Carrying timbers, took to #3 seals. Carried them 3 breaks, felt pain shoot down back.
Was using a rake to clean belt and it got caught in roller. Handle hit him on right side of hand.
Was lifting timbers and pulled his back. The conditions were good, no mud or water. Was not reported until 8-3-09.

Working on stoppings, crossing over gob pile, stepped on loose rock, rock went out from under foot and to keep from falling on top of some H beams, EE grabbed a hanging roof bolt and pulled shoulder. Accident 6/14/08 never became reportable until 7/9/08.
He was sitting in truck at lunch time and blacked out and fell from passenger side of truck and hit the ground.
