Employee was going to pull the miner cable. When he reached down to get hold of the cable, his left thumb was cut on a discarded razor knife blade. Cut required 3 stiches.

Employee was operating a shuttle car when his hat was pulled off his head as he crossed under a cable hung from the roof. This startled him and caused him to turn too sharply as he attempted to go into the crosscut. His left shoulder came in contact with the rock rib.

The employee complained of back pain after lifting and turning and doing normal work of a scoop operator on March 3. He continued to work through April 27th without incident. He began missing work on April 28th due to back pain. He has a history of back problems. This is an occupational illness from repetitive motion trauma.

Employee was operating a small battery-powered vehicle. The vehicle struck a bag of rock dust, causing the steering wheel to spin and the steering knob to strike the employee's right hand.

An unintentional roof fall was discovered in the No. 9 return entry. The fall was near spad no. 3952 and was approximately 18' wide x 20' long by 4' thick. The area had been previously bolted with 42" fully grouted bolts. The fall will not be cleaned up as the return was routed to the adjacent entry.

The employee had just finished adjusting the ventilation curtain preparation for minng. He crawled outby to the right inby corner of the intersection and was on his knees talking with another employee. A piece of roof rock measuring approx. 60"x60"x1-1/2"to 3" fell at angle from between two rows of bolts. The rock struck the employee on the back and shoulders.

Employee was assisting in the removal of a planetary assembly from the continuous miner. He was attempting to let pressure off a hydraulic "grease jack". He loosened a grease fitting on the jack and it blew off, striking him on the forehead.

A roof fall was discovered during an examination at the mine. The fall was located in the return aircourses on the 3 right panel. It measured approximately 30' in length, 20' wide and 4' thick. The area had been previously bolted with 42" fully grouted bolts. There were no injuries, no damage to equipment, and ventilation was not affected.

As the continuous miner was completing a pushout, a piece of roof rock fell near the pushout and fouled the machine's cutter head. This prevented the continuous miner from being readily removed from the area. Sometime later, as the machine remained in this position, additional roof material fell onto the continuous miner. There were no injuries.

Employee and a co-worker were using a chain hoist to pull belt, when the hoist hook pulled loose and the chain and hook recoiled, striking injured on the right side of upper torso.

The injured employee was positioned alongside the roof bolting machine operator's deck and had finished conversing with the operator who was in the deck. The operator started the machine and the injured employee was caught between the deck of the machine and the rib.

The roof sagged against the frame of the roof bolt machine in the #2 face for a span of approx. 12' in length and 12 to 14' in width. An area of approx. 8' X 19' wide of unsupported (unbolted) roof fell directly in front of the machine at the face. (After the initial call-in report, further investigation revealed this was not a reportable incident.)

Injured and a coworker were riding a battery powered 4 wheeled vehicle in the primary intake entry. The employee was operating the vehicle. As they were traveling, the hood of his jacket snagged on a metal roof support strap and this pulled his head toward the roof. His ear contacted something on the roof resulting in a laceration.

A fall of roof rock occurred in the No. 2 to No. 1 intersection near spad location 2032. The fall measured approx. 25' long x 18' wide x 4' thick. The area had been supported with 42" bolts. This area had previously been cribbed off. There were no injuries, no effect on ventilation and no equipment involved.

Employee was trying to pull stuck drill steel from the drill hole. The steel suddenly came loose out of the hole and he caught his left little finger between the steel and the drill pot.

A roof fall was found in the intersection of No. 4 entry one break inby spad location 1850. The fall was approx. 20'x 20'x 4' high. There were no injuries and no equipment damage. Ventilation was not affected and travel was not impeded. It was above the anchorage zone.

A ROOF FALL OCCURRED NEAR SPAD LOCATION 1869 IN #7 ENTRY. FALL WAS APPROX. 75' LONG X 20' WIDE X 4' THICK. THERE WERE NO INJURIES; NO DAMAGE TO EQUIPMENT. THE FALL WAS ABOVE ANCHORAGE. IT DID NOT AFFECT VENTILATION OR IMPEDE PASSAGE.

Injured was working at #3 tailpiece aligning belt structure when the belt structure separated causing him to mash his right middle finger. This resulted in a non-displaced fracture of the finger.

A roof fall occurred in the #5 entry approximately 15' inby 1615. The fall is about 70' long 20' wide and 5' high. The fall will not be cleaned up at this time. The area was cribbed off prior to the fall.

Superisor was assisting in installing a ripper motor on the miner when the motor slipped, causing his hand to be caught between the motor and miner frame. He sustained a fracture to the ring finger on his left hand and received medical treatment on his finger on 6/28/05.

Employee was attempting to tighten lug nut on a 3 wheeler when the ratchet slipped causing his hand to strike the fender of the ride. He was diagnosed as having a non-displaced fracture to his hand. He has not missed any work and is still performing mechanic duties. His work schedule is 4 on 2 off.
