Slabcamp Coal

A roof fall was discovered in the primary escapeway near spad 15550 on No. 5 Belt at No 18 Break. Work crews were withdrawn to an area outby the fall. Fall measured approximately 40 feet long x 20 feet wide x 10 feet high.

While exiting scoop operator's deck, employee stumbled and fell on right hand injuring ring finger. Resulted in a fracture.

While repairing scoop, miner was using a utility knife to open the outer jacket of a cable, knife slipped causing a laceration on employee's left hand resulting in three (3) stitches.

Roof fall discovered in primary intake escapeway. Fall measured approximately 75' long x 20' wide x 5' high.

Injured was hooking a chain to a belt sled, the sled teetered pinching the right hand between the mine floor and the belt sled. Resulted in a laceration requiring seven (7) stitches to the right middle finger.

While operating a lifting jack, employee was assisting in letting the No. 6 mantrip down, when the jack bar slipped striking employee in the face on the right side causing a laceration to the inside of the jaw and a fractured nose.

A piece of roof rock measuring approximately 36 inches long X 36 inches wide X 1.5 inches thick fell and struck employee on the head.

Airway examiner discovered roof fall at spad 15409 adjacent to the No. 5 Belt at 18 CC in the no. 10 entry. Fall measured approximately 75 feet long x 20 feet wide x Unknown height (pulled anchorage).

While walking in track entry, employee stepped on track rail, slipped and fell landing on elbow and left arm.

Employee was shoveling the 2-A tailpiece placing material on a 2-A belt. Employee stated the shovel broke causing a sharp pain in the lower right portion of employee's back & right leg.

While cleaning up in #7 entry on the #1 Section, a piece of draw rock fell from between bolts striking employee on arm causing a 4 inch laceration. Resulted in 10 stitches.

Piece of draw rock fell striking employee on right wrist causing a laceration resulting in 4 stitches.

While setting up a pre-fabricated metal overcast, the structure collapsed striking the injured on left ankle resulting in a fracture.

EE was on a ladder when the legs kicked out and EE fell on EE's right arm and broke EE's wrist.

Employee was operating a rail mounted 15-ton locomotive. While traveling underground at approximately 8-9 break on No. 2 Belt, the locomotive wrecked and employee struck their head on a canopy post.

While riding in mantrip, injured's foot struck an accumulation of ballast that was pushed up when the mantrip derailed twisting EE's ankle resulting in a sprain.

While handling belt structure, injured cut EE's left thumb. Resulted in three stitches.

Roof fall was discovered on No. 5 Belt at 18 Break in intake.

Employee was handling bags of hand dust when they felt a pop in the left forearm. Incident was determined reportable on 04/27/2020.

Employee was carrying bottom conveyor idler and slipped in mud causing employee to fall. The bottom conveyor idler fell on EE's right arm causing a fracture to EE's wrist.

A Claim for Occupational Pneumoconiosis due to exposure to coal dust.

An unplanned roof fall occurred on the #1 section at spad 16053 bk #9 in the belt entry. The fall measured 40' long, 18' wide, and 5' thick.

An unplanned roof fall occurred on the #1 Section 1 Break outby the last open break in the #1 entry inby the loading point. The fall measured 70' long by 19' wide. The height of the fall was 10'.

An unplanned roof fall inby the loading point occurred on the #1 section #8 right crosscut 3 breaks inby survey station 16054. The fall occurred during idle operations, and the roof strata covered a Fletcher DDR roof bolter. Dimensions of the fall were 25'L X 18'W x 7H.

Mantrip going over dip and brakes didn't work. Neither did the dead man. Mantrip derailed.

Employee was hanging high line cable, EE felt a burning/tingling in the groin area. The injury resulted in a hernia.

Employee was hanging ventilation curtain from a roof bolter. The employee lost footing and jumped from the bolter. The employee landed on feet, but felt a pain in lower back.

Employee was loading 16' T5 channel. When picking up the outby end of the channel and turning, the employee felt a pain in the right shoulder.

A non-injury accident occurred on the mines raw coal stock pile. An employee crossed an unknown void in a Cat D9 dozer causing the dozer to fall into the unknown void entrapping the miner.

Employee was standing in the intersection, when a piece of draw rock fell striking their left foot. The rock measured 24"x 25".

Employee was hanging miner cable, the cable pulled away from EE causing a burning sensation in EE's right shoulder.

Employee was installing lower head bolts on a continuous miner. When aligning the bolts, ee was using a hammerer. While swinging the hammer the employee slipped and struck left index finger. The employee continued to work and reported to the doctor on 9/11/2018. After and x-ray was performed, it identified a minor fracture to the left index finger.

Employee was walking by dumping point, employee stepped into a hole causing a sprain to the left knee. Employee had continued pain and followed up with employee's physician, a torn meniscus was discovered. The employee was take off of work for surgery 6/20/2017. This will also be the first day missed since the accident.

Employee was cutting out a rock box. Metal from the rock box struck the employee on the back of the right hand. This caused a tearing laceration that required 3 sutures.

An unplanned roof fall occurred on the #1 pillar section inby the loading point in the #4 entry 40' inby spad #12628. Measurements on the fall were 40' long x 30' wide x 4.5' to 8' high.

EE was operating a scoop in preparation for construction of a permanent ventilation control in the #9 entry. While operating the scoop, a broken piece of ventilation control, known as a pogo stick entered the operator's deck of the scoop striking the EE in the face near EE's right eye. Causing a fracture to EE's nose and allowed debris to enter right eye resulting in a scratch.

Claim for Occupational Pneumoconiosis due to coal dust.

Employee was walking the face, when striking head on a coal brow. The employee worked the shift, and the next prior to reporting to a doctor for treatment on 9/9/2016.

Claim for Occupational Pneumoconiosis, due to exposure of mine coal dust.

Employee was standing at a storage insert looking at mine electrical cable. The employee was struck by a Johnson Industry Super track rubber tire personal carrier. This caused the employee to fall, resulting in a broken leg just above the right ankle.

A roof fall occurred in the Old In-active panel at 38 brk. on 5 mile 1st left panel. The fall measured approximately 20'x180x4-5' high & did not impede ventilation or travel. The fall has been breakered off & will not be rehabilitated.

The operator was notified on 7/11/2016 that the former employee (last exposure date worked 9/2/2014) has been diagnosed with Occupational Pneumoconiosis.

Loading waterline onto sled slipped and fell injuring left shoulder.

Re-installing motor onto rail when blocking kicked out striking the employees right foot toe area pinching big toe and toe beside it, pinch resulted in a fracture to big toe.

An unplanned roof fall occurred in the #6 and #7 (intake) entry along the Northwest Mains (4 belt) at break #12, one break inby SS #9042. This fall was 6000 feet from active workings. The fall measured 20 feet L x 140 feet W x 8 to 10 feet tall. Ventilation was not affected, passage of persons was affected.

The mine operator has been notified that the former employee (last exposure date 11/15/13) has been diagnosed with occupational pneumoconiosis.

An unplanned roof fall occurred on the north east mains, #2 belt break #27, #4 to 5 entry survey station #02978. Area of fall was developed 1-4-11, and supported with 48" resign bolts, it is 6600' from active section. Fall measured 75' wide x 19'L x 4-8'H. Passage of persons was affected, ventilation was not affected, fall area will be rehabilitated.

Employee bent down to pick up a rock, he felt a pull in the lower back. he continued to work until 12/30/14. He was taken off of work for rest.

An unexplained roof fall occurred on the #1 MMU 002-0 miner section. The area was in #5 entry, between break #6, SPAD #9169, and break #7, at spad #9191. The fall was approx. 60'long x 18'wide x 5' to 8' thick

A piece of draw rock fell from the top striking employee's hand causing severe laceration, and break to hand.

Working on the #1 roof bolter, he was on his knees when a rock fell out in between 2 bolts striking the employee in the head. The rock then broke and hit the employee in the hand causing a laceration that required 2 sutures.

Employee was unloading a bolter ATRS beam, when it slid out of the scoop bucket on top of his right foot. Employee's first day away from work was 8/18/14.

Employee was hanging miner cable in the #9 entry at spad #9061. The employee felt a sharp pain in his back. The employee returned to work for his next scheduled shift. After his shift on 7/31/2014 he reported to a doctor where he was taken off of work. The first away from work was 8/1/2014.

Employee was kneeling down removing rock out of the walkway, when he felt a pop in his knee. The employee returned to work, and worked until 7/20/14. He returned to the doctor with continued pain in his left knee. The doctor took him off work on 7/21/14.

Was using a crib block to drive an axle in the #2 roof bolter, when he hit the axle with the crib block it bounced back striking him in the mouth. This resulted in a split lip that required 27 sutures.

Floor was wet and muddy, while carrying roof bolts plates to supply bolter. Employee slipped and smashed finger between roof bolt plates and bolter. This resulted in a 3/4" laceration to left middle finger that required four sutures.

While traveling down track, mantrip came off track. While jacking up ride, EE could not get leverage with jack bar so he started using slate bar. He thought he heard the jack "click" lock into place so he took his hand off of the slate bar. It swing up striking him in the hand and also hitting him in the face. Resulted in bruised hand & 3/4" laceration requiring 6 sutures.

Employee stepped from the back of walk thru roof bolter to the mine floor. He slipped while exiting the roof bolter resulting in a sprain to his right ankle. He was released by the doctor to return to work however the employee stated he was unable to perform his regular job at full capacity therefore the employee has been assigned restricted work activity.

Employee was taking his glove off to eat lunch. His glove got caught on an inner latex glove worn to prevent chafing. When the glove caught, it jarred his hand into a stack of roof plates causing a laceration on the back of his left hand. The laceration was deep enough for 3 sutures. Note: the employee did finish his regular shift and then went to doctor.

An unplanned roof fall was found by Block Foreman in the #6 entry at crosscut #21 of the 1st right panel off of #5 belt on #2 section. The fall was approximately 20' length X 22' width X 12' high. No injuries or entrapment involved. No escapeways, equipment or ventilation effected. This is an abandoned panel and the #6 entry is not traveled.

Employee was walking backward hand rock dusting entry when he stepped into DBP scoop bucket and slipped and fell striking his right hand on the edge of the scoop bucket which resulted in a fracture (crack) to one rib. (x-ray)

An unplanned roof fall above the anchorage zone occurred in the face of the #4 entry of the #1 section (MMU 002). The fall was approximately 50ft long x 18ft wide x 10ft high. No injuries or entrapment was involved. The Fletcher Roof Bolter was covered by the fall. The entry travelway was involved in fall and ventilation to face involved. No escape way was affected.

Employee was attempting to splice conveyor belt at #8 conveyor belt take-up. He was using a utility knife (razor blade) and the knife slipped cutting the side of his right index finger.

Employee dropping miner cable that was hung with rope hangers. As he lowered the cable the hook on the ripe caught the web between his fingers resulting in a laceration. He continued to work with sutures and the on 7/25/11 he started missing days worked due to the injury.

Employee and a co-worker were going to install door in stopping-removing plastard block as he went to remove a block the block above it slid out, a thin layer of block bond attached to the block was thin and sharp enough to cause a bad avulsion on his thumb.

An unplanned roof fall has occurred in the #3 entry between crosscuts 16 and 17. (#5 belt). Fall was approx 10'thick x 15' wide x 55' long. Fell inby dumping point resulting in K-order and change of RCP. No injuries involoved and the fall did not affect ventilation or travel ways.

An Unplanned roof fall has occurred in the #6 entry between crosscuts 13 and 14. (#5belt) 4' to 6' thick X 17' wide X 30' long. timbered and dangered off 2 days prior to the roof fall, no injuries involved with fall and the fall did not affect ventilation of travel ways.

Unplanned roof fall on #4 belt in the track entry (#4 entry) at spad #707. Fall was approx. 55' L. X 4' Thick X 20' W. Area bolted with 6' resin bolts, no injuries or entrapment involved. Fall was in secondary escapeway and fell on track, high voltage cable and the trickle dust cable.

An unplanned roof fall at #26 brk. on #2 belt, #1 entry 20' into the cc.. 2 to 1. Fall was approx. 5' thick X 35'l. X 19' w. Area was developed in 01/11. Occurred 2000' out-by the section feeder. No entrapment or injuries involved. No EQ., escape way, travel or ventilation involved.

Unplanned roof fall occurred inby feeder on MMU 004-0, from 7 right crosscut to the #9 entry. Fall was approximately 95 ft length, 19 ft wide and 8 ft in height. No injuries, entrapment or equipment involved. Last open crosscut ventilation affected and travel way. Developed 1/31/2011.

An unplanned roof fall was discovered on old #1 section (old #8 Belt Panel) one break outby the last line of crosscuts. Fall was from #5 entry to #8 entry. Date of development was 6/2010. No injuries or entrapment involved. No ventilation, escapeway, passageway or equipment involved. Fall was approximately 115 ft long x 19 ft wide x 19 ft high.

Member was helping install a stopping. He was throwing blocks over to Members that were constructing stopping, when he turned to throw one he caught his left little finger between block in his hand and the stopping being built, resulting in a fracture to his little finger on the left hand. The finger was splinted.

EE injured his left shoulder while lifting bottom roller back into hanger. He refused medical treatment at end of shift. He was contacted at 11 am & still did not need treatment. At 2 pm he called mine site & stated he was not going to seek medical attention. He called back around 8 pm & stated he had gone to doctor & would not return to work on his next shift.

Unplanned roof fall in return on 1st left panel off of #8 belt in #8 entry. Fell from #10 Brk to 11 1/2 Brk (134' long X 8-10' high X 18-20' wide) between spad #2373 & 2384. No injuries, no equipment involved, no ventilation affected, no travelway affected. Date of development 6-29-2010.

Employee was walking around power center & slipped on roof rock that had fallen out then tripped on cable and fell to the ground. He threw right arm out to catch himself & injured his right wrist & arm.

Employee had moved up scoop charger and was hanging extra charger cable when he felt something pull in his groin area.

EE was unloading 6" water line off flat car with scoop. One joint of water line became caught on scoop ram. He attempted to free it with a slate bar. The pipe popped loose striking the slate bar which resulted in the bar striking his finger. The result was a dislocated finger with medical treatment only of a splint applied. No days lost.

Employee was driving a spad nail into rib hanging a curtain. A corner of the rib rolled off striking the employee on the right hand. The result was a small laceration on the knuckle of the right hand which required sutures only (no days lost) Medical treatment only.

An unplanned roof fall occurred at #8 bck on the #4 belt and #7 entry that measured approx. 50' L X 18' W X 7' H. No injuries were reported. The area was developed on 2/27/09.

EE was walking to mantrip when he tripped over a rock and fell striking his forehead and nose on a rock causing a laceration to forehead and bridge of nose.

Employee was operating the continuous miner. He stopped to advance his line curtain and as he was attempting to hang the curtain, he lost his footing and caught the brattice hook with right middle finger. The result was a laceration to his finger that required sutures.

An un-planned roof fall occured in the Subcamp mine at brk #26, inby spad #1622 that measured approx. 160' long X 18' wide X 4-8' high. No injuries were reported. The area was developed on 12/16/09.

While loading a fusion machine into the bed of a pickup truck, EE was lowering the handle of the machine, causing it to pinch his left middle finger. The injury to his finger required part of the distal phalange to be amputated. This accident was not received at Marfork until 10-23-2009

Member was in the process of building a 7' high stopping. Right corner was tied in and he was stacking block across the top when the middle blocks fell out and struck him on the shoulder, causing it to become dislocated.

Claim for Occupational Pneumoconiosis due to mine coal dust.

EE & a co-worker were building a stopping in the #2 entry, the seam height is 10'4" at this location and is on a hill. The right side of the stopping was stacked almost high enough to wedge. EE climbed down from the ladder to retrieve wedges when the stopping fell, striking him on the right hand.

Member was pulling welder cable. His knife was clipped to his belt and had accidently been left in the open position. As he pulled the cable the knife made contact with his right pinky finger and made a small laceration.

Member was in the process of cutting a zip tie from a roll of curtain. Member had roll laying on his leg. While applying force, knife slipped and lacerated his right thigh. 21 sutures were required.

Member was dragging miner cable as he was pulling, his fact became tangled in the cable and he tripped and struck his forehead on the boom of a shuttle car.

EE was walking up the #6 entry to the face, when he stepped in a hole and twisted his right ankle.
