Employee smashed left pinky and ring fingers on metal plate. The tips of the pinky and ring finger were amputated 5 days later.

Quality Control employees was removing two 5 gallon buckets of sample material from the back of the truck at the same time and strained EE's back. EE did not report to EE's supervisor until 5 pm on 01/28/20

EE was tasked with operating a 980G loader to build a new berm to allow drilling and blasting crews access to set up a new shot. The EE cleared material and opened the area to an exposed highwall edge, then backed up, towards the edge of the highwall, causing the machine to fall to the bench below, sustaining injuries from the impact.

Connector was positioning a motor base for rigging to be removed. Connector was using a stever bar (pry bar) to leverage motor base.

EE was working inside a bin installing ceramic wear plates, using a steel hammer to set the plates in place. As the EE tapped the plate in place, a small shard of steel broke off of the plate and struck the EE in the left eye.

Employee was handing a handrail to another employee and lost grip. The handrail was being handed up when employee lost grip and the clip on handrail hit employee on the bridge of nose. (Handrail pipe w/clip scratched employee's nose).

Employee was walking from one side to another around a rebar form and made a misstep and scraped both shins and had to 5 stitches on left shin.

Employee was mounting cooling unit onto crusher unit, when cooling unit shifted and pinched left ring finger, resulting in a broken left ring finger with a laceration.

EE was walking toward the shop, slipped on a rock on the frozen ground, fell to the ground and dislocated a shoulder.

An employee was walking down a stair case and asked an employee on the catwalk above to hand them their radio. The employee on the catwalk attempted to lower the radio via the mic cord. The employee on the catwalk let go of the radio causing the mic cord to hang up on the hand rail. This caused the trajectory of the radio to alter and strike the employee in the mouth.

Employee was maintaining a material chute when the steel spring back and pinched EE's finger.

Employee slipped on ice/snow. The resulting injury was treated as first aid initially. Employee informed that doctors are now recommending surgery. Surgery scheduled and employee set to return to work soon.

The employee was transferring from one rail car to the next when he slipped and fell landing on his shoulder. The employee reported pain in his ribs and shoulder. An MRI was taken on 07/17/15 that indicated a rotator cuff injury. Surgery is scheduled for 07/31/2015. The employee has been on light duty but it has not offset his normal job duties.

Doing daily pre-shift inspection on a Cat 777 haul truck. Employee tripped and lost his balance and fell. Tried to catch himself, landed on left hand and felt pain in left shoulder. Dr. restricted use of shoulder for one week and gave him a sling to wear

Employee was holding a bar while another employee was hitting it with a sledge hammer. The hammer glanced off the bar and struck employee in the mouth. Employee's top lip needed stitches and he has a cracked tooth that is going to need a root canal before repairing.

Employee was walking down stairs when he felt a pop and burning in his left knee around the front of his knee cap. He had sharp pain when walking, pain and swelling in his left knee with a dull ache while sitting. Physician found medial joint tenderness. Follow up exam indicated the need for surgery, performed 9/2/05.

At the time of the incident employee was removing cutting edges from the bucket of a CAT 992C loader. To prevent the square shoulder of the bolt from slipping out of its locking lug while removing the nut he would hold the bolt in place while engaging the impact wrench. When the impact wrench was engaged the bolt slipped out of its lug and spun, cutting his hand.

EMPLOYEE WAS LOADING A LARGE PIECE OF IRON INTO THE BACK OF A PICKUP & BUMPED HIS KNEE. THE NEXT DAY AT WORK IT HAD STARTED TO SWELL. THIS IS BEING REPORTED AT THIS TIME BECAUSE THE EE & THE C OMPANY WERE UNSURE AS TO WHETHER THE INJURY WAS WORK-RELATED.

EMPLOYEE WAS WORKING ON REPLACING WORN SCREEN PANELS. HE MADE A SINGLE CUT ON A METAL BAR WITH TORCH. SPARKS IGNITED FABRIC ON LEATHER GLOVES CAUSING 2ND DEGREE BURNS ON APPROX 2 SQUARE INCHES ON THE BACK OF HIS LEFT HAND.

STEEL BEAM FROM SCREEN DECK WAS BEING CUT WHEN ALL OF A SUDDEN IT CAME LOOSE STRIKING EE.

THE EE HAS NOTICED ONGOING WRIST PAIN. HE SAW A PHYSICIAN ON 8-17-01 WHO TOOK HIM OFF OF WORK FROM 8/20-9/4 TO REST IT. IT WAS NOT DIAGNOSED AS A WORK-RELATED INJURY UNTIL 10-10-01 AT WHICH TI ME IT WAS DIAGNOSED AS TENDONITIS DUE TO REPETITIVE MOTION. EE WAS A ON LIGHT DUTY FROM 9/4/01 UNTIL 10-10-01.

EE NOTICED PAIN IN HIS LEG. THE DOCTOR ORDERED AN MRI & HAS DETERMINED THE PAIN IS CAUSED BY A RE-OCCURANCE OF A HERNIATED DISC, AN INJURY THATOCCURED ON THE JOB IN 1998 WHEN EE STEPPED IN A HOLE & TWISTED HIS BACK. HE WILL UNDERGO A FUSION ON 1/1/01 & IS EXPECTED TO BE OUT OF WORK FOR THREE TO SIX MONTHS.

EE WAS DOING A LOT OF SHOVELING AT THE END OF THE WEEK HE NOTICED REDNESS & SWELLING & PAIN. THE DR DIAGNOSED TENDONITIS RECOMMENED LIGHT DUTY

A BREAKER SHORTED OUT AND EXPLODED. TWO EES THAT WERE STANIDNG IN FRONT OF THE SMALLER BREAKER SUFFERED MILD TO MODERATE FLASH BURNS TO THE EYES AND FACE. ON EE WEARING SAFETY GLASSES RECEIVED A MINOR FLASH BURN THAT REQUIREFD FIRST AID TREATMENT IN THE FORM OF IRRIGATION. THE 2ND EE, WHOD WAS NOT WEARING SAFETY GLASSES, RECEIVED MODERATE FLASH BURNS AND LESIONS TO HIS EYES.
