Employee inside Screen deck removing frozen agg. material from belt with pry bar. Chunk came free & struck the employee in the jaw, chipping a tooth. We were informed on 12/3/24 that the employee had dental work done. Reporting within 10 days.

Employee is claiming carpal tunnel syndrome in right wrist due to years of doing maintenance work & operating a large loader. Employee is having surgery.

An employee and a co-worker were performing maintenance on a generator and were taking off the electrical panel. During this process the panel slipped and the employee cut his palm on a tab located on the backside of the panel. He sought medical treatment and received stitches.
Employee was doing general repair/maintenance & welding. Sometime in the morning his eye became irritated. He sought treatment on 2/20/14 & a foreign body was removed from his eye. He was given antibiotic ointment.
