Injured employee was lifting five gallon buckets of material from a pump area on the plant. EE felt abdominal pain. EE reported to supervisor that EE had a pre-existing hernia. EE was transported to an ER where EE was treated for the pre-existing Umbilical Hernia. The physician placed EE on lifting restrictions and has recommended surgery to be scheduled in the next few weeks.

The injured employee (IE) was participating in first aid/CPR/AED classroom training when IE experienced a personal medical event and subsequently fell to the concrete floor striking face requiring medical treatment beyond first aid. IE was transported to a local ER where IE was treated for a nasal fracture and sutures to the nose.

The injured employee (IE) began feeling discomfort in the left eye several hours after the end of the shift. During IE's shift, the IE was grinding on metal plates while wearing the proper PPE. The IE was treated by a local ER clinic and eye doctor after a metal shard was found imbedded in IE's left eye. The metal shard was removed and prescription antibiotics were given.

Wind picked up and blew dust into the employee's eye around EE's safety glasses. EE was evaluated the next day by a Dr. and found to have a minor abrasion. EE was prescribed prescription eye drops.

EE was tasked with operating a 980G loader to build a new berm to allow drilling and blasting crews access to set up a new shot. The EE cleared material and opened the area to an exposed highwall edge, then backed up, towards the edge of the highwall, causing the machine to fall to the bench below, sustaining injuries from the impact.

EE was working inside a bin installing ceramic wear plates, using a steel hammer to set the plates in place. As the EE tapped the plate in place, a small shard of steel broke off of the plate and struck the EE in the left eye.

Employee was handing a handrail to another employee and lost grip. The handrail was being handed up when employee lost grip and the clip on handrail hit employee on the bridge of nose. (Handrail pipe w/clip scratched employee's nose).

An employee was walking down a stair case and asked an employee on the catwalk above to hand them their radio. The employee on the catwalk attempted to lower the radio via the mic cord. The employee on the catwalk let go of the radio causing the mic cord to hang up on the hand rail. This caused the trajectory of the radio to alter and strike the employee in the mouth.

The employee was transferring from one rail car to the next when he slipped and fell landing on his shoulder. The employee reported pain in his ribs and shoulder. An MRI was taken on 07/17/15 that indicated a rotator cuff injury. Surgery is scheduled for 07/31/2015. The employee has been on light duty but it has not offset his normal job duties.

Employee was holding a bar while another employee was hitting it with a sledge hammer. The hammer glanced off the bar and struck employee in the mouth. Employee's top lip needed stitches and he has a cracked tooth that is going to need a root canal before repairing.

Employee was walking down stairs when he felt a pop and burning in his left knee around the front of his knee cap. He had sharp pain when walking, pain and swelling in his left knee with a dull ache while sitting. Physician found medial joint tenderness. Follow up exam indicated the need for surgery, performed 9/2/05.

At the time of the incident employee was removing cutting edges from the bucket of a CAT 992C loader. To prevent the square shoulder of the bolt from slipping out of its locking lug while removing the nut he would hold the bolt in place while engaging the impact wrench. When the impact wrench was engaged the bolt slipped out of its lug and spun, cutting his hand.

EMPLOYEE WAS WORKING ON REPLACING WORN SCREEN PANELS. HE MADE A SINGLE CUT ON A METAL BAR WITH TORCH. SPARKS IGNITED FABRIC ON LEATHER GLOVES CAUSING 2ND DEGREE BURNS ON APPROX 2 SQUARE INCHES ON THE BACK OF HIS LEFT HAND.

IT WAS NOT DIAGNOSED AS A WORK-RELATED INJURY UNTIL 10-10-01 AT WHICH TI ME IT WAS DIAGNOSED AS TENDONITIS DUE TO REPETITIVE MOTION. EE WAS A ON LIGHT DUTY FROM 9/4/01 UNTIL 10-10-01.

A BREAKER SHORTED OUT AND EXPLODED. TWO EES THAT WERE STANIDNG IN FRONT OF THE SMALLER BREAKER SUFFERED MILD TO MODERATE FLASH BURNS TO THE EYES AND FACE. ON EE WEARING SAFETY GLASSES RECEIVED A MINOR FLASH BURN THAT REQUIREFD FIRST AID TREATMENT IN THE FORM OF IRRIGATION. THE 2ND EE, WHOD WAS NOT WEARING SAFETY GLASSES, RECEIVED MODERATE FLASH BURNS AND LESIONS TO HIS EYES.
