Unplanned roof fall occurred outby Spad #1118 and one crosscut outby #1 seal. Fall was approximately 35 feet long X 19 feet wide X14 to 16 feet in height. Unplanned fall occurred in the number 7 entry. No injuries or entrapment involved. No escape way, ventilation, walkway, or equipment involved. The unplanned roof fall was on northeast mains.

Unplanned roof fall occurred #7 entry at Spad 2121 and extending inby 1 crosscut. Fall was approx. 19' W. x 80' L. x 10' H. No injuries, entrapment or Eq. involved. No escape way, ventilation or travel way affected.

Employee was standing in intersection watching a continuous miner water line being pulled through a hole in bottom of a stopping. The end of the water line at a coupling caught. The water hose suddenly became free and struck the employee on the left lower leg.

Unplanned roof fall occurred at bottom of slope #4 entry at spad #206 track entry. The fall was approximately 30 ft. long x 18-19 ft. wide x 15 ft. high. No injuries or entrapment or equipment involved. No ventilation involved. Secondary escapeway blocked and it was changed around the fall.

Unplanned roof fall occurred #6 Belt Intake at spad #1423. Fall was approximately 100 ft. length x 20 ft. wide x 8 ft. height. No injuries, entrapment or equipment involved. No ventilation, escapeway, or travelway involved. Area was developed 11-30-09. Fall was 4600 ft. from face.

An unplanned roof fall was discovered 3A Belt Intake at spad 1127. Fall was approximately 80 ft. length x 20 ft. wide x 8 ft. height. No injuries or entrapment was involved. No ventilation, escapeway or travelway involved. Area was developed 10/22/08. Fall was 3800 ft. from section.

Unplanned roof fall occurred #1 entry at spad 1108. Fall was approximately 150 ft. length x 18 ft. wide x 10 ft. height. No injuries, entrapment or equipment involved. No escapeway or travelway involved. Ventilation partially blocked in return. Area was developed 02/17/09. Fall was 3600 ft. from face.

Unplanned roof fall occurred at #10 c.c. on the #3 belt the c.c. to the right at spad #241 between entries #2 and #3 it is approx. 40' long, 8 to 10' high and 8' - 10' thick. No injuries or entrapment involved. However it did fall on a diesel forklift. No ventilation or travel way involved.

Unplanned roof fall occurred break #13 #6 entry - in old #6 belt panel. Inby spad 1750. Approximately 20 ft wide X 20 ft long X 18 ft high. No injuries or entrapment involved. No ventilation, equipment, escapeway or travelway involved.

Employee stated that he stepped down from conveyor belt line and his knee twisted sideways. Nothing on mine floor at the area, he stated his knee just went sideways. He continued to work however pain continued and he ceased work.

An unplanned roof fall above the anchorage zone occurred measuring approximately 14ft high x 80ft long x 19ft wide in the right return entry one break inby spad 1876 of #6 a belt. No injuries, entrapment or equipment involved. Ventilation was affected on section no escapeway was affected and travel way was blocked.

Employee was cleaning along 4A belt line on the offside. He picked up a piece of rock weighing approximately 10-20 lb and twisted around to throw on conveyor belt line when he felt pain in his lower back.

An unplanned roof fall occurred (extension of old roof) at Spad# 828, 3A belt in #7 entry. Fall was approx. 12'h. X 19' W. X 40'L. No injuries, entrapment or equipment involved. No ventilation ,travel way or escape way involved.

Ventilation involved by 1 Kennedy panel was blown out in Kennedy stopping which short circuited a portion of intake into area of old works - MSHA has investigated

An unplanned roof f all has occurred at brk 33 of 4 belt in the #7 entry. The fall approx. 70' L. X 17' H. X 19' W. There were no injuries involved.

An unplanned roof fall has occurred at brk. 39 on 1st N.W. Panel. The nearest spad to this location is #1322. Fall was approx. 17' high x 18' wide x 50' long, there was no injuries involved with the fall.

Employee was walking in walkway along the track when he tripped and fell landing on his knee against track rail. The result was a fracture to his knee cap.

An unplanned roof fall has occurred at brk. 3 of 4A belt in the #1 entry. The nearest spad to this location is #1829. Fall was approx. 16' high x 18' wide x 25' long. There was no injuries involved and the return was re-routed around the fall area.

An unplanned roof fall has occurred at brk. 7 of 4A belt in the #3 entry. The nearest spad to this location is #1592 approx. 100' outby the fall. Fall was approx. 14' high x 18' wide x 25' long. There was no injuries involved with fall and it did affect ventilation or travelways.

Employee was shoveling loose coal from mine floor onto 4-A belt conveyor line at number 6 break when a piece of coal rolled from the inby corner, on the walkway side of the belt. It struck him on the left leg just above the ankle.

An unplanned roof fall has occurred at brk. 16 of 1st S.W. Panel. The nearest spad is #1141. Fall was approx. 25' long x 18' wide x 12 to 15' high. There were no injuries involved and the fall did not affect ventilation or travelways.

An unplanned roof fall has occurred at 34 brk. in the main return air course 40' outby the return overcast in the #2 entry at spad #1271. Air course was for both #1 and #2 sections the fall measured 17' high x 19' wide x 20' long there were no injuries involved.

An unplanned roof fall has occurred at brk. 18 of 1st S.W. Panel. The nearest spad is 50' inby fall spad #1158. Fall was approx. 25' long x 18' wide x 12 to 15' high. There were no injuries involved and the fall did not affect ventilation or travelways.

Employee had bent an eight foot resin bolt due to seam height. The roof bolt would not seat in drill chuck. He hit rotation and the roof bolt struck him in the right wrist resulting in a fracture.

Employee was cutting a ventilation curtain that was around the continuous miner head. The knife he was using cut through the ventilation curtain and struck the employee in his right leg above the knee. He received 4 sutures and returned to work at full duty 4/30/2011 (no days lost).

Two employees were working on buggy seat (raising seat up). IE was leaning in pulling on seat so co-worker could put pins in. He felt something in back pop.

EE was walking to the #1 entry and walked into a roof bolt hanging from the top and hit the top of his head. The area was dry and the height was around 8-9'. The bolt hanging down was 67" from the floor of the mine.

In the intersection at brk. 41 of old #4 belt in the #1 entry. The nearest spad to this location is #1590. This spad is approximately 80' outby the fall.

Employee was putting a rub rail back on operators side of shuttle car and pinched his middle finger on his left hand between rub rail and tram motor. Result was fracture to end of finger. No days were lost due to the incident, returned to work at full duty. Medical reportable only.

A panel of mesh wire (screen) had stuck to another piece, the other roof bolter operator jerked on wire to free. When wire camed loose it had an upward movement and struck the injured employee in the face, knocking off safety glasses and hard hat. The result was a scratch to the left eye.

An unplanned roof fall was found in an area where persons do not regularly work or travel. Between break #4 & break #5 of 1st NW Panel in #2 entry (spad #851 appr. 60' inby fall). Fall was appr. 70'L x 19'W x 12 to 14'H. No injuries or entrapment involved. No ventilation, escapeway, walkway or equipment involved. Fall will not be cleaned up, will be breakered & dangered off.

An unplanned roof fall was discovered a break #29 of 1st NW Panel in the #1 Entry at spad #1213. Fall was approximately 75ft in length X 19ft wide X Approximately 12ft to 14ft in height. There were no injuries , entrapment or equipment involved, no ventilation escape way or travel way affected. Supplemental support was installed in area and fall will not be clean up.

Pinsteel steel stopped up, tried to get steel down, had left hand on drill pot, steel fell out striking him on the back of hand.

Unplanned roof fall occurred in North East Main Panel at BK#10 in the #7 entry spad #239. Fall was approx. 112' L. X 19' W. X 12-14' H.Date of development was 10/07. No injuries, entrapment or equipment involved.

Unplanned roof fall occurred in Sub-Mains "A" panel off of 1st NW panel BK #4. Fall was approximately 25' in length x 19' wide x 14' to 16' high. Date of development was May 2009. No injuries, entrapment or equipment involved.

Unplanned roof fall occurred in BK #34 of 1st NW panel Fall was approximately 40' long x 19' wide x 14' to 16' high. Date of development was January 2009. No injuries, entrapment or equipment was involved.

Unplanned roof fall occurred in 1st N.W. Panel in #9 Entry at spad #1414 (brk 4 of #4 entry belt line in the right return entry). Approximately 90 ft. length x 19 ft. wide x 14 ft. to 16 ft. high. The date of development was November 2008. No injuries, entrapment or equipment involved. Stoppings will be re-routed to re-establish the right return air course.

Employee was assisting in installing bottom belt roller. Another employee picked end of roller up & pulled EE slightly forward straining his back.

Employee was helping hang water line. When he pushed water line to roof, his left thumb came into contact with pizza pan and caused a laceration.

Section foreman was bent down in the #4 entry, approx 4' inby the outby corner, next to the right rib. Scoop operator was backing into #4 to clean entry in front of feeder. Scoop operator did not see section foreman because the foreman was bent down picking up a notepad. Battery tray end of scoop struck foreman and fractured left femur, pelvis and ribs.

EE was assisting another member in changing a broken water coupling. EE was reaching for the broken piece of coupling to move it out of the way and struck top of his hand on broken edge causing a laceration to the top of his left hand.

EE was next to the rib in the process of cutting the bottom off of flypad when a rock measuring 9"x6"x3" rolled out of the rib and struck him in the back of the head.

An unplanned roof fall occurred in the #2 Gas Mine at spad # 445, break # 35 that measured approximately 90' long x 18' wide x 15' high. No injuries were reported. The area was developed on 2-6-08.

Employee had almost completed loading a shuttle car. A rock measuring 91" x 13" x 11.5" fell from top and landed on the boom of the miner. It traveled the conveyor to the shuttle car. Since the car was almost loaded the rock slid off the side of the shuttle car and struck employee in the left leg.

EE and another member were changing a tailshaft on a C/M. EE had one end of the tailshaft in place, while setting the other end the tailshaft slipped out and fell striking the middle and ring fingers on left hand.

EE was in the process of moving cables during a power move. He was on the 3rd step of the ladder when he was struck by the cables. He fell onto his left side fracturing 6 ribs.

Member was helping hang miner cable. A piece of rock fell from top, struck his hard hat, causing safety glasses to make a small laceration on bridge of his nose and fracturing right side of nose.

An unplanned roof fall occured in the #2 gas mine at 49 brk, one brk. outby spad #550. The fall started in the #1 entry and continued across to #6 entry. The fall measured 425' long x 20' wide x 10-15' high. No injuries were reported. The area was developed 3/25/08.

An unplanned roof fall occurred int he #2 gas mine, in the #2 entry near spad #497. The fall measured 30' in length, 28' width and 11 to 14' in height. No injuries were reported. The area was developed on 3/5/08.

EE was helping hang a 20' joint of 6" water line. EE was holding water line over his head while another EE was hanging chains around it. The waterline came dislodged from coupling and it fell stricking EE on the head causing injury to his head and back.

Member was pushing 16' J channel straps onto roof bolter. His right hand slipped, causing a laceration that required 5 sutures.

Employee was helping move cable. When he bent over to pick up the cable, a rock that measured 8" long, 1.5" thick and 4" wide fell from between a row of bolts and struck him on the head behind the left ear causing a laceration.

Member was picking up some metal that had sharp edges. Hand slid across edge and cut little finger.

Member was cutting a piece of plastic tubing with a razor knife. One end of the tubing was resting on leg and knife slipped off and cut side of right thigh.

An unplanned roof fall occurred at spad 34. Fall area developed December 2006. Approximately 4000' outby the working section. Fell in #2 entry. Fall measured 23'W x 30'L x 8'High. No accidents reported.

Member was standing/walking by right rear of the machine when the bolter operator started tramming the machine forward, the bolter tried to climb over a rock which skewed the machine caddy corner, pinning him into coal rib.
