Employee was troubleshooting the MODCO Pod in product handling area when an air pulse blew lime dust into left eye. EE rinsed EE's eyes multiple times during the day and after shift change sought medical attention where EE was prescribed eye drops.

Miner conducting a work area inspection. Miner looked up at approaching pickup headlights and slipped on a patch of ice. X-ray taken 12/12/2024 no fractures and released to full duty. MRI referral was made on January 1/24/2025. 1/27/2025 rotator cuff tear was diagnosed. On 2/3/2025 referred to Orthopedic specialist for surgery.

Product loader was on back of trailer when trucker started moving forward. Steps hooked on truck hatch pulling steps under catwalk and employees legs became entangled. Broken right ankle, broken big toe, and lost index toe on right foot.

Employee opened the kiln firing door to check the grates. This caused some dust to stir up and it got in employees eye. Employee flushed eye at work but later went to the clinic. Employee was given prescription eye cream.

Employee was adjusting roller when pallet exited conveyor and rolled over their hand. Contusion on right hand. Nothing broken.

Employee was performing maintenance on a conveyor belt when a foreign body entered eye.

Employee was stepping off of forklift, felt slight twinge in right knee. Worked for several hours while knee was sore. After taking a break for lunch employee was unable to bear weight on leg. Waiting for doctor diagnosis.

Employee was changing air pads and laid a impact tool down. The tool got dust in the back of it so when the employee went to use the tool, the air blew the dust out of the back of the tool and into the employees eyes. Employee was wearing safety glasses.

When stepping out of vehicle in the morning prior to shift, employee slipped in the parking lot. It had snowed the previous night and was snowing still that morning. Strain to right knee.

EE was lifting rubber belting out of basket. The basket swung and bumped EE in the back of right calf. At the time the EE stated that it didn't hurt and EE hardly felt the bump. The next morning the EE has a large lump in muscle so EE visited doctor and doctor put EE on restrictions. Contusion to muscle. Blood thinner medicine could have caused bruising.

Removing a piece of brick that was wedged in the end gate of the dump truck. End gate shut catching employee finger between latch and bar of end gate. 19 stitches required.

Employee was using a grinder with a 6" cut-off wheel. Employee had finished the cut, released the trigger on the grinder and was moving to set the grinder down on the ground. The wheel was still spinning as the grinder was shutting down, employee inadvertently touched it against the back of left hand and it cut though the glove and caused a small laceration. 2 stitches required.

While operating a crane a wind storm suddenly popped up. This storm kicked up a bunch of dust in the path of employee. EE was wearing safety glasses and other personal protective equipment. During the wind storm some dust got in the left eye.

Employee was replacing bolts to secure a handrail when he got a piece of lime scale or dust in his eye. Right eye scratched.

Employee was poking hydrated lime out of the ball mill and stuck his finger on a bolt. Chip in bone and finger splinted.

Employee was moving railcars and the trackmobile jumped the tracks causing the employee to be thrown around in the cab of the trackmobile. Employee experienced pain that did not get better. Had MRI on 10-30-14 that revealed a Lumbar compression fracture.

Employee was welding in close quarters with 3 other coworkers. Weld flash to eyes.

Employee was removing wet dust from a dribble chute. When employee went to push the flap gate shut the employees his left thumb was caught in between the pipe and the flap gate. X-ray revealed a fracture.

Employee was helping co-worker remove a metal seal from a 55 gallon drum. When employee pried the metal seal apart the seal popped up and cut his finger. Employee received a laceration to his left pinky finger that required 3 stitches.

The employee was preparing to drill holes in the Kiln 1 cooler pipes. Employee went to get additional PPE and when he was getting back into the cooler, employee realized he had something in his right eye. Employee doesn't know exactly how particle got in his eye. Right eye abrasion.

The lime transferring system shutdown, plant personnel tried to reroute the lime. The hot lime sitting on the conveyor caused the belt to start on fire. Plant personnel tried to extinquish fire and call the fire department at 18:06. Plant management was contacted at 18:53, came to the plant to assess the situation and contact MSHA at approximately 19:00. Fire was out around 21:00.

Employee was loading a bulk truck. The employee started winding up the loading boot when dust particles entered his eye. Employee was wearing safety glasses.

Employee was carrying a portable propane tank when he slipped and fell. Employee has a cut on his right index finger and broke his right leg.

A small fire had started on the night shift but was quickly extinguished with a CO2 fire extinquisher. The burnt gear lubricant reignited and could not be extinquished immediately.

Employee was using a wheelbarrow to clean up in the Modeco building. When the employee went to dump the wheelbarrow full of lime into the skidsteer, he slipped and fell. The wheelbarrow tipped over and spilled lime onto him. Employee had lime burn to right eye.

Employee was helping take the coal mill apart during a shutdown. While taking it apart, the bottom gear housing became wedged. The employee tried to unwedge the gear housing and strained his lower left back.

Employee was emptying the lime out of the bucket of a small front-end loader. While the employee was driving the small front-end lader, lime dust blew back into hif ace. He suffered a lime burn to his right eye.

While accompanied by experienced miner, the injured employee was shutting off the water control valve to shutdown the hydrator process. A pipe thread failure on the 3 way valve released lime slurry onto upper body and face.

Very windy outside and lime dust was blown into right eye.

Employee was cleaning slurry pump; strained muscle in back.

Employee was bagging product and got dust in eye.

Employee was driving a forklift with a load of pallets on it. Employee misjudged being past the corner and when he turned to the left, employee hit cement pillar causing him to bruise his abdomen on the steering wheel.

Employee was loading boxcar and while cutting some cardboard he cut left middle finger. Received 2 stitches.

Miner was installing new roofing. Miner was adjusting the roof sheet when a piece of metal from drilling entered right eye.

Employee was bagging pure cal lime; while attempting to put a bag on spout #3, the spout "tripped" before bag was on securely, causing lime to go into his eye.

Employee was raking ash on the #1 kiln floor when he strained his groin muscle.

Employee was blowing lime dust off himself with a blower hose. Something flew into his eye. Employee did seek medical attention; has a corneal abrasion. Rec'd a prescription for his right eye.

Employee was cleaning out hydrate trough; steam from the hydrating lime went into his face and under his safety glasses.

Employee was walking past the firing floor cooling fan which was "cooling" and therefore blowing air/lime dust. Some dust got into employee's left eye. Employee sought medical attention at 23:40 and was discharged at 12:25 a.m. on the 21st of June.

Employee was attempting to clear a bagger spout with a metal rod. While checking to see if spout was open, hydrated lime shot out of spout. The lime deflected off the cross member of the seat causing some lime to get into employee's left eye. Employee flushed eye out but after 45 minutes it still was bothering him. E'ee went to emergency room.

Employee was fabricating a roof for the waste oil tank containment. While using the wire wheel, a piece of metal went into his left eye.

Employee was carrying 5 gallon pail of waste/dust on 9/7/05. Employee slipped on oil on the steps while going down. He felt a slight pain in lower back. On 9.9.05, he re-injured back while lifting/reaching forward to position mixer box in place.

Employee was walking towards the stone screener. While walking, he stepped on a stone and twisted his left knee. ** Employee began missing 2/3/2006 for surgery. He is back but still on restricted duty.**

Employee was sweeping the floors in kiln #2 basement. As he was sweeping, dust got into his eye. He was wearing safety glasses at time of accident. He did flush eyes out immediately, but as the night progressed, his right eye became more irritated.

EE WAS USING A UTILITY KNIFE TO CUT THROUGH AN AIR HOSE ON A RAILCAR. HE WAS PULLING THE KNIFE TOWARDS HIM; THE KNIFE SLIPPED OFF THE HOSE & STRUCK HIS LEFT INDEX FINGER. EE WAS WEARING GLOVES BUT THE KNIFE CUT THROUGH THE GLOVE & CUT HIS FINGER. EE DID SEEK MEDICAL ATTENTION; "SUPER GLUE" WAS USED TO SEAL THE WOUND.

Employee was setting up the bulk bagger on a bin with a co-worker who was above lowering a power cord to be plugged in. The cord hit the employee's hard hat; a chunk of dust slid off his had and into his right eye. He flushed out his eye but it was still irritated so he sought medical attention.

Employee was helping with the slurry pump maintenance in the hydrate plant. Employee felt irritation in his left eye so he flushed it out at the plant. At the end of the day it was still bothering him so he went to the emergency room.

EE was cleaning out lime build up under a tail pulley so he could unload railcars. EE was kneeling in water and wet lime for approximately two hours. He mentioned to his supervisor that his knees were sore. On 2/11 he told his supervisor he was going to the doctor for sore and raw knees.

Employee was lifting an overhead hoist onto a beam. The weight of the hoist caused a shooting pain in his lower left and upper shoulder area of his back.

EMPLOYEE WAS ATTEMPTING TO REMOVE LIME BUILDUP FROM THE TUBE MILL WITH AN AIR CHISEL. HE WAS ON HIS STOMACH WHEN THE CHISEL BROKE THRU THE HARDENED LIME. THIS CAUSED A CLOUD OF STEAMY LIME TO GO ONTO HIS FACE AND ENTER BOTH EYES. HE RINSED HIS EYES WITH WATER AND EYE SOLUTION BUT COULD NOT REMOVE ALL THE LIME PARTICLES.

Employee was emptying the pebble lime bin; he opened the actuator valve and lime came spraying out. Lime entered his right eye. He rinsed out his eye for 15 minutes at the plant and eye drops were applied also. He did seek medical attention because there were perticles still in.

WHILE EMPLOYEE WAS BAGGING PURE CAL LIME, TWO BAGS BLEW OFF THE SPOUT CAUSING LIME TO BLOW INTO EMPLOYEE'S FACE. HE WAS WEARING SAFETY GLASSES; HOWEVER, LIME DID GET INTO HIS LEFT EYE. EMPLOYEE FLUSHED OUT EYES AT THE PLANT SITE. HIS LEFT EYE CONTINUED TO BE RED AND IRRITATED SO HE DID SEEK MEDICAL ATTENTION.

EMPLOYEE WAS HELPING TO INSTALL NEW SCREENS ON THE S131 SCREENER. HE WAS USING TWO PRYBARS TO LEFT THE SCREENER DECK HOUSING. HIS FOOT SLIPPED CAUSING HIM TO LOSE CONTROL OF THE WEIGHT. HE ALLEDGEDLY FELT A SHARP PAIN IN HIS LOWER BACK.

Ee & coworker were lifting a 200#, 20' long ash rake up west side of rotary. As it was being placed on rotary floor, ee felt his back go out. This caused him to release his grip on ash rake. He sat down: when he tried to stand up, he fainted, fell & hit his nose & forehead on coal mill exhauster fan base. He cut his nose & bruised his forehead. He was taken by ambulance

While ee & coworker were lifting a 200#, 20' long ash rake up to the rotary floor, coworker's back went out causing him to release his grip on rake. This caused the ash rake handle to strike ee's hand: This action caused his middle finger to be pinned between handle & window ledge. He applied 1st aid @ the plant but later was seen at St Agnes ER. His middle rt finger is broken.

EE WAS REMOVING ASH RING FROM #2 KILN WITH A CHISEL. A PIECE OF ASH WENT INTO HIS LEFT EYE. EE DID RISNSE HIS EYE. AFTER A FEW DAYS, HE NOTICED THE EYE WAS STILL RED AND SORE. ON 5-14-04 HE DECIDED TO GET MEDICAL ATTENTION. HE RECEIVED THREE PRESCRIPTIONS.

When he was unloading lime, the line became blocked. While dislodging the blockage, he opened the valve hatch without totally depressurizing the tank causing lime to blow back into his face. Lime dust entered both eyes and onto his forehead. His eyes were flushed at the plant but were still irritated. He sought medical attention; received a rx for vicidon and anitbiotic.

Ee was bagging pure cal lime. The bag broke & lime flew onto ee's face. Lime dust got under safety glasses & into his right eye. Eye was flushed out at plant site but eye was still irritated. Ee did seek medical attention. His eye was flushed out again & he was given a prescription for medicated eye drops.

WHILE ATTEMPTING TO INSERT SHAFT UNDERNEATH THE HYDRATE MIXER BOX, LIME DUST FELL INTO EE'S RIGHT EYE. EE DID RINSE HIS EYE WITH WATER AND AN EYE SOLUTION, BUT COULD NOT REMOVE DUST PARTICLES. HE DID SEEK MEDICAL ATTENTION. EE ALSO RECEIVED A PRESCRIPTION FOR ACULAR MEDICATED EYE DROPS.

EE WAS IN THE PROCESS OF ATTACHING THE BOOM TO THE LOADER BUCKET. HIS GLOVED LEFT HAND, LITTLE FINGER BECAME CAUGHT BETWEEN THE BOOM & THE BUCKET. THE LITTLE FINGER HAS A LACERATION. EE DID SEEK MEDICAL ATTN.

EMPLOYEE WAS ATTEMPTING TO OPEN THE SLIDE PLATE ON THE GROUND LIME SCREW CONVEYOR. HE WAS POUNDING ON THE PLATE WITH A HAMMER WHEN IT CAME LOOSE CAUSING A BURST OF LIME DUST. LIME DUST GOT INT O BOTH EYES. EMPLOYEE IMMEDIATELY FLUSHED HIS EYES. HE DID SEEK MEDICAL ATTENTION.

EE WAS DRIVING THE LOADER ON 12-22-03 FOR EIGHT HOURS EACH DAY. HE FELT A SHARP PAIN ON HIS RIGHT SIDE UNDER HIS RIB CAGE AND DOWN HIS FRONT RIGHT LEG. EE WENT TO THE EMERGENCY ROOM ON 12-23-0 3.

WHILE DESCENDING THE STAIRS IN THE HYDRAT PLANT. EE S FOOT SLIPPED ON THE BOTTOM EE THEN FELL FORWARD ONTO THE HYDRATE FLOOR. HE DID NOT FEEL ANY PAIN AT THAT TIME. ON 12-9-03 HE DID FELL A DU LL PAIN IN THE MIDDLE OF HIS BACK. THE PAIN IS STILL THERE AS OF TODAY 12-11-03.

EE WAS ATTEMPTING TO BREAK THE MERRICK AIRLOCK WITH THE CHAIN STILL ON. HE WAS USING A PIPE WRENCH, HIS RIGHT HAND SLIPPED OFF THE PIPE WRENCH. THIS ACTION CAUSED HIM TO SMASH HIS RIGHT MIDDLE FINGER. HE RECEIVED A SMALL CUT BELOW THE NAIL AND MASSIVE BRUISING OF THE NAIL.

EE WAS TRYING TO REALEASE GUARD OFF THE MERRICK CHAIN. WHEN HE PULLED ON THE GUARD, THE LIME THAT HAD ACCUMULATED ON THE COVER CAME OFF, CAUSING LIME DUST TO ENTER EE LEFT EYE.HE FLUSHED OUT H IS EYE BUT AFTER A FEW HOURS HIS EYE BECAME MOREIRRITATED. EE DID NOT SEEK MEDICAL ATTENTION.

EMPLOYEE WAS DUMPING HOT ASH THAT WAS IN HOPPER USING FORKLIFT. HOPPER WAS LOW TO THE GROUND. ASH FLEW BACK AND BURNED RIGHT FOREARM. HE RINSED HIS ARM WITH COOL WATER. HE WAS TAKEN TO CLINIC AND TREATED WITH FIRST AID CREAM, HYDROCODINE AND ETODOLAC TABLETS.

EE WAS DUMPING A HOLDING TUB FULL OF HOT PEPPLE LIME WITH A FORKLIFT. AS IT STARTED TO DUMP, A CLOUD OF HOT LIME DUST FLEW BACK TOWARD EE. THE HOT LIME DUST WENT ON HIS LEFT FACE AREA, LEFT ANKLE AND BOTH KNEES. EE DID RECEIVE MEDICAL ATTENTION: HE RECEIVED TYLENOL W/CODEINE AND MEDICATED BURN CREAM.

EE WAS DESCENDING FROM A FIXED LADDER. HIS LEFT HAND SLIPPED: EE FELL APPROX. 4 FEET TO A CONCRETE FLOOR. EE THOUGHT HE HEARD A "CRACK" AND FELT PAIN IN RIGHT ANKLE. TWO FELLOW EMPLOYEES STABI LIZED ANKLE: ICE PACKS PUT ON AND EE TAKEN TO EMERGENCY ROOM.

EE WAS LIFTING A 55 GALLON OIL DRUM (FULL) TO AN UPRIGHT POSITION. THE OIL DRUM STARTED TO TIP OVER, SO THE EE PUSHED ON THE DRUM TO SET IT STRAIGHT. AS HE DID THIS, HE NOTICED A TIGHTNESS IN THE LEFT SIDE OF HIS GROIN AREA. EE WENT TO A DOCTOR ON 5-28-03.

EMPLOYEE WAS RE-INSTALLING SIDE GUARDS ON A LIME CONVEYOR (C-157). WHILE DOING THIS, SOME LIME DUST GOT BEHIND EMPLOYEE'S SAFETY GLASSES. EMPLOYEE DID SEEK MEDICAL ATTENTION.

EE WAS BAGGING 10# LAWN & GARDEN LIME. WHILE THE BAG WAS FILLING, THE BAG EXPLODED CAUSING LIME TO SPRAY ONTO EE'S FACE. EE WAS WEARING PROTECTIBE EYE COVERING, BUT LIME STILL ENTERED RT. EYE.

IT IS ALLEGED WHEN EMPLOYEE WAS WEIGHING 70# MORTAR BAGS HE WAS LIFTING THE 70# BAG OF MORTAR AND FELT A SHARP PINCHING PAIN IN HIS LOWER LEFT SIDE BACK AREA. HE HAS PAIN WHEN HE TURNS OR LIFT S HIS LEGS; ESPECIALLY LIFTING HIS LEFT LEG.

IT IS ALLEDGED WHEN EE WAS CLIMBING A LADDER TO FILL A DUST TRUCK, HIS RIGHT KNEE MADE A LOUD POPPING NOISE. APPROX. 1/2 HOUR LATER HIS KNEE WAS SWOLLEN. EE COMMENTED HE FELT LIKE LIQUID WAS G ATHERING AROUND THE KNEE CAP. EE SOUGHT MEDICAL ATTENTION: HAS APPT TO SEE ORTHOPEDIC DR ALSO.

EE WAS STEPPING DOWN FROM THE PAYLOADER. HE FELT A PULL IN HIS LEFT LEG.

EE WAS OBSERVING ANOTHER EE PERFORMING MAINTENANCE ON THE NORTH BAGGER SPOUT. THE OTHER EE ACTIVATED THE SPOUT WHICH RELEASED MORTAR. THIS MORTAR SPRAYED ON THE BAGGER FRAME & INTO EE'S RIGHT EYE. EE DID SEEK MEDICAL ATTENTION.

EE WAS USING A UTILITY KNIFE WHILE UNTYING BULK BAGS. WHILE CUTTING THE INSTIDE STRAP, HE CUT HIS LEFT INDEX FINGER. THE CUT REQUIRED STITCHES.

IT IS ALLEGED WHEN EE WAS GETTING OFF THE TRACKMOBILE, HE SLIPPED AND FELL AGAINST THE TRACKMOBILE. HIS RIGHT SHOULDER AND RIGHT ELBOW ARE STIFF AND SORE. EE HAS GONE TO DR ON 8-15-02, HE IS O N RESTRICTIONS WITH A POSSIBLE RORTATOR CUFF INJURY.

WHEN EMPLOYEE WAS EMPTYING THE #1 DUST BIN, HE FELT PAIN IN HIS LOWER MIDDLE BACK WHEN HE PULLED ON THE BIN. HE SOUGHT MEDICAL ATTENTION.

EE WAS USING A SLEDGE HAMMER TO BREAK UP A PIECE OF KILN ASH. WHEN THE HAMMER FACE CONTACTED WITH THE PIECE OF ASH, THE EE FELT SOMETHING POP IN HIS LEFT WRIST AND FELT PAIN.

EE WAS USING MIRACLE BAGGER TO FILL 50# BAG WHENBAG BLEW OFF SPOUT. LIME ENTERED HIS LEFT EYE.

EMPLOYEE WAS SHOVELING LIME FROM THE HYDRATE ELEVATOR PIT. WHILE SHOVELING, EMPLOYEE FELT A PAIN IN THE LOWER MIDDLE BACK.

AN EMPLOYEE WENT TO TELL VICTIM SOME INFORMATIONON A RAILROAD CAR; WHILE HE WAS SPEAKING, THE VICTIM TOLD HIM HE WAS HAVING CHEST PAINS AND THATHIS RIGHT ARM WAS GOING NUMB. VICTIM WAS IN THE HYDRATE DEPARTMENT WATCHING THE SLURRY PUMP AT THIS TIME. THE COMPANY IS NOT ANTICIPATING THIS TO BE A WORKER'S COMP CLAIM.

IT IS ALLEGED WHEN THE EMPLOYEE WAS WELDING ON THE U-TUBES, HE FELT SOMETHING ENTER HIS LEFT EYEIT WAS VERY WINDY AND WHILE WELDING, PARTICLES FROM THE U-TUBE WERE BLOWING BAKC INTO EMPLOYEE'S FACE. HIS LEFT EYE DID NOT BECOME IRRITATED UNTIL APPROX. 2:30PM AND BECAME WORSE AS THE NIGHT PROGRESSED. ON 9-13-01 HE COULD NOT OPEN HIS EYE

EMPLOYEE WAS TIGHTENING A CHAIN COME-ALONG TO REMOVE A COOLER PIPE FROM KILN #2 COOLER. THE COOLER PIPE CLAMP JAW BROKE. THE CLAMP FLEW BACK AND STRUCK EMPLOYEE IN THE LEFT SIDE OF HIS ABDOMEN . EMPLOYEE WENT TO THE EMERGENCY ROOM. CAT SCAN PERFORMED AND MEDICATION PRESCRIBED.

EE WAS ATTEMPTING TO STOP A BAG OF MORTAR FROM GETTING CAUGHT IN THE APRON OF THE MORTAR PLANT BAGGER. HE USED HIS RIGHT FOOT TO ROLL THE BAG SO IT WOULD NOT GET CAUGHT. IN DOING SO HIS RIGHT FOOT BECAME CAUGHT BETWEEN THE ANGLE IRON BY THEBAGGER SEAT AND THE BAG OF MORTAR.

IT IS ALLEGED EE INJURED HIS MIDDLE/SIDE OF HIS BACK WHILE SHOVELING MUD/DIRT IN THE RECLAIM AREA. WHILE SHOVELING, HE STATES HE FELT A SHARP PA9IN IN THE MIDDLE AND SIDE AREA OF HIS BACK. HE IMMEDIATELY QUIT SHOVELING AND WENT TO SEEK MEDICAL ATTENTION. MEDICAL CENTER DR. PRESCRIBED NAPROFYN FOR INFLAMMATION AND VICODIN FOR PAIN.

IT IS ALLEGED WHILE EMPLOYEE WAS LAYING BRICK IN KILN #2, HE HURT HIS BACK. PAIN GOT PROGRESSIVELY WORSE AS DAY WENT ON. HE DID NOT SAY ANYTHING TO THE OTHER WORKERS UNTIL LATER IN THE DAY. DI D NOT WORK 4-4-01.

EE WAS INSPECTING THE TRUCK HE WAS DOING MAINENANCE ON WHEN HE TRIPPED OVER A PALLET LYING ON THE GROUND. HE FELL ONTO A CONCRETE SLAB. HE TRIEDTO CATCH HIS FALL BUT HIT HIS RIGHT FINGER ON A PIECE OF IRON ON THE CONCRETE SLAB. HIS FINGER BECAME SWOLLEN. HE DID NOT SEEK MEDICAL ATTENTION; FINGER IS FRACTURED.

WHEN EMPLOYEE WAS MOVING A FULL 55 GALLON DRUM OF OIL WITH THE HAND CART, HE FELT A SLIGHT PAININ HIS BACK WHEN HE TIPPED THE BARRELS DOWN.

WHILE EMPLOYEE WAS WORKING IN THE KILN PULLING AND LIFTING PAILS, HIS LEFT SHOULDER AREA BEGAN TO HAVE SHARP PAINS. EMPLOYEE WENT TO A DOCTOR ON 11-3-00, RECEIVED A PRESCRIPTION, HAD X-RAYS DONE. HE HAS SCHEDULED AN APPOINTMENT WITH AN ORTHOPEDIC CONSULTANT ON 11-14-00.

EMPLOYEE WAS INSTALLING A CATWALK UNDERNEATH THE TRANSITION OF THE ELEVATOR. A GUST OF WIND BLEW AND LIME ENTERED BOTH OF THE EMPLOYEE'S EYES. HE FLUSHED OUT HIS EYES, HOWEVER, THE LIME WAS ST ILL LODGED IN. EMPLOYEE IS SEEKING MEDICAL ATTENTION.

WHILE EMPLOYEES WSERE USING A 100 TON POEWR PACK TO PUSH THE RAM PINS OUT OF THE CLEVIS. WHILE DOING THIS, THE SOLID SAFT USED TO PUSH THE PIN OUT, THE SHAFT POPPED OUT OF PLACE ADN HIT EE IN THE UPPER RIGHT CHEST AREA AND LOWER RIGHT SHOULDER AREA. HIS CHEST AND SHOULDER ARE RED AND SCRATCHED.

EMPLOYEE WAS WORKING IN KILN #2 CLEANING OUT STONE AND ASH. HE WA SPULLING A RAKE THROUGHOUT THE DAY. HE FELT OKAY AT 2:30 PM WHEN HIS SHIFT ENDED. FRIDAY MORNING WHEN HE AWOKE, HIS RIGHT SIDE , LOWER BACK MID SECTION AND RIGHT SHOULDER WEREVERY SORE AND STIFF.

EE WAS PULLING A BRACKET ACROSS THE MODCO ROOM FLOOR AND FELT A PAIN IN HIS RIGHT SHOULDER AND BACK. EE DID SEEK MEDICAL ATTENTION.

WHILE TWO EES WERE TAKING OFF THE COVER OF THE TUBEMILL, EE COMPLETED TAKING THE COVER OFF WHEN THE PRESSURE FROM INSIDE THE TUBEMILL BLEW LIME DUST INTO EE'S LEFT EYE. EE FLUSHED OUT HIS EYE THROUGHLY AND APPLIED HIS OWN EYE DROPS. HE DID NOT SEEK MEDICAL ATTENTION.

EMPLOYEE WAS HOLDING A ROD WHILE OTHER EMPLOYEESWERE WELDING ON THE ROD. INJURED WAS 10-15 FT AWAY FROM THE POINT OF WELDING, HOWEVER, HE SUSTAINED FLASHBURN IN HIS RIGHT EYE. EMPLOYEE DID SEE K MEDICAL ATTENTION.

EMPLOYEE IS EXPERIENCING PAIN & A BURNING SENSA-TION WHILE WORKING IN THE MORTAR PLANT; THE PAINIS IN HIS LEFT FOREARM AND ALSO SLIGHTLY IN HIS RIGHT FOREARM; EMPLOYEE STATES HE HAS DIFFICULTY HOLDING THINGS; SEEMS HE IS DROPPING THEM FRE- QUENTLY. EXPERIENCES PAIN WHILE PALLETIZING. STATES THIS IS STILL FROM ACCIDENT REPORTED ON 4/25/97.

WHILE AN EMPLOYEE AND THE INJURED WERE LOOSENING UP THE BOTTOM PLATE ON THE MIXER BOX WITH THE VISE GRIPS, INJURED GOT THE PLATE LOOSE AND THE PRESSURE RELEASED OUT AND THE VISE GRIPS FLEW BAC K AND HIT INJURED IN THE LEFT HAND PALM AREA BY THE THUMB, DEEPLY CUTTING HIS LEFT PALM AREA BELOW THE THUMB, STTITCHES WERE GIVEN.

WHILE EMPLOYEE WAS USING A TIN SNIPS TO CUT THE METAL BANDING ON THE BOXCAR, THE METAL BAND AFTER BEING CUT SNAPPED UP AND CAUGHT INJURED IN THE RIGHT HAND ON TOP OF THE THUMB AREA CAUSING A D DEP CUT. HE REQUIRED 6 STITCHES TO THIS AREA.

EMPLOYEE WA HELPING CLEAN OUT THE GRADING ON THEDOUBLE BELL DECK. WHILE POUNDING THE ROCK THROUGH THE TRADING, A STONE CAME UP. THIS STONE STRUCK THE EMPLOYEE'S MOUTH CAUSING IT TO BLEED. HIS FRONT TEETH WERE CHIPPED ALSO.

EE WAS SHOVELING STONE BY THE SCREENER WHEN HE FELT A SHARP PAIN IN HIS LOWER BACK AREA. THE PAIN WAS ALSO SHOOTING DOWN HIS LEFT LEG. HE CONTINUED WORKING AND THE PAIN GOT PROGRESSIVELY WORSE .

EE WAS TRYING TO PICK UP THE HITCH FOR THE WATERWAGON AND THE HITCH WAS FROZEN TO THE GROUND AND DID NOT MOVE. WHEN EE WAS PULLING ON IT, HE IMMEDIATELY FELT A SHARP PAIN IN HIS RIGHT GROIN AR EA. HE WENT TO A DR AND THE DR STATED THAT IT WAS A HERNIA AND HE IS SEEKING MORE MEDICAL ATTENTION ON 2/16/00.

WHEN EE WAS DONE LOADING A PEBBLE TRUCK, HE WAS WALKING TO THE MIRACLE PLANT AND SLIPPED ON THE ICE, RESULTING IN PULLING A GROIN MUSCLE.

WHEN LIFTING, PULLING, ETC EE IS EXPERIENCING A BURNING SENSATION, TIGHTNESS AND SORE TO TOUCH IN HIS LEFT ELBOW AREA. ON OCCASION HE DOES EXPERIENCE THIS SAME SENSATION IN HIS RIGHT ELBOW ARE A. THIS REPORT OF THIS TYPE OF INJURY HAS ALSO BEEN SUBMITTED ON REPORT IN 4/25/97.

WHEN EE WAS WORKING IN THE KILN ON 1/24/00 AND 1/25/00, HE WENT TO THROW BRICKS THROUGH THE KILN ON THE MORNING OF 1/25/00 WHEN HE FELT A SHARP BURNING PAIN/SENSATION IN HIS RIGHT AND LEFT SHO ULDER AREA. THE PAIN AND BURNING SENSATION IS STILL PRESENT AND INTENSE AS IT WAS AT FIRST. HE IS SEEKING MEDICAL ATTENTION ON 1/28/00.

WHILE EE WAS JACK HAMMERING IN THE KILN ON THE ANCHORS, A PIECE OF ANCHOR FLEW UP AND HIT HIM ON THE RIGHT HAND SIDE OF HIS JAW AREA CAUSING A CUT THE SIZE OF 3/4". WENT TO SEE DR ON 1/25/00 A T 2:45 PM.
