EE WAS INSTALLING 0-RINGS ON THE CONE CRUSHER. HE WAS PUTTING OIL ON THE BOTTOM WAREBRASS WHEN HIS RIGHT THUMB CAME INTO CONTACT WITH A SHARP EDGE CAUSING HIS INJURY.

EMPLOYEE AND CO-WORKERS WERE WORKING ON THE JAW CRUSHER AT THE QUARRY, SHORTLY BEFORE NOON EMPLOYEE WAS WITNESSED FALLING FROM THE SUPPORT STRUCTURE TO THE GROUND. EMPLOYEE WAS TREATED AT THE HOSPITAL. DURING THE ACCIDENT INVESTIGATION IT WAS DETERMINED THAT EMPLOYEE HAD CROSSED OVER A HANDRAIL ONTO A PIECE OF UNGUARDED CATWALKING. THE STRUCTURE FAILED AND THE EMPLOYEE FELL 15'.

EMPLOYEE WAS HOLDING TENSION ON FLYWHEEL COUNTERWEIGHT WITH A PRY BAR, WHILE ANOTHER EMPLOYEE INSERTED BOLTS IN HOLES. THE WEIGHT OF THE FLYWHEEL/COUNTERWEIGHT SHIFTED AND THE BAR SLIPPED STRI KING THE EMPLOYEE'S FINGER BETWEEN THE FLYWHEEL AND THE BAR.
