An unplanned roof occurred in the intake air-course/intake escape way one crosscut from spad #44. The fall is the entire width of the entry. Height - unknown. There were no injuries or entrapment. Blocked primary escape way but escape way has been rerouted.

An unplanned roof fall occurred, there were no injuries or entrapment. The location of the fall was in the return airway at the #11 & #12 northeast main seals. The #11 seal was completely blocked. The measurements of the fall were approx. 18' wide, 14' high at a length of 60'.

Employee was standing in intersection watching a continuous miner water line being pulled through a hole in bottom of a stopping. The end of the water line at a coupling caught. The water hose suddenly became free and struck the employee on the left lower leg.

Employee had bent an eight foot resin bolt due to seam height. The roof bolt would not seat in drill chuck. He hit rotation and the roof bolt struck him in the right wrist resulting in a fracture.

Employee was cutting a ventilation curtain that was around the continuous miner head. The knife he was using cut through the ventilation curtain and struck the employee in his right leg above the knee. He received 4 sutures and returned to work at full duty 4/30/2011 (no days lost).

A panel of mesh wire (screen) had stuck to another piece, the other roof bolter operator jerked on wire to free. When wire camed loose it had an upward movement and struck the injured employee in the face, knocking off safety glasses and hard hat. The result was a scratch to the left eye.

Scoop operator did not see section foreman because the foreman was bent down picking up a notepad. Battery tray end of scoop struck foreman and fractured left femur, pelvis and ribs.

EE was assisting another member in changing a broken water coupling. EE was reaching for the broken piece of coupling to move it out of the way and struck top of his hand on broken edge causing a laceration to the top of his left hand.

Employee had almost completed loading a shuttle car. A rock measuring 91" x 13" x 11.5" fell from top and landed on the boom of the miner. It traveled the conveyor to the shuttle car. Since the car was almost loaded the rock slid off the side of the shuttle car and struck employee in the left leg.
