Employee was holding a 20lb sledge hammer against the connecting link of a flight on the conveyor chain (14-15 miner) Co-Worker was striking the ring with a 4lb hammer onto the connecting link when he missed and struck injured on the right ring finger.

Employee was pulling on a chain fall that had been wrapped around a conveyor belt to raise it up to take tension off a channel pan that was to be changed out. In the process, the hook slid down the change and hit the employee in the knee, causing pain and swelling as time progressed.

EE was walking underground slipped on a ledge in the bottom and fell to the ground hurting his back. Note: It was never mentioned in the accident report that he hurt his Right knee. He had told the E.R. he injured his knee during the accident. 2nd Note: He continued to work until 03/12/2013 before he went off for surgery on his knee.

Injured was dragging old belt rollers from the belt line to the track entry and pulled his left shoulder. He filed a notice only report and missed no work. On Sept. 4, 2012 he missed work fo r surgery on that shoulder. Original accident was 9/16/11 and employee continued to work. He hurt it again on 6/4/12. Due to continued aggravation, he decided to have surgery.

Scoop operator was unloading timbers off a pallet approx. 5' off the ground when a timber fell off the pallet and landed on his right foot. Timbers were seven feet long and were in a scoop bucket. He is off work due to swelling of the foot.

While hanging a fly pad curtain, EE struck his left index finger with his 20 oz. claw hammer. He was not wearing the required metacarpal gloves. He drove to Hospital and received one suture for the laceration. He returned to work on his next shift.

Move crew was pulling cable slack toward the face during a power move. Employee thought the cable was going to become tangled. He attempted to straighten it out as it was being moved when the cathead caught his right hand. He received a laceration that required six stitches. He returned to work on his next scheduled shift.

While laying track on #1 section, employee was using a bar to push a rail over. He then used his hand to push the track tie down as he flipped the rail. The rail landed on his left hand just above the thumb. He was placed on restricted duty for two days due to the contusion. No further injuries were found.

Forklift operator raised a portion of ballasted track so the supplyman could run a chain under the rail and tie off a flatcar. The fork caught under the ball of the rail and the fork bounced loose striking the supplyman in the face. He received fractures to the right eye socket and cheek area. He is off work for treatment.

Bathhouse attendant was sweeping the bathhouse and fell out the back door. The height was 18-24" and the steps were removed while putting an addition on the building. He twisted his left knee but refused any medical attention. He missed no work until March 12, 2012.

Member did not cease work. On May 29, 2009 it was learned that he had seen a doctor and diagnosed with a hairline fractures to four ribs. He said he felt fine and refused to be off work.

Member was preparing to fuse two pieces of pipe together when the piece he was holding slipped free and flew up and struck him in the face causing a small laceration just under his nose which needed 3 stitches to close.
