An unplanned roof fall occurred at this mine and was found by the owl shift on their way outside to the surface. The fall occurred on #1 Belt at 25 break and extended toward the intake stopping line at SS#: 1390. There were no injuries due to this accident.

Witness to the accident stated that when shutting the miner off to hook up a rope on the miner, the witness heard the injured employee call out for help. The injured employee was caught between the boom of the miner and the coal rib where the employee sustained a fractured L1 vertebra.

Employee stated EE was loading bolt supplies in the scoop bucket at the supply hole when EE slipped on oil that was in the scoop bucket and fell on EE's right foot. EE was treated at Raleigh General Hospital where EE was treated for a sprained right ankle.

Employee stated that employee was stepping down from the walkway of the bolter to the ground at the back of the machine when employee stepped on a rock and rolled ankle. Employee was treated for a fractured left ankle and released to return to full duty on 3/11/2024, employee's next regular scheduled shift.

Employee stated that they and a co-worker were removing supplies from a rail mounted flat car and had removed one pallet from the flat car with a chain. When the employee tried to step back on to the flat car they slipped and their leg was now on the ground between the scoop bucket and side of the flat car. at this time the scoop rolled forward pinching their left calf.

Employee stated had walked up to check on the drill crew when a chain hoist struck EE in groin area. The drillers were attempting to tighten pipe with 2 pipe wrenches and a chain hoist hooked into the wrenches. Pipe wrench broke causing the chain hoist to strike the employee.

Employee stated had been dropped off to help make a belt splice. Employee stated put a new blade in the knife, and was on knees squaring the belt with a utility knife when the knife came out of the belt and struck EE's right leg just above the knee, causing a 2-3 inch laceration.

Injured employee stated EE was operating the left Fletcher bolter on the #1 section, stated that EE was installing 10ft cable bolts in the intersection between entries 3 & 4 at #251 break, stated EE was 6ft in depth for a 10ft deep hole, stated that when bolt partner dropped pot it caused stress on EE's steel causing it to break and therefore strike EE's left pinky finger.

The rock measured approximately 4'L x 2'W x 0" to 2"T. This resulted in a strain and restricted work days.

Employee stated was operating a scoop and encountered a communication and junction box hanging from the mine roof. Employee used left hand to hold the cable away from the canopy while operating the scoop. Employee's L index, middle, and ring fingers were caught between the canopy of the scoop and the rib, resulting in an amputation and lost work days.

Employee stated EE was standing inside the bucket of the scoop and loading supplies onto the roof bolter. Emptied a wooden pallet and turned to throw it out of the scoop bucket. As EE turned EE's feet slipped and EE fell over the side of the scoop bucket resulting in sprains to multiple areas of EE's body and restricted work days.

Employee bent over to throw the track switch located on the surface of the mine yard. Employee states foot slipped in mud, causing pain in lower back. The incident became reportable on 9/30/2016 when the employee began missing work.

Employee stated was changing scoop batteries. As EE turned the battery bell, a metal tag with a sharp edge cut left index finger. This resulted in a laceration, and lost work days. Sutures were used to repair the wound.

Employee was walking on the mine yard when he became ill and collapsed to the ground in an unconscious state. He was transported to Raleigh General Hospital where he was pronounced deceased. The actual cause of death is unknown at this time. The M.S.H.A. was notified and an investigation was conducted by the agency.

Employee stated that he was using a slate bar to pry a plate that was caught in a pivot point on the bolt machine when the bar slipped causing him to strike his hand on the frame of the bolt machine. This broke the nail and bone on the tip of his right index finger.

Employee stated that he was attempting to hang the line curtain on a curtain hook where the top was too high for him to reach from the ground. The employee jumped up to snag the curtain on the hook, but cut his right index finger in the process requiring 6 sutures.

Employee stated that while he was attempting to place a roofbolt into the drill pot, the plate became lodged in the drill steel guide. When the employee attempted to remove the fouled plate, his right thumb was pinched between the plate and the drill guide, resulting in a fracture.

Contractor was loading truck after work at mine site was finished and stepped off tail gate and slipped on mud and twisted ankle. It was his right ankle and he was unable to drive to ER therefore an ambulance was called.

Contract labor emplyee stated that as he was walking in front of the bolt machine being trammed to break 43, he had stopped to observe a peice of draw rock when the bolt machine tire came in to contact with his left foot. The contract laborer was walking on the off side of the machine when the incident occurred.

Employee was standing on a ladder installing fly pads in the #2 entry. While hanging the fly pads, the scoop operator trammed through pads striking the ladder, causing him to fall.
