While conducting repair work on the feeder chain located on the Workman Creek Coal Handling Facility, injured was attempting to use a metal bar when the bar slipped striking the left eye brow, resulting in 6 stitches.

Employee was installing a bolt for a shuttle car anchor when EE lost EE's footing and fell resulting in a bruise and soreness. EE continued to work until 11/03/2019 when EE was taken off work due to infection that resulted from the injury.

Employee was taking a scoop off charge. As he was trying to lower the battery lids, the lid got caught on the fire suppression rack above the batteries. When he tried to lift the rack out of the way the lid fell on his right hand crushing his index finger.

While Employee lifted lid to the shuttle car cable reel the SC operator pulled forward causing the lid to fall smashing Employee's left ring finger. Employee was transported to Raleigh General x-rays were taken and no broken bones. Employee was scheduled for surgery at CAMC Memorial Division for skin grafts to repair the finger.

He was driving shuttle car from the continuous miner toward feeder, attempted to readjust himself and tires hit hole in roadway causing him to hit canopy with hardhat jamming his back. He continued working 2 weeks, went on vacation for 1 week and began missing work on 7-8-2013. Company conducted investigation to confirm validity and found it valid on 7-18-2013

Employee was loading 60lb rail off of a forklift onto the rail carrier. Forklift would not raise high enough to clear the standards on rail carrier due to height. Employee was lifting one end of rail at a time when rail fell smashing his left ring finger. Employee was treated at Raleigh General and was diagnosed with a slight fracture and received six (6) sutures.

Employee attempted to hook chain on spreader bar. He grabbed the slack end of chain trying to pull chain to get fully hooked. He pulled chain and the chain slipped out of spreader and caught his left index finger in link of chain. Employee was treated at CAMC General Division where he underwent surgery to repair tip of finger. Employee was diagnosed with a crushing amputation.

Employee was pulling cable out of gob. As he bent over to pull cable again he felt his back pop and fell to the ground. He was transported to Raleigh General Hospital where he was diagnosed with muscle spasm in his back.

An unplanned roof fall occurred at survey spad 3106 #3 entry inby dumping point. The roof fall was approx. 40'x18'x7'. The initial 103(j) Order was modified to a 103(k) Order. Both Federal and State agencies approved a clean up plan to rebolt and clean roof fall. On June 17 the 103(k) Order was lifted to resume normal production. No injuries was reported or no equipment involved.

Employee was building a stopping in #6R across from feeder. A piece of rock broke loose from the rib striking employee on the right shoulder and right hand. Employee was transported to Raleigh General Hospital. Xrays were taken and was told to follow up with a specialist for further evaluation.

As employee was moving the rub rail on a 14CM15 Continuous Miner he felt a pulling sensation in the lower left abdominal area. Employee was diagnosed at Med Express with a hernia on 5-17-11. Employee returned to work on restricted duty from 5-18 to 5-24. The hernia was repaired on 7-27-11 which the Safety Department was not notified of his surgery until 8-3-11.

Employee and helper was removing a rock off of the roof bolter. As Employee pulled the rock it fell striking him on the right foot. Employee was transported to Raleigh General Hospital by Whitesville Ambulance. Employee received xrays and CT scan. He was diagnosed with a contusion on the top right foot. Employee was given restricted duty until the swelling went down in his foot.

Member walked up and asked the miner man how much more he lacked so he could come across and clean that side of the section and the cut where the miner was fell in on the miner and a piece of rock slid down and hit member on the ankle. He was taken by ambulance to Raleigh General where he met Analabs for a drug screen and was scheduled for surgery on 1/29/2011.

Member was putting a pump on the 488 scoop. The relief valve was tied back and it came loose and smashed his finger between the pump and relief valve. Member immediately ceased work to go to Whitesville Clinic and had 3 stitches put in his left hand, ring finger.

An EE was running the scoop and scooped up the pressure pump in order to move it. The door on the scoop deck was not shut and injured EE went to shut the door. Other EE thought that IE had completely moved out of the way of the scoop, but he had not. Other EE began backing up the scoop bucket and hit IE and pinned him against the right rib. He sustained injuries to his knee.

Employee was attempting to install a U joint in a shuttle car drive shaft. While using a large screw driver to pry it into the yolk he slipped causing his right hand to hit the frame resulting in a fracture to the tip of his right ring finger.

When attempting to raise the top cover of the belt tail piece, it slipped out of employee's hands causing it to fall on his left hand that resulted in an injury that required amputation of the distal phalange of his left ring finger.

A fall of the mine roof was discovered on 6/3/08 at approx 11:45 am. The size of the fall is 80 ft long x 19 ft wide x 10-15 ft high. There were no injuries and no airways blocked.

Member was picking up trash after building stoppings. He put some of the plastic bands that hold cinder blocks together in a plaster bucket. One of the bands popped out of the bucket and went behind his safety glasses and scratched his eye.
