On July 26, 2023 a non-injury roof fall was discovered approximately 60' in-by spad # 319 in the #4 entry on the NF #5 Panel. The fall is located both in-by and out-by crosscut 8. The fall is approximately 100' in length and approximately 5' to 6' thick.

EE stepped down from the center walkway onto the bolting platform on the operator's side. The back of the employee's forearm came in contact with an 8' T3 strap causing a 2" X 2" laceration.
