Employee reportedly collapsed and was having trouble moving legs and arms on right side of body (symptoms associated with stroke). Transported to local ER and treated for dehydration/potassium deficiency. Employee working their first shift from days off, not considered work related and given full duty release to work. MSHA Hotline was notified just in case.

When shovel dipper was coming out of the bed of the truck, a rock came out of the dipper and struck the dovetail, jarring the driver. Driver was taken to the ER and given time off, missing next shift, fully released on 11/03/2025.

While installing banjo bracket and arm on the shovel sticks, using a come along, the come along chain failed and the bracket and arm contacted the employee's right wrist area. EE was taken to the ER, diagnosed with a fracture and taken off work. CURRENTLY ONGOING

As employee was making their rounds in the grinding tunnel, a grinding ball rolled down the tunnel, striking them in the left calf. EE was taken to the ER, diagnosed with a contusion and placed on restricted duty. CURRENTLY ONGOING.

While conducting cell maintenance, electrolyte got in employee's boot. Employee completed shift and went home, called and reported irritation, and next day requested medical attention. Employee was given work restrictions. Currently ON GOING.

Employee was cleaning the basement at the lime plant and steam burnt a small hole through EE's boot, causing blistering on EE's left foot.

Employee was dressing lead anodes for cell maintenance. While installing side insulator (PVC) with a rubber mallet, the left side of the anode slipped off the strongback hook, striking the employee in the neck area. EE was taken to the ER and taken off work 2 days for concussion protocol.

During cell maintenance, employee was in the process of repositioning lead anodes back into the cell when one shifted, pinching the employee's right middle distal between the anode and another surface, degloving their fingertip. Employee was seen by an orthopedic and taken off work.

Employee was preparing to remove a tire on an articulating truck, and when placing the bead breaker in position, caught the tip of index finger between the bead breaker and the rim of the tire. Lacerated finger, requiring sutures to close.

Employee was lifting a return idler to carry down the tunnel when they felt a pain in lower left abdomen. Employee was taken to the ER, diagnosed with muscle strain and referred to PCP. PCP placed on restrictions. CURRENTLY ONGOING.

Employee had returned from taking samples, entered information, stood up from the chair and experienced a sharp pain run from lower left leg to lower hip area. Employee was evaluated by an ERT member then taken to the ER for evaluation. Employee was taken off work until evaluated by PCP.

Employee was working restricted duty from a previous personal injury, failed to remember not to pull with right arm, and used the grab bar while mounting pickup and dislocated shoulder. Employee was taken to the ER, referred to ortho and taken off work. Currently ongoing.

After completing shift, employee was walking to the parking lot, after a light rain shower, and while attempting to walk around puddles slipped but didn't fall. They reported off work that evening due to pain, was seen at the clinic the next morning and placed on restrictions.

Employee had finished servicing the dozer, climbed back on to remove lockout lock, slipped and jerked their shoulder. They refused treatment, but the next day took themselves to the ER, was taken off work until 5/27, was reevaluated and released to work

Employee was assisting another employee install a solenoid on a water truck when EE reached up to install bolt and felt EE's right shoulder dislocate. EE was taken to the ER, taken off work and referred to an orthopedic specialist. CURRENTLY ONGOING. (has dislocated 3 times prior elsewhere)

The conditions contributing to the injury or illness are primarily related to environmental factors (strong winds, sand, and dust) at the mine site, which led to aggravation of previous asthma for the individual despite the use of respiratory protection (N95).

Employee was pressing on cell line when bar fell in and splashed liquid in eyes. EE was taken to the ER, prescribed multiple application antibiotic, referred to ophthalmologist, who in turn took EE off work for next shift.

Employee was lifting a box and smashed right middle finger between the box and rack/shelf. On 12/9, EE was prescribed an antibiotic for infection, placed on light duty. X-rays ordered. Currently ongoing.

As an articulating truck was backing towards the drill pad at 5 mph, driver failed to see a dozer, striking the rearend. Initially, both operators stated they weren't injured. However, the next morning, the dozer operator complained of neck and shoulder pain. EE was taken to the clinic and placed restrictions. Currently ongoing.

Haul truck operator was dismounting the truck; at the second to the last step, EE's foot missed the step, causing EE to lose balance. EE grabbed the handrail and felt and heard a pop in EE's shoulder. EE was taken to the emergency room and given restrictions. Currently ONGOING.

Employee working in mill basement slipped and fell in mud, landing on left arm. EE immediately informed EE's supervisor and initially declined a medical examination. After bruising appeared on EE's arm, EE and supervisor elected to seek medical treatment on 11/11/24. Employee had acute, traumatic, comminuted fracture of left ulnar olecranon and styloid processes.

Employee was in the field to conduct surveying; while walking to area to survey, they tripped on a rock and fell against some 3" in HDPE pipe, injuring their shoulder. Employee was taken to the ER, diagnosed with a dislocated shoulder and referred. Employee was given restrictions the site will accommodate. Currently ongoing.

Central dispatch notified site of a fire at an abandoned house on an outlying area of mine property, AKA Apache Tejo south of Hurley, NM. ERRT members and Hurley VFD responded, flames were completely out by 4:00 a.m. and fire watch continued. No mining operations were involved or threatened, just located on land the company owns.

Employee was getting out of their service truck and twisted a knee. Employee did not report the event until the next day when their knee swollen and sore. Employee went to the ER and taken off work. Currently ongoing.

While conducting a PM on a light plant, employee was trying to lay the mast down when it became stuck. EE grabbed the bottom of the mast, and it folded back and lacerated EE's right middle finger. EE was taken to the ER, received stitches and released to work.

The driver picked up one of the wheel chocks then felt a twinge in right shoulder. The pain wasn't bad at the time, felt discomfort. After leaving the mine, the driver felt bicep roll up then started to hurt.

Employee was cutting cathode samples when EE moved the table for the next cut. The employee had both hands placed on the table with EE's thumbs placed underneath while in motion. The employee's right thumb was pinched in between the wheel and the table, diagnosed with a contusion, taken off work. Currently ongoing.

Employee was dismounting a water truck and rolled an ankle as they started down the first set of steps, catching themselves before they fell. They initially refused treatment and would ice at home. The next day, it was still painful, and they went to the ER, diagnosed with a fractured ankle and taken off work. CURRENTLY ONGOING.

A D10T dozer component was being repaired and the component tipped over striking the employee. Prior to the event the component was removed from the machine and was placed on blocks and was being dissambled. As parts were removed, the component became off balance.

Employee was exiting the pickup, as they stepped out, they missed the running board with one foot and the other foot got caught under the seat, pulling their thigh muscle. They missed one shift, then released to next rotation, and returned to work full duty.

Employee was training on overhead crane press when they were walking on cell two. Employee was looking back talking to the mentor, when they tripped and fell on hands and knees. Employee was checked by ERRT and finished the shift. Next shift they called off due to back pain, was seen at the clinic and taken off work. Currently ongoing.

Operator was in the process of opening a gate valve on a 6" HDPE pipe and was sprayed with raffinate, which got in EE's eyes. Employee flushed repeatedly but continued to have discomfort. Was taken to the ER, given drops, restricted from driving, and referred to an ophthalmologist. Currently ongoing.

Crate fell from forklift forks. Employee attempted to push crate back onto forks and injured bicep.

While reaching down for a radio on the bottom shelf, employee felt a sudden pain in EE's lower back. Employee went home the remainder of the day, and then on days off. On 6/30, pain persisted, and employee went to the ER, was referred to PCP and taken off work. CURRENTLY ONGOING

While trying to remove an inner tire on a forklift, employee was swinging a sledgehammer and struck the end of right index finger, lacerating & fracturing it. Employee received 4 stitches and given restricted work duties. Currently ONGOING.

Employee was stepping off the 20T crane when EE experienced pain in lower right quadrant, where EE previously had a hernia repaired. EE was taken to the ER and diagnosed with a recurrent right inguinal hernia, referred to PCP/Surgeon. Currently ONGOING.

While fabricating starter sheets, employee's arm was feeling fatigued from pulling hangar bars, so started using the other arm, pulling across their mid-section. When rotating for another bar, felt a pop and sharp pain in lower back. Taken to ER, off work until seen by PCP.

While harvesting cathodes, EE used hook to move hangar bar on strongback and felt pain in lower back, was taken to clinic, released to work. EE worked two more rotations. Was off for personal reasons, then on 5/7/24, received WSR from PCP, taking EE off work as of 4/9/24. Currently ongoing.

Employee had just arrived at the office and told a co-worker was experiencing chest and arm pain. Was evaluated by ERRT and transported, met by ALS and flown to a cardiac hospital in Las Cruces NM. Courtesy notification to MSHA was made. Employee had surgery and should return next week. Personal - Code 8.

Employee was dumping trash, and the dumpster didn't latch. Employee got out of the forklift and went back to the dumpster and tried pushing it down and EE's hand slipped, and EE's right wrist was smashed between the dumpster and the forklift frame. EE immediately reported it to EE's supervisor.

IE was retrieving parts for feed trunnion insert, upon returning they turned around and caught the end of the feed card shroud and tripped. Employee stuck right shoulder on ground.

Employee descending the rod truck was stepping down using the rear tire and wheel hub when EE missed the step causing EE to land awkwardly and twist EE's ankle.

Employee was working above the cell line passing material to a crew member. Employee was wearing full PPE causing some sweat to drip down in eye causing burning to the right eye.

Torque arm for the backstop on Sag2 Conveyor seized, causing the arm to swing around and grazed employee's right arm. EE was taken to hospital and taken off work until cleared by PCP. Currently ONGOING.

Employee was working as crane helper, and reached down to grab a hangar bar and felt a sharp pain in low back. Employee was taken to the emergency room and taken off work until seen by PCP. Currently ONGOING.

On 08/16/23, employee notified supervisor that on 8/15/23 EE had stepped down from a pump pedestal and twisted EE's knee. EE felt some discomfort, and the pain progressed throughout the day and into the night, and EE requested to be seen by a doctor. EE was taken to the clinic, given restrictions and referred to EE's Orthopedic specialist. Currently ongoing.

Employee was pulling hangar bars during harvest, bent down to load bars on bottom row of new bar rack, and felt a cramp in lower back, which turned into a sharp pain. EE was taken to the clinic, given restrictions. Currently ONGOING.

Employee was ripping and relaying shovel pit when employee lost a ripper point. Employee got off and picked it up and got it out of the way. The point was hot and caused 2nd degree burns on employee's hand. Employee was taken to the clinic and given restrictions. After days off, returned for follow up and given full release.

Employee placing bucket protectors in metal bin, while removing lifting strap once placed, the protector shifted, pinching employee's hand just below pinky finger, lacerating it. Employee received 4 stitches and one week of restrictions.

While operating crane, anode hit the cap block, which came off and hit employee's middle finger, causing a small cut near knuckle. Observed by ERRT member and wanted no further treatment, then worked 2 more shifts. On 6/15, employee noticed swelling and went to the ER, diagnosed with cellulitis secondary to cut, and notified company on 6/16/23.

While assisting electricians pull rope through pipe, employee was standing outside the UTV, waiting for the rope when EE lost footing on the uneven surface, and EE's legs went out from under EE, causing EE to do the splits, straining left buttock and hamstring. EE was taken to hospital, taken off work last day of rotation, to return full duty the following rotation.

While performing shovel pit clean up, dozer slipped on a rock operator was trying work out, causing a jerking motion. Employee began to notice soreness in their neck later in the shift. Employee was taken into the ER, and taken off work until the next rotation, causing them to miss one shift.

Employee was washing down grinding media. When EE went to step forward, EE slipped on a ball and fell, injuring their knee.

Folding deck fell and pinched employee on right forearm. This resulted in swelling to the forearm, no fracture.

Employee was assisting another employee in carrying a 2X16 form. When the employee stepped back, they felt and heard a pop in their right heel.

Employee noticed a rash on hands after wearing latex or nitrile gloves all day. Was taken to the clinic and diagnosed with contact dermatitis and returned to work. Different gloves are being used.

While straddling a 2" hose, attempting to connect it to the organic tote truck, employee stepped to the left, tripping on the hose. When employee reached out to brace self, hitting left arm on a concrete wall, employee injured the arm. Employee was taken to the ER and referred to an orthopedic specialist. Currently ONGOING.

Employee was struck in the right elbow by a piece of the carbide steel while standing 4 to 5 feet from the drill rig. The crew were attempting to tighten the casing hammer with the rig's breakout wrench, when a tooth from the jaw broke off and flew the 4 to 5 feet and passed through their long sleeve shirt and entered their inner right elbow, causing a small puncture wound.

Employee was assisting in flagging off a potential icy area, when employee slipped and fell, landing on right forearm. Employee was taken to the ER, referred to a specialist. Currently ONGOING.

Employee was pulling cathode slippers, and when stepping forward onto a bar, they slipped, and stumbled onto the grating, landing on their side. They were taken to the ER, given restrictions and referred to their PCP. Employee was released on 02/21/23, but is on their scheduled days off, will update when they return.

Employee was operating a forklift moving used tires to be transported later in the day. When EE was done with the task, EE stated EE hopped off the Forklift twisting right ankle.

After assisting picking up a slipper, employee stepped on a drain cover, which was loose, causing their foot to fall into the drain, twisting knee. They were taken to the ER, referred to a specialist, and is currently off work. ONGOING.

Employee was task training co-worker on service truck when they hit an icy patch on the haul road and began to slide. When they hit dry ground, they fishtailed and then rolled over. Employees were able to exit truck and call MayDay, taken to ER, examined, given one day off. This employee saw PCP and taken off work through 2/3/23.

As haul truck was backing into the shovel, dipper made contact with the bed, jarring the haul truck operator. Initially operator refused medical attention, but after about 3 hours, requested to be taken to hospital. ER referred to PCP, PCP took operator off work. Currently ongoing.

EE was proceeding on the ground floor of the column cell when EE had tripped and fell, causing right wrist to be fractured.

Employee was walking across the leach stockpile when they rolled their ankle. Employee went to the clinic, diagnosed with a sprained ankle and given restrictions that cannot be accommodated at this time. ONGOING

Employee was exiting mobile porta-john, misjudged step as EE stepped down and fell, cutting knee just below the knee cap. EE was taken to the ER where EE received two stitches, and released back to work.

Employee was mounting the FM22 on the passenger side when employee experienced a personal medical condition and fell to the ground, hitting head.

Employee was doing traffic control for Coring Job when EE saw the Core Machine was stuck, and the operator was having trouble dislodging the Core Machine. EE begun to help dislodge the machine. When it came loose it sprung up hitting EE's Right Shoulder knocking EE to the ground.

The employee was setting up a blast pot unit with two crew members. As they were untangling the air hose(bull hose), the hose slipped from employee's gloved hand and smashed a finger on the other glove and which was pushing off on the unit. The result was two splits on their finger requiring stitches. The result was "No Restrictions" by the hospital.

Driller was removing bolts from the bonnet on the drill head. The driller was using a ratchet with an 3/8-inch drive Allen socket trying to remove the bolt. The bolt was tight and when pushing on the ratchet, the bolt suddenly broke free and is when the drillers shoulder was jolted. Medical treatment sought 11/11/2022.

Employee was descending a 50-ton truck and slipped off the bottom rung of the boarding ladder, fell backwards and hit head on the ground, causing a laceration which required one staple to close.

Employee was exiting the main security gate when EE fell down the stairs and landed on the pavement below. EE had put arm out to brace, and felt pain in the wrist and arm. EE waited to notify the supervisor and was taken to the ER, and has been taken off work until seen by a specialist.

Employee was troubleshooting the Peri after reports of actuator problems. There was feed on the grating, and employee's foot slid then caught, which jarred employee's knee. Employee worked the rest of shift, then attempted to seek medical attention on 9/7, finally seeing a PA on 9/8, and was given restrictions. Currently Ongoing

Employee was conducting a PM on a boom truck and fell to the ground, causing a pulmonary contusion, fractured clavicle, and rib. Employee was taken to the ER, hospitalized overnight, released and is currently off work. ONGOING. MSHA was immediately notified out of an abundance of caution.

Employee was scanning cathode bundles before loading on a truck, when EE stepped onto the bundle to unfold the band. As EE was getting down, slipped, and grabbed the band, lacerating EE's left middle finger, which required sutures to close.

One of our employees was climbing a fixed ladder on the Sag 2 Feed Cart, when the bottom supports of the ladder collapsed; pinning EE's left hand in-between the ladder and the upper support. 1 broken finger with 9 stitches between 3 of the fingers on EE's left hand.

Employee was placing starter sheet in missing cell slot, when the hangar bar and attached starter sheet struck employee's right foot. Employee was examined by a first responder and returned to work, finishing the shift. The next evening, employee felt enough pain employee went to the ER, rather than work, and was taken off pending PCP review.

EE was unloading scaffolding from bed of truck stepped on a tire to reach them and foot slipped to the ground.

Stripping drip line off the slope. Drip line came loose employee lost balance, causing a sprain to the right ankle.

Employee operating the overhead crane during harvest. While traveling to wash tank a cathode fell off the strongback onto the cells. Employee attempted to move cathode off of cell, and felt a pop in right forearm/bicep area.

Employee was pulling plastic on the stockpile, when EE lost EE's footing and fell. EE initially got up and went back to work. Later, after leaving work, EE went a PCP, and taken off work due to potential fracture below knee and MCL strain. Currently ongoing.

Crew were removing walk way flooring of the rig. Trainee driller was under the deck on a 6' A frame ladder loosening bolts when the ladder kicked out from underneath them. EE put a gloved left hand out to stop themselves, and the hand made contact with the steel casing rails and cut the left palm open. EE received 8 stitches to the left palm and was released back to full duty.

As employee was descending stairs, toe of right boot caught on last step, and EE's left foot went to the floor, causing EE's left ankle to twist. EE was taken to the ER, diagnosed with a fracture. Currently ongoing.

Employee was shoveling up lime in the slaker basement, then moving it about 10-12 feet away. After dumping the wheelbarrow, employee made their way back, set the wheelbarrow down, and felt a sharp pain in lower back. Currently ongoing.

3/4" plate was held into position with another person, adjusted plate on floor to weld in place, after adjusting plate felt pain in left lower abdomen. Which followed with consistent pain. Awkward position for a long period of time.

Employee was leaning over a pipe to open a drain valve, and when they pulled up on the valve handle, they felt pain in lower back. Employee was taken to the ER and given light duty, which the company is accommodating.

While working on a haul truck, employee stepped wrong in a parking ditch while picking up tools and felt pain in lower back.

Employee was installing dripline on the 7265. EE cut the dripline with a pair of pruning shears, placed them in their pocket, and when EE bent over, cut the right side of EE's abdomen. The cut required stitches to close, and restrictions. Ongoing.

One of our employees was using a pry bar to line up a false bottom in a rougher row cell when the bar slipped crushing EE's pinky finger in between the bar and the cell wall resulting in a broken pinky finger on EE's right hand.

Employee was getting onto a platform when EE's hand slipped, causing EE to fall back and land on a gang box, causing injury to EE's lower back and wrist.

When entering the employee parking lot on 10/07/21, employee hit a pot hole and twisted EE's back. EE reported the condition at the morning tailgate. On 10/12/21, employee requested time off work to seek chiropractic treatment, and time off 10/13/21 to see employee's PCP about pain back EE claims is from hitting the pot hole in EE's personal vehicle.

While troubleshooting a launder level transmitter, the employee tripped and injured knee. Employee continued working, and at end of shift asked for medical attention. Employee was taken to the clinic and given restrictions. Currently ONGOING.

Employee slipped exiting out of the cab of the backhoe, missed the grab handle and fell backwards, hitting head on the ground. Employee was taking to the ER, got 3 staples, and released to work.

Employee was pulling (hangar) bars and felt a warm sting in their back. Employee continued to pull bars until they said it hurt too much. Employee was taken to the clinic, given restrictions the site is accommodating. On Going.

Employee tripped on raised grating, and felt a pinch in their back. Employee initially refused treatment, worked the 19th, then on the 20th requested medical attention. Employee was taken to the clinic, and was given restrictions which are being accommodated. ON GOING

Haul truck operator was spotting at shovel when the shovel suddenly lost power, and the dipper dropped on the dovetail, jarring employee. Employee was taken to the ER, and released. A week later, employee concerned with residual pain, requested to be seen again, was given physical therapy, which employee attended the first time on 9/27/21, and will have a total of 6.

Employee began noticing some cramping, had requested squencher drinks, then shortly thereafter supervisor discovered the cramping was worse. EE was taken to the ER, given IV fluids for dehydration and released to next shift.

Repetitive motion injury. Soft tissue injury

On 8/27/21, WC carrier notified by email that our employee claimed a work related injury. EE slipped, but did not fall, while on the sample circuit, causing pain to EE's left knee that EE had injured during personal activity in 2016. Currently ONGOING.

Prior to normal shift starting, employee arrived early and was walking from pump house to the water truck, tripped and fell in a seated position onto the ground. Employee worked the shift, but the next day woke in pain, went to the ER and was taken off work, until further evaluation by PCP. ONGOING

At the start of shift August 14th, employee reported having back pain as a result of hitting a hole on the road to the crusher the previous shift. It had been raining on and off all day. Employee was taken to the ER, and given a full release for 8/17/21.

Employee came to work 08/14/21 and reported having back pain as result of hitting a hole in the road on the 44-shovel run, near the entrance, by the spanners, the previous shift. Employee was taken to the ER and given a full release for 08/17/21.

While pulling hangar bars out of the rack, one was wedged and employee states put some extra force in pull and felt a pop in right shoulder. Employee was taken to the clinic, referred to Ortho, taken off work and is currently ongoing.

While working the UPS shipment, employee felt a pop in the wrist. When traveling home, noticed it had swollen and went to the ER. EE was referred to an ortho, who gave EE restrictions, currently ongoing.

Employee had been assigned as a spotter in the pit, sitting in an air-conditioned man van. Employee noticed a low tire, called for tire person, assisted in the tire change. Employee got in the van, called dispatch saying employee felt overheated and they initiated a May Day. The employee was taken to the ER, diagnosed with Heat Stress. Treated & released.

Employee stepped on an elevated (10"-15") ergonomic platform when it suddenly dropped, causing EE to jar EE's back. Currently ONGOING.

Mechanic was changing out a slack adjuster on the 731 haul truck, and after a while began to feel pain in their back and left leg. Employee continued working, but reported ongoing pain the next morning. Employee we taken to the ER, given meds, missed the 23 & 24, and has restrictions. Currently ONGOING. Aggravation of pre-existing condition.

Mechanics were installing drive belts on the 32PP07 feed pump, were jumping a belt to its correct position on the pulley when employee got the tip of EE's pinky caught between the belt and pulley shiv, and rolled it through to the other side, crushing EE's fingertip. Taken to the ER, taken off work, referred to ortho, currently ONGOING.

On 5/14, employee called off work stating they'd rolled an ankle near the banding table on 5/12 around 9:30. EE didn't think much of it, even worked a full shift 5/13. Felt they needed to rest it on 5/14, first report to company. Company sent them to the clinic, where they were given restrictions. Currently ONGOING.

While driving down the tailing ramp, water truck hit a rut, causing front tires to jerk, and caused pain in driver's shoulder.

Employee was returning to pickup after a PM at the sewer plant, stepped on a rock on some grating and rolled ankle. Employee was taken to the clinic, diagnosed with a sprain. Employee was given restrictions which the company is accommodating.

Employee was spotting under shovel, did not stop in time after hearing shovel horn, and hit the dipper with the truck bed. Employee reported to the supervisor at end of shift, and refused medical treatment. Start of shift 03/19/21, employee requested medical treatment, was given restrictions the site is accommodating.

As employee was walking to the plant from the parking lot, EE slipped on black ice, landing on EE's back pack. EE initially refused treatment. EE notified operator on 1/28/21 EE was seeing EE's PCP, operator followed up with Occ Med. EE was given 1 day restrictions, full release 2/1/21.

Driver States: I started to climb out of the cab and I had my Right foot on the top step and right hand on the grab bar and left hand on the door and missed the step coming down with my left foot and I fell onto the ground from the cab. I fell on my left side when I made contact with the ground.

Employee was exiting service truck, as they stepped down they felt a pop in right knee. Employee was taken in to be checked, was released with restrictions and referred to ortho. EE was seen by Ortho 8/28/20 and given a full duty release.

Employee got into the back of a pick up bed to fix an issue with a buggy whip, as EE stood up from a crouched position EE lost bearings, got light headed and took some steps backwards and misjudged the end of the pick up bed, fell on back and hit head. EE was checked by a Supervisor, refused medical treatment. On 07/09/2020 Employee went to ER, taken off work one day.

Mechanic was using a drill to drill out a hole in a plate mounted in a vice. The drill bit caught, causing the employees hand to contact the corner of the plate. This caused a laceration on the hand. EE was taken to the ER and taken off work.

Was doing shop cleaning when EE tripped over oil drain cart and fell straining left shoulder.

While assisting in replacing a lower roller shaft on the ES43. As the shaft was pushed in, it broke the keeper welded on the inside, which struck a mechanic on the left arm, above the wrist. Taken to ER, released back to work; went to PCP several days later, referred to specialist, given restrictions for 2 wks. ONGOING

Employee drove haul truck through pot hole, jarring back. Employee continued to operate truck until later in the afternoon when pain began to bother them. Employee was taken to the clinic, and was given restrictions. Delay in accommodating, causing 2 lost work days.

Warehouse employee was relocating three 55 gallon drums in back of bed of truck to balance load. The drums of oil and DEF were being delivered to customers outside of mine. Initial injury appears to be a strained back.

Employee was driving an empty haul truck when EE hit a hole in the road that jarred EE's back. EE received first aid treatment and continued working. The following day EE reported to work stating EE was stiff and sore, and requested to see a physician. EE was given restrictions, CURRENTLY ONGOING.

9:00-9:05, blade operator requested supervisor or mine tech. When mine tech arrived, employee stated employee thought employee was having a heart attack. MayDay was called, taken to ER. 9:46 site notified no heartbeat, 10:55 declared deceased. Courtesy call to MSHA, inspector investigated for chargeability, stated likely personal illness at investigation close out.

Employee was removing pillow block from apron feeder, the block fell landing on end of left middle finger. Employee did not report until 11/22/19, was taken to the ER and diagnosed with distal fracture, and given restrictions.

Employee had left office headed to employee parking lot, walking on grating by the warehouse, when EE's foot got caught on a stake holding the grating. EE fell, landing on EE's left knee. Taken to the ER, referred to specialist, diagnosed with fractured patella. Currently ONGOING.

As haul truck driver was walking around the haul truck, conducting end of shift inspection, EE slipped and fell down, and felt ankle pop. EE was taken to the ER, has a displaced fracture, taken off work. CURRENTLY ONGOING

Employee was looking left, checking mirrors when the truck rocked, tweaking EE's neck. EE had pain in the neck and shoulder, and couldn't turn head. EE was taken to the ER, taken off work until 9/20 full release, and suggestion for F/U with PCP. Was released 9/24 at MMI. Next shift is 9/26 graves.

Attempting to lower mast on light plant, caught fingers between mast and frame, cutting middle finger. Examined by first responder, taken to the ER, received sutures to close. Returned to work, no restrictions.

Employee was driving an empty haul truck back to the shovel when the employee hit a bump and jarred back. Employee was taken to the ER and taken off work.

On 8/21/19, employee claims they had leg cramp from getting out of a pickup on 7/25/19. Company highly questions validity of claim for many reasons, and after investigation, can neither prove or disprove validity. Filing in good faith since employees claims it happened at worksite. Currently Off work.

Employees changing out corner shroud; employee struck shroud with hammer, felt something hit EE's leg, and noticed it was cut. EE was taken to the ER, where an embedded piece of metal was removed. No sutures.

Employee was disconnecting an airline from a pump, when the hose came off, spraying degreaser in employee's face. Employee flushed face and eyes, was taken to the ER, and referred to an optometrist, who took employee off until released to full duty 7/30.

Employee stepped off dozer, onto rock, and rolled ankle. Employee was taken to the clinic the next morning, given restrictions company is accommodating. Currently ongoing.

Employee reported to supervisor that EE had swelling, itching and burning sensations on both legs from the toes to above the ankle, that had begun about a week prior. Taken to the ER, diagnosed with dermatitis and MRSA staph. Employee was placed on restricted duty, then released on 7/9/19.

Employee was exiting on the SW side of the pond when EE slipped and fell down and landed in the trench.

Moving a jack under the machine. Gloved hand was pinched between hydraulic tank and jack handle. Finger laceration.

Employee was installing drive belts on the #3 ball mill feed pump. While rolling belts onto the sheave, caught fingers, fracturing ring finger. Taken to ER, given restrictions.

Employee was driving up a ramp after being loaded, felt like the truck may have bottomed out, reported to the supervisor, had EE checked by first responder. Employee waited until the next day and sought medical attention, was taken off that shift due to lateness in day before shift.

Employee was collecting metallurgical samples from blast hole, when lifting 2 buckets, one in each hand, felt twinge in back. Told supervisor next day after feeling more pain in one area. On 4/15, reported increased discomfort and requested medical treatment. Was given restrictions initially, then sent for MRI, taken off work as 4/24, pending therapy. ONGOING

Employee was starting to climb a small berm, as they started to step they felt a sharp pain in the left calf.

Employee was welding wear bars at the primary crusher. At the end of shift ee didn't note any issues, after getting home, ee notice vision issues with left eye. EE went to optometrist and was taken off one day of work.

Employee was helping pull cable out of road below the 46 shovel. While pulling a portion, they heard a pop in the right knee. At 4:30 a.m., they requested medical treatment. Employee was taken to the ER, and referred to a specialist.

Employee slipped and fell on ice at queue point, causing a closed fracture of lower leg. Currently on-going.

As one mechanic was swinging a hammer on a Victaulic clamp, on the six swing, the hammerhead came off striking another employee's hardhat. That employee was standing approx. four feet away. Employee sustain a laceration, was taken to the ER, and taken off work for one day, possible concussion, no stitches.

Employee was tightening a bolt on bumper when wrench slipped and top of EE's hand hit the bumper and cut the hand.

Employee states was jostled in man van headed to haul truck, causing pain in shoulder. Employee reported it at 5:30 in the evening, was taken to ER, and taken off work one day.

Haul truck brakes had locked up; a mechanic was called. Mechanic had parked in front of haul truck. When ee completed repairs, driver was to back up to test brakes. Driver stated truck began rolling forward, over berm, hit service truck which caught on fire, which in turn caught the haul truck on fire. NO ONE WAS INJURED.

Employee had completed pre-shift on service truck, turned on PTO and compressor to check fluids, when the hose came off the ball valve, hitting the side of EE's face, knocking off safety glasses and hardhat. EE was taken to the ER, diagnosed with corneal abrasion, and released to next scheduled shift on 1/19/19.

Employee was loosening a bolt on an excavator during radio installation. EE was using a breaker bar, and when the bolt loosened, they felt a pop in the shoulder. EE was taken to the clinic, given restrictions, and released to work. Currently ongoing. MRI pending.

Due to engine failure, employee thought haul truck was on fire, and in an attempt set off fire suppression system, injured/bruised wrist. EE was taken to ER and given a full release (Fri., start of days off). On Monday, morning of next shift, contacted company, complained of ongoing pain. Taken for further evaluation, given restrictions, pending MRI, currently ongoing.

Employee was descending stairs, slipped on mud and while trying to catch self from falling, strained right shoulder. Employee was taken to the ER, referred to an ortho, who then prescribed a sling, and restrictions the company cannot accommodate, pending an MRI. Currently ongoing.

Employee was in a laydown yard, tripped over uneven ground, and hit ribcage on a rock. EE saw PCP, who didn't have X-rays done, but took employee off work. Employee requested 2nd opinion, was x-rayed on 11/28. X-rays were negative, however, physician did not release to work until 11/29/18. One day lost time.

While cutting out a broken liner on the SAG2, a rubber lined "pulp lifter" began to smoke and caught fire. May Day was called, plant evacuated. The fire exceeded the 30 minute time frame, MSHA notified by phone. Fire put out by rotating the mill and spraying with water. No injuries, minimal damage.

Haul truck operator notified supervision of eye irritation, an EMT responded and flushed eye, employee thought the debris was out. Before next shift, employee reported irritation was worse, went to the ER, was given a prescription, and taken off work 2 shifts.

Employee was opening a 10" manual gate valve at the tailing pond when EE's hand slipped off the valve, causing EE's shoulder to contact the valve. EE was taken to the clinic for evaluation, and given restrictions of no use of left arm.

Coolant line on haul truck failed, spraying coolant on deck. Employee downed the truck, and as they stepped out on the deck, waiting for the mechanic, stepped on non skid resistant area, slipped and fell. EE was taken to the ER and referred to PCP, given restrictions which can not be accommodated. F/U 9/20/18.

Employee was conducting EE's pre-shift, stepped in a mud hole and felt pain in EE's heel. EE continued to work, but later reported to EE's supervisor. EE was taken to the ER, x-rays negative, given restrictions.

IP was helping to trip pipe, leaned forward to secure drill pipe to drill string. Left foot slipped off side of drill head and IP fell forward, to the left of drill head. Left hand that was holding onto the drill pipe hit against old pipe wrench backup plate, catching left ring finger between the drill pipe and the plate.

Contractor shoveling concentrate and lime paste toward vacuum hose to be vacuumed up and disposed of. Lime and water came in contact with employees pants and caused irritation and lime burns at below the knee.

IP was removing core from core tube. Approximately 3' section stuck, so with core tube on core slide, struck it with leather hammer. While beating the core out the tube fell off the slide and struck left foot resulting in contusions.

Hit gloved hand with 5 lb dead blow hammer lacerated finger that required 8 stitches.

Sliding compressor with FMI employee to the stairs, lifted the compressor into doorway and felt a tug/pop in the rib. Was taken to urgent care to be checked out by GVE Supervisor.

While working on the fuel pumps at a fuel dock, employee felt an irritation in left eye. EE thought it was an eyelash, but was also wiping sweat with the sleeve of coveralls. At some point, flushed eye several times. After going home, went ER, employee had an embedded metal fragment. EE was taken off work 2 days.

Employee was walking through the 73/74 sump area to collect a sample when employee slipped but caught self. In doing so, employee injured knee. Employee reported to supervisor, was seen by ERT, then went to the ER, taken off work until seen by specialist. Currently ONGOING.

Employee was working in flotation during a shutdown, and begin feeling like EE had heat exhaustion. EE stopped for an hour break, and replenished fluids. At the end of the shift, EE reported this to the supervisor. The next morning, EE thought EE was better, but about 9 am began to feel light headed, was taken to the ER and given IV fluids. Released to work.

Employee was spray painting a stake, miss-stepped on uneven ground, slipped but caught self and twisted back. EE continued working, but later felt a pop and more discomfort, was taken to the ER, referred to PCP. Has been given sedentary duty. Currently ONGOING.

Employee was training on a new water truck, and as EE was exiting the cab, lost EE's footing. EE reached for the handrail and EE's arm slipped between the two rails, cutting EE's lower on arm on a zip tie holding some cables. Laceration took 4 stitches to close.

Pit mechanic was exiting a pickup when their foot slipped on the running board. Mechanic felt pain in the lower back when reaching for the grab bar. Mechanic was taken to the ER, referred to their PCP. PCP initially released to full duty 6/6, at follow-up on 6/4, released with restrictions. Currently ONGOING.

Employee was moving 50# clay sacks and felt pain in the lower back. EE finished the shift, but the next day before returning to work, went to the clinic, and was given restrictions. EE went for a follow-up on 5/29, and was taken off work. ONGOING

While cleaning up after job, employee reached to open the skidsteer door and felt a pop around belly button. That night ee felt pain and a bulge. It felt better next day, so ee went to work. After a few hours notified supervisor of increased pain, taken to ER, diagnosed aggravation of pre-existing umbilical hernia. Surgery to repair 4/25/18. Currently ongoing.

Employee was collecting samples, descended stairs backwards, at the bottom stair, ee placed right foot twisted to the right, then ee turned to the right, and buckled knee. Taken to the ER, referred to specialist, MRI shows meniscus tear. Currently Ongoing.

Employee was working in the shop, turned quickly to reach for something, and felt EE's calf tighten up. On 4/3 employee sought medical attention and was taken off work. Currently ongoing.

Employee slipped while conducting area inspection, landed with foot under them, and felt pain. Employee was examined by an ERT member and opted to go to the ER where employee was diagnosed with a fracture.

On 3/10, sx/ew employee told supervisor of a rash on foot that had begun on or about 2/1, that had turned into sores. EE was taken the doctor, and taken off work until 3/23.

After first dipper was placed in haul truck, material rolled down from shovel bank, striking the filter housing, and jarring the driver. Driver initially refused treatment, but the next day prior to the shift, went to the ER and was taken off work until 2/16, a day off.

While fabricating starter sheets, standing on grating, the grating separated, causing employee to roll ankle. Employee was taken to the clinic and taken off work 1 week. On 2/15, employee returned to PCP, was given a boot, and taken off an additional week. Currently ONGOING.

Mechanic was helping install tracks back on a drill, watching the track and idler for clearance, EE heard a pop, then felt something hit above left eye. EE didn't realize was hurt until EE wiped face and saw blood. After EE got home, felt something there, went to PCP and a small piece of metal was removed. PCP gave EE a day off work.

Employees were replacing feed pump on ball mill, and while putting v-belts back on, employee got 4th and 5th fingertips caught between belt and motor sheave, fracturing them. Employee was given restrictions the company is unable to accommodate. Currently ongoing.

Employee chasing threads on shovel keeper bail on vise on service truck. Wrench slipped and employee fell to the ground, on shoulder. Initially refused treatment; 1/8/18 requested to be seen at Orthopedic clinic, taken off work pending MRI, results rec'd 1/12/18, referred for surgical repair. ONGOING.

Employee was walking up the steps at GO annex, failed to raise foot high enough, tripped and fell on elbow. Was taken to the ER, X-rays negative, was returned to work. 1/9/18 employee requested further treatment due to ongoing pain; taken to PCP, given restrictions and therapy. ONGOING.

Haul truck and dozer operator lost sight of each other and made slight contact (no damaged to equipment reported.) ERT members conducted precautionary exam on operators, and dozer operator's vitals were elevated. Employee was sent to the ER and diagnosed with cervical strain, taken off work one shift, then started long days off.

Employee was turning valve to dump pump box on re-grind mill and felt pain in shoulder. EE was seen by a first responder, requested further medical treatment, seen by PCP, taken off work until next shift rotation. EE came to work, claimed EE couldn't do the work, sent to PCP, taken off another week. On going.

While doing clean up in front of the sag mill, employee was pulling a wash water hose to wash down the area. Later at home, moving EE's shoulder around EE felt grinding, and claimed it was due to work. This event was questionable, but after a full investigation, and receiving medical records which allows an injury in line with the work, the claim was accepted.

Employee, from a standing position, reached for backpack, felt tightness and pain in lower back. Employee refused to be seen by an ERT member, requested to leave work. Employee drove self to the ER and was taken off work until 11/24/17.

Employee had finished cleanup, was walking towards the stairs when they slipped and felt their knee pop. EE iced it for two days, then requested to see their PCP. EE was given restrictions the company cannot accommodate. CURRENTLY ONGOING.

Employee was rolling up window on haul truck when ee felt a pop in shoulder. EE requested an ice pack, then later requested further medical attention. EE was taken to the ER, given restrictions, and referred to a specialist. CURRENTLY ONGOING.

Employee was reaching to get rope from the cable truck platform, with one hand on the rail, when a chock on top of the rope smashed employee's finger between the chock and the rail, causing a closed distal fracture.

Dozer operator was pushing material on stockpile when they backed over a rock, jarring them, and felt pain in the left lower ribcage. Employee requested to be seen by an ERT member, initially declined further treatment. Later they requested to be taken to the ER. Employee was given 2 days off work.
