Injured EE was asked by other EE to assist with equipment install at crusher. In attempting to remove 150lb guard for install, injured EE hand was in line of fire when other EE lowered guard thinking they were out of the way. Injuries: Fracture right ring finger, lacerations to right ring and middle fingers rq. stitches

Employee cut left index finger while attempting to slice through a grease line with a utility knife

Employee was repairing a drive belt, and attempting to loosen a nut, using a crescent wrench. While employee was applying pressure to loosen the nut, employee felt a pain in right shoulder.

While cutting square tubing with band saw, dead end fell off roller saw horse and kicked up end being cut, smashing finger tip between saw and pipe.

Associate was climbing down the extension ladder when the associate missed the second step from the bottom straining the right hip.

Dumping small lab hopper and associate hit the frozen sand with hand to free material. The frozen sand came out and the hopper weight shifted and hopper lid stuck right hand index finger, peeling skin back. Taken to urgent care and received 5 stitches.

Two associates were rotating a steel frame. The frame was pulled to the edge of the work bench and one of the associates reached to grab the frame when it fell to the ground. His small left finger was pinched between steel frame and table top resulting in a fracture.

Employee was holding a piece of steel with his hand when cutting with the band saw. Broken teeth on saw grabbed the steel, forcing steel forward and pinching of finger between steel being cut and steel laying next to band saw. Wrapped finger and received stitches at urgent care clinic. Vice not used to secure steel while cutting and hands should have been kept clear.

Doing morning housekeeping. Bent forward and lower back went out.

Employee tried to kick a frozen chuck of sand loose. Kicked the ice and felt a pain in his back.
