It was sleeting when subject was descending a metal grate staircase when they slipped. Subject grabbed the railing and heard a pop in right shoulder and felt pain. Subject also felt pain in right ankle. Subject has returned to work but is awaiting medical follow up. Subject is employed by Yaggy Trucking and not a miner. Yaggy trucking does not have a contractor ID number.

Subject was changing the bags in a baghouse on 3/13 and 3/16 which involved a lot of bending and lifting. Subject had discomfort in EE's back and thought resting it would be fine, but still had pain days later. Subject reported it on 3/19/2026. Subject has been going to a chiropractor to help with the discomfort.

A pipe was rigged up to be placed when the weight shifted and the subject's hand was smashed between the pipe and a pump. The subject suffered a broken left ring finger.

While using a punch to dislodge a backstop bushing from a conveyor gear reducer, subject struck a finger with the hammer. Subject did not think it was bad until they noticed the bone was moving the next day.

Associate was using an electric rail car opener when the reaction arm slipped from the rail car body and struck the associate in the head. The associate was confused and had brief memory loss. Associate suffered a concussion.

We were removing pipe from the clients well. Crew was using a dong tong (large pipe wrench) utilizing a sling on a ram to help break the coupling loose. The sling slipped off the wrench causing the wrench to move back toward employee. Employee twisted EE's knee when this happened. Employee has been seen and is currently on light duty working in our shop.

Associate was changing a tooth on a bucket when they struck the tip of the tooth with a hammer causing a sliver of metal to break off. The sliver of metal was embedded in their cheek.

While cleaning a car at the rail clean-out, the associate used a bar instead of a car opener on a stubborn hard rail door. While doing this, the associate struck EE's right leg/shin with the bar and suffered a puncture wound to the right leg/shin area. Associate went to clinic and received 2 x-rays, (1) stitch and a tetanus shot.

Miner was cutting tape off a wire lead to change a motor. Miner cut a slit in the top of the tape/lead and the knife cut through quicker than usual and the knife cut miner's forearm.

Associate was working on a pump in the primary wash area. Coworker noticed the associate sweating a lot and not responding to questions that the coworker was asking the associate. Temperature was 55 degrees outside. Associate seemed confused and not acting right.

Employee had a an audiogram that showed a Standard Threshold Shift. EE also had a second audiogram to confirm the findings of the first. The Associate Farms, Hunts and uses a Chainsaw in personal life without proper hearing protection, so this hearing loss is most likely due to high non-occupational noise exposure.

Miner was in a crouching position to clean out under a railcar. When miner came out under the railcar, miner could not straighten knee that felt locked up in a bent position. Miner had surgery on 6/10/2019.

At 7:30 AM an articulated haul truck crossed a dike which caused the road bed to fall into a hole under the road which is located on the top of the dike. At 10 AM the secondary wash plant shut down to reduce water entering pond 2. At 2:15 PM the diameter of the hole changed so was reported to MSHA as an unstable impoundment.

Miner was using a ¾ inch ratchet to operate a gate on the S3 structure. When the miner pushed down on the ratchet, their pinky finger was around the end of the ratchet handle and the ratchet slipped out of the modified shaft end causing their finger to be caught between the guard and the handle.

The miner was fueling the haul truck from the fuel tank. The miner turned around to talk with another associate and tripped over the large orange wheel chock falling on left knee cap. The miner had surgery on December 7, 2017 and is expected to be off work two to four weeks.

While changing an elevator bearing, the miner went to assist the other miner on the opposite side of the elevator. EE's body was then out of position and they noticed a shoulder problem when upper range of motion was attempted. Miner had surgery on 06/25/2018 since shoulder did not heal properly over time.

Miner was checking the proper sequence of rail cars. The miner did not realize that ee had traveled past the end of fall protection lifeline. This stopped the miner and caused body to turn and fall onto the side of the rail car. Miner did not miss work but was sore so went to doctor on day off 2/15/17. X-rays showed cracked ribs. No other medical treatment was given.

The miner was using a Leatherman to cut plastic pipe. The Leatherman slipped and cut the inside of the miner's left thigh. The miner received a cut resulting in five stitches.

At the bi-annual occupational health physical, the physician noticed problems with the miner's wrists. Upon further medical evaluation on 10/1/2012 the miner was diagnosed with carpal tunnel syndrome in both wrists.

Miner was loading railcars and at the time of the incident crossing from one railcar to the next. The lanyard from the fall protection system is rolled out ahead of the miner. In this case the rollers attached to the lanyard which rolls on the overhead cable caught on a splice. When this happened the associate fell backward on the catwalk of the car behind.

Associate was filling the water truck. The valve to fill the truck was over grown with weeds and he had to clear them out. He thinks he touched poison ivy on his arms and it spread to his stomach.

Associate called work from his home residence after his work shift. Associate was working outdoors two days in the warm rail yard. He reported not feeling well. Upon the phone call we encouraged him to seek medical attention. He saw a doctor that afternoon and learned he had another medical issue going on.

Associate was running the 834 dozer for extended periods of time the past few months. The control level for the blade has not been working the way it should. This exposed the associate to repeated actions with his fingers to accomplish what should be a smooth continuous movement.

Installing a fender on haul truck box assembly. Associate was standing on an elevated platform to position fender. Fender was swung into position prior to him mounting the platform stairway. As he was aligning bolt holes to fasten the fender it became wedged in the assembly. When fender unstuck it hit associate causing him to overcenter his weight and platform to tip over.

Replacing fasteners on 980G loader bucket cutting edge assembly. One of the fasteners fell down not in contact with blade anymore so associate had to reach through and push the fastener in place to remove and replace it. The worn sharp edge of this fastener cut through the glove and left middle finger.

No specific event. Working at rail loadout car cleaning site installing 136 pound grating sheets and 24 inch belting in between rails. Utilized a material handler to do heavy work and physical work consisted of positioning the grating sheets in place with two associates.

Working in a scissor lift, got off of it ot carry angle iron. As he was walking with the angle, his safety harness lanyard got caught on the heal of his boot, which caused him to fall foward over some pipe and land on the ground.

Associate was assisting to move and lift a 32 foot fiberglass ladder from the scrubber building. The ladder was located in a tight area and was entangled in an upper unistrut. While one associate was working to release the upper portion, the associate on the lower side twisted and injured his lower back.

Excavator had been removing material from the toe of the slope which surrounds Pond 7. While taking a break, material began to crumble off the slope. The operator attempted to move the excavator away from the slope, but while attempting to do this, the excavator was engulfed with material and flipped partially on it's side.

Mine associates left the main office area and traveled to the mine work area at 6:20 a.m. One EE notified another EE at 6:25 a.m. by FM radio the mine office trailer had been destroyed by fire. The fire occurred sometime over the weekend. The mine was not in operation and no injuries. MSHA was contacted at 6:34 a.m. to report the fire. Cause has not been determined.

Associates were closing a difficult discharge gate on a rail car. Associate reached in to adjust the lock at the same time his team mate gave leverage to a bar to close the door. The associate's right index finger got pinched between the lock and rebar.

Associate was installing right steering cylinder on 988G loader. When positioning the front end of the cylinder, the rear portion came off the mount and cylinder shifted pinching his left middle finger between the chain and the frame of the loader.

While operator was running the dozer, the air conditioner vent came loose and fell down. The operator attempted to replace the vent cover without turning off the air conditioner and his hand hit the fan blade which is about five inches inside the unit.

Associates were welding pipe. A tension problem for pipe clamped into position. The associate got out of crane to assist the ground man. The associate was unbolting tension using his left hand which was near pinch point. At the same time other associate made adustment with hydraulic level to remove tension. The level spiked up then down pinching thumb between metals.

DOCTOR DIAGNOSED TENNIS ELBOW ON 12/31 & THOUGHT HIGH IMPACT WRENCH MAY HAVE CONTRIBUTED.

AFTE R THE APPOINTMENT HE RETURNED TO WORK BUT THEN WENT HOME. THIS IS A REOCCURRING INJURY. LAST 7000-1 REPORTED ON 9/8/03 AND HE WAS RELEASED ON 10/10/03.

AS HE APPLIED PRESSURE TO THE DOOR, HE FELT A PAIN IN HIS LOWER LEFT ABDOMEN. LITTLE BY LITTLE THE PAIN IS GROWING. HE SOUGHT MEDICAL TREATMENT ON 11/25/03. HE WAS PLACED ON NO PUSH/PULL RESTRICTIONS UNTIL 12/16/03.
