Eagle Butte Mine Coal

On a wet slick road a scraper operating at 10 MPH began to slide. The scraper stopped abruptly as it went from wet to dry conditions. Operator hit EE's back on the arm rest resulting in back strain. No damage to scraper.

On 9/2/23 Employee reported pain in back with no event, was evaluated and released to return to work. Pain got worse and employee went back to doctor. Doctors' notes received 10/10/23 & MRI revealed multi-level spinal degeneration and prior compression fractures in the lower back.

Employee has a previous injury that has progressively worsened over time. Employee went to orthopedic doctor and was diagnosed with a herniated disc and pinched nerve in L-5 of the spine and was taken off work until physical therapy could help resolve this condition. No event at the mine occurred to cause this issue.

Team member got out of company truck and had sudden pain going down left leg while standing. Outside conditions at the time were sunny and dry.

On 4/1/23 Miner slipped and fell on ice, diagnosed as PCL sprain will heal on its own. Miner reevaluated as knee was not improving and had an outpatient procedure performed to repair PCL on 8/31/23 and allowed to return to work but prescribed to go to physical therapy. No days were missed from this injury.

On 2/9/23 Electrical Planner slipped on ice walking around substation repair. No treatment. On 2/24/23 not improving went to doctor and diagnosed as sprain. No improvement and MRI taken with no findings. Another MRI taken 3/8, on 3/16 diagnosed as a muscle tear. Outpatient procedure performed on 3/16 applied brace, released to work and prescribed physical therapy.

Miner was performing utility tasks and moving shovel trail cable, when picking up cable felt pop in lower back. Miner went to ER and diagnosed with a muscle strain. Returned for follow up and physical therapy was suggested. Miner went to physical therapy on 7/16/22 for muscle rehab.

Rubber tire Operator was pushing material off Dump 1 in the west pit. The Operators hand came off the joystick controller and struck the console resulting in a fracture to the right hand 5th metacarpal bone.

Employee was loading skid steer fork attachment into skid steer bucket. The attachment got hung up on cutting edge bolt. When employee pulled on fork attachment, it came free and slid into bucket pinching the employee's hand between the attachment and bucket. After taking glove off employee could see this caused a laceration to the employee's hand requiring stiches.

Team Member stepped off rail car ladder onto ground and felt a pop in their heel.

Employee was standing on a two step ladder, using air gun to clean off coal dust off the prep hood. As the employee was stepping down from second to the first step, right foot missed the first step causing employee to fall. The employee reached right hand out to break the fall causing a hair line fracture to wrist.

A mechanic was under the front of a truck trying to repair a coolant leak. In an attempt to tighten the original clamp first, the clamp stripped and became loose. The hose connection came apart, and hot coolant poured out of the hose onto the employee's chest, forearm, and face. Employee was taken to the ER, referred to a burn center, treated for 1st and 2nd degree burns.

Electrician was adjusting power cable on elevator when the leather of the back of the electrician's left boot was pinched between toe guard and floor. This resulted in a laceration that required 10 stitches. Was given a work as tolerated release and missed no scheduled shifts.

On 9/28/2021 pumper pulled a piece of 6-inch poly pipe out of the mud, felt something in right shoulder region crunch/pop. Pumper stated no pain was felt, decided not to report anything to supervisor. On 9/30/2021 the pumper experienced pain in the shoulder region and decided to report the incident to supervisor. Pumper began receiving chiropractic care on 10/01/2021.

Mechanic had cut a piece of all-thread. While cleaning off the excess thread with the wire wheel, the wire wheel caught the piece and smashed EE's left index finger between the piece of all-thread and guard resulting in a laceration to the tip of the finger. Sutures were required to close the wound. Mechanic received a work as tolerated release and returned to work.

Employee exiting the porta john, pushed the door open, wind caught the door slamming it on employee's right hand. ER evaluation x-ray was negative with a work as tolerated release, second evaluation at the Coalition could not confirm or rule out a small fracture. Was given a splint to immobilize 2 fingers and given a work as tolerated release. No scheduled shifts missed.

Employee was opening a lid on the truck to reload it with product. Lid was stuck, so employee reached under the lid with fingers to pry it open. The lid did not open and pinched fingers in a scissor-like format, resulting in a laceration to the tip of right ring finger. Received 1 stitch.

Employee was removing a pin from a hydraulic cylinder with a hammer, felt a pop in shoulder and experienced some pain. Was evaluated at CCH and received a work as tolerated release. An MRI determined a torn rotator cuff. Surgery was performed on 01/21/2021 which is the first scheduled shift the employee has missed.

Guard shack attendant stood on a chair to reach an upper shelf, fell landing on left shoulder. Was diagnosed with a re-aggravation of a previous injury to the left rotator. The employee was going to be given a work as tolerated release but requested to be removed from work activities until released post operation on 9/10/2020. Surgery will follow when it can be scheduled

Employee was walking at the west spoon fuel island and tripped on uneven ground, stiff armed the ground with left hand. At the time of the report EE stated EE was fine and that EE just wanted to report it.

When replacing the control arms on the front of 21.4567 service truck the mechanic was using a pry bar to get the control arm into place when the bar slipped off the control arm causing the control arm to fall on EE's left thumb causing a fracture to the tip of the thumb bone.

Mechanic leaned an 8 ft ladder up against the outside right front haul truck tire to perform an inspection. EE was descending the ladder, the ladder slipped and fell to the ground, the ladder landed on the mechanics right hand and they landed on top of the ladder. This caused a dislocation injury to the right hand. Employee had surgery on 6/2/20 to repair the injured hand.

Employee was removing the water truck tank drain hose, the hose popped loose from the fitting striking the employee in the mouth. The impact resulted in a contusion to the lower lip and chipped the front left tooth. The tooth will need to be capped and the employee did not miss any scheduled shifts.

Employee arm-fingers swelled 1 hour after using torque wrench, taken to ER received work as tolerated release. 4/01 ESM notified, employee would need shoulder surgery 05/06. ESM disputed claim, 06/05 WC review surgery payment denied. 9/29 ESM notified by State of Wyoming Medical Commission honored in favor of employee WC claim, shoulder injury linked to incident on 01/15/20

Operator tramming track dozer 26641, encountered a rock caused operator to inadvertently set park brake. Machine suddenly stopped, operators left shoulder jammed causing a shoulder strain. Employee was given a work as tolerated release 1/2/2020, then on 1/9/2020 notified the mine that the MRI showed rotator cuff tear, taken off work 1/9/2020. Begin physical therapy 1/13/2020.

Employee was in the office, experienced chest pain and slumped over. Rescue team administered CPR and used an AED. EMS was called to the mine and took over the first aid, employee was transported to the hospital, diagnosis was cardiac arrest and admitted to the hospital.

Technician was drilling holes in a plate, stopped the drill press to clear the metal shavings from the drill bit and a piece of metal went through glove resulting in a laceration to finger. The employee received 9 stitches to close the wound and released to work as tolerated.

Plant Tech was removing ice from a conveyor head roll area. Employee used a bar to remove the ice the bar was pulled into the head roll and frame pinching the techs left hand between the bar and the frame work causing a little finger injury. The employee had surgery to remove part of the distal bone from the little finger. No scheduled shifts were missed.

Injured employee was assisting an employee in removing the engine from a haul truck. They were removing the spreader bar when the shims shifted, the injured employee then stuck left index finger in a bolt hole to prevent them from falling to the floor. As the last bolt was removed the spreader bar and shim pack shifted and pinched the IE's finger between the shims and frame

Mechanics had removed the old edges and cleaned the surface for the new ones. While lifting the edges a bar is used through the bolt hole so it is easier to carry. The edge slipped on the bar and pinched the mechanics finger between the bar and the edge resulting in a laceration. Employee received 3 sutures in EE's left finger work as tolerated and returned to work.

While washing, operator looked up, a small chunk of coal fell and landed between EE's face and safety glasses, causing coal dust to contact the eyeball. Operator was taken to the ER and small specks of coal dust were flushed out of the eye. Employee was given eye drops and pain medication with a work as tolerated release and returned to work.

Loading a trailer, a gust of wind caused the trailer doors to close, hitting EE's face, causing lacerations to EE's face.

Operator dismounting a motor grader, missed the step & tried to catch self with left hand & ended up twisting shoulder & heard a popping noise. Employee was diagnosed with a ruptured bicep tendon & rotator cuff tear on 10/05/18. Employee has not missed any scheduled shifts & has scheduled surgery for January 2019. EE had surgery 1/21/19 & will miss upcoming scheduled shifts.

Employee slipped on the ice walking to a crew van causing pain in EE's right knee. The employee had a non work related surgery on this knee in Nov 2017, an MRI was performed on 3/6/18 which resulted in knee surgery on 3/20/18 with a diagnosis of a recurrent symptomatic medial meniscus tear. The first day the employee missed work was on 3/19/18

Warehouse employee was boarding a fork lift, employee's shoes had some mud on the bottom, employee slipped and bumped right knee. Initially it appeared the knee was just bruised. On 2/27 the doctored order an MRI because knee continued to be sore, the MRI determined a torn meniscus, the doctor ordered physical therapy. The first physical therapy visit was on 3/13/18.

Operator positioning a light plant on the bench, when EE got out of EE's pickup EE rolled EE's ankle. EE felt popping and pain was evaluated by the EMT then taken to the hospital. X rays revealed EE had fractured EE's right fibula. The employee was given a splint and crutches then sent home.

Utility person was moving a shovel, the shovel cable horn flipped over jerking the rope the utility person had a hold of which caused a pop in the employees left elbow area. An MRI showed the bicep muscle was torn, on 08/29/2017 the employee was prescribed a brace, surgery is scheduled for approx. 09/05/17. The employee has not missed any scheduled shifts until after surgery.

Employee was hooking up a 6" suction line to a pump. Employee felt pain in lower back and was looked at by EMT. Employee denied further treatment at the time it was reported. Employee notified Employer on 7/17/2017 that ee had been seen by the VA in Denver and diagnosed as a muscle sprain. Employee began physical therapy on 7/18/2017 and missed no scheduled shifts.

Mechanic was adjusting a jack stand, EE put EE's middle finger in the adjustment hole to help lift the jack stand, the jack stand came loose and fell pinching EE's finger in the hole causing a laceration that required stitches. The mechanic was released to work as tolerated and returned to the mine to finish the shift.

Fueler was driving fuel truck and drover over a windrow. This caused the steering wheel to jerk and pull EE's left arm. Fueler reported the incident and the EMT looked at EE's arm. EE returned to work. On 8/8/17 the employee informed the mine that EE has been taken off work and will be receiving surgery due to injuries from the incident.

Mechanic was cleaning up a fuel spill, attempted to pick up an oil absorbent boom that was frozen to the ground and right knee popped and was unable to put weight on the knee. Mechanic had been having problems with this knee and was receiving professional treatment prior to this incident.

Plant employee was changing out a roll on the storage conveyor belt, the roll slipped pinching the employee's middle finger resulting in a chipped bone & laceration to the end of the middle finger. The end of the middle finger received sutures, the finger nail was re-attached with sutures. The employee was given a work as tolerated release and returned to work immediately.

Rubber tire dozer operator was operating a dump, the operator was making cuts on the dump surface to re-grade the dump and created a hump on the dump surface. The operator backed over the hump and jarred back resulting in a pinched nerve in back. The employee will need physical therapy and is expected to return to work on 01/27/17.

Operator dismounted rubber tire dozer. Slipped on ice. Employee felt they did not need any medical care at that time. On 4-3-17, employee informed company that on 3-31-17 employee received results of x-ray and MRI. Employee will be receiving physical therapy treatment on 4-10-17, for continued numbness in left arm. To date, employee has not missed any scheduled shifts.

The injured employee was working on the front strut of a CAT 793 haul truck. The EE was moving the pillow block when it rolled back and bent fingers back, injuring wrist.

Water truck driver reported a noise while steering to the right. Pit mechanic installed new grease lines, the mechanic instructed the water truck driver to steer the wheels from side to side as grease was added. As the wheels were turned to the right the mechanic was pinned between the tire and frame causing injuries to the mechanic, 4 broken ribs and a broken collar bone.

The fueler was fueling the track hoe while walking around the track hoe the fueler stepped on a rock rolling left foot forward causing a bone fracture in left foot

Track dozer operator pushing down in the cast and was backing up over the coal rib which jarred the operator resulting in a back strain/sprain

A truck driver's hand was pierced by a pressurized narrow stream of hydraulic fluid from a trailer. This happened while unhooking truck from trailer.

Employee was removing pin from steering cylinder to the bell crank. As pin came out, cylinder shifted forward and pinched finger between cylinder and bell crank. This smashed the finger.

Operator was doing a walk around inspection on blade 3500 & struck hardhat on circle assembly frame. They reported to supervisor & said that they felt ok with slight pain & just wanted to report incident. On 2/26/2016 they reported to safety & human resource that they were still having pain. On 4/5/16 was seen & diagnosed with bulging disc in neck, & placed on restricted duty

Mechanic was putting away a tie rod off of a CAT 789 haul truck. Six days later they informed their supervisor that they thought they received a back injury putting away the tie rod. The EE's medical records indicate a history of back pain and sciatica which is what the EE is being treated for. Medical reports also indicate that sciatica can be caused by herniated/slipped discs.

Employee was operating a road grader blading a haul road, backed the machine up and ran over a large rock jarring the operator which caused the operator to experience pain in the neck and shoulder area. The doctor diagnosed this as a neck strain/sprain. Prescribed pain medication and physical therapy.

Operator was backing a haul truck under shovel to be loaded and contacted shovel bucket. Impact caused a strain/sprain to operator neck and back region.

Operator had dismounted the blade and was walking away from the machine when he slipped on the snow covered ground landing on his left knee. The MRI showed a tear in the cartilage

Employee was doing a clean-up in front of the shovel, when the moldboard caught a rock stopping the blade's forward motion jarring the employee's neck and back.

Employee was dismounting a Cat track dozer and lost his footing spraining his left knee. It was determined on 10/12/2015 when he finally saw an orthopedic specialist that he tore his ACL & MCL.

Haul truck operator twisted knee dismounting HT. A 2015 audit of employee injuries we discovered employees initial knee injury was on 4/19/15. It appeared to be a First Aid injury. First work day missed was 5/22/15, surgery 10/29/15.Reporting delay waiting on medical doc's determining degenerative or new injury documentation traced the injury back to a work place injury.

A haul truck contacted a roadside berm causing the truck to tip on its side, fracturing his left wrist.

While dismounting a motor grader/blade, employee missed a step and fell to the ground.

Electrician was leaving electrical shop and wind gusts of 40+ mpg shut the door on his finger, fracturing the tip of his finger

After power washing trucks, employee complained of left wrist pain

While employee was assisting another employee replace the bearing on a tail roll, she got debris in her left eye.

Torquing bolts on push blade when fingers were caught betwee the blade and the acuator for the pneumatic torque wrench causing fracture and laceration requiring stitches to two fingers.

The 651 scraper traveling down a ramp at approximately 8 m.p.h. Scraper started riding up the side of the parallel berm, went over the top of the berm, and down the other side 30 feet stopping at the bottom of the other side. The machine had no damage and the employee was wearing his seat belt.

Employee lost his balance, slipped and fell back onto right hand and wrist resulting in a fracture.

Employee was pushing a hand tool cart in the shop area and felt pain in her lower abdomen

While handling a scrap piece of wire rope, strand punctured leather glove and finger of EE causing a puncture/tear that required sutures.

While installing a nut on the end of a hydraulic ram, EE cut the tip of his finger on the frame resulting in 6 sutures. Machine was locked and tagged out. EE was wearing leather gloves.

Employee was working on top of a bench placing a marker stake near a highwall when the ground under him collapsed, dropping him to a muck pile.

EE stepped out of pickup onto uneven ground, twisting ankle.

Employee struck head on cyclinder head requiring four staples behind the ear.

While employee was operating shatter box, he was distracted and inadvertently placed his hand in a pinch point, resulting in a laceration with finger tip fracture. Employee had not followed procedure and did not close the lid to the box while it was in operation.

While operating a front-end loader, a chunk of coal roller over the top of the bucket and damaged the windshield. The operator received small lacerations.

The area of shot coal was restricted from access to all mine site personnel because of a K order that was issued on 07/25/12. The shot coal spontaneously combusted and started to produce flames on 08/17/12.

At approximately 1:30 am, a dozer operator reported observing pieces of debris raveling off the East highwall in the East pit. A supervisor observed over the next 90 minutes, portions of the East wall experienced a gradual block slough. No injuries or equipment damage occurred as all personnel and equipment were clear of the area. 7000-1 is files under protest.

Grass fire on mine site. Flames were observed for 30 minutes. 21.3 acres of grass burnt.

Conditions were normal, pump was operating correctly. Worker was introducing anti-freeze to the line feeding into the pump. Worker was wearing requited safety equipment including safety glasses and hard hat. A ball valve was not fully closed and when air pressure was introduced to the line, the anti-freeze blew up into the workers face and eyes.

Employee was lifting compressed gas cylinder.

EE went to talk to anther EE that using a drill. EE felt something enter their eye when they were walking up to or standing by the EE using the drill. EE was given prescription medication making it a MTC.

Technician was changing brake pads on a pickup. While loosening one of the bolts on the caliper, the bolt came loose suddenly and handle of the ratchet pinched his finger, causing a laceration.

Pneumatic hand grinder slipped and cut ee left knuckle.

Small piece of metal in eye while grinding. Was Wearing goggles and face shield.

EE was taking a break when they noticed something in their eye from welding and grinding early in shift. EE was given prescription medication and had a rust ring removed.

Haul truck drove though a rough spot on haul road, jarring the operator. Pain meds were prescribed.

Loading holes and a piece of coal dust flew into right eye.

EE was assisting another EE placing wooden mats in front of portable racks. In the process of setting the second mat the EE's noticed a chunk of clay was in the location for the mat. When the EE tried to roll the chunk of clay out of the way by hand he slipped causing his arm to brush against the wooden mat. EE had a splinter embedded into his arm.

Employee was walking across shop bay when the employee tripped and fell, breaking the left wrist.

Employee was performing repairs on a shovel revolving frame during air arcing and welding metal particles entered his eye.

While pulling a water hose in the wash bay, technician slipped and fell. The fall resulted in a finger fracture.

While changing out a crowd motor, a Welder's finger was pinched between a roller and a crowd gear case. This resulted in a one inch laceration requiring stitches.

2 Employees were welding on same piece of metal within 5' of one another under silo #2. One employee received flashburns to both eyes.

Worker was driving in wood posts for silt fencing with hammer and missed. His left index finger was pinched between the hammer handle and the top of the post.

A welder was using a chain anchor to remove a drill bit from a drill steel. He was hit on the knee with a sharp edge on the chain causing a one inch laceration on his knee. The laceration required four stitches

Due to an undetermined medical issue, a technician felt ill after a break. He felt dizzy and nauseous and cut his lip when he fell on the deck of a haul truck. Blood tests ruled out occupational causes.

A dozer operator was pushing coal cleanings into a berm in a drilling and blasting area. While backing to reposition, the operator misjudged her distance from the crest and backed off the highwall. The dozer fell approximately 33 feet onto a slough pile. The operator was dignosed with one ruptured disc.

While using a jack to change a tire the tech pinched his thumb between the jack and the truck tow hook.

Individual was lifting a body pin into the truck pivot bore and strained his back. Incident occurred on January 29, but individual did not seek treatment until February 2nd, at which time he visited a chiropractor.

The final cut had been made on the 96" conveyor belt in prep for vulcanization. Employee stood up, remaining in a bent over position in the 42" space. While he was backing out from under the belt he inadvertently pressed the blade of his razor knife into his thigh causing a puncture wound about 2 cm long through the epidermis, stopping barely into the dermis (2 sutures).

A dirt particle or small piece of debris entered the employees left eye. The particle was flushed out, but the eye became infected, requiring prescription medication for treatment.

At the time of the incident the tech was installing an oil filter base on a CAT 793 truck. As the tech was starting the first bolt the ladder he was standing on partially folded up causing him to quickly step off the ladder. On the way off the ladder the tech's left hand was caught between the fill tube and the filter housing and lacerated.

At the time of the incident the technician was moving a workbench in order to clean under it. As the tech was pushing on the bench one of its wheels dropped into a divot in the floor. As he pushed harder to dislodge the wheel his left foot slipped on some oil causing him to fall against the bench and twist his back.

While mounting backhoe, employee lost his footing and stepped back to the ground. When he did, his right foot was in pain. He went to EMT and then to ER.

The employee is a haul truck driver who has been diagnosed with a degenerative back condition. Repeated trauma from equipment operation has caused increase pain.

Slipped on the ice and hit the back of the head causing a slight concussion. No lost time accident, but was given a prescription for pain medicine.

The operator was pulling under the shovel with a coal truck to get loaded. The service braves were applied to abruptly causing the truck bed to bounce on the frame. The jarring motion caused pain in the upper part of the back.

The employee was operating a haul truck and getting loaded when the discharge of a bucket of material from the shovel rocked the truck jarring the operator. The haul truck operator reported back pain shortly after the incident.

The operator's haul truck was being loaded by a rubber tire loader. Material surged from the loader bucket as it was dumped and caused the truck to rock. This motion jarred the operator causing soreness to the lower back.

Truck operator sustained a neck and shoulder strain when the bottom of an electric shovel door contacted the truck bed.**It was originally believed that this injury was minor and 1st aid only. Injured had ongoing discomfort and saw a Dr. and on 8/142007 was restricted from returning to work.**

While conducting an equipment pre-shift examination the employee twisted her right knee.

The employee was pulling on a stuck trip rope cable on a shovel bucket. As the cable came loose it sucked his left hand down and pinched his left hand ring finger between the cable and the cable pulley. He sustained a laceration on the tip of the finger which required twelve stitches to close.

The employee was pulling on a water pump primer hose. Unexpectedly, the hose moved abruptly. The employee lost her balance and hit her head on the primer bowl creating a gash requiring stitches in her right eyebrow.

While climbing the boarding ladder on a Cat 793 haul truck, the employee experienced a sharp pain in the right shoulder.

EE was replacing the rear suspension cylinder on a CAT 793 truck. As he was using a sledge hammer to drive the top strut pin from its bore he felt a pain in his right wrist. He did not report the incident at that time. On 9/22/06, as he was loosening/tightening the wear plate retaining bolts on a 24M motor grader, using a 3/4" impact wrench, he again experienced pain in wrist.

Glove snagged on hinge while closing door-pinched 4th finger of right hand between door & frame-smashed fingernail causing cut and bone fracture.

Employee was unloading a portable generator from the bucket of a backhoe loader. As he moved the unit he sustained a strain in the groin area. On 4-27-06 after a doctors visit it was determined that he had a hernia.

As EE was coming down the front ladder of a CAT 793 truck, a welder working 15 feet away struck an arc, causing him to see "spots". The next morning his eyes were swollen and felt like they had something in them. He went to the doctor and was diagnosed as having corneal ultraviolet burns,

At the time of the incident employee was installing a final drive cover on a CAT 793 truck. As he was using an overhead hoist to move the cover, the threads on the bolt attaching the lifting link to the cover stripped, allowing the cover to drop and strike employee on his right foot.

Employee had hooked cable to container, lifted up, into grooves of hopper. He was making a sweep with the blade when the container popped out of grooves and swung to the right where employee was operating controls.

An employee was lowering a clothes basket from the ceiling as another miner walked behind. The basket struck the other employee on the top of the head causing a slight concussion and a laceration which required two staples to close.

The employee was pounding out a latchbar insert for a shovel bucket using a sledge hammer. A small piece of steel splintered off the insert and penetrated the inside of the right thigh. An incision was needed to remove the metal fragment and four stitches were required to close the affected area.

MAINTENANCE TECHNICIAN SLIPPED ON A WET STAIR WHILE DESCENDING FROM A DOZER. HE RECEIVED A LACERATION ON HIS RIGHT ELBOW WHICH REQUIRED STITCHES

A PIECE OF STEEL FELL FROM A STORAGE RACK STRIKING A MAINTENANCE TECH. ON HIS LEFT CALF. THIS RESULTED IN A 4" LACERATION.

WHILE OPERATOR WAS ATTEMPTING TO WASH WINDOWS ON 992 LOADER HE LOST HIS GRIP AND FELL FROM THE DECK RESULTING IN TWO BROKEN RIBS. IT WAS RAINING AND OPERATOR DID NOT UTILIZE BEST PRACTIVES FOR CLEANING WINDOWS IN 992

While repositioning upper rail for removal, the rail slipped off of the house rollers pinching ee's right foot against a track pad.

Operator was closing shovel cab door and caught tip of left ring finger. He received a hairline fracture to the tip of his finger.

(DEATH - NATURAL CAUSES) WHILE WAITING FOR HIS NEXT JOB ASSIGNMENT, EMPLOYEE FELL TO THE GROUND.

A FIRE STARTED ON BELT TIRE OF LORC-2. CONVEYOR WAS TOTALLY DESTROYED AND DAMAGE OCCURRED IN TOP TWO FLOORS OF BATCH LOADOUT BUILDING. AFTER INVESTIGATION, WE FEEL A PEN THAT WAS COVERED AND HIDDEN, AND NO ONE WAS AWARE OF ACCUMULATED COAL FINES AND STARTED FIRE AT WEIGHT TRANSFER TOWER.

EE WAS DUMPING TRUCK AT HOPPER. THE BED WOULD NOT RAISE OVER 6". SHE DECIDED TO LOWER BED CAUSING IT TO FALL BACK DOWN. THE IMPACT CAUSED A NECKSTRAIN.
