Employee was setting up to take 1st lift out of block. Injured bent over to throw miner slack (cable) next to rib, when without warning, Rib Rolled off and struck him on the lower left leg and ankle pinning him between the miner and the rock. went to hospital and received X-rays RX.

Employee was traveling along the track on 4belt. When a piece of rock (measuring 4'X 2.5' X4") Fell from between the bolts and struck the Employee in the right shoulder knocking him off the ride. Injured went hospital and received a Diagnosis of Injury to the head, sprain of shoulder, contusion and a Concussion.

An unintentional roof fall has occurred 7 breaks inby surface at spad 3939. The roof fall was approximately 100ft x 18ft x 5ft. There were no injuries reported and no equipment involved.

Employee felt a pop in his right elbow as he was throwing a piece of curtain on the flat car. There was a piece of shuttle car cable tangled in the curtain. Employee was treated at MedExpress and x-rays indicated joint effusion with a possible fracture.

An unplanned roof fall occurred on the Stallion mains inby spad 3973 located in the face on #2 section. The falls specific location is in the last open crosscut from #3 entry to #7 entry. It starts from 1 break inby #3 entry and runs through 6 right crosscut.

An unplanned roof fall occurred at spad #1054, in #1 entry in the old works. Ventilation and passage of persons were not affected. Area was initially developed in 2010 and was supported by 6' resin bolts. Length of the fall was 40ft long X 18ft wide X 7ft high. Area will not be rehabilitated and all intersections have been timbered off. Federal and State agencies were notified.

Injured was setting supplemental roof support. He was holding the jack that a beam was resting on. Hand was resting on the tightening ear of the heintzman jack. While other employee was tightening his jack, the jack that injured was holding rolled, smashing his ring finger (no ring on).

Roof Fall in the #1 entry (return) along #11 belt measuring 100'long by 19'wide and 6'- 7' high. Area was bolted with 48" resin bolts in conjunction with 72" torque tension bolts. All approaches have been timbered off and no plans at this time to clean up.

An unplanned roof fall occurred at spad #936, in #3 entry up in the old works. Ventilation and passage of persons were not affected. Area was initially developed in 2010 and was supported by 6 foot torque tensions. The length of the fall was 20ft x 20ft x 8ft high. Area will not be rehabilitated and all intersections have been timbered off. Federal and State agencies were notified.

Walking across the section, injured tripped over a cable and caught himself. In doing so he put pressure onto his left shoulder and it became dislocated. Doctors put it in place and gave him 3-5 days of light duty.**Restrictions did not prevent the employee from doing ALL of his regular duties**

A unplanned roof fall occurred at Spad#1448 in the #6 Entry on the Old#2 Left Panel. The size of the fall was 80'L. X 18'W. X 3-5' H. Passage of person was not affected, ventilation was affected. Area was developed on Jan. 2011 and was originally supported resin roof bolts. Area has been dangered off, and will not be rehabilitated. Federal and State Agencies notified immediately.

Two electricians were lifting a small power box onto the top of a kva box on the yard at the mine site. The small box shifted and caught the index finger of the left hand of one man. He received a laceration that required four stitches. He was wearing leather gloves without metacarpal protection.

A unplanned roof fall occurred in the 1st panel in the #7 entry at spad 1686 in the return air course. Fall is approx. 5,700' from nearest active section. Fall will not be rehab. Passage of person and ventilation was not affected. Fall meas. 160'X20'X4' area was initially supported with 5' torque tension roof bolts. Federal and State agencies were notified immediately.

Employee was splicing a shuttle car cable. He laid the cable on his thigh and began cutting, using a utility knife. The knife slipped and cut his thigh. The laceration required three stitches. He returned to work on his next shift.

Employee was using a chain hoist to raise the top conveyor belt to change a top of piece of belt structure. The hoist came loose and fell onto his right hand near the thumb. His hand was laying on the belt rail at the time. He received a fracture. He was not wearing his metacarpal gloves. He retuned to work on his next shift.

Miner operator and two shuttle car operators were hanging the miner cable. The miner operator was on his knees and over-stretched while lifting the cable. He complained of his back hurting. He was transported to CAMC where he was diagnosed with a muscle strain in his back. He was given two days off.

EE was using torches to remove the cable stand off the right continuous miner. While cutting, hot slag went into his left ear. He continued working until Aug.28,2011. He sought medical attention on the third day after becoming light headed. Doctors said he had burned his eardrum and the ear was infected. He is off work receiving treatment.

EE was riding mantrip with the crew to #2 section. He was riding with the operator instead of the passenger compartment. The mantrip was not over crowded. EE said he was just riding there. He reached for his tobacco and his head hit the mine roof causing neck pain. He was released from CAMC hospital and told to remain off work a couple of days to rest his neck.

Employee was taking down MRS cables. The cables had been hung with nylon hangers with two hooks. He removed one hook. The hanger had been wrapped around the cable several times. The weight of the cable caused the hanger to spin free. The cable hook hit his nose. He had six stitches to the left nostril. He missed no work.

Power knocked on CM (JM6432). Operator and scoop man were tightening the waterline swivel near a damaged place in the cable. Someone reset the circuit breaker and cable went phase to phase creating an arc. The scoop operator may have received. a shock when the arc occurred as he stumbled away from the cable. EE was kept in the hospital two days for observation.

While replacing the rub rail on the miner, electrician felt pain in his back. Tests were negative and he was released from the hospital emergency room. He missed work and went back to the doctor. He was excused from work for three days.

Electrician was at the section power center drilling a larger hole in a ring terminal. He was holding the terminal with his left hand and the drill in the right. The drill bit caught his glove and lacerated his left thumb. He continued working and later sought medical treatment. The cut required 7 stitches. The safety department did not receive the report until 2/15/11.

Employee was taking a left-handed lift on the #2 pillar section. He backed the miner out to check a leaking boom swing hose. He and a miner helper were on the offside of the miner checking the leak when a rib rolled and struck employee on his lower left leg. He received multiple fractures to the left foot. *Doctors had to amputate below knee.

Loading structure into scoop bucket, EE told the operator to pull ram in to tighten pull chain. EE had his index finger in back of ram. He had swelling to his finger but x-rays revealed no injury. He stayed off for a follow up with specialist and no injury was found. No therapy or treatment has been scheduled. Due to soreness, he remains off work.

Employee was walking to the mantrip when he tripped and fell. A pick hammer he was carrying pierced his left hand between his thumb and index finger. He drove to CAMC and received six stitches. He returned to work on his next shift.

EE was unloading a pallet of roof "pizza" pans with the forklift. He dropped the load, breaking the pallet and scattering the pans. He said he was throwing a temper tantrum and slinging the pans to theside. One of the pans cut his left middle, ring and small finger. He went to Boone Memorial and rec. 2 stitches on the middle finger.

Employee was walking around the mantrip which was parked at the end of the track in a spur for the new section. He was going for electrical tape. He heard a flyboard crack and then rock knocked his hardhat off. The rock hit his left shoulder and then rolled down to the back of his leg. His leg and foot were bruised. He was instructed to keep it elevated for three days.

EE was on the Brookville Mantrip heading underground when he felt he was having a heart attack. He was transported to Raleigh General Hospital where test confirmed he had a heart attack. His Dr advised him that he is not able to perform physical labor and we are currently trying to place him in a mine office or similar position.

Crew was spot bolting outby face in #2 entry. EE was operating the helper side of a Fletcher RRII. He swung the drill out to spot a bolt between the outside bolt and the rib, when loose rock fell onto his right foot. X-rays revealed fractures to ankle. Surgery on 3/28/10. A plate and screws were inserted. Rock measrued 5"X12"WX30"L. Wearing tiger-tip metatarsal boots.

Member was working on a brookville pump motor coupling. He was in the process of removing a coupling off of pump shaft by using a chisel and hammer, upon striking the chisel with the hammer, chisel bounced and struck member on the left cheek. Member finished shift and went to a medical facility and received 3 stitches.
