Black Castle Mining Company, Inc. · Fall to the walkway or working surface

Injured employee was securing equipment to the lowboy when he slipped and fell causing a fracture to his tail bone. He continued to work his shift but went to the ER afterwards where the diagnoses was made. He was released to return to work the following day.

An unintentional roof fall was discovered on the #2 Section in the #2 Entry Outby Spad 99. The fall measured approximately 8ft. long x 6ft. wide x 3 1/2ft. high. No injuries resulted from this occurrence.

Injured employee's left hand was struck by a rock that fell from the mine roof causing a laceration. He was seen and treated at the ER and received 7 sutures.

Injured employee was exiting the Scoop when the Scoop door expectantly closed smashing his left hand causing a simple distal fracture to his finger.

Injured member was operating the roof bolter when he placed his left hand on the canopy and smashed his middle finger between the canopy and roof resulting in a laceration requiring 5 sutures and a distal fracture of the left middle finger. He was released to return to work.

Injured member was cutting an overcast on the #3 section in the #3 entry when a piece of rock fell or was thrown back and struck member in the left shoulder (Clavicle)causing a laceration. He was transported to the ER via EMS. He received 24 sutures and was released to return to work on his next shift.

Black Castle Mining Company, Inc. · Struck by falling object

Injured member was rolling up the wash down hose on the continuous miner when a rock fell from between the roof bolts and struck him on the right side of his body injuring his right shoulder and legs. He was diagnosed with multiple contusions and excused from work.

Injured employee was pulling slack on the continuous miner cable to make a splice when he felt pain in his lower back. He immediately ceased work and was trasported to the surface to an awaiting ambulance. The ER physician diagnosed injured with a strain/sprain injury of his back and was excused from work for 1 week.

Injured employee was walking the roadway on #1 section when he stepped in a rut and twisted his lower back. He saw his primary care physician 9/28/10 and was referred for an MRI on 10/18/10. On 10/21/10 MRI revealed bugling disc in his lumbar region of his lower back. He has been excused from work until further notice.

Injured employee was spraying off the continuous miner when he stepped backwards and tripped over the rub rail on the miner. When he fell he struck his lower right back against the handle of the rub rail. The ER physician diagnosed him with a bruised kidney and bruised ribs and was excused from work.

Injured employee was loading rail ties onto a flat car, when he smashed his right hand between the tie and metal portal door, causing pain and a laceration. X-Rays revealed a fracture of his right little finger and he received 4 sutures in that finger. The physician has excused the injured from work until 10/25/10.

A roof fall was discovered in the #7 entry on #2 section near spad 731. The fall measured approximately 40ft long by 20ft wide by 5ft high. No injuries resulted from this fall.

Injured employee was installing roof bolts when the drill steel became lodged in the chuck. During his attempt to free the steel he accidently hit the rotation and the steel swung out and struck his left hand and elbow. He completed his shift then went to the ER were he was diagnosed with 2 small fractures in the center of his hand. He returned to work on his next shift.

Injured employee was bolting on the #3 section when the bolt he was installing bent and struck him on his left forearm. On 02/22/2010 Orthopedics diagnosed him with a Ulna fracture.

Injured member was installing impact rollers in the #1 tailpiece when the lid on the side of the tailpiece slid off and fell onto members right foot. Injured member was diagnosed with a Right Foot Contusion and was placed on restricted work activity.

Injured employee was moving timbers from the roadway and leaning them against the rib when one of the timbers (6" x 6" x 8') fell over and struck him on the inside of his right elbow. He was transported to the ER by EMS. X-Rays were negative for fractures. Diagnosed with tissue bruising. Employee suffered an injury to the same elbow in 2003 at another company.

EE was recutting the roof to allow equipment to travel with the miner in the #4 entry when a rock approximately the size of a soft ball bounched off the miner and struck EE on the left forearm causing a laceration. EE received 6 sutures.
