. Upon striking the chisel, it recoiled in the employee's hand, underneath the face shield, striking employee's tooth. All appropriate PPE was being utilized.

EE was repairing a fuel line on the gooseneck of a scraper and when descending the fender ladder, cut his buttock on the edge/corner of the engine compartment door. The door had been opened for troubleshooting and maintenance. The cut required stitches.

EE was pulling slack in a come-along when he slipped on ice causing his right kneee to twist.

Employee was operating a demolition saw. Employee was cutting a vertical concrete wall when the saw "kicked back", striking the employees faceshield, knocking his faceshield up, and cutting his lip and nose.

EE was assisting the move of a disassembled dozer track frame. The EE was picking up cribbing and placing it on track frame, when another person did the same from the opposite side. As they placed their cribbing at the same time, the EE's finger was pinched between cribbing blocks, resulting in a fractured finger tip.

EE was trouble shooting air conditioning system on a Cat 24H motor grader. When checking air flow in an air duct, the employee did not realize the exposure to a moving fan. When retracting his hand it caught the fan, injuring two fingers. The finger tips were cut and fractured requiring medical treatment.

EE was hanging the ring of a tow chain onto a cable skid. EE failed to notice the chain was frozen down and as the ring went over the hanger, it pinched the tip of EE's finger. Stitches were required.

An overburden truck driver was exiting his truck for break time and while walking down staircase tripped on third step from the bottom. He tried to catch himself with left arm on handrail and injured his shoulder. Staircase was clean and in excellent condition.

A hydraulic failure of a coal truck spill protector allowed the spill protector to contact a personnel lift and push it over. The employee was wearing a safety belt in the personnel lift. The lift tipped over and the employee's leg was fractured when the lift contacted the floor.

EE went to clean up under the hoist motor coupling and was going to give the motor coupling a quick wipe because it was a ripping grease onto the floor. It caught her glove somehow and pulled her finger into the bolt hole. She felt a tug and a sting and when she looked. the tip of her finger was gone.

After returning tools to a tool box, the EE turned to walk away and felt/heard a pop in his leg. EE was not performing a task or carrying anything at this time. The EE's achilles tendon was torn. Surgery was required.

In changing out of his work shoes in the employee locker room, the employee strained a muscle in his back. At Coteau Properties request, he saw a local physician @ 10:30 am 04-23-08, who advised him to not go back to work till Monday, 04-28-08.

EE was performing welding operations on bucket and went to assist mine employee in removing a bucket tooth. As EE approched, the mine employee struck the part with a hammer and a small fragment flew off and struck EE on his forehead, between his hardhat and his safety glasses, causing a small cut that required 3 stiches.

Washing under rear of haul truck, he lost balance while turning water pressure on.

Employee was grinding overhead and wind blew some trash into his left eye.

While reaching to turn off a defroster fan, an EE's r/h middle finger was cut, when it came in contact with the fan blade.

While grinding a particle became imbedded in an employees left eye.

While air arcing and grinding, a particle became embedded in the employee's right eye.

When cutting the top off a cardboard box the cutter came through the box corner and cut employee's knee. The cut required one stitch.

Welder received a minor burn to his left eye while working on the revolving frame of a dragline.

EMPLOYEE SLIPPED ON A SLOPE BY A LOADING DOOR ON A DRAGLINE THAT HAD GREASE ON IT.

INJURED AND ANOTHER PERSON REPLACED FRONT WHEEL ON FRONT WHEEL ASSIST TRACTOR. AFTER TIGHTENING WHEEL BOLTS THEY LOADED 6 IN X 8 FT WOOD POSTS IN BACK OF PICKUP. WE ARE NOT SURE WHICH OF THESE ACTIONS CAUSED HIM TO STRAIN HIS LOWER BACK OR BOTH DID.

WHILE UNLOADING PIPE FROM A FLATBED, THE PIPE SLIPPED AND CHIPPED TOOTH

CHANGING CUTTING EDGES ON A SCRAPER, THE HOISTING CHAIN CAME LOOSE FROM A LIFTING EYE AND HIT EE ABOVE THE EYE, STITCHES WERE REQUIRED.

While working on 509-107 dozer, ee bumped a steel pin. The pin tipped over onto his hand, fracturing his left index finger.

WHILE LOADING OBJECTS ONTO THE FORKS OF A FORKLIFT, THE FORKLIFT OPERATOR LOWERED THE FORKS ONTO INJURED EMPLOYEE'S FOOT. HIS TOE WAS FRACTURED.

AN ENGINE COMPARTMENT FIRE RESULTED FROM A MECHANICAL FAILURE ON 526-518 KRESS COAL HAULER DRIVELINE.

Employee was spot facing tub with a hydraulic magnetic drill. EE was raising drill head up when shirt sleeve got hung in the drill pulling hand into drill, cutting the top of left hand. 14 stitches.

WHILE POSITIONING A SPOOL OF ELECTRICAL WIRING ON A TRAILER, THE LOAD SHIFTED & FELL ON EE'S HAND. THE CUT REQUIRED STITCHES.

EE's finger was cut when it became caught in a motor grader door. The cut required one stitch.

EE RECEIVED A CUT ON HIS RIGHT HAND THUMB WHEN SECURING THE HOOD SUPPORT ON HIS PERSONAL VEHICLE. THE CUT REQUIRED STITCHES.

EMPLOYEE RECEIVED A CUT ON HIS MIDDLE FINGER, RIGHT HAND WHEN ALIGNING A PIN INTO THE CENTER SHIFT YOKE ASSEMBLY ON A BLADE. THE CUT REQUIRED STITCHES.

EE PINCHED HIS RIGHT INDEX FINGER WHEN MOVING AN I BEAM. AN X-RAY SHOWED THE FINGER TIP WAS FRACTURED.

EE WAS GRINDING ON THE LEFT SIDE TOP OF THE TUB IN THE RAIL PATH AREA. THE WIND BLEW A FILING INTO THE EYE. 3

EE WAS LEAVING THE WORK AREA AND HE SLIPPED ON A PIECE OF MACHINERY COMING OUT OF THE FLOOR, HURTING HIS LEG.

EE WAS WORKING ON HIS KNEES ON A DRAGLINE TUB FOR SEVERAL DAYS. HIS KNEES BECAME INFLAMED AND SORE OVERTIME. THE ACTUAL EVENT TOOK PLACE AROUND MAY 10, 2003 BUT THE EE DID NOT REPORT THE INJUR Y UNTIL JUNE 9, 2003.

EE WAS USING A BACK PACK SNOW BLOWER. HE SLIPPED & FELL ON ICE. SIDE SHIELD ON GLASSES CUT HIS FOREHEAD. HE RECEIVED 6 STITCHES.

EE PARKED HIS TRACTOR ON TOPSOIL THAT HAD BEEN DISCED, SEEDED AND MULCHED. HE GOT OUT TO WALK AROUND IT AND WHILE DOING SO HE STEPPED ON A ROCK AND TWISTED HIS ANKLE. HE WAS TAKEN TO THE EMERG ENCY ROOM AND HAD X-RAYS, IT WAS DETERMINED HE HAD FRACTURED TWO BONES JUST ABOVE HIS ANKLE. MULCH COVERING THE GROUND HAD A FACTOR IN THIS AS IT PROBABLY COVERED THE OBSTACLE.

WHILE FILLING 5 GAL BUCKET WITH GRAIN AND HANDING IT OUT OF GRAN BIN TO ANOTHER PERSON OUTSIDE, THE COMBINATION OF LIFTING AND TWISTING STRAINED HIS LOWER BACK. AFTER SEEING A CHIROPRACTOR HE WAS TOLD TO STAY OFF WORK FOR TWO DAYS AND SEE HIM THEN. SO FAR THE ESTENT OF HIS INJURIES AREA COUPLE OF VERTEBRAE OUT OF PLACE AND VERY SORE MUSCLES.

EE WAS CUTTING A BOX OF RAGS OPEN WITH A UTILITY KNIFE WHICH HAD A DULL BLADE. THE KNIFE SLIPPED AND CUT INTO HIS L ARM AND STITCHES WERE REQUIRED.

WHILE INSTALLING A FAIRLEAD SHEAVE ON THE DRAGLINE, THE SHEAVE SLIPPED AND EE HIT HIS HAND WITH A HAMMER. HIS KNUCKLE WAS FRACTURED. HIS FIRST DAY MISSED WAS 7-12-02.

EE WAS CLEANING ON A SHOVEL THAT WAS NOT RUNNING. WEATHER CONDITIONS WERE WARM & MUGGY.

WHEN DISMOUNTING A RUBBER TIRE DOZER, EE FELL OFF OF THE ACCESS LADDER SUSTAINING A CUT ON HIS RT. LEG. THE INJURY REQUIRED 2 STITCHES.

EE WAS SWEEPING OUT THE CAB OF A TRUCK & SOMETHING BLEW INTO HIS EYE. THE PARTICLE WAS EMBEDDED IN HIS LEFT EYE.

WHILE INSPECTING TOOLS, A HEAVY OBJECT SLID AND FRACTURED EE'S FINGER.

EE TWISTED HIS ANKLE DISMOUNTING A DOZER.

WHILE LOADING A SET OF DOZER PADS IN THE BACK OF A PICKUP, HIS FINGER WAS PINCHED BETWEEN THE DOZER PAD AND THE PICKUP BOX. HIS FINGER WAS FRACTURED.

EE HAS BEEN EXPERIENCING SOME WRIST PAIN AND HAS BEEN DIAGNOSED WITH CARPAL TUNNEL SYNDROME. DOCTORS CANNOT SAY IT IS WORK CAUSED, BUT IT IS POSSIBLE. HIS FIRST DAY MISSED WAS 2-8-02.

A FIRE WAS NOTICED IN THE AUXILARY SHOP BY THE "DAKOTA SECURITY". THE EMERGENCY TEAM ASSISTED BY THE FIRE DEPT. AND DAKOTA GASIFICATION CO. EXTINGUISHED FIRE BY 4:00 PM. THE STATE FIRE MARSHA LL RULED THE FIRE AS ACCIDENTAL: ELECTRICAL IN THE WIRING TO A LIGHT FIXTURE NEAR THE CEILING.

WHILE WORKING ON A LADDER THE LADDER SLIPPED AND EE FELL 4' TO THE DRAGLINE FLOOR. HIS ARM, LEG, AND SHOULDER WERE BRUISED.

EE'S FINGER WAS FRACTURED WHEN IT WAS PINCHED WHILE PLACING A DUFF NORTON JACK UNDER A PIECE OF EQUIPMENT. STITCHES WERE REQUIRED.

WHILE BACKING AN OVERBURDEN TRUCK DOWN A SLOPE TO DUMP THE LOAD, THE TRUCK OVERTURNED ONTO ITS SIDE, CAUSING A BRUISE TO EE'S LEG AND HIPS. HER FIRST DAY MISSED WAS 7/24/01.

EE SLIPPED AND TWISTED HIS RIGHT KNEE WHILE PERFORMING MAINTENANCE WORK ON A PIECE OF MACHINERY. THE INJURY WAS DIAGNOSED AS MUSCLE STRAIN AND HE CONTINUED TO WORK WITH SOME DISCOMFORT UNTIL 1 2-14-01 AT WHICH TIME HE HAD SURGERY.

EE WAS SLIDING MATERIAL THRU THE TABLE OF THE SAW WHEN HIS LEFT FOREFINGER CAME IN CONTACT WIT6H THE SAW BLADE THAT WAS STILL MOVING. HE WAS WEARING LEATHER GLOVES.

EE'S FINGER WAS FRACTURED WHEN THE JACK HE WAS USING SLIPPED AND HIS FINGER WAS PINCHED BETWEEN THE JACK AND THE DOZER FRAME.

HE SLIPPED ON THE ICE & INJURED HIS ROTATOR CUP IN HIS LEFT SHOULDER.

EE CUT HIS LEFT RING FINGER WHEN LOWERING A CABLE TREE. THE CUT REQUIRED STITCHES.

A COAL HAULER WAS TRANSPORTING COAL FROM THE COAL PIT TO THE CRUSHING FACILITY WHEN THE OPERATOR NOTICED THE ENGINE COMPARTMENT AREA WAS ON FIRE. THE FIRE SUPRRESSION SYSTEM AND EMERGENCY RESP ONSE TEAMS EXTINGUISHED THE FIRE IN 45 MINUTES. INITIAL INVESTIGATION POINTS TOWARD A MAJOR ENGINE FAILURE AS THE CAUSE OF THE FIRE.

WHILE RAISING A LIGHT PLANT MAST, HIGH WINDS CAUGHT THE LIGHT PLANT HOOD CAUSING IT TO CUT HIS LIP. FOUR STITCHES WERE REQUIRED.

WHILE REMOVING DRAGLINE BUCKET TEETH, THE JACK HAMMER SLIPPED OFF THE PIN AND PINCHED EE'S THUMB. THE CUT REQUIRED STITCHES.

WHILE REMOVING A BOLT, THE SOCKET CAME OFF THE IMPACT WRENCH AND HIT HIM IN THE FOREHEAD. THE CUT REQUIRED STITCHES.

CLIMBING OUT OF MUSTANG. STEPPED ON BUCKET TO JUMP DOWN ABOUT 11/2FT & FOOT SLIPPED DUE TO MUD ON SHOE, FELL AGAINST ROOF OF MUSTANG & HIT TOOTH ON THE ROOF

EE HAS BEEN EXPERIENCING SOME WRIST PAIN AND HAS BEEN DIAGNOSED WITH CARPAL TUNNEL SYNDROME. DOCTOR'S CANNOT SAY IT IS WORK CAUSED, BUT IT IS POSSIBLE. HIS FIRST DAY MISSED WAS 9/26/00.

THE TIP OF EE'S FINGER WAS CUT WHEN USING A UTILITY KNIFE TO REMOVE TY-WRAPS FROM AN OBJECT. STITCHES WERE REQUIRED.

WHILE REMOVING AN OVERLOAD STOP FROM THE AXLE OF A TRUCK, THE OVERLOAD STOP FELL AND SMASHED EE S FINGER. THE BONE WAS FRACTURED AND THE CUT REQUIRED 4 STITCHES.

WHILE ATTEMPTING TO LOOSEN A STUCK BOLT WITH A PIPE WRENCH, THE BOLT CAME LOOSE CAUSING EMPLOYEE TO LOSE HIS BALANCE, FALL, AND SPRAIN HIS WRIST.

WAS WORKING OUTER PROPEL, LEFT HAND SIDE AND WAS LEAVING THIS AREA. SHE CRAWLED OUT OF THE AREA AND WENT TO STAND UP AND HIT HER SHOULDER ON A PIECE OF STATIONARY IRON THAT WAS USED AS A BRACE FOR A PLATFORM.
