EE was changing screens inside screen box and felt a back strain. EE self-cared until 10.13.2023 when they sought medical attention. EE was placed on restricted duty Eff 10.13.2023 and is currently treating and working within restrictions.

Injury was climbing up into the loader and hit the ladder with his left knee. He did not report the injury to us as a workplace injury until 05/26/09. This is a questionable work comp case because of conflicting information to the doctor and employer so is under investigation. I will update you when the case is denied or confirmed as a workplace injury.

The employee was stepping off truck on uneven ground after loading and felt a sharp pain in the lower abdominal area. He went to the doctor and was diagnosed with a pulled abdominal muscle. He was released to full duty on 08/30/06. Employee was released from medical on 08/29/06. This employee does not work in the pit.
