Green Bay Lime Kiln Metal/Non-Metal

Employee was closing the lids on top of a railcar when they tripped and fell forward. The employee was wearing their fall arrest and using a retractable lanyard. When they fell their arm was caught by the lanyard, resulting in a shoulder separation. Employee had surgery on 11/14/2023. Worked until then.

Employee felt a sharp pain in their right shoulder while pulling ash. Employee continued to work in full capacity until which time surgery was required on shoulder. Surgery was performed on 3/15/2023 to their shoulder.

Employee fell down a flight of 10 metal stairs in the Hydrating building. Employee failed to firmly plant foot on first step and failed to maintain 3 points of contact. Employee suffered minor bruising, lacerations and friction burns to their knees, shins, left cheek, left elbow, left shoulder, neck and right hand.

Employee went to use hand sanitizer and while pushing down on the nozzle the hand sanitizer was forced upward and into the employee's right eye. Employee noted a stinging in their eye. On 12/28/2020 the employee went to an optometrist and was prescribed an antibiotic eye drop.

As Graymont was not a party to this accident, Graymont does not have detailed information for this accident. Per the attached, Graymont maintains that this accident is not MSHA jurisdiction, is not chargeable to Graymont or mining industry, continues to be investigated by OSHA, was not a Graymont employee, & occurred outside of Graymont's privately held mine site.

Employee slipped on ice while exiting vehicle in parking lot. The employee landed on their elbow. Initially, employee did not report an injury. On 3/22/2019, employee reported to work with note stating they could not return to work for 2 weeks. On 4/15/2019 the injury was determined to be work related.

In the process of building a set of steel saw horses and an approximate 4' piece of 4X4 tube steel was standing vertical on the bench when it tipped and struck employee's left thumb between the steel tabletop and the piece the employee was handling.

Employee was on a back side of a cooler repairing steel, grinding, cutting and welding in dusty conditions. He was fine when he left work but the next morning his eye was irritated and felt like something was in it.

Employee opened lid of hydrate plant pugmill to inspect discharge end. When he closed the lid hydrated lime dust blew into his right eye. He was wearing safety glasses at the time. He immediately flushed the affected eye at the plant and was brought to the emergency room for additional flushing. He did not miss any work over the incident and returned to his normal work duties.

Employee was walking to parking lot when a wind gust blew lime into eye. Employee flushed eye. Additional medical treatment included prescription for the eye.

Employee was diagnosed with bilateral carpal tunnel syndrome. Employee is having surgery to repair. Date and time of accident is the date and time employee turned in paperwork showing the diagnosis.

Employee was using a 4 inch grinder to cut a piece of plastic. The grinder wheel grabbed the plastic and came into contact with employees left middle finger. The employee sustained a broken finger and a laceration.

Employee was removing chunks from G289 airlock with a rod. While employee was unplugging the G289 airlock; dust blew out of the pipe and into employees eyes.

Employee was pulling ash with the chunk puller and his left hand was pinched between an i-beam and the chunk puller causing a hand contusion with hematoma.

Employee was walking across the plant, when he slipped on the muddy and wet pavment. Employee fractured right wrist.

While employee was placing a bulk bag on the hydrate bagger, he overextended his reach which caused a muscle stain in his lower back.

Employee was pulling the ash can up with a hoist to empty the can. While employee was holding the chain to guide the ash can his thumb ran into the pulley. Employee's right thumb was cut and bruised.

Employee was raking ash with the ash puller, when hot lime dust was pushed out the door. Employee had a lot of lime dust hit face, left eye red and swollen.

Employee was using a front end loader to transfer stone from the stockpile to the reclaim system. The stockpile was frozen and snow covered in several areas. The employee undercut the stockpile attempting to reach thawed and thus free flowing stone. The stockpile overhead sloughed off and engulfed the loader into the cab.

Employee was pulling ash with ash puller and roller rest from the grates using the front pulling door. Employee struck his right hand against a burner pipe joint. The right little finger (pinky) is bruised and sore. Employee did seek medical attention on 1/30/07.

Employee was "pulling" ash with ash puller. When he lifted the ash puller, he felt a strain in his left elbow. He sought medical attention on 7-24-06; his left arm is in a sling; restricted to lifting 20#, cannot climb, occasional twisting motion allowed and seldom can squat or push/pull.

Employee was using a metal rod to clean the O2 analyzer. The system pulsed which caused hot air and dust to go onto his face. Lime and hot air combined to irritate both eyes; causing them to become red, puffy and water.

Employee was using a shovel to clean under the C32 elevator. He felt something "give" in his lower back while shoveling. (shift started previous night)

Employee was walking to the dust collectors when something blew into his right eye. Right eye became red and irritated with time. Went to a doctor and rec'd acular eye drops on 10/21/05.

Employee was using a prybar to pull off grates. Prybar slipped and hit employee on his chest and left side. Caused bruising and cuts to his chest. Em'ee started work the night of 10/18 at 6:30 p.m.

Employee was cleaning hot lime out of the #2 kiln basement. He put the hot lime into a barrel to haul out of the basement.

Employee was stepping into the skidsteer when her foot slipped causing her to fall against skidsteer with her right hand. This action bent her ring and pinky fingers backward.

EE was raking chunks of coal, while raking chujmks he twisted his back to toss chunks into a barrel. Ee felt pain in his lower back.

Employee was helping to install a sump pump near the hydrate plant; em'ee moved a pallet and slipped in water. The left boot filled with water, which reacted with lime and caused a burn to the top/left side of left foot.

Ee was attempting to dislodge a chunk of lime from the bottom of a baghouse. The hot dust that was hung up poured out onto his left boot. The extreme temperature of the dust caused the boot leather to become burning hot. The heated leather caused burns to his left foot. Ee did seek medical attention on 10-16-04.

EMPLOYEE WAS CHECKING RAILCAR TO SEE IF BOTTOM DOORS WERE CLOSED. HE STEPPED OFF CURB. HIS LEFT ANKLE GAVE OUT AND TURNED INWARD. EMPLOYEE DID SEEK MEDICAL ATTENTION: LEFT ANKLE SPRAINED.

EMPLOYEE WAS USING A WIRE WHEEL GRINDER. THE GRINDER HIT A PIECE OF METAL, KICKED BACK, HITTING THE EMPLOYEE ON RT. SIDE OF CHEST. EE DID NOT SEEK MEDICAL TREATMENT UNTIL 9/9/04.

EE WAS MIXING REFRACTORY USING A SHOVEL AND HOE. WHILE MIXING SHE FELT A "SNAP" IN HER RIGHT FOREARM. EE DID SEEK MEDICAL ATTENTION ON 5-11-04.

Employee was performing his normal duties as a hydrate operator. EE was feeling pain three hours prior to starting his shift. he did mention this fact to his supervisor.

EE WAS WALKING PAST THE CASE SKIDSTEER. HIS FOOT CAUGHT THE EDGE OF THE LOADER BUCKET. THIS CAUSED EE TO LOSE HIS BALANCE AND FALL FACE DOWNONTO THE GROUND. EE DID NOT SEEK MEDICAL ATTENTION UNTIL MARCH 1, 2004.

EMPLOYEE WAS CLIMBING DOWN THE LADDER ON A RAILCAR. HIS FOOT SLIPPED OFF THE LAST RUNG CAUSING HIM TO FALL AGAINST AN ADJOINING RAILCAR WITH HIS LOWER TORSO. HE THEN FELL TO THE GROUND LANDING ON HIS LEFT SIDE. HE DID NOT SEEK MEDICAL ATTENTION UNTIL FEB. 2, 2004.

EE WAS CLEANING OUT BOTTOM OF ELEVATOR. THE ELEVATOR DOOR CAME LOOSE, SWUNG DOWN AND HIT EE ON HIS HEAD.

EMPLOYEE WAS USING A GARDEN SHOVEL TO LOAD ASH CHUNKS AND STONE INTO A WHEELBARROW. HE FELT PAIN IN THE RIGHT SIDE OF HIS MID/LOWER BACK, WHILE HE WAS LIFTING/ TWISTING.

EE WAS MOVING A PALLET OF LIME WITH THE FORK LIFT. LIME DUST ENTERED EYE CAUSING EYE TO BECOME SORE.

EMPLOYEE WAS LEANING OVER A RAIL TO PUT BOLTS ON A PLATE. HE HAD A FEELING OF NUMBNESS AND THEN A SHARP PAIN IN HIS ABDOMEN WHEN HE STOOD UPRIGHT.

EE STATES HIS RIGHT WRIST IS SORE FROM WORKING OVER THE "YEARS". COULD NOT INDICATE ANY PARTICULAR DATE OF INJURY.

EE WAS LOADING HOT LIME MATERIAL WITH SKID STEERINTO THE BUCKLEY. UPON ENTERING THE PILE, THE HOT DUST AND STEAM ENGULPED HIM. HE RECEIVED BURNS AN HIS FACE ARM, AND CHEST AREA.

EMPLOYEE SLIPPED PARTIALLY INTO A CATWALK GRATE OPENING. HE HAS A LACERATION ON HIS LEFT LOWER LEG THAT REQUIRED SUTURES.

EMPLOYEE WAS PULLING AN EMPTY BULK BAG OUT OF A BUNDLE OF BAGS. HE FELL BACKWARDS ONTO THE CONCRETE FLOOR, EE HIT HIS LEFT ELBOW/ ARM ON THE FLOOR.

EE WAS IN THE PROCESS OF "KNOCKING DOWN" KILN ASH. EE TRIPPED OVER AN ASH PILE: HIS LEFT KNEE WAS OVEREXTENDED. MEDICAL ATTENTION WAS RECEIVED.

EE WAS SWEEPING THE PREHEATER FLOOR WITH A BROOM. DUST AND DEBRIS ENTERED HIS RIGHT EYE.

EE WAS WORKING AROUND THE PLANT DURING A SNOW STORM. WEATHER CONDITIONS WERE WINDY: DUST & LIME WERE BLOWING AROUND. A PIECE OF LIME BLEW IN HIS MOUTH ONTO HIS TONGUE.

EMPLOYEE WAS USING A RAKE TO PULL ASH CHUNKS FROM THE COOLER GRATES. EMPLOYEE FELT PAIN IN LOWER BACK FROM BENDING PULLING ACTION.

EE WAS BAGGING LIME.LIME DUST WENT THROUGH HIS CLOTHING, THE COMBINATION OF LIME DUST & THE HIGH TEMPERATURE CAUSED A CHEMICAL BURN TO HIS LOWER RIGHT BACK

WHILE PULLING 2X8 BOARDS OUT OF FEED END OF #1 KILN, THE BOARD FELL INTO THE HOPPER. THIS CAUSED THE OPPOSITE END OF THE BOARD TO BOUNCE UP. THE BOARD HIT EE IN THE CHEST AND PINNED HIM AGAINS T THE DOOR. EE EXPERIENCED CHEST PAINS AND SORENESS.

WHILE CLEANING OUT THE BAGGER PIT, LIME GOT INTO EE'S EYE RIGHT.

EE WAS RESTACKING 50# BAGS. HE PICKED ONE OFF THE FLOOR AND FELT PAIN IN HIS LOWER BACK. EE DID SEEK MEDICAL ATTENTION.

EMPLOYEE WAS CLEANING OUT THE SILO PRODUCT BINS, WHEN LIME FELL ON THE BACK OF HIS NECK. EE DID SEEK MEDICAL ATTENTION. HIS SHIFT STARTED ON 8/15/01 AT 6:30 PM UNTIL 6:30 AM ON 8/16/01.

WHILE PULLING HEAD PULLEY ONTO C-114 CONVEYOR, EE FELT PAIN IN HIS LEFT SHOULDER.

EMPLOYEE WAS USING A TWISTING MOTION TO MOVE EMPTY BAGS FROM THE LEFT SIDE OF THE BAGGER TO THE RIGHT SIDE. HE FELT A SHARP PAIN IN HIS LOWER BACK. EMPLOYEE IS SEEKING MEDICAL ATTENTION.

EE WAS PULLING ASH CHUNKS WITH A RAKE. HE NOTICED BACK PAIN. EE WENT TO THE DOCTOR ON 5-18-01.

EE WAS CLEANING UP A PILE OF LIME THAT SPILLED IN THE #1 BASEMENT. WHILE LIFTING, HE FELT A SHARP PAIN IN HIS LOWER BACK.

WHILE PEROFRMING MAINTENANCE WORK, THE EE STEPPED ON A CATWALK AND THE GRATING GAVE WAY. EE FELL THRU WALKWAY AND LANDED ON A PIPE BELOW. EE DID SEEK MEDICAL ATTENTION.

EE WAS STACKING BAGS: HE WENT TO GRAB A BAG THATWAS CAUGHT ON THE CONVEYOR BELT. HIS FINGER CAMEIN CONTACT WITH THE CONVEYOR BELT. HE DID SEEK MEDICAL ATTENTION.

EE CALLED IN ON 1-30-01 COMPLAINING OF PAINS IN HIS CHEST. HE ALLEGES WHILE HE WAS PULLING CHUNCKS OUT OF THE #1 BASEMENT HE FELT PAIN IN HIS NECK, SPINE AND CHEST. THE PAIN IN HIS CHEST CONTI NUED, SO HE WENT TO DR. HE HAS FRACTURED RIB.

EE WAS ENTERING THE DUST COLLECTORS. HE SLIPPED ON THE LADDER AND FELL FORWARD INTO THE DUST COLLECTOR. HE LANDED ON HIS RIGHT HAND. EE DID SEEK MEDICAL ATTENTION.

EMPLOYEE WAS CLEARING ASH CHUNKS FROM THE KILN COOLER GRATES. HE EXTENDED THE RAKE WITH HIS RIGHT ARM AND HIT HIS ELBOW ON THE SAFETY RAIL WHEN THE RAKE SLIPPED. EMPLOYEE DID NOT SEEK MEDICAL ATTENTION.

EMPLOYEE STUMBLED WHILE WALKING AND FELL FACE DOWN INTO A METAL POLE. EMPLOYEE DID SEEK MEDICAL ATTENTION.

EMPLOYEE WAS TALKING TO SOMEONE IN THE SHOP AREA. WIND PICKED UP AND BLEW LIME DUST IN HIS EYES. NOTICED IRRITATION LATER AND WENT TO HOSPITAL.

WHILE FILLING BULK BAGS OUT OF BIN 43, THE LIME DUST PUFFED UP. THE WIND BLEW THE LIME DUST UNDER EMPLOYEE'S SAFETY GLASSES. EMPLOYEE TAKENTO THE EYE CLINIC. MATION REC'D PRIOR TO 2/14/01 TO INDICATE ANY TREATMENT BEING GIVEN OR ANY DIAGNOSIS

EE MISSED A STEP WHILE CLIMBING DOWN FROM THE PREHEATER. HE NOTICED SWELLING AND PAIN APPROX ONE HOUR LATER. EE WAS TAKEN TO HOSPITAL.

WHILE TORCHING A HEATING PIN ON THE PAYLOADER BUCKET, THE EMPLOYEE TURNED TO LOOK AT THE PIN. AS HE TURNED THE TORCH MOVED ACROSS THE TOP OF HIS LEFT HAND.

EE WAS TOSSING ASH CHUNKS FROM THE ACESS DOOR OF KILN #2. HE FELT A SHARP PAIN IN HIS LOWER BACK.

EE WAS LIUFTING A 50 LB. BAG TO THE TOP OF THE PILE. HE FELT A PAIN IN HIS CHEST.

EMPLOYEE FELL OFF HIS TRUCK WHILE TARPING HIS LOAD. HE FELL FACE FIRST ON THE PAVEMENT. HE WAS TAKEN BY THE RESCUE SQUAD TO THE HOSPITAL. CORPORATE OFFICE NOTIFIED UMTHUN TRUCKING IMMEDIATELY AND WE CORRESPONDED HOSPITAL INFORMATION TO THEM
