Roundbottom Powellton Deep Mine Coal

Employee was holding a 20lb sledge hammer against the connecting link of a flight on the conveyor chain (14-15 miner) Co-Worker was striking the ring with a 4lb hammer onto the connecting link when he missed and struck injured on the right ring finger.

The employee along with some help was hanging Miner cable and water line across the entry for shuttle car to run. Injured felt a sharp pain through his right shoulder and down his back.

Employee was walking two breaks outby the section loading points in the neutral on the belt line and fell over a rock. Radio microphone and tag reader struck him in his right side

Employee has been diagnosed with occupational pneumoconiosis. the safety department received notification of his diagnosis on April 28 2015.The information to complete the 7000-1 was received on May 4 2015 by the HR department.

As Employee was cutting belt the knife slipped causing a laceration to his right leg-thigh. Employee was treated at Raleigh General Hospital where he received 10 sutures

Employee was moving top structure out of the way, stacking it against the rib. A piece of structure Employee just stacked fell over striking his right middle finger resulting in a fracture and seven (7) sutures. On April 1, 2015 Employee had surgery to repair right middle finger.

While moving feeder cable Employee stepped on a rock twisting his right ankle. Employee was treated at Med Express in Beckley and was diagnosed with a fibula fracture.

While Employee was shoveling material he felt a pain in his lower back.

Employee was pulling on a chain fall that had been wrapped around a conveyor belt to raise it up to take tension off a channel pan that was to be changed out. In the process, the hook slid down the change and hit the employee in the knee, causing pain and swelling as time progressed.

Employee was troubleshooting the RCM when a piece of draw rock fell striking Employee in the head. He was treated at Beckley Appalachian Regional Hospital where he received nine (9) staples in his head.

Employee was helping unload high voltage cable when a loop came down striking Employee in the face. An internal investigation has been conducted and has been waiting on medical documentation.

Employee was placing pogo sticks behind line curtain after cleaning. The pogo stick came loose and the top screw struck Employee in the palm of right hand. Employee was treated at MedExpress where he was diagnosed with a laceration and a hairline fracture.

While plastering stopping on track, Employee was stepping down from motor Employee was using as platform due to excess height, when Employee slipped and felt pain in his right knee. (non-displaced fracture) Employee did not miss work but received second opinion and was diagnosed with non displaced fracture

An unintentional roof fall has occurred on the #1 Section intake primary escape way outby spad 1708. The roof fall was approximately 20ft x 20ft x 12ft. There were no injuries reported and no equipment involved.

While helping build a crib, Employee was carrying a crib block when he tripped and fell onto another crib block dislocating his left shoulder. Employee was working over his scheduled shift.

Employee bent over to pick up continuous miner cable and waterline and felt a sharp pain in his lower right back.

While tramming a mantrip outside Employee had something flew into his left eye. Employee states he was wearing safety glasses and that it resulted in a scratch on his eye.

An unintentional Roof Fall has occurred on the #3 Section 2nd panel off 4 left spad 10180. The fall was approximately 140ft x 20ft x 8ft. There were no injuries reported and no equipment involved.

An unintentional roof fall has occurred on the #1 Entry (face) West Submains 27ft inby spad 6838. The roof fall was approximately 27ft x 18ft x 5-6ft. There were no injuries reported and no equipment was involved.

While striking coupling with a hammer, coupling came up and hit Employee in the mouth causing a laceration to his lip. Employee was treated at MedExpress where he received four (4) sutures in his upper lip.

An unintentional Roof Fall has occurred on the Old #1 Section #7 Entry on Bleeder Panel 30ft inby spad 1759 to one break outby 1798. The fall was approximately 500ft x 20ft x 8ft. No injuries were reported and no equipment was involved.

An unintentional Roof Fall has occurred on the Old #1 Section #2 Entry on Bleeder Panel inby corner of spad 1830 to inby corner of 1829. The fall was approximately 90ft x 20ft x 8ft. No injuries were reported and no equipment was involved.

An unintentional Roof Fall occurred on the Old #1 Section #2 Entry on Bleeder Panel spad 1814 to outby corner of spad 1830. The fall was approximately 90ft x 20ft x 8ft. There were no injuries reported and no equipment was involved.

An unintentional roof fall has occurred on the West Submains spad 6838. The fall was approximately 80ft x 18.5ft x 12ft. No injuries were reported and no equipment was involved.

While Employee was helping load 10' roof jacks, Employee felt a pain in his lower back. Employee was treated at Raleigh General Hospital. The Safety Department has learned of Employees physical therapy treatments which occurred during February 25, 2014 through April 8, 2014.

An unintentional roof fall has occurred in 1st Left Panel North Submains #1 Entry 1 Break inby/outby spad #8719. The roof fall was approximately 90' x 20' x 5'-6'. There were no injuries reported and no equipment involved.

An unintentional roof fall has occurred in 2nd Left Panel North Submains #9 Entry 2 breaks outby spad #9158. The roof fall was approximately 120' x 20' x 5'-6'. There were no injures reported and no equipment involved.

Employee has been diagnosed with Occupational Pneumoconiosis. Marfork Safety Department received notification of the diagnosis on April 22, 2014.

An unplanned roof fall occurred in an intersection in the primary escape way intake #6 entry spad number 3022. The fall measured app. 40'long X 70'wide X 6'high.

Injured was going to his four wheeler to retrieve a tool, as he came back to the scoop charger he stepped on a rock, measuring 15"long 9" wide and 4" thick twisting his left ankle resulting in fracture.

Employee has exercised his rights to occupational pneumoconiosis. On 06/04/2013 a chest x-ray showed small opacities over the lung zones with a profusion of 1/0. Note: Employee retired 09/11/2012

Injured was cleaning grease away from bearing on #5 belt take-up with a stick. The key steel on the bearing caught the piece of wood causing the stick to fly back striking Injured in the right ear resulting 22 suture' s

Injured was walking along beside miner, sliding his hand along the miner when his Right hand caught a burr cutting his hand below right thumb resulting in 6 sutures.

Injured was helping load supplies, picked up bundle of plates and cut through right glove and cut palm where little finger meets hand.(Received 8 sutures)

Lafferty was walking down a hill, slipped and strained his left knee. **Employee did not miss any work until May 22, 2013 when he had surgery on his knee.**

Changing out a bottom roller on a conveyor belt and hurt his back.

EE was cutting tape off of a shovel handle with a utility knife. The knife slipped from the shovel handle striking him on the inside of his left arm.

White was pulling a pump out of a roadway and felt pain shoot down his lower back. **Addendum ~ Employee did not start missing work until 3/9/2013**

A unplanned roof fall occurred at this location. Fall was located at #162 Break in the #3 entry, 10,800' from the active 001/002 section. Area was originally supported with 5' torque tension bolts. Fall measured 60' L. x 20' W. x 6' H. Did not affect ventilation or passage of persons. Federal and State Agencies notified.

EE was walking underground slipped on a ledge in the bottom and fell to the ground hurting his back. Note: It was never mentioned in the accident report that he hurt his Right knee. He had told the E.R. he injured his knee during the accident. 2nd Note: He continued to work until 03/12/2013 before he went off for surgery on his knee.

An unplanned roof fall occurred on #3 section panel 800' from the face @ spad #8136. Area was originally mine in Sept 2012 and was supported with 8' cable bolts and 5' tourque tensions. Fall measured 20'L x 18'W x 10'Thick. No injuries or equipment involved. Did not affect passage of persons or ventilation. State and Federal agencies notified immediately.

Employee was loading top belt rollers into the scoop bucket and heard a pop in his right shoulder, followed by pain. This is an old injury that he has been having issues with.

An unplanned roof fall occurred at spad #5977, in #4 entry, 34 panel. Ventilation and passage of persons were not affected. Area was initially developed in 2011 and was supported by 5 foot torque tensions. The length of the fall was 100ft x 18ft x 6ft high. Area will not be rehabilitated and all intersections have been timbered off. Federal and State agencies were notified.

Employee shoveling #4 tailpiece and twisted to throw coal on belt without turning his entire body. He felt pop in left knee. Previously hurt knee 8/15/12 but it was file only. He is off with torn ligaments.

Area was initially developed in 2008 and was supported by 5 foot torque tensions. The length of fall was 30ft x 18ft wide and 3ft high. Area will not be rehabilitated and all intersections have been timbered off. Federal and State agencies were notified.

Injured was dragging old belt rollers from the belt line to the track entry and pulled his left shoulder. He filed a notice only report and missed no work. On Sept. 4, 2012 he missed work fo r surgery on that shoulder. Original accident was 9/16/11 and employee continued to work. He hurt it again on 6/4/12. Due to continued aggravation, he decided to have surgery.

Employee stated he was cleaning under the belt when a jack became dislodged and struck him on the back of the head.

While shoveling spillage onto #6 conveyor belt, injured's shovel got caught between the belt and mine roof, jerking his body. This caused pain in his upper back and neck. He is off work seeking treatment.

Scoop operator was unloading timbers off a pallet approx. 5' off the ground when a timber fell off the pallet and landed on his right foot. Timbers were seven feet long and were in a scoop bucket. He is off work due to swelling of the foot.

While hanging a fly pad curtain, EE struck his left index finger with his 20 oz. claw hammer. He was not wearing the required metacarpal gloves. He drove to Hospital and received one suture for the laceration. He returned to work on his next shift.

While riding the Brookville mantrip to the section, EE chipped a tooth when the ride ran off the track at spad#6595. The trip came to a sudden stop and EE hit his mouth on the handle bar in the passenger compartment. He refused to seek medical treatment.

A unplanned roof fall occurred in the #5 Entry of the North Sub Mains at spad #6471,5200. The size of the fall was 30'L. X 20'W X 7' H. Ventilation was not affect, passage of persons Area has been breakered off , and will not be rehabilitated. Federal and State Agencies were notified immediately.

Move crew was pulling cable slack toward the face during a power move. Employee thought the cable was going to become tangled. He attempted to straighten it out as it was being moved when the cathead caught his right hand. He received a laceration that required six stitches. He returned to work on his next scheduled shift.

Employee stated he was using a slate bar to pry against a car to get it back on track when the bar slipped and struck his right leg.

While laying track on #1 section, employee was using a bar to push a rail over. He then used his hand to push the track tie down as he flipped the rail. The rail landed on his left hand just above the thumb. He was placed on restricted duty for two days due to the contusion. No further injuries were found.

Forklift operator raised a portion of ballasted track so the supplyman could run a chain under the rail and tie off a flatcar. The fork caught under the ball of the rail and the fork bounced loose striking the supplyman in the face. He received fractures to the right eye socket and cheek area. He is off work for treatment.

Employee was bolting an overcast one break inby the #1 Section feeder. He had his right hand on the bar attached to the canopy when he slipped on some rock. He did not let go when he fell and twisted his upper body. His shoulder twisted in the process. Doctors gave him a few days of restricted duty.

A unplanned roof fall occurred in the #6 Entry at spad # 2322. Fall is 7,200' from active 001-0/002-0 section. Area was initially supported with 5' torque tension bolts. Fall meas. 20' L. X 20' W. X 6' H. Ventilation and passage of persons were not affected. Fall will not be rehabilitated. State and Federal agencies were notified immediately.

Bathhouse attendant was sweeping the bathhouse and fell out the back door. The height was 18-24" and the steps were removed while putting an addition on the building. He twisted his left knee but refused any medical attention. He missed no work until March 12, 2012.

Operator was bolting in 5R on #2 section using 12' cable bolts when a rock measuring 21" X 10" fell from between bolts striking his left shoulder. He received a laceration that required two stitches. The height was 11' and the top mixed. He missed no work.

A unplanned roof fall occurred in the #1 Entry at spad # 6731. Fall is 500' from active 003-0/004-0 section. Area was initially supported with 5' torque tension bolts. Fall meas. 20' L. X 24' W. X 6' H. Ventilation and passage of persons were not affected. Fall will not be rehabilitated. State and Federal agencies were notified immediately.

A unplanned roof fall occurred on the #2 section in the #3 entry at spad 6839. Fall occurred 80' from the active #2 section. Fall measured 20'x20'x7' area was initially supported with 5' torque tension and 8' cable bolts. Ventilation was not affected, passage of person was affected. Area will be rehabilitated. Federal and State agencies were notified immediately.

Electrician was walking by the #3 section power center when tripped over power cables. He twisted his right knee. The incident was reported as a notice only with no injuries, treatments or doctor visits. In February 14, 2012 he sought medical treatment and missed work for surgery to his knee.

The off-side roof bolter was drilling a hole, when a rock fell onto his canopy and busted apart. A piece of the rock hit his left ear causing a laceration that required fourteen sutures at CAMC General Hospital. He was instructed to take five days off work.

The chief electrician was removing a side shield off a 14-cm15 Joy miner. He was letting the shield free-fall when his finger got caught in the handle. The middle finger of his left hand was broken at the first joint and the nail ripped off. This required four stitches. He returned to work on his next scheduled shift.

The roof bolter operator was using a utility knife to remove the straps from a bundle of roof bolts, when the knife slipped and lacerated his right thumb. The cut required four stitches. He returned to work on his next shift.

Electrician was replacing a battery plug when a rock measuring 5'x3'x3 1/2" fell from the roof striking him on the back of head & neck area. He drove himself to Raleigh General and the doctors gave him a full release. He returned to work & continued to work until 9/27/2011. He went to his doctor who gave him two days off with muscle relaxers. He returned to full duty on 9/30/2011.

Operator went to the back of the bolt machine and picked up a box of resin. He threw the box onto the machine and felt a pull in his left shoulder. He continued bolting and finished that cut. He left work and sought medical treatment at Raleigh General. Drs there advised him to take 1 day off. He RTW on 8/29/11 after missing one scheduled shift. He did not report this until today.

Fireboss was riding a rubber tired ride on the track along #1 belt when he attempted to steer the ride off the track. The tire got caught on the track and caused the steering wheel to spin out of control The steering knob hit his right hand around the pinky causing a fracture. He was treated and released. He returned on his next shift.

Electrician was installing a speed reducer on #10 belthead. To see if the reducer mounting holes were aligned, he put his rt index finger in the bolt holes. Was cleaning rockdust out of the holes. Another worker decided to shift the reducer with a slate bar and in so doing he lacerated the finger. He was taken to CAMC and received sutures. He RTW on his next shift.

While swinging the drill pot, the operator had his right hand on the drill head and mashed his middle finger between the pot and frame of the bolter. He was wearing gloves. He went to Raleigh General and received ten stitches. He returned to work on his next shift.

Employee was at #8 belt heads line splitter waiting for power to be set up outby his location. When power was supplied to the kva it created an arc. He was standing about 5' from flash. His eyes were flushed and he was given eye drops. He returned to work on his next shift. The arc was created by a loose phase lead.

Beltman/fireboss was breaking up a large rock using a sledge hammer. He felt pain in his lower back and hips. He was working alone. He was transported to CAMC General by ambulance. Doctors gave him a light duty slip. He did not report to work and has been off work since the accident.

Roof fall occurred in the #3 enrty at spad #1576. Fall occurred 5600 feet from the active face on the 001-0 MMU. This area was developed March 2008. Area was originally supported with 5 torque tension bolts. Roof fall did affect ventilation and passage of persons. Fall will be rehabilitated. Fall 20w X 20l X 8H. Federal and state agency were notified immediately.

Employee was running the scoop when his canopy caught the bolter cable. He used his hand to push the cable clear of the canopy when the scoop rolled, pinching his finger between the canopy and the mine roof. He drove to Appalachian Regional Hospital and received ten stitches. He returned to work on his next scheduled shift.

Electrician was helping scoop operator change batteries. EE had female end of jumpers and was connection it to scoop when wiring pulled loose inside bell creating an arc. Palm of his left hand was burned. Was treated and released at CAMC. He returned on his net shift. He followed-up at St. Marys Burn Clinic. The doctor removed dead tissue with no infection and released him.

EE, electrician was unloading parts from the man trip at the end of the track rail on #3 section. EE crossed the track on the out-by end and caught his foot under the track rail. The newly installed track was not ballasted. EE hyper extended his foot and rec. a bruise. He has not RTW and does not respond to requests for communication regarding the incident.

On the right handle cover - pulled cover outward it fell to ground and pinched fingers between handle & cover.

A unplanned roof fall occurred in the # 5 entry of the 001-0 section, this fall was 40' from active face. Fall measured 30' L. X 20' W. X 6' H. No eq. or injuries involved. Area was supported with 60" resin bolts along with 10' cable bolts. Fall occurred in Neutral Entry, ventilation and passage of person were not affected. Federal and State Agencies were notified immediately.

Employee was cutting a piece of conveyor belt. The knife slipped and cut the end of his left thumb. He drove himself to the doctor after work. According to him, he received six stitches. He has not missed work.

While making a walk around inspection of the surface facilities, the security officer slipped and fell on the ice striking his left shoulder on the ground. Security Officer completed his shift and refused transport and treatment.

EE was in the number 4R crosscut hanging flypads with co-worker. He bent over and cut the flypad 3/4 way through & stretched to finish the cut. The knife slipped and cut his left forearm. He was treated by EMTs and transported to Charleston Area Medical Center. He received eight stitches. He worked until 10/27/10 restricted until no work available as light duty.

Crew was recovering track rail on #2 section. They were using a scoop and chain hoist to lift the rail onto the rail car. One rail caught the car and jumped the upright standard (8"). EE turned to get out of the way. The rail caught the calf of his right leg. Tests were negative. Worked restricted until 10/27/10 and then off for surgery to repair muscle.

A unplanned roof fall occured in the #2 entry on the #2 section, in the #2 entry 500' from active face. Fall measured 20' L. X 20' W. X 5' Thick. No Eq. or injuries. Roof Fall did not affect ventilation and or passage of persons. The fall will not be rehab. Area was developed 9/10/10. Roof support consisted of resin roof bolts. Federal and State agencies notified immediately.

EE was rock dusting #7 beltline. EE was moving the hose under the belt to cover the offside. EE backed up and the hose became twisted and hit the fresh waterline. Dust hit his face and got into his eyes. He said he was wearing glasses. He came out and drove home before anyone could investigate. EE said his corneas were scratched. Doctor reports have not been rec. to confirm.

EE was replacing a bucket jack on a scoop. He noticed a flag pin had become fouled behind scoop ram blade. He tried to remove bent pin when a splintered piece of metal penetrated his right thumb.

Employee was pillaring right lift #2 entry on #1 section. A rib rolled out and struck his lower left leg. He continued to work and refused treatment. Due to swelling he later went to a doctor. Tests revealed a fractured left foot.

The move crew foreman wanted to move shuttle car out of way to make belt move. He pulled 15' cable off reel because pump didn't start. He told another EE to start car and everyone was clear. Reel began to take up and cable hit foreman, knocking him down. He said his right leg and lower back felt numb. X-rays were negative. He remains off work.

EE and two others were hanging 4" water discharge line. The line was wet and covered with rockdust, it slipped and struck him on the head/back of neck. Other end anchored w/hoist to prevent falling. He refused medical attention. 2 others helped with hanging. Went to Dr. and referred to orthopedic. Diagnosis, back strain on 8/17/10. He is seeing a chiropractor.

Miner operator and M.R.S. operator set up in #2 Block to begin pillaring. The right rib popped and rolled hitting M.R.S. operator on right calf. After checking roof and rib again, they loaded out first lift before end of shift. On way to mantrip, M.R.S. operator said his leg was wet and discovered a tear in his calf. He was transported to hospital for sutures.

EE was jacking up belt head with a chain ratchet. The chain broke with EE holding the chain. The jerk caused him to twist his lower back.

On April 22, 2010, the mine experienced an accident when water came from a overlaying mine (#2gas seam) and began flooding the magnum mains inby the mouth of section. The mine experienced an inundation of water causing all mining on the round bottom number 1 section and the miners to be withdrawn. Federal and Stated agency were notified immediately

An unplanned roof fall occurred in the #5 entry on the #1 section @ Shonk Round Bottom. The fall occurred in the #5 entry spad #3989-4002. Fall measured 20' long x 17-18' wide x 4-8' thick. Area was previously supported by 5' Torque tension bolts with 8' cable bolts in intersections. This fall occurred 500' from the active 001-002 section. Area was originally developed 12/23/2010.

A unplanned roof fall occurred in the #6&7 crosscut on the 001-0 section, fall occurred spad #1706, 7600 feet from active face . Roof fall was in the intake, ventilation and passage of person was affected. Area was developed 4/08 and roof support consisted of 5' torque tension and 12' cable bolts all entrance have been breakered off fall measured 240'LX20'WX6'H

Shonk #1 belt which is an overland conveyor caught on fire at the take up area. No injuries. Caused extensive damage to conveyor system.

An unplanned roof fall occurred in the #2 entry @spad #3569, approx.4,000'from active face.Roof fall measured 80' long X 20' wide X 7' thick. Fall occurred in the return airway. Airway was not affected and will not be rehabilitated. State and Federal agencies notified immediately. This area was developed 9/15/09, area had been supported by 60" Torque Tension bolts.

On 9/9/09 at 8:30pm an unplanned roof fall occurred on the #1 section. Roof fall was in the #1 entry at Spad #3527. Roof fall measured 90'L X 20'W X 9-10' in thickness. This fall occurred 500 feet from the active face. Ventilation or passage of persons was not affected.

Bolting 6 Right, moved bolter up and fell going around the head of the machine. Fell on his right thumb and broke it. He refused an ambulance but went to the doctor and learned the thumb was broken.

Member bent over helping electrician work on miner when a piece of drawrock struck him in the back shoving him into the frame of the machine. The rock measured appx. 3' x 3' and less than 2" thick. Member did not cease work. On May 29, 2009 it was learned that he had seen a doctor and diagnosed with a hairline fractures to four ribs. He said he felt fine and refused to be off work.

Employee just finished bolting 2 right. He removed his gloves, placed his hand on top of the canopy and pulled himself upright to check bolt pattern. A rock, measuring 26" x 10" x 12 - 2 1/2" fell a distance of 18 inches, striking member's right hand. (12 stitches)

Employee stated he was carrying a belt splice board to the mantrip, turned to the left and heard 3 "pops" in his right knee.

Member was carrying crib blocks from scoop bucket to miner. While carrying cribs member slipped on a rock and twisted his right ankle. Member drove himself to hospital. Follow up with orthopedic surgeon on 11-21-08.

Employee was riding into the mine on a track mounted mantrip. When the mantrip reached the section, the brakes failed to hold and it rolled back down the track and jumped track causing a fracture to his back.

Brookville battery powered manbus arrived on #2 section. Operator hit e-stop & knocked power. Bus started to roll & operator did not re-energize machine. Bus traveled down grade for approx 400' where it derailed in a curve & hit rib head-on. Injured ee, a passenger, was transported to CAMC where he received six staples to close a cut to the head. He lost no days due to his injury.

An unplanned roof fall occurred at the number 3 track entry and number 2 scoop raodway, 1 break outby spad 1553.

An unplanned roof fall occured in the #4 entry near spad #1837 fall measured 20' wide by 80' long by 6' high no persons or equipment was involved ventilation was not affected roof was supported with 5' 3/4" fully grouted torque tension bolts. Fall will be rehabilitated.

While moving the continous miner on #1 section in the #5 entry. EE became pinned between the boom of the cont. miner and coal rib. EE was transported to ER where he was diagnosed with a collasped lung and contusions, x-rays revealed no broken bones and his condition is stable.

An unplanned roof fall occured just inby spad #1845 in the #5 entry the fall measured 150' long by 20' wide by 5' high no equipment or persons were involved ventilation was not affected a clean up plan was submitted and the fall has been rehabilitated.

An unplanned roof fall occurred in the #2 section inby spad # 1747. Fall measured 70' long X 20' wide X 8' high. No equipment or persons were involved and ventilation was not affected. The fall will not be rehabilitated, all approaches have been breakered off.

An unplanned roof fall occurred at break 151 spad 1630. Fall measured 80'L by 20'W and 4-8'H. No persons or equipment was involved. Ventilation was not affected,a clean up plan has been submitted to MSHA.

EE was bolting top in 3-R crosscut he had lowered his drill steel from the hole and was reaching to get it when a piece of slate measuring 12"Wx31"Lx3" thick fell from the roof stricking him in the left forearm, causing a laceration that required 10 stiches.

Employee stated that while lowering the canopy on the roof bolter he was not paying attention until the canopy struck him in the head. He was diagnosed with post-concussion syndrome.

An unplanned roof fall occured at # 1 Section # 5 Entry near Spad # 863. Ventilation and passage of persons were not affected. No equipment was involved and no personal injuries reported. Fall measured 30' in length, 20' width and 4' high. All approached have been breakered off and will be rehabilitated.

Member was lifting a drill pot on a flat car when he slipped and fell striking his left hand on the ground resulting in a small fracture to his left wrist.

Member was pushing a scoop tire by hand to be put on scoop. The tire fell over and pinched his right pinky finger in between the scoop tire and the fender of the scoop, causing him to receive a small fracture to the tip of his finger.

Employee was drilling into a 2"x4" piece of lumber when the drill bit hung up in a knot in the wood causing the drill handle to spin, catching his hand in a pinch point.

Member was preparing to fuse two pieces of pipe together when the piece he was holding slipped free and flew up and struck him in the face causing a small laceration just under his nose which needed 3 stitches to close.
