An employee, assisting with shovel maintenance, entered the man lift at the shovel house level to descend and retrieve a needed part. After entering the man lift, the support arm broke, causing the basket and employee to fall to the ground. An investigation is ongoing. Amendment: Employee passed away 6/27/19.

Employee was standing next to wrecker trailer and reached up on the trailer to retrieve EE's earmuffs. When EE stepped back, EE stepped on the welder power plug connection, twisting EE's knee. EE was taken to the ER and given restrictions that cannot be accommodated at this time. Currently ONGOING.

Employee operating a dozer when it came down dig face backwards, resting on cab. Employee released seatbelt and fell against cab, causing discomfort in shoulder. Taken to ER, released to next shift; before RTW, saw PCP, taken off work until seen by surgeon from prior back/neck surgery to be cleared. ONGOING.

The employee tripped and fell off last night around 9:30 PM, EE called to report when EE was back in the El Paso terminal, then EE called the nurse this morning to notify of having some difficulty to breathe. I asked them to please notify the mine due to the injury.

Employee reportedly collapsed and was having trouble moving legs and arms on right side of body (symptoms associated with stroke). Transported to local ER and treated for dehydration/potassium deficiency. Employee working their first shift from days off, not considered work related and given full duty release to work. MSHA Hotline was notified just in case.

When shovel dipper was coming out of the bed of the truck, a rock came out of the dipper and struck the dovetail, jarring the driver. Driver was taken to the ER and given time off, missing next shift, fully released on 11/03/2025.

During cell maintenance, employee was in the process of repositioning lead anodes back into the cell when one shifted, pinching the employee's right middle distal between the anode and another surface, degloving their fingertip. Employee was seen by an orthopedic and taken off work.

As an articulating truck was backing towards the drill pad at 5 mph, driver failed to see a dozer, striking the rearend. Initially, both operators stated they weren't injured. However, the next morning, the dozer operator complained of neck and shoulder pain. EE was taken to the clinic and placed restrictions. Currently ongoing.

Employee working in mill basement slipped and fell in mud, landing on left arm. EE immediately informed EE's supervisor and initially declined a medical examination. After bruising appeared on EE's arm, EE and supervisor elected to seek medical treatment on 11/11/24. Employee had acute, traumatic, comminuted fracture of left ulnar olecranon and styloid processes.

Central dispatch notified site of a fire at an abandoned house on an outlying area of mine property, AKA Apache Tejo south of Hurley, NM. ERRT members and Hurley VFD responded, flames were completely out by 4:00 a.m. and fire watch continued. No mining operations were involved or threatened, just located on land the company owns.

While conducting a PM on a light plant, employee was trying to lay the mast down when it became stuck. EE grabbed the bottom of the mast, and it folded back and lacerated EE's right middle finger. EE was taken to the ER, received stitches and released to work.

Employee was dismounting a water truck and rolled an ankle as they started down the first set of steps, catching themselves before they fell. They initially refused treatment and would ice at home. A D10T dozer component was being repaired and the component tipped over striking the employee. Prior to the event the component was removed from the machine and was placed on blocks and was being dissambled. As parts were removed, the component became off balance.

While harvesting cathodes, EE used hook to move hangar bar on strongback and felt pain in lower back, was taken to clinic, released to work. EE worked two more rotations. Was off for personal reasons, then on 5/7/24, received WSR from PCP, taking EE off work as of 4/9/24. Currently ongoing.

Employee was dumping trash, and the dumpster didn't latch. Employee got out of the forklift and went back to the dumpster and tried pushing it down and EE's hand slipped, and EE's right wrist was smashed between the dumpster and the forklift frame. EE immediately reported it to EE's supervisor.

IP was removing core from core tube. Approximately 3' section stuck, so with core tube on core slide, struck it with leather hammer. While beating the core out the tube fell off the slide and struck left foot resulting in contusions.

Employee was working in flotation during a shutdown, and begin feeling like EE had heat exhaustion. EE stopped for an hour break, and replenished fluids. At the end of the shift, EE reported this to the supervisor. The next morning, EE thought EE was better, but about 9 am began to feel light headed, was taken to the ER and given IV fluids. Released to work.
