Rare occurrence of a stone fragment chipped off and hitting employee in the forearm causing a cut that needed stitches

Employee was chipping rocks with hammer and chisel and a piece of metal fragmented off of chisel and entered employee's right thigh and became embedded there

Miner was attempting to re-attach a blower guard in the loader. EE's finger got too close to the blower fan and was cut by the fan blade.

Miner developed trigger thumb of right hand from doing repetitive motion tasks over 20 year career repairing mining equipment.

Miner has worked entire career at this mine repairing heavy and light equipment. The repetitive motion after years of using tools to repair mining equipment has caused carpal tunnel symptoms in right and left wrists and hands.

Miner was climbing down the ladder on a 35 ton haul truck. The ladder was broken and EE's foot slipped through the ladder while the ladder flipped up into the wheel well. EE fell backwards onto the ground 3 feet below. EE landed on EE's back with EE's leg stuck in the ladder. EE did not submit EE's injury report until 02/08/2022.

Employee was leaving for the day when they slipped on ice in the parking lot and twisted a knee.

Miner was picking up a piece of stone from the conveyor belt of a stone splitter. When EE grabbed the stone, the belt stopped and EE twisted right wrist.

Employee was lifting 12" cut stone and felt a pull in lower right abdominal area. The pain would not subside so employee was put on a smaller thin line machine.

Miner was using a hammer and chisel to break a piece of stone when a piece of steel from the chisel flew off and embedded in EE's leg.

Miner was carrying a piece of flag stone when the stone slipped out of left hand and fell on the tip of the middle finger of right hand.

Miner was stacking a piece of stone on a pallet when a finger was pinched between two pieces of stone. Miner received a crushing injury to the tip of the little finger and the finger nail bed.

Miner was filling the propane tank on EE's forklift from a bulk propane tank. As EE disconnected the filling hose, the pin in either the filling nozzle or the forklift propane tank stuck open allowing propane to escape under pressure. The propane hit the back of the miner's hands and caused freeze burns resulting in skin loss on the back of the right and left hands.

Miner was stacking stone on a pallet. When EE bent over, EE felt pain in lower back.

Miner was using a hammer and chisel to split a piece of stone when miner struck left hand with the hammer. Miner bruised left hand in the thumb area.

Miner was facing stone with a hammer and chisel when EE felt pain in the groin area. EE was diagnosed with a hernia.

The miner was lifting and stacking stone when EE felt a pain in lower back.

Employee was stacking stone on a pallet when employee experienced pain in the lower back and groin. Employee had been feeling symptoms for two to three weeks prior to reporting injury but the pain became more intense so employee reported it.

Injured miner was changing the outer rear tire of a semi-trailer when another miner lowered the floor jack. The trailer came down and pinched the injured miner's hand between the tire and the trailer.

Miner was trimming a piece of flag stone when the stone tipped and fell pinning the miner's finger between the stone and a pallet.

Miner bent over to pick up a piece of flag stone and strained back.

Miner was sizing a piece of stone using a hammer and chisel. EE swung the hammer very hard and lost control hitting the chisel with a glancing blow. EE's hand smashed into the stone cutting the right index finger and causing a fracture of the right index finger.

Miner was late for shift. Had just punched in and was walking to the job site and slipped on the wet pavement and fell. Miner broke fall with right hand and injured hand.

Miner was stacking stone coming out of the hydraulic splitter. As employee was picking up a piece of stone from the conveyor belt, the belt reversed and pinched a finger between the stone and part of the machine.

Miner was splitting a stone with hammer and chisel and struck finger with the hammer.

The employee, a loader operator, was trying to dislodge a stone from the tire of loader with own pocket knife. The pocket knife collapsed and cut the tip of little finger on right hand.

While working on the parking brake of a pickup truck, the employee started to feel pain in the left elbow. The employee has been working as a mechanic at the mine for 23 years.

Miner was using a hammer and chisel to split a stone in extremely cold temperatures. A small shard of steel fragmented from the chisel and lodged in right leg.

Employee was carrying a small piece of stone and slipped on the ice. EE did not fall but in struggling to keep balance, ee strained groin and abdominal muscles resulting in a hernia requiring surgery.

Miner was splitting a piece of stone with a hammer and chisel when he experienced pain below his right shoulder blade. He is right handed and was hammering with his right hand.

A worker was removing cut stone from a cutter when he lost his concentration on the task. He lost his grip on a large piece of stone which then fell on his right hand causing a fracture of his thumb.

Employee finished his shift, went home, had dinner, spoke with his family, went to bed. Woke up with back pain sometime on 08/01/2014 and went to the emergency room. Was treated and released to return to work with restrictions.

Worker was splitting a stone using a hammer and chisel when the stone fractured trapping his gloved hand and the #4 finger of his right hand causing a open fracture of the finger.

Worker was operating a loader removing stone from a ledge. He felt his back tighten and soon it became painful. This injury was NOT reported to the company. The worker went to the doctor and was put on restricted duty.

Worker was splitting a stone using a hammer and chisel when a small piece of the chisel broke off and struck him on the inside of his left leg just under his knee. First aid was given for a small puncture wound. Worker was sent to the Dr. on 09/30/2013 and put on restricted duty.

Worker was lifting and carrying a piece of stone to be stacked on a pallet when he lost his balance and fell on his right side causing a strain. No lost time, reassigned to restricted duty.

Worker was splitting stone using a hammer and chisel when he felt a pain in his Left wrist. Reported the injury and was treated with ice. 06/11/2013 reported for work and requested to see a Dr. went to clinic. (sprained wrist) restricted, NO LOST TIME!

Worker was lifting and stacking stone when he felt a pain in his lower back. He continued to work for the rest of the day (04/15/2013) reporting his injury the following morning 04/16/2013. Went to clinic-lumbar strain-restricted duty and meds.

Worker was lifting/carrying a piece of stone when he felt a pull/pain in his right hip traveling down to his leg/foot. Diagnosed with a right SI sprain.

Worker was taking stone from a conveyor belt to be stacked on a pallet. When he removed a piece of stone another piece tipped and hit the index finger on his right hand pinching the finger tip between the stones.

Worker was splitting a stone a when a small chip of stone flew up and struck the left side of his forehead causing a small cut. First aid was given and a bandage was applied. On 01/29/2012 the worker went to the Dr. claiming vertigo, headaches and blurred vision. He was given meds and restrictions. This incident is under investigation for possible workers compensation fraud.

Worker was bending over to pick up a piece of plastic wrap and felt a pain in his back. Reported the injury, went to the Dr. and was given restrictions and pain meds.

Employee has worked for the company for 26 years. He has operated a loader using a joy stick to operate forks and bucket. Employee was diagnosed with carpal tunnel syndrome on 8/1/2011. His surgery was on 8/12/2011. He was off the day of the surgery. He returned to work on 8/15/2011 with meds but no restrictions. On 8/19/2011 he was seen by his Dr. and put on restrictions.

Worker was climbing in to a loader when his foot slipped, he caught himself with his right arm injuring his shoulder.

Worker was trimming vines and weeds using a hand trimmer when he cut the tip of #4 finger of his left hand.

Worker was replacing the carrier bearing on a semi. tractors drive shaft. He removed but did not secure the drive shaft per. policy. The drive shaft slipped striking him on the top of his head causing a laceration.

Worker was lifting a piece of stone 2'X 4" when he lost his footing and slipped. He twisted to regain his balance and injured his R wrist.

Worker was walking and stepped on a small piece of stone causing him to twist on his right knee causing a sprain to the knee.

Worker was stacking stone. Felt a pull/pain on his left side upper rib cage. Went to Dr. = rib strain. Meds & restricted duty. No lost time.

Worker was splitting stone using a hammer and chisel. A small piece of steel broke from the chisel and lodged in his left leg just below the knee.

Worker was bent over splitting stone using a hammer and chisel. He felt a pain in his lower back and left leg. Injury was reported but the worker did not inform the company that he wanted to see a Dr. until 10/01/2008. Worker has a lumbar strain. No restrictions, no lost time. Meds. only.

Worker was stacking stone; the stone slipped and fell on his left hand causing fractures to the tips of fingers #2 & 3.

Worker was moving a stone to be cut. Stone slipped and pinched his left thumb between this stone and another, causing a fracture to the thumb.

Building on mine property was over-exposed to carbon monoxide gas from an improperly tuned forklift. Worker was taken for medical treatment. No work time was lost.

Worker had been working on a saw, he was reasigned to catch stone from a conveyor and stack the stone on pallets. He asked to go back to the saw and was told NO. Soon after his back started to ache, he could not remember an injury. Dr diagnosis: low back pain.

Worker stated that he was lifting a large stone from a conveyor to a pallet and felt a strain in his back. He did not report the injury until 10/26/2007. He felt that the injury would heal itself.

Worker was lifting a piece of stone he felt a pull in his left groin area resulting in a groin strain.

Worker was using a hammer and chisel to dress stone when a small piece of steel broke from the hammer and entered his right thumb. Dr removed steel and stitched wound.

Worker was wrapping a pallet of stone with shrink wrap. He pulled hard on the wrap, causing the roll to slip from his hands. The roll struck him in the face and hit his safety glasses. Safety glasses struck right cheek, causing a laceration below the right eye, requiring 4 stiches to close.

Worker was removing stone from a table; stone broke in half, worker tried to catch the stone causing a strain to his right shoulder. Worker was placed on light duty. On 07/30/2007 worker requested to seek medical attention.

Worker was moving a piece of stone; he tripped and fell feeling pain in his leg and lower back. He did not report his injury to the company until 07/30/2007. He was placed on company-ordered light duty. His pain continued; on 08/20/2007 he was sent to the clnic diagnosed with lumbar strain. meds and therapy. restricted duty.

Employee was moving a piece of stone when he pinched his left hand between the stone he was moving an another piece of stone. He treated the injury with ice for one week then went to the clinic. Employee has a contusion to the hand; doctor perscribed meds, to reduce swelling. No restricted, no lost time.

Worker was lifting stone from a conveyor belt to be stacked on a pallet, he felt a sharp pain in his side, which caused an existing hernia to become larger. Worker will be having surgery to repair this hernia on 07/05/2007

Worker was moving large pieces of stone, when one fell, hitting the little finger of his right hand, causing a closed fracture to the finger.

Employee reported to me that he has been having pain in his right shoulder for some time; he feels that it is a work-related repetitive motion injury.

EE WAS CATCHING A STONE AND A CHIP OF THE STONE BROKE OFF AND WAS STUCK INTO HIS EYE. (UNDERNEATH HIS SAFETY GLASSES).

Employee was catching. When he grabbed another pallet, it slipped out of his hand and it scraped down the outside of his right ankle.

EE WAS CATCHING STONES AND STACKING THEM ONTO PALLETS. HE STEPPED ON A PIECE OF STONE AND TWISTED HIS ANKLE. DR DIAGNOSED A LATERAL ANKLE SPRAIN TO THE RIGHT LEG.

WAS MOVING STONES ONTO PALLETS, AND HE GOT HIS FINGER CAUGHT BETWEEN TWO STONES. 4TH FINGER ON HIS LEFT HAND. THE DR. CALLED IT A CRUSH INJURY\LACERATION.

DANIEL WAS SPLITTING A STONE AND HE HIT HIS LEFT KNUCKLE WITH THE HAMMER. DR DIAGNOSED A CONTUSION/LACERATION TO HAND.

EE WAS SANDBLASTING AND ANOTHER EMPLOYEE WAS MOVING A TUB OF STONE AND HIT HIS IN THE HIP AND SHOULDER. DR DIAGNOSIS STRAIN AND ABRASION.

EE WAS DOING HIS NORMAL WORK ACTIVITIES, AND HE GOT SOMETHING IN HIS EYE & IT WASN'T GETTING ANY BETTER. HE WENT TO URGENT CARE AND HE HAD CORNEAL AND CONJUCTIVAL FOREIGN BODIES REMOVED FROM HIS EYE.

EE stepped on a stone and twisted his knee. it is sore and he just wants to make sure that it is o.k.

EE was stacking a stone onto a pallet and the sharp edge of the rock cut his right forearm.

EE WAS STACKING STONES ONTO A PALLET AND HE PINCHED HIS FINGER BETWEEN TWO STONES CAUSING HIS LEFT RING FINGER TO FRACTURE.

EE was chiseling a piece of stone and a chip flew. A piece of the chisel got lodged into his neck. The dr removed the metal sliver from his neck.

Employee was cutting some steps and half of the stone fell onto the top of his left thigh.

Employee was stacking stones and palletizing them when he dropped a stone onto the top of his left foot. Dr. result was a contusion dorsum left foot.

Alberto was stacking stones and a stone fell on top of another stone and his finger got between them. Right pinky finger. Open Fracture.

He was stacking stones onto a pallet, when he slipped over a stone and felt a crack in his mid-back area. He was diagnosed with a mid back sprain.

HE SAID HIS WRIST JUST STARTED TO GET SORE (STACKING STONE ONTO PALLETS).

WAS CLEANING THE TANK COVERS AND HE SLIPPED. HIS BACK WAS SORE AND HE HAD SOME NUMBNESS/TINGLING IN HIS ARMS.

CAUGHT HIS ELBOW ON THE SHARP EDGE OF STONE. HE NEEDED STITCHES.

SAID HE WAS STACKING STONE ONTO PALLETS AND HE FELT A PULL IN HIS BACK.

HE HURT BACK THROWING A STONE DOWN TO CUT.

EE WAS BREAKING A PIECE OF WOOD IN HALF AND A PIECE OF THE WOOD FLEW UP AND CUT HIM UNDER THE LEFT EYE. HE WANTED TO GO AND GET IT CHECKED OUT.

EE CAME TO ME IN THE BEGINNING OF NOVEMBER 2003. HE SAID HE DROPPED A STONE ON HIS FOOT A COUPLE MONTHS AGO. NOW IT IS HURTING AND HE NEEDED TO GET IT CHECKED OUT.

EE WAS PUTTING A STONE ON A GUILLOTINE, AND HE DID NOT LIFT THE STONE HIGH ENOUGH, WHEN HE THEN SMASHED HIS LEFT RING FINGER AND PINKY FINGER BETWEEN THE STONE AND THE MACHINE.

A STONE CHIP BROKE OFF AND CUT HIS LIP.

HE WAS CATCHING A STONE WHEN HE FELT PAIN IN HIS STOMACH AREA. ABDOMINAL WALL STRAIN WAS DIAGNOSED.

DROPPED A STONE & IT FELL ON HIS PINKY FINGER

EE WAS MOVING A STONE---HE DROPPED IT ON HIS FINGER

HE WAS MOVING A STONE TO PUT ON A PALLET AND THE STONE CUT HIS FOREARM.

LACERATION - WAS KNOCKING OFF PALLETS & A STONE EDGE CUT HIS FOREARM. HE RECEIVED 8 STITCHES.

WAS MOVING STONES AND ANOTHER EE DROPPED A STONE. WHILE EE TRIED TO GET HIS FOOT OUT OF THE WAY HE PULLED HIS HAMSTRING.

HE SAID HE WAS MOVING BIG ROCKS AND PUTTING THEM ON PALLETS. INJURY DATE WAS 7/28/03. HE WENT TO THE DOCTOR ON AUG. 6, 2003.

HE WAS MOVING STONE TO A PALLET AND STONE FELL, LANDING ON THE TOP OF HIS LEFT FOOT.

HE WAS ON HIS MACHINE AND IT MALFUNCTIONED...THE BLADE CAME DOWN TOO QUICKLY. WHEN HE GOT HIS HAND AWAY FROM THE BLADE HIS LEFT INDEX FINGER GOT TRAPPED BETWEEN THE STONE AND THE HEAD OF THE M ACHINE. THAT TOOK THE TIP OF HIS FINGER OFF.

WAS LIFTING A HEAVY STEEL BAR WITH 4 OTHER GUYS, AND HE FELT A PULL IN HIS STOMACH AREA.

HE PICKED UP A ROCK AND IT CUT HIS FINGER. HE GOT AN INFECTION IN THE CUT.

STRAINED HIS LOWER BACK WHILE MOVING A STONE.

WENT TO DR ON JUNE 30TH. STONE SCRAPED HIS LEFT ANKLE. HE REPORTED TI TO ME. BUT JUST PUT ON A BANDAGE CLEANED IT UP. ON 6-30-03 HE CAME BACK AND SAID IT WASNT GETTING ANY BETTER. HE WEN TO TH E DR. HE HAD INFECTION.

SAID HE GOT OUT OF LOADER AND FELT A SNAP IN HIS RIGHT KNEE.

WAS KNOCKING DOWN STONE & HE TWISTED WRONG.

HE WAS CHISELING ON A LEANING SLAB OF STONE AND WAS STANDING IN FRONT OF IT (INSTEAD OF THE SIDE OF IT) WHEN IT SLID DOWN AND PINNED HIS RIGHT FOOT UNDER THE STONE AND BETWEEN THE GROUND. HE B ROKE HIS FOOT.

WAS WORKING OUT ON THE BLOCK WHEN HIS CHISEL SLIPPED AND HE HIT HIS RIGHT THUMB WITH HIS HAMMER.

HE WAS POURING CEMENT, LEVELING AND DRILLING HOLES FOR RE-BAR, PULLING & PUSHING A STRIKE BAR.

EMPLOYEE WAS CATCHING A STONE. WHEN HE LIFTED IT HE TWISTED HIS BACK AND FELT PAIN IN HIS LOWER BACK.
