The injured and helper were attempting to move and load a 4' diameter shop fan when the fan housing swiveled on the fan base pinching the tip of the little/pinky finger on the right hand that required sutures.

An unplanned roof fall occurred outby the idle #2 section on 7/16/2012. There were no injuries or equipment damage due to the fall and there were no escapeways blocked. The fall was in the return and an alternate airway has been established. This is an idle section and the equipment has been moved outby to the #16 tailpiece.

The injured employee was cleaning off the miner when a rock fell from the top striking him in the head above the right ear causing a laceration that took 5 staples to close. The employee also struck his right elbow on the machine and x-rays were taken and results were negative.

An unplanned roof fall occurred at the Imperial Mine on 4/26/12 on the #2 section in the face of #5 entry intersection and through the crosscut 5-6 at spad #8803. There were no injuries or equipment damage due to the fall. The fall will be cleaned up according to the company fall clean up and securing plan. Imperial will also follow guidelines put in place by MSHA & the WVOMHS&T.

Employee was cleaning front windshield on the loader when he went to take a step and foot slipped and fell. Foot slippped off 90 degree fitting on hoses in center of loader. His foot slipped down about 1' and badly twisted right knee.

The area around the fall has been supported by cribs & timbers and all approaches have been dangered off. The fall dimen. are 27' L. X 17' W. X 8' H. This area was dev. on 2/27/11 with approx. 180' of cover.

An unplanned roof fall occurred on 8/29/11 in the #2 zone #3 entry intersection and through the 3-4 crosscut at spad #699. There were no injuries or equipment damage due to this fall. This area was developed in 12/2006.

While roof bolting the face of the #8 entry employee was preparing to drill a hole. The drill steel was placed against the mine roof and a piece of roof rock fell striking the employee in the chin causing a laceration which required 4 stitches to close the laceration.

Employee was installing a roof bolt and during the rotation process a small slither of metal off either the bolt or the bolt plate became airborne and struck the employee in the right eye.

EE was prying a rock off the miner with a slate bar when the rock broke off causing EE's hand and slate bar to slam into the miner smashing his thumb. EE went to the doctor and had an X-ray done which revealed 2 small bone chips on the knuckle of his thumb.

EE was kneeling down to work on a power center when a rock from the top fell striking EE on the head above the left eye. EE continued to work the rest of his shift and went to the doctor and received 9 stitches to close the wound.

An unplanned roof fall occurred in the 1st left off old #11 belt 3 blks inby spad # 710 in the old face area. This area was developed on 12-15-06 with approximately 160' of cover and the roof is supported with 6'fully grouted resin bolts.

Employee hit middle finger on right hand while trying to get pie pans apart and cut finger. This accident occurred on 11-18-10 but did not become restricted duty until 12/1/10.

An uplanned fire occurred at Imperial on the surface when a contractor owned front end loader caught fire in the cab of the machine. The fire was put out by mine personnel using fire extinguishers. Fire departments responded to the scene. The loader was not in use at the time of the fire.

There were no injuries or equipment damage as a result of the fall. There are no plans to clean up the fall however all approaches around & leading to the fall will be dangered & timbered off.

A unplanned roof fall was dicovered at 10:10 a.m. on 9-3-10 by the Mine Foreman in the # 5 zone return at # 11 blk in the 2-1 crosscut. There were no injuries and the fall did not block an escapeway or a regular travelway. All approaches to the fall have been dangered off and timbered and additional timbers have been set around the fall area.

EE was using a hammer to strike a drill chuck on a roof bolter trying to free it up when a piece of metal for the chuck broke off became airbourne and embedded into his left hand. Ben was seen a St. Joseph's Hospital and surgery was required to remove the piece of metal from his hand.

A roof fall was discovered at approximatley 12:30 p.m. on 5-24-10. The fall was approximatley 20' long x 20' wide and 6' high. Exact location of the fall is: # 8 entry, #2 zone , #62 block crosscut 1 crosscut inby spad #447. Mine Foreman discovered the fall.

Injured was standing in an upright position and was reaching above his head to hang a ventilation line curtain, when he fell back against the continuous miner which caused a pain in his back. Injured continued to perform his normal duties, without restrictions, and began to perform restricted work on 12/28/2009 making it reportable on that date.

Employee was in an upright position operating a Fletcher DDO-15 Roof Ranger Roof Bolter, when he slipped on a piece of rock on the mine floor. In an attempt to break his fall, he extended his right arm out and when he landed, he received a strain to the right wrist.

Employee was extending the ventilating curtain in the working face. He was reaching upward to attach the line curtain to a curtain hanger that was attached to a roof bolt that was through a pizza pan. As he hung the curtain on the hanger, the back side of his left ring finger came in contact with the hanger causing a laceration to the finger requiring 5 sutures to close.

He had completed drilling a hole and was bringing the drill steel out of the hole when a small piece of roof rock fell and struck him on the right side of the upper lip. The wound required 3 sutures to close. He returned to work on his next scheduled shift without restrictions.

The injured was operating a radio remote controlled continuous miner when a rock fell inby the last row of roof bolts, it rode through the last row in a manner that it struck the injured as he truned to get back, resulting in an injury to his neck and a fracture to his left ankle.

Employee and co-worker were removing broken pins from feeder/crusher shear pin plate with punch and hammer. When injured attempted to strike punch, being held by co-worker, he missed the punch with the hammer and struck the punch with the index finger of his right hand, causing a laceration that required 9 sutures to close. He returned to work on his next scheduled shift.

End loader operator had got off the loader to move the coal stacker. When he was walking back to the loader he fell on the frozen, slick ground. When he was uprighting himself, he struck his head on a metal storage rack causing a strain to his neck.

A previous fall had been supported with a tunnel liner. More rock fell from the previous fall cavity and caused the existing liners to bend, blocking the secondary escapeway. Area was last traveled at or about 5:00 AM.

Employee was operating a Fletcher Roof Ranger on the left side. Upon completion of drilling he was in the process of removing the drill steels from the hole when a small rock fell from the roof and struck him atop the left hand causing a fracture to the metacarpal bone.

Employee and co-workers were installing a 400? piece of mine belt to the number 11 belt. The employee was in a squatted position, on his knees, cutting the belt by pulling the knife toward him. As he pulled the knife, it came from the belt and struck him in the right thigh causing an incision.

The employee was in a bent over position, with his knees slightly bent, getting ready to pick up the continuous miner cable, when he felt a pain in his lower back. The employee had not begun to lift the cable. He told his co-workers and supervisor that his back was hurting and continued to work the remainder of the shift.

The employee was standing on the right side of the Fletcher DD0 15 Roof bolter. As the bolter trammed past him to set up to begin a row of bolts, the right front tire ran over the side of a rock that was laying on the mine floor, measuring approximately 10? Long by 10? Wide by 5? thick, causing the rock to become an airborne projectile and struck the employee.

He was striking the bit insert with a hammer when a sliver of metal came from the metal insert and lodged behind his right knee cap. He was treated at St. Joseph's Hospital in Buckhannon, WV. The metal was removed by St. Joseph's Hospital

Employee was tramming a Joy 14cm-15 radio remote continuous miner into the face of #6 entry. He stopped the miner and bent over to handle the miner cable when a rock approx. 19'w x 30"l x 1/2" to 2 3/4" thick fell from between two roof bolts and struck him on top of the head, which was protected by a hard hat.

Employee was platering a stopping. He stopped to get a drink from his dinner bucket. When he returned to the work site to complete the plastering of the stopping a rock approx. 18" long x 10" wide x 6" thick fell from the 8' high mine roof and struck him on the head and neck.

Employee was walking in an entry where a shuttle car was parked. As he began to cross the shuttle car cable by beginning to step on the cable, the shuttle car operator simultaneously started the pump motor on the shuttle car, causing the cable to quickly come upward from the mine floor. The cable struck the bottom of the injured's foot.

Employee was cleaning the #8 entry with scoop. He got out of the scoop to roll up a line curtain when a rock appr. 2' by 2" fell from between the right rib and the 1st bolt on the right side, striking him atop the right foot, causing a fracture to small toe and next toe.

EE was attempting to hang a line curtain on a previously installed curtain hanger nail that protrudes from under a roof bolt plate. The top was high so he placed the line curtain in his hand and jumped upward to place the curtain on the protruding nail. His right index finger contacted the protruding nail causing a laceration that required 7 sutures to close.

The operator started to tram forward and at the same time he steered the scoop to the left, which caused the injured's lower legs to be caught between the batteries on the scoop and the batteries on the mine floor. This resulted in contusions to the calf area of both legs.

Employee was helping unload supplies from a scoop onto a roof bolter. When he placed the oversized plates across the roof bolter they came in contact with long boards already laying on the bolter and one of the boards came in an upward direction and struck the employee on the nose. The wound was closed at the hospital using an adhesive.

Employee and co-worker were beginning to bolt first cut out of crosscut. Employee was on helper side of roof bolter. ATRS was set lightly against the roof. Injured asked operator to back bolter up 2-3 inches. When he did a rock fell from the roof and struck the employee on the upper back and knocked him down causing him to strike his left knee against the mine floor.

Employee was putting plastering material on a stopping. To reach the top of the stopping, the employee was standing on two (2) eight inch cinder blocks. The employee slipped off the blocks causing a pain in his lower back. He went to the Dr. on 6-18-07 and x-rays were negative to any fractures and was prescribed restricted duty.

Employee had previously visually inspected the roof. A ventilation line curtain blocked his view of the rock dislodging and falling.

He had completed drilling the rib side hole in the roof and was reaching back to get his roof bolt when a piece of rock measuring 25"LX18'WX2"T fell from the roof and struck his right forearm. X rays negative. Employee later fainted making it an emergency.
