Miner was washing TT1.They shut the nozzle off and pulled on the hose. The miner was holding the hose 2 feet away from the nozzle. The nozzle appears to have contacted a beam and turned on. When it energized, it flipped the nozzle up impacting the nose and mouth area resulting in a laceration at the base of the left nostril. Miner received three stitches on the laceration.

A coal oxidizer was located in the top of Roland in West Cast pit area. The oxidizer is at the top of the coal seam under the cast wall and will require more than 30 minutes but are currently in the process of getting equipment into the area to extinguish.

On 3/2/24 Miner was helping unhook a cable from a truck. The cable shifted unexpectedly and hit the Miner's right thumb. On 3/5/24 the Miner's thumb was diagnosed as fractured, and an outpatient procedure and physical therapy will be performed. Miner was allowed to return to work and perform normal duties.

On a wet slick road a scraper operating at 10 MPH began to slide. The scraper stopped abruptly as it went from wet to dry conditions. Operator hit EE's back on the arm rest resulting in back strain. No damage to scraper.

On 9/2/23 Employee reported pain in back with no event, was evaluated and released to return to work. Pain got worse and employee went back to doctor. Doctors' notes received 10/10/23 & MRI revealed multi-level spinal degeneration and prior compression fractures in the lower back.

Employee has a previous injury that has progressively worsened over time. Employee went to orthopedic doctor and was diagnosed with a herniated disc and pinched nerve in L-5 of the spine and was taken off work until physical therapy could help resolve this condition. No event at the mine occurred to cause this issue.

On 4/1/23 Miner slipped and fell on ice, diagnosed as PCL sprain will heal on its own. Miner reevaluated as knee was not improving and had an outpatient procedure performed to repair PCL on 8/31/23 and allowed to return to work but prescribed to go to physical therapy. No days were missed from this injury.

On 2/9/23 Electrical Planner slipped on ice walking around substation repair. No treatment. On 2/24/23 not improving went to doctor and diagnosed as sprain. No improvement and MRI taken with no findings. Another MRI taken 3/8, on 3/16 diagnosed as a muscle tear. Outpatient procedure performed on 3/16 applied brace, released to work and prescribed physical therapy.

Miner was performing utility tasks and moving shovel trail cable, when picking up cable felt pop in lower back. Miner went to ER and diagnosed with a muscle strain. Returned for follow up and physical therapy was suggested. Miner went to physical therapy on 7/16/22 for muscle rehab.

Rubber tire Operator was pushing material off Dump 1 in the west pit. The Operators hand came off the joystick controller and struck the console resulting in a fracture to the right hand 5th metacarpal bone.

Employee was loading skid steer fork attachment into skid steer bucket. The attachment got hung up on cutting edge bolt. When employee pulled on fork attachment, it came free and slid into bucket pinching the employee's hand between the attachment and bucket. After taking glove off employee could see this caused a laceration to the employee's hand requiring stiches.

Employee was standing on a two step ladder, using air gun to clean off coal dust off the prep hood. As the employee was stepping down from second to the first step, right foot missed the first step causing employee to fall. The employee reached right hand out to break the fall causing a hair line fracture to wrist.

A mechanic was under the front of a truck trying to repair a coolant leak. In an attempt to tighten the original clamp first, the clamp stripped and became loose. The hose connection came apart, and hot coolant poured out of the hose onto the employee's chest, forearm, and face. Employee was taken to the ER, referred to a burn center, treated for 1st and 2nd degree burns.

Electrician was adjusting power cable on elevator when the leather of the back of the electrician's left boot was pinched between toe guard and floor. This resulted in a laceration that required 10 stitches. Was given a work as tolerated release and missed no scheduled shifts.

On 9/28/2021 pumper pulled a piece of 6-inch poly pipe out of the mud, felt something in right shoulder region crunch/pop. Pumper stated no pain was felt, decided not to report anything to supervisor. On 9/30/2021 the pumper experienced pain in the shoulder region and decided to report the incident to supervisor. Pumper began receiving chiropractic care on 10/01/2021.

Mechanic had cut a piece of all-thread. While cleaning off the excess thread with the wire wheel, the wire wheel caught the piece and smashed EE's left index finger between the piece of all-thread and guard resulting in a laceration to the tip of the finger. Sutures were required to close the wound. Mechanic received a work as tolerated release and returned to work.

Employee exiting the porta john, pushed the door open, wind caught the door slamming it on employee's right hand. ER evaluation x-ray was negative with a work as tolerated release, second evaluation at the Coalition could not confirm or rule out a small fracture. Was given a splint to immobilize 2 fingers and given a work as tolerated release. No scheduled shifts missed.

Employee was opening a lid on the truck to reload it with product. Lid was stuck, so employee reached under the lid with fingers to pry it open. The lid did not open and pinched fingers in a scissor-like format, resulting in a laceration to the tip of right ring finger. Received 1 stitch.

Employee was removing a pin from a hydraulic cylinder with a hammer, felt a pop in shoulder and experienced some pain. Was evaluated at CCH and received a work as tolerated release. An MRI determined a torn rotator cuff. Surgery was performed on 01/21/2021 which is the first scheduled shift the employee has missed.

Guard shack attendant stood on a chair to reach an upper shelf, fell landing on left shoulder. Was diagnosed with a re-aggravation of a previous injury to the left rotator. The employee was going to be given a work as tolerated release but requested to be removed from work activities until released post operation on 9/10/2020. Surgery will follow when it can be scheduled

When replacing the control arms on the front of 21.4567 service truck the mechanic was using a pry bar to get the control arm into place when the bar slipped off the control arm causing the control arm to fall on EE's left thumb causing a fracture to the tip of the thumb bone.

Mechanic leaned an 8 ft ladder up against the outside right front haul truck tire to perform an inspection. EE was descending the ladder, the ladder slipped and fell to the ground, the ladder landed on the mechanics right hand and they landed on top of the ladder. This caused a dislocation injury to the right hand. Employee had surgery on 6/2/20 to repair the injured hand.

Employee was removing the water truck tank drain hose, the hose popped loose from the fitting striking the employee in the mouth. The impact resulted in a contusion to the lower lip and chipped the front left tooth. The tooth will need to be capped and the employee did not miss any scheduled shifts.

Operator tramming track dozer 26641, encountered a rock caused operator to inadvertently set park brake. Machine suddenly stopped, operators left shoulder jammed causing a shoulder strain. Employee was given a work as tolerated release 1/2/2020, then on 1/9/2020 notified the mine that the MRI showed rotator cuff tear, taken off work 1/9/2020. Begin physical therapy 1/13/2020.

Employee was in the office, experienced chest pain and slumped over. Rescue team administered CPR and used an AED. EMS was called to the mine and took over the first aid, employee was transported to the hospital, diagnosis was cardiac arrest and admitted to the hospital.

Technician was drilling holes in a plate, stopped the drill press to clear the metal shavings from the drill bit and a piece of metal went through glove resulting in a laceration to finger. The employee received 9 stitches to close the wound and released to work as tolerated.

Plant Tech was removing ice from a conveyor head roll area. Employee used a bar to remove the ice the bar was pulled into the head roll and frame pinching the techs left hand between the bar and the frame work causing a little finger injury. The employee had surgery to remove part of the distal bone from the little finger. No scheduled shifts were missed.

Injured employee was assisting an employee in removing the engine from a haul truck. They were removing the spreader bar when the shims shifted, the injured employee then stuck left index finger in a bolt hole to prevent them from falling to the floor. As the last bolt was removed the spreader bar and shim pack shifted and pinched the IE's finger between the shims and frame

Mechanics had removed the old edges and cleaned the surface for the new ones. While lifting the edges a bar is used through the bolt hole so it is easier to carry. The edge slipped on the bar and pinched the mechanics finger between the bar and the edge resulting in a laceration. Employee received 3 sutures in EE's left finger work as tolerated and returned to work.

While washing, operator looked up, a small chunk of coal fell and landed between EE's face and safety glasses, causing coal dust to contact the eyeball. Operator was taken to the ER and small specks of coal dust were flushed out of the eye. Employee was given eye drops and pain medication with a work as tolerated release and returned to work.

Operator dismounting a motor grader, missed the step & tried to catch self with left hand & ended up twisting shoulder & heard a popping noise. Employee was diagnosed with a ruptured bicep tendon & rotator cuff tear on 10/05/18. Employee has not missed any scheduled shifts & has scheduled surgery for January 2019. EE had surgery 1/21/19 & will miss upcoming scheduled shifts.

Employee slipped on the ice walking to a crew van causing pain in EE's right knee. The employee had a non work related surgery on this knee in Nov 2017, an MRI was performed on 3/6/18 which resulted in knee surgery on 3/20/18 with a diagnosis of a recurrent symptomatic medial meniscus tear. The first day the employee missed work was on 3/19/18

Warehouse employee was boarding a fork lift, employee's shoes had some mud on the bottom, employee slipped and bumped right knee. Initially it appeared the knee was just bruised. On 2/27 the doctored order an MRI because knee continued to be sore, the MRI determined a torn meniscus, the doctor ordered physical therapy. The first physical therapy visit was on 3/13/18.

Operator positioning a light plant on the bench, when EE got out of EE's pickup EE rolled EE's ankle. EE felt popping and pain was evaluated by the EMT then taken to the hospital. X rays revealed EE had fractured EE's right fibula. The employee was given a splint and crutches then sent home.

Utility person was moving a shovel, the shovel cable horn flipped over jerking the rope the utility person had a hold of which caused a pop in the employees left elbow area. An MRI showed the bicep muscle was torn, on 08/29/2017 the employee was prescribed a brace, surgery is scheduled for approx. 09/05/17. The employee has not missed any scheduled shifts until after surgery.

Employee was hooking up a 6" suction line to a pump. Employee felt pain in lower back and was looked at by EMT. Employee denied further treatment at the time it was reported. Employee notified Employer on 7/17/2017 that ee had been seen by the VA in Denver and diagnosed as a muscle sprain. Employee began physical therapy on 7/18/2017 and missed no scheduled shifts.

Mechanic was adjusting a jack stand, EE put EE's middle finger in the adjustment hole to help lift the jack stand, the jack stand came loose and fell pinching EE's finger in the hole causing a laceration that required stitches. The mechanic was released to work as tolerated and returned to the mine to finish the shift.

Fueler was driving fuel truck and drover over a windrow. This caused the steering wheel to jerk and pull EE's left arm. Fueler reported the incident and the EMT looked at EE's arm. EE returned to work. On 8/8/17 the employee informed the mine that EE has been taken off work and will be receiving surgery due to injuries from the incident.

Mechanic was cleaning up a fuel spill, attempted to pick up an oil absorbent boom that was frozen to the ground and right knee popped and was unable to put weight on the knee. Mechanic had been having problems with this knee and was receiving professional treatment prior to this incident.

Plant employee was changing out a roll on the storage conveyor belt, the roll slipped pinching the employee's middle finger resulting in a chipped bone & laceration to the end of the middle finger. The end of the middle finger received sutures, the finger nail was re-attached with sutures. The employee was given a work as tolerated release and returned to work immediately.

Rubber tire dozer operator was operating a dump, the operator was making cuts on the dump surface to re-grade the dump and created a hump on the dump surface. The operator backed over the hump and jarred back resulting in a pinched nerve in back. The employee will need physical therapy and is expected to return to work on 01/27/17.

Operator dismounted rubber tire dozer. Slipped on ice. Employee felt they did not need any medical care at that time. On 4-3-17, employee informed company that on 3-31-17 employee received results of x-ray and MRI. Employee will be receiving physical therapy treatment on 4-10-17, for continued numbness in left arm. To date, employee has not missed any scheduled shifts.

The injured employee was working on the front strut of a CAT 793 haul truck. The EE was moving the pillow block when it rolled back and bent fingers back, injuring wrist.

Water truck driver reported a noise while steering to the right. Pit mechanic installed new grease lines, the mechanic instructed the water truck driver to steer the wheels from side to side as grease was added. As the wheels were turned to the right the mechanic was pinned between the tire and frame causing injuries to the mechanic, 4 broken ribs and a broken collar bone.

Mechanic was putting away a tie rod off of a CAT 789 haul truck. Six days later they informed their supervisor that they thought they received a back injury putting away the tie rod. The EE's medical records indicate a history of back pain and sciatica which is what the EE is being treated for. Medical reports also indicate that sciatica can be caused by herniated/slipped discs.

Worker was wearing requited safety equipment including safety glasses and hard hat. A ball valve was not fully closed and when air pressure was introduced to the line, the anti-freeze blew up into the workers face and eyes.

The final cut had been made on the 96" conveyor belt in prep for vulcanization. Employee stood up, remaining in a bent over position in the 42" space. While he was backing out from under the belt he inadvertently pressed the blade of his razor knife into his thigh causing a puncture wound about 2 cm long through the epidermis, stopping barely into the dermis (2 sutures).

At the time of the incident the tech was installing an oil filter base on a CAT 793 truck. As the tech was starting the first bolt the ladder he was standing on partially folded up causing him to quickly step off the ladder. On the way off the ladder the tech's left hand was caught between the fill tube and the filter housing and lacerated.

At the time of the incident the technician was moving a workbench in order to clean under it. As the tech was pushing on the bench one of its wheels dropped into a divot in the floor. On 9/22/06, as he was loosening/tightening the wear plate retaining bolts on a 24M motor grader, using a 3/4" impact wrench, he again experienced pain in wrist.
