MAN WAS DRIVING 922B LOADER DOWN A ROAD WITH A STEEP GRADE. FOR UNKNOWN REASON, LOST CONTROL AND THE LOADER TURNED OVER AND THE RAPS SAFETY CAB FAILED AND COLLAPSED CAUSING FATALITY.
