Employee was using a swivel socket to remove bolt. The pin came out of the socket and struck employees left index finger and thumb.

EE was helping to move the forks on a fork loader his hand was caught between the forks and the back board of the loader.

Walking off push beam in table, put his left hand on table for balance. When he stepped off push beam a piece of metal broke off in employee's left palm.
EE was putting the belly pan back on the miner when the lifting eye broke off letting the belly pan drop approx. 16" landing on the employees legs.
