There was a roof fall in 18H 3 entry xc 5 intersection. It fell on the belt line and monorail in the entry. The fall was roughly 10' high, 30" long, and 20' wide. No injuries, no trapped miners, and no disruption to ventilation on the longwall face.

Employee was shoveling along side the crusher when a rock 28" x 30" x 4 1/2" fell and landed on their ankle. Causing a sprain/strain to their ankle.

Roof fall at 17hg return 3 entry xc 5. Roughly 10' high 20' wide and 30' long.

Employee was walking down a walkway stepped down off a step planted their foot and slipped. Causing their ankle to roll.

while turning power back on during a planned outage the generator for hoist shut down causing the hoist computers to shut down. This required the hoist computers to be rebooted causing the sync switch to fault out. The hoist was down for 37 minutes.

There was a roof fall in the tailgate of the longwall measuring 15ft to 20ft long and 7ft to 8ft high. The fall did not restrict ventilation, cause injury, or impede escapeways.

The miner was cleaning up the walkway when EE picked up a large rock and felt a pop in EE's back.

Employee was gritting their teeth while loosening a bolt. Employee chipped and bottom tooth.

There was a piping failure in the southeast corner of the lower impoundment that allowed water to flow out into the evaporation lake.

Employee was sprayed with 50% caustic. The caustic contacted the corner of left eye.

EE was hammering a bearing sleeve. The bearing broke and struck EE's hand causing a laceration to EE's finger

Slip, Trip, Fall, in the plant while walking across damaged concrete. EE fell and broke EE's clavicle

Employee was tightening 2 1/2" bolts on a pump when EE felt pain in EE's right shoulder

8 shaft hoist went down for an electrical fault. No one was on the cage or injured when the fault occurred.

Employee was removing a tag off a chain valve when EE felt a pop in EE's left shoulder.

Spacer plate for the cable handler slid and smashed EE's finger against the shear resulting in a right ring finger tuft fracture.

a fire was reported at the land fill pit. no injuries occurred.

there was an above anchor point fall above the overcast near 2 northwest pump station. the fall damaged the overcast causing it to outgas.

Employees were drilling the hole when the pipe started to slip. Employee grabbed another wrench to put it on the pipe, while employee was setting the wrench they had a hold of the other pipe. The wrench slipped and the one they were holding accidentally grabbed and smashed their hand between the wrench and the mass of the drill.

Employee was carrying pin rollers when they tripped and fell on their shoulder. Employee tripped over grating outside the 4shaft airlock.

Madsen (contractor) Front end loader operator was cleaning material out of mono loadout silo 2. The material above the entryway came down on the front of the front-end loader and pushed the steering column and dash down into the operators' legs trapping EE in the loader.

While welding grating the bin over flowed dropping a small rock into the hood contacting right eye.

Laying on right forearm when felt a pop in right shoulder while welding.

Employee was bending over to set the impact down after bending over and tightening bolts.

Employee was walking on platform lost balance. Employee attempted to catch self and felt a tear in bicep area.

Employee was operating the shear cutting out the Headgate, when a rock the size of a baseball came out of the panline and hit them in the left knee.

Lowering a piece of grating and felt a pop in right elbow. Seen at on-site medical facility and released. Still felt pain a few days later and went to personal physician on 5/27/25 where it was found EE had a torn right bicep

Slip and fall when pulling wrench off bolt. Fell and landed on a rock in the employee's lower back.

Employee was putting drill steel in the chuck while hand was in a pinch point and closed the centralizer on their hand.

Employee was walking on an uneven surface and felt pain in knee, sought medical attention and was released back to work. On 05/10/2025 employee was still experiencing pain sought medical attention and was put off of work until an MRI can be performed.

The 5 hoist mantrip had an unknown electrical issue which deemed it inoperable all miners were evacuated.

Employee wash unplugging a caustic line and got liquid on their pants causing a caustic burn. Employee went for a follow up appointment on 04/26/2025 and was put off work for one day.

The operator was adjusting the lime drag when they pinched a finger, resulting in a laceration that required 11 sutures.

Two maintenance employees were working to replace broken bolts on the longwall stage loader chain flights. One employee was swinging a hammer to free the flight and the other employee was pulling the chain and was struck by the hammer requiring 13 stitches.

Had a roof fall above the anchor points in the return of the tailgate of the longwall.

Employee was evaluating materials and found a roof fall above the anchor points at 299 15HG #1 crosscut.

Employee was evaluating areas for material storage and found a roof fall above the anchor points at 458N in #1 room.

Supervisor was walking the 14hg area of the mine to ensure that there was no equipment, or tools that needed to be removed before the stoppings were installed. During EE's inspection EE noticed that the top was looking like it was deteriorating. EE contacted engineering and when they arrived, they walked the area and discovered the roof fall.

Had a power trip on the primary man hoist (8 shaft) and one of our secondary shafts was locked out for maintenance. Power was restored to the primary man hoist at approximately 8:15PM.

HVAC Technician was working on HVAC unit on above ground concrete pad on 12/10/24 that was covered with ice. The technician slipped and jammed shoulder when EE went to catch self. The technician tried taking care of it for a few weeks, but it was still hurting on 1/3/2025 so EE went to seek medical care and was issued work restrictions and prescribed medication.

Employee was changing out a leaf spring & when it broke free it pinched employee's hand requiring 13 stitches.

Employee was removing a bolt from a junction box when EE received a shock to EE's left hand.

Employee tripped landed on EE's hand sustaining a fracture.

Employee rolled EE's ankle when walking into a building causing a sprained ankle.

8 Shaft hoist went down while taking the night shift crew into the mine. A ground fault issue on the surface caused an undervoltage event. There were 2 faults that occurred on the hoist motor. The first was found relatively quickly, but the second one took some time to find. Therefore, the crew was stuck on the cage for over 30 minutes. We notified MSHA at 11:25PM.

While working in the bucket elevator the employee stepped down on to a beam, causing the employee to lose balance. Employee reached with left arm which dislocated employee's shoulder.

Employee went to grind on a piece of pipe, when the cord got entangled, went across employee's hand causing a laceration requiring 9 sutures.

The 8 shaft hoist went down due to an electrical issue after troubleshooting, the electrical issue was resolved. There was one miner on the man hoist when it went down and was hoisted out at approximately 10:20PM.

Employee bumped a valve handle on an airline and came into contact with the debris in the line.

We experienced a power trip on the PPL's mainline that caused the underground mine to lose power. There were approximately 27 miners underground until secondary power was established at approximately 2:15AM.

Employee was ascending stairs, tripped on a step, and stuck hand out and it went through glass light. Employee sustained a laceration to the right index finger requiring 6 stitches.

Employee was putting away tools, climbed up on the tool truck, felt a shock as EE's right knee touched the tool trip. EE reactively stood up and hit head on the mine roof. Employee was taken to the ER as a precautionary measure and fully released on 6/19/24. Due to persistent shoulder pain, EE received an MRI and was put off work for further medical evaluations on 7/29/24.

While prepping a pipe penetration another employee was grinding on a piece of stainless pipe in the immediate area. Employee immediately felt something in eye.

7 Shaft hoist went down due to electrical issues with two miners on the cage the electrical issue was resolved, and the miners were hoisted out at 10:15am.

Miners were receiving elevated levels of CO and as a precautionary measure, evacuated the mine. Approximately 10:44PM miners found that there was a belt fire located at 2nd 54 drive. The area was smoldering when found the miners applied water to cool the affected equipment and flagged the area.

Double tie line trip on Rocky Mountain Power caused a power outage at 8 shaft rendering the man hoists inoperable. Power was restored at 10:53AM all miners were hoisted out at that time.

Employee smashed right hand loading structure onto a trailer requiring 3 sutures.

Employee was blowing out drag conveyor and got dirt in their eye that caused irritation. EE was given a prescription.

A roof fall occurred above the anchor points at 592 South XC 168, area was flagged off and MSHA was immediately notified.

Employee was turning valve, felt pain in shoulder, after MRI was put on light duty.

Employee was dismantling a cable handler, when the cable handler slipped pinching employee's finger causing a laceration and partial amputation of the pinky finger.

Employee was opening a valve when the handle slipped off, hit their tooth requiring dental repair.

Elevator tripped at Mono Utilities contractor was stuck in the elevator and released at 6:52AM. Safety was notified at 7:25AM and contacted MSHA at 7:35AM.

Employee was steadying box when the box fell, pinching EE's hand causing a fracture. Employee was returned to work with no restrictions later referred to a specialist and prescribed work restrictions on 11/1/23.

Employee was removing protective covering from blade and cut a finger which required stitches.

Employee was training the belt and strained shoulder, was prescribed restricted duty.

EE was hydroblasting a heat exchanger in the bicarbonate unit. At approximately 15:08, the barrel of the shotgun failed and broke at the mfg weld. This caused EE's hands to rise and hit EE's faceshield with the shotgun trigger assembly. This action caused EE's faceshield and hard hat to strike EE's glasses causing a cut to EE's face that required stitches.

Employee was working on a pump, when EE opened a drain valve and was sprayed with hot liquor causing burns on the neck and forearm.

Employee rolled ankle on mine floor was given a full medical release on 09/04/2023. On 10/10/2023 went to see an orthopedic and was put off work for two weeks.

Employee was walking to the cage and rolled their ankle causing it to dislocate.

Employee was working on removing guarding and received a laceration on the expanded metal requiring sutures.

Employee was walking on grating when the grating shifted causing the employee to fall on left knee. Employee was given work restrictions and medication.

Roof fall at xc 144 592 west roadway above the anchor points L-15' x W-20' x H-7. Access nor ventilation is blocked and, there is no active work inby the fall. The roof fall is barricaded off.

Employee was assisting another employee hold packing around shaft and received a laceration requiring 5 stitches.

Employee was hammering on a crossover and injured EE's shoulder. Employee was fully released back to work that day. Employee saw another medical provider that put EE off work on 5/9/23.

Three employees were stuck in the 8 shaft cage for approximately 3 hours.

Employee was moving belt; belt slid and impacted the employee causing a cervical sprain.

Employee was working on a pump when employee smashed a finger between a belt and the pulley.

Employee was removing an agitator blade and sustained a laceration to leg requiring 8 stitches.

Employee was working on a crusher insert, when employee cut a piece of metal, it folded in on employee's hand, causing a 3rd degree burn to employee's wrist.

Employee was loading a portable A/C unit into an elevator when employee's hand was smashed between A/C and elevator doorway, causing a laceration requiring sutures.

Employee was using a die grinder and a foreign object got in EE's eye. Employee didn't report the incident until 9/21/22.

Employee was lifting a case of water and injured lower back.

Employee was pulling a cylinder off a piece of equipment, the load shifted, smashed EE's thumb requiring stitches.

Employee was using a floor drill when EE's glove caught on the chuck causing an injury to EE's left thumb.

Employee was pushing a work platform into place when they felt a pop on left bicep. Employee tore their bicep tendon.

While off-loading lime, the hose became plugged. Driver attached the line to the mine air to unplug the blockage. EE failed to bleed the pressure before opening up. The pressure blew lime hitting EE in the right arm and right side of EE's abdomen Burning the skin.

Employee smashed finger between a shim and housing causing a tuft fracture to right index finger.

Employee was walking under a valve, hard hat moved the valve into the open position, causing a thermal burn on employee's back. Employee was seen by medical providers on 7/23/22 and received a full release. Employee went back to the medical providers and was put on restricted duty on 7/28/22.

Employee was exiting a shuttle car when the employee contacted the door & received an electric shock. Precautionary tests were performed and employee was released with no restrictions. Employee followed up w/medical provider and was put off of work.

A contained fire started on an overcast. Was extinguished with handheld fire extinguishers at 1:52PM.

An employee was operating a miner when EE's foot was smashed between two pieces of metal in the cab. The miner sustained a fracture in their foot.

Mechanic was removing a piece of sheet metal that shifted and smashed EE's finger requiring 7 stitches.

A mine mechanic received a shock while removing mud out of the cable reel rollers on a shuttle car. Although it was reported that the mechanic briefly lost consciousness, when medical responders arrived the mechanic was alert, oriented, and vital signs were normal. The mechanic was further evaluated at the hospital and was fully released back to work.

Power tripped causing 8 shaft man hoist to lose power entrapping one individual for more than 30 minutes.

EE was descending steps, stumbled, and fell into a truck causing a dislocated shoulder.

EE was releasing a chain and pinched finger causing a laceration requiring 4 sutures.

Double tie line trip on the mainline took out the power underground and the hoists were not operating.

EE was struck by an actuator piston on the left side of the face. EE sustained fractures and loss of vision in the left eye.

EE was standing in a crosscut when a rock was thrown from a shuttle car tire causing a laceration to the lip and a chipped tooth.

While taking down scaffold, injured employee had elbow stuck between runner and brace they were taking down.

Employee was recovering conveyor belt structure when a part of that structure fell on right arm, pulling on shoulder. EE was placed on restricted duty.

EE was using a vice to hammer a U joint, struck thumb which required x-rays and two sutures.

Lost power on PPL mainline causing a total power outage at the underground mine rendering the hoists inoperable.

Employee finished measuring parts in high winds, removed safety glasses and cleaned face. Employee got a metal sliver in eye when cleaning face. Started to rust and had to be removed by doctor.

Over a sump the roof fell into the sump. No disturbance to ventilation, access, or employees.

As EE was jacking up a jeep to change a flat tire, the jack started to shift. The employee was steading the jack and released the jack handle which snapped back on their thumb causing a fracture.

While employee was installing a ventilation panel, employee twisted EE's back while repositioning the panel, causing a back pain. EE was given medical treatment for the back pain.

While coiling a 1" hose onto a hose station, the hose kinked, employee attempted to get the kink out of the hose and employee's hand contacted the valve, resulting in a sprained finger.

On April 30th while lifting a heater, an employee experienced pain in their shoulder. The employee continued to work but the pain did not ease. EE consulted an orthopedist and had an MRI which diagnosed an injury requiring medical treatment. The employee has been off work since 5/19/21, making this a recordable injury.

Employee was stepping out of a personal vehicle in the parking lot, slipped and fell and is receiving medical treatment for an injury to EE's left knee.

Employee was pulling bags off a belt and felt pain, employee was diagnosed with a groin strain.

Employee was traversing from a rail car to the mezzanine, put EE's right hand on the safety gate bar, the bar fell striking EE's right ring finger.

A roof fall was discovered at 8 head gate.

A fire was reported at the 8 shaft landfill, fire department and on-site emergency teams responded. Fire was extinguished at approximately 4:47AM.

Employee was pulling the tender in went to tie up and pulled hand into metal cleat causing a fracture to the pinky.

The involved employee (IE) was passing a scaffold pole to a co-worker that was working from the scaffold. Due to the tight area IE had to pass the pole up next to a cable tray. As the co-worker began to lift the pole the pin snagged on the cable tray. This resulted in the pole being dropped and struck the right hand of the IE causing a fracture.

Employee was manipulating a valve when employee was sprayed with hot liquid resulting in burns.

Employee was repairing a door closure mechanism when struck in the head requiring 4 stiches.

EE was on the back of the truck tarping the load. EE began climbing down when EE's foot slipped off the step and hurt EE's knee.

Employee fell in shower room. Employee referred off work for follow up medical evaluation after X-ray negative.

EE was observing a roof bolter put in a bolt, when a piece of rock fell out of the roof cutting EE's arm requiring 16 stitches.

Employee was cutting cable jacket, cut forearm requiring 5 sutures.

The suspected cause of the breach was piping under the embankment from the dissolution of deca crystal below the embankment. As the water escaped to the Evaporation Lake, the flow eroded the earthen embankment. It is suspected recent heavy rains contributed via excessive dilution of the material stored in the impoundment.

Employee was beating in a pin with a hammer, pin fragmented and metal sliver went into arm.

Employee was washing down with hot water when the hose ruptured causing a burn.

EE was entrapped in a man lift for more than 30 minutes due to suspected electrical issues.

Removing angle iron welded to a roof, co-worker was grinding the welds and IE was prying the angle iron up with wedges and a hammer. IE grabbed the angle iron with left hand out of the swing path when IE felt a metal sliver in thumb. After hitting the wedge IE stopped and removed glove to find out it cut IE's thumb.

Employee smashed finger between a bit wrench and a mitt causing a fracture to the finger.

Employee went to step off of work platform and jammed thumb.

EE was washing a piece of equipment when the EE sustained a cut on the thumb from a metal burr.

EE was operating a forklift and ran over a piece of wood, felt a pain in the lower back, received medical treatment.

EE was prying on a drag chain, pry bar slipped and EE's elbow contacted a sprocket requiring 4 sutures.

Employee was assisting with a job when ee was struck by a wire rope on hand causing 7 sutures.

Hanging sheet metal.

A roof bolter operator was beginning to center-bolt a freshly cut entry, the operator was struck on the hardhat by a loose rock. The rock pressed the operator's head into the levers on the bolter's hydraulic control bank.

EE was opening 1" valve and reported pain in shoulder two days later.

Roof bolting drill steel fell out of roof and contacted right ring finger.

EE was leveling expando rails when two rails made contact and fell from an elevated position onto EE's right foot.

EE turned to talk to a co-worker, while standing in the change room, slipped and fell sustained a laceration requiring 7 sutures.

EE was operating the forklift while IE was rigging the pipe to lift. IE adjusted the synthetic sling twice trying to lift the pipe at the desired angle. One end of the pipe was still on the ground and IE grabbed the pipe and lifted it up to put a tagline on it. The pipe swung into a support pillar 8" away pinching IE's hand in between.

EE was splicing cable and sustained a laceration to left pinky.

While roof bolting a rock fell and struck the hard hat of the EE, pushing hat onto EE's glasses, which caused glasses to lacerate EE's nose.

EE was walking from the underground parking area to cage to hoist out. EE stumbled and fell to the ground, twisting right knee.

EE went to retrieve a rope/metal thimble assembly from the deck of the dredge which is used to pull the dredge out of the lake. As EE stood up, the thimble slipped out of EE's hand, striking the top of EE's left foot.

Employee was lifting box, lost control of box and it fell striking right ankle.

While trying to lay down Drisco pipe, the end piece slid on dirt and bumped into the employee. It knocked employee down and in the fall, employee sprained knee and ankle.

EE was respooling port winch cable when cable strand caught glove and pulled finger into winch.

EE mechanic jumped in to help another employee drag a two inch suction hose down the longwall face. EE lost balance and fell striking forehead on the toe of the roof support. As a result of the fall, the EE received a laceration to the forehead above the eyebrow requiring four sutures.

Employee was diagnosed with an umbilical hernia by EE's doctor. Employee did not report incident at work but later communicated that EE believed it occurred while actuating gates under a rail car. It is probable that hernia was created by the repetitive stresses associated with opening gates under the rail car.

While breaking a nut loose with a breaker bar and socket the nut broke free and foot rolled under body causing a fracture to the right ankle.

Employee was rotating a sheave block to uncross the two cable lines. When the sheave was rotated the weight of the cable caused the sheave to roll a couple of inches. EE right hand was pulled into the sheave as ee was holding onto the cable. Result was the amputation of the tip of ring finger, right hand

Employee was turning wrench to loosen a bolt, wrench slipped, hitting employee in the lip requiring stitches.

Employee stepped into hot liquor causing a burn on the right foot.

Mine operator was backing up a 915 load haul dump with forks after setting a piece of face conveyor. As the operator was backing up the corner of the cab caught on a two inch lip on the rib, which caused the cab to break loose from the machine, pushing the operator against the steering wheel. The employee sustained injuries to the chest, left shoulder and left arm.

Main power loss to the 8 shaft facilities caused power loss to the 8 shaft hoist. One miner was on the hoist when power was lost and was trapped until power was restored. The miner was trapped for a total of 45 minutes.

EE was rodding a #1 feeder on 3 silo cb-2701 belt. The feeder was plugged off and EE continued to rod and a chunk broke loose and fell on the rodding bar twisting and jarring EE's back.

Installing pipe overhead, using a chain wrench to apply forward pressure. Chain wrench grip slipped, causing pipe to fall off of pipe jacks, striking employee on the left shoulder. Based on preliminary medical notes the injury is a potential non-displaced scapular fracture.

Before regular work shift started employee tripped and fell in parking lot requiring 5 stitches to right hand, 3 stitches to the right side of the head and a dislocated finger.

Employee was walking across a slab of cement near the rail road tracks. Went to step down and felt a pop in left knee and was unable to bear weight any more.

Rail fell on finger pinching it against trailer.

EE employee started to count cars at high line switch. Employee noticed some cars that had separated. Before employee could get down from the rail car the two cars joined causing an impact and throwing the employee to the ground.

Was back flushing the lime system and sustained burn to upper back and shoulders.

Grating was loose and employee stepped onto the grating causing employee to lose footing and fall through. Employee sustained some abrasions and a sprain of the right ankle.

Employee reported strain to bicep.

Employee felt strain in groin area while rodding out chunk from feeder. Further understanding of this case is pending until medical evaluation and notes have been received and reviewed.

While changing ear muffs the broken wire cut across the left index finger, eight stiches were required.

EE was walking through mud, slipped and felt a pop in right knee. EE was medically evaluated, including MRI, which revealed no ligamentous injury, but did reveal prepatellar bursitis (chronic illness with mild degenerative changes).

EE stated EE was grinding metal pipe on 1/28/16. Employee made no report of injury on 1/28/16, but went to emergency room around 7:00 pm on 1/28/16, where a metal shaving was removed from EE's eye. EE was also seen by opthamologist on 1/29/16. EE made a report of injury to supervisor on 1/29/16.

EE was walking an incline, slipped on metal grating and taken to onsite medical facility and returned to work without restrictions. Due to ongoing additional medical evaluations and investigations, this injury was determined to be work related on 2/8/16.

EE was using hydraulic equipment when the wrong function was activated causing the hammer to extend instead of swing, EE pinched right hand between the hammerhead and controls, EE went to ER. EE had a broken hand and received 4 stitches. Determined to be Reportable to MSHA on 02/05/16.

EE was walking North into the fire pump room at Mono Power. EE caught heel on a raised portion of the cement (3/8") causing EE to trip and fall onto right shoulder. This injury was determined reportable on 02/09.

EE began to water lance bundle tubes. At approx. 2245 ee inserted the lance into the 3rd tube and the nozzle hydraulic backed due to material build-up causing the lance to exit tube EE was unable to release foot pedal & dump pressure on lance water jet went across EE's left hand causing a laceration. EE was taken to doctor where EE received medical treatment (stitches).

Bolter operator was drilling gas hole and had 2 5' augers in the roof. EE lowered drill head to install 3rd auger and continue drilling. When EE lowered it the second auger came out of the roof striking roof bolter operator on the right index finger. EE was transported to ER for Treatment.

While using a jack, the handle slipped and struck the right side of the employees face causing a laceration, black eye, and concussion. Employee was transported to the Emergency room for treatment.

Team member was walking and slipped on ice and hyper extended his left arm.

EE was putting in a support crib and tightening it up by striking a wood wedge with a hammer while on a two step ladder, ladder shifted and employee lost balance hitting his left index finger with the hammer. Injury was not determined recordable until 12/3/15 due to time delay in receiving medical records.

EE was removing bore miner free bits with a bit bar when he injured himself. He hit the bit with the bar and lost his grip and hit himself with it in the forehead causing a laceration requiring 5 stitches.

EE was lifting a top roller which weighed over 80 LBS, when his arm gave out. Employee was transferred to MHSC for treatment. Followed up with specialist and determined recordable on 11/13/15.

Employee experienced severe back pain while changing clothes and transported to emergency room. Employee adamant this was not a work related incident and made no report of injury. Employee later diagnosed with compression of nerves at bottom of spine and work restrictions imposed.

While working at Mono Employee was stepping down off step ladder, slipped and fell to floor. Right arm was straight out bracing his fall and the employee felt it "pop. EE was sent to ER in town and medical documentation suggested an operation was needed to repair the injury.

Two workers were cleaning the inside of the #6 boiler scrubber ducts. EE took a step back to get out of co-worker's way when he stepped into the drain piping being used to flush material from the scrubber ducts. Because of the angle that his foot entered into the pipe, he was unable to remove it for about an hour until a section of pipe was removed. No injuries resulted.

Employee received 1st aid on 2/17 reporting pain in right hand. Incident not reported to Supervisor. On 8/24, employer advised that EE's personal physician had diagnosed carpal tunnel syndrome & imposed work restrictions. EE reports he does about 8-9 hours per week of extracurricular activities that can contribute to carpal tunnel & no relation to reported injury established.
