EE was in the process of removing a J-Bolt from a bottom hanger using a 1/2-inch impact to remove the loop nut. J-Bolt swung around and hit EE's finger.

Employee was attempting to couple a diesel locomotive to a lowboy railcar. Employee used a railjack to raise the drawhead of the lowboy, the drawhead fell off the railjack and caught employee's middle and ring fingers on the right hand between the two.

EE was using a utility knife when the knife slipped and cut EE's left middle finger and index finger. EE was not wearing cut resistant gloves as required by company policy. EE stated that EE had just removed EE's gloves to get a better grip. EE received 5 stitches in EE's left middle finger.

EE was assisting Co-Worker who was attempting to remove a cat pad pin, EE was holding a cut off roof bolt that had been modified for this task while Co-Worker was striking it with a 10lb sledgehammer. The hammer, after striking the head of the roof bolt, ricocheted up striking EE in the left eye. This resulted in a laceration above the left eye.

Injured employee was drilling a test hole when the steel became lodged in the top, while attempting to free the steel it fell out of the hole striking the left ring finger resulting in a laceration that required 10 stitches.

While mining the #3 entry the shuttle car couldn't make the turn to get out of the cut, EE walked to the back of the shuttle car to see if EE could help. The shuttle car pulled forward to reposition to try and make the turn. EE walked in between the shuttle car and the outby rib the operator didn't see EE and pinned EE between the shuttle car and the coal rib.

The employee was using a 4 pound sledge hammer to drive a boom pin out of a continuous miner, the miner removed glasses after they fogged up and begin to drive the pin out when a piece of metal came off the pin and embedded into right eye.

Employee was working on putting a conveyor chain together on the company's 310 shuttle car EE was swinging a sledge hammer installing a connector link and felt a sharp pain in EE's stomach EE was diagnosed with a ventral hernia.

EE pulled a bottom roller out from under the belt and stood it up and placed it on EE's shoulder and tried to stand up and carry the bottom roller. When EE stood up EE felt a sharp pain in EE's lower back and left arm and leg.

The owl shift was in the process of making a belt move when they heard the top working on the left side of the section around 5:00 am the roof fall occurred it fell for approximately 90 ft from number 1 over to the edge of the number 3 entry no one was injured and there was no equipment involved.

EE was preparing to set some timbers in the belt entry when EE noticed some loose top where the timbers was to be set. EE pulled the loose top and was shoveling it up when a piece of coal rolled out from the rib striking EE in the back and left elbow and left foot. EE finished the shift and worked for 4 days before being placed off work for 2 weeks.

After placing a jumper in the 4 mains belt EE started releasing the 3 ton chain hoist to let the pressure off the belt clamps. EE had worked the hoist 2 cranks when the belt broke outby the belt clamps allowing the clamps to fly forward knocking 2 top rollers loose on the belt pinning lower left leg between the belt structure.

Employee stated EE was in the process of hanging fly pads in the #4 entry, on the Unit 2 Section. In the process of EE hanging a fly pad from the 6" ladder, the ladder tipped over, and EE fell to the ground. This caused EE to land on EE's head, shoulder, and back. Note: EE was standing on the bottom 2nd step of the ladder when this incident occurred.

In the process of drilling a hole into the top to install a roof bolt a piece of rock measuring 36" Wide X 22" Long X 8" Thick fell out from between the roof bolts and struck the injured employee on top of EE's right foot.

Employee stated EE and another employee backed the #420 Roof Bolting Machine out by the No. 7 face after bolting it on the Unit 2 Section. The employee stated EE was going to walk around the machine to check EE's dust box. In the process of walking around the machine EE slipped and fell landing on EE's shoulder resulting in a shoulder sprain.

Employee stated EE and another employee were attempting to make a belt jumper at 3 Head with a piece of extra belt. In the process of squaring up the belt, the knife came out from the belt, and cut the employee in EE's leg. This employee had to receive 4 stitches.

Employee stated that ee was operating the #420 Fletcher Roof Bolt Machine on the Unit 2 Section. In the process of placing the drill steel into the drill pot a piece of rock fell from the mine roof and struck the injured employees hand. This caused one of the employee's fingers to be fractured.

Employee stated that they were installing a new tail piece directly under #2 Mains Head. The crew had accomplished that task and was in the process of pulling the belt back together. In the process of pulling the belt back together a bolt head broke off of a roof bolt striking the employee in the top of the head. The employee received 4 stitches.

EE stated Continuous Miner lost power so EE walked down to power center. When they arrived section electrician was trouble shooting C/M cathead. Electrician then said they were about to put the power in on C/M. In the process of putting power on the machine a cable splice grounded, causing the cable to blow up beside the injured employees face.

Employee stated ee and another employee were attempting to load a mass jack into a scoop on the Unit 2 section. In the process of lifting the mass jack into the scoop bucket, the jack slipped causing the employee to smash finger between the jack and the scoop bucket. The employee sought treatment and received 10 stitches.

Employee stated ee was assisting some other employees to unload a scoop tire from a mantrip. They were using slate bars to unload the tire. In the process of unloading the tire it flipped over into the scoop bucket they were loading it into. This caused the employee to smash finger in between the slate bar and scoop bucket.

Employee stated that EE was attempting to drag a 110V water pump with a 2 inch plastic discharge line. When the employee was attempting to move the pump and the discharge line across the track, EE stepped on the metal track rail which caused EE to fall, landing on EE's right hip.

Employee stated EE was preparing to make a belt splice on the 3 Mains belt line. EE had finished hooking up the chain ratchets and was going to step down off of the belt. In the process of stepping off of the belt, the employee stepped on a baseball size rock which caused EE to roll EE's ankle.

Employee stated ee smashed the tip of right index finger when ee attempted to toss a crib block. EE index finger was smashed between a crib block and side of a scoop. Employee received stitches and modified duty starting 11/14/2017.

Employee stated employee was on knees attempting to pry on the ball valve of the shuttle car so employee could add oil. In the process of prying with a slate bar employee felt a sharp pain in employee's lower back. Employee was placed on restricted duty on 10/17/2017

Employee stated ee was operating the tail motor hauling supplies to the section. The lead motor was pulling the supply cars up an incline, the lead motor stalled and rolled back striking the tail motor. Employee stated ee struck shoulder inside the operators compartment causing pain to shoulder. Note: Employee did not notify management until the 9/8/2017

Employee was moving cables out of the way to pull draw rock down that was over the power center on the 1 Left section. In the process of moving the cables, a piece of draw rock fell from the mine roof and struck the employee in the head and foot.The rock measured 27" X 18" x 2".

Employee stated as EE was drilling the hole a piece of draw rock fell from the mine roof and struck the employee on the left forearm. The rock measured 30" long X 10" wide X 1" thick and fell from approximately 2".

Employee stated ee was going to load supplies onto the scoop on the 4 Mains section. In the process of walking toward the scoop the employee slipped on a roof strap which was on the ground. This caused the employee to lose balance, fall to the ground, and twist back.

On 6/3/2017 an incident occurred when a track switch was not thrown back to the straight. The On coming day shift crew did not notice the switch thrown to the curve and the mantrip took the curve striking a parked motor. The employee suffered a back strain as a result. Note: Employee did not go on light duty until 6-13-2017

Later in the day he went to the ER, x-ray was negative. He continued to have soreness and was referred to an orthopedic on 1/6/15 where an x-ray revealed a fracture. He was placed on light duty.

EE was putting up a 8' bolt in the #5 entry while holding plate his hand was caught between scicorres clamp and plate resulting in a cut to his right hand.

Miner operator was cutting in the No 2 entry when his cable was sliding under the miner. The operator hooked the cable with his right heel of foot to pull the cable out of the way. He felt a sharp pain going down the side of his right leg.

EE was climbing a ladder to get to the top of a overcast, when the ladder began to tip over. He jumped from the ladder landing on his left ankle. He went to the doctor and x-rays revealed no fractures. EE started missing work on September 2 when he was diagnosed with ligament damage.

An unintentionall roof fall occured above anchorage at the Mains Section in the no. 7 face. The fall measured 7' high by 70' long by 20' wide. No injuries occured and No ventilation or travel was blocked by the fall.

Employee was removing a shuttle car tire. As the tire fell of the lugs, it struck the employee in the right ankle resulting in a fracture.

EE had finished his shift and was walking the off side of the belt with another EE to the outside. Between the no1 & no2 enrty is a crossunder for the belt. EE was in front of injured EE and crossed under the belt at the crossunder first and then heard EE scream. He returned to EE and was told by EE that his hood got hung and pulled his left arm into the belt.

Employee was trying to locate a hydraulic leak on equipment by holding cardboard in the area of the leak. When the hydraulics were activated, his hand was in line with the hydraulic leak resulting in a cut finger.
