While changing a gear case on the continuous miner the pin cap became hung. A belt hanger was welded to the cap and a chain ratchet was being utilized in an effort to remove the pin cap. After applying tension the belt hanger separated from the pin cap striking the injured in the face.

A 1990 Ford F-150 owned by the driver/mine employee and carrying one additional mine employee crashed when driver lost control after the truck's brakes failed. The employees' shift was over, they were leaving mine property, they were not performing any mine related work activities and the accident was not caused by mining conditions, activities, or equipment.

EE was fixing the parking brake on the #1 mantrip. While working, the shield fell and struck him in the back of the head, causing him to hit his face on the mantrip. The accident caused a minor laceration across the bridge of his nose.

Employee was in the process of replacing shield over the pump compartment on a shuttle car. He placed the shield on top of the frame of the shuttle car. The shield began to slide off, as he attempted to catch it he lost his balance and fell backwards on the ground holding the shield. He experienced soreness and tenderness in his groin, knees, and back.

He accidentally grabbed hold of the rotating drill steel causing tissue damage in his right shoulder. EE continued to work until going to Raleigh General Hospital on 06-22-10 and was told the damage would require surgery.

This is a worked out panel, requiring weekly exams approximatly 4000' from an active section. Location of the falls are at spad numbers 3650, 3653, 3688, 3831, and 3662. No injuries or equipment involved.

EE was backing a 35C Fairchild Scoop down the #3 entry along the #2A beltline. At the #7 crosscut there were five fly boards lying to the side of the entry. The rear tire of the scoop ran over one of them, causing one end of it to flip up and enter the operator's deck, hitting EE in the ribs and back pinning him in the deck.

While roof bolting EE attempted to shove a 48" resin bolt up he was holding on to. While spinning the bolt, he pushed it up. His glove became tangled, causing a fracture to four fingers. He refused emergency transportation and transported himself to Raleigh General Hospital Emergency Room where diagnosed with the fractures and referred to a surgeon.
