EE climbed up on to an excavator track and grabbed on to the door handle. The door was not latched, it swung open. As a result he fell to the ground landing on his right arm and shoulder, resulting in a break to his arm.

WORKER BANDING STONE ON PALLET, STONE SLIPPED OFF OF PALLET AND FELL ON FOOT. RIGHT FOOT FRACTURE.
Worker was splitting large piece of stone when the top layer slid off of main piece & hit his arm. Right radius fracture - required surgery.
