White Buck Coal Company · Over-exertion in lifting objects

EE complained of pain in low back after uncoupling and handling belt structure. The accident occurred in the belt entry, there was no mining, and mining equipment has been removed due to the mine being permanently closed. A report was made online on 9/24/14 (attached). Upon reviewing 3rd quarter results it was noted that the report failed transmission due to an error of the writer.

Walking to lock out main mine fan door, thick ice under the snow. Employee slipped and fell twice while traveling to the fan door causing pain in his right shoulder.

Roof fall 20' outby spad # 2020 in #6 entry off #7 belt flight. The fall measured approx. 30'x20'x7'. The area was supported with 4' resin bolts and no plans at this time to clean up the fall. All approaches to the fall have been breakered off and dangered off.

Switching seats in shuttle car, employee grabbed handle and when he pulled himself up he felt a pain in his shoulder

Employee reported un-witnessed accident, stating he slipped on bathhouse floor, he finished shift and went to doctor next day, employee had MRI on 07/16/2012, no tears or deformities were found, however a cyst was found. Operator has determined no accident occurred, submitting for MSHA review, as Occupational Illness

pulling slack on 2" waterline, a clamp on waterline caused a laceration to right thumb of employee, employee went to Braxton county memorial hospital and received five stitches

CM operator was pinned between Coal rib and standoff on the CM. He was airlifted to Charleston General hospital, no fractures, contusions only.

EE was driving client's vehicle he was 3 miles away from assigned post. He said engine stopped and he lost brakes and power steering, vehicle went into a 7 ft. ditch. No injuries reported. When supervisor got to site, engine was still running.

Employee holding a punch to drive out a stuck bolt when a small piece of metal off the punch broke off when struck by a hammer. The piece of metal hit right forearm causing a small laceration and imbedded in arm. No stitches at time of injury but due to swelling in arm doctors decided to remove metal on 08/17/11, at this time five stitches was required.

Employee was shoveling belt when he felt a pain in his lower back. He visited his family doctor for pain and continued to work until 05/14/2011 when he returned to doctor because of continuing pain. Doctor has placed employee off work until he can see a specialist.

Continuous Miner was turning Left Break in #5 Entry. While loading shuttle car a piece of rock came off of the boom of the miner and struck the employee on the right side of his face. A laceration on the right cheek bone required 4 stitches.

EE was setting bits on the Miner in the #6 entry inby spad #1334. He turned around to put an old bit in a bucket and took a catch in his back. He was transported to Summersville Regional Hospital by ambulance where the ER doctor placed him off work for a week to rest his back.

EE was moving a cement pallet out of his way when he slipped/tripped over some plastic wrap that was on the mine floor. EE pulled some ligaments in his lower back. He did not go off of work until 10/16/2009.

Employee stated that he was helping another man load metal stopping panels onto trip to take underground. He slid his hand down the panel to turn it and cut his finger through the leather gloves he was wearing.

EE was operating shuttle car and heading to the feedback to unload. He was lashing the SC cable due to the anchor being one break inby the feeder. While EE was back lashing the cable, sprocket broke causing the reel to lock up resulting in the cable coming into the operator's deck striking EE's left arm and caused arm to strike the canopy.

Roof fall occurred over the weekend. The fall occurred in the #1 entry LT return qirway, the fall was approx. 65'L x 18"W and 41/2'T. The fall was rebolted and cleaned up, extra support was installed in the fall area.

EE was using slate bar to turn a 33' long rail, he was being assisted by another EE from the opposite end of the rail. As they were turning the rail the slate bar slipped from EE's hand resulting in the slate bar hitting him on the right side of his head. 2 stitches were required to close the wound.
