Employee has been diagnosed with Carpal tunnel disease. He had surgery on 7/17/12. We were notified on 8/23/12 by Worker's Compensation.

Member had been in the process of applying gravel to the haul road, as he was back dragging the road, he got to close to the berm and went into the sump. Member continued to work until 2/21/11 when he went to the doctor and was taken off work until 3/1/11.

While tightening a bolt with two hand wrenches, he felt a sharp pain in his ribcage. Upon X-raying his chest, it was discovered that he had fractured a rib due to overexertion while attempting to tighten the bolts.

Employee was handling a filter pot header pipe attempting to place the first bolt in the flange when the pipe slipped and caught one of his fingertips on his right hand against the filter pot. He was wearing leather work gloves.
