Miner was loading in a left handed cross cut which laid down hill toward the outby area of the mine. As the car left the miner, the car slid into the rib pinching the operator's left arm between the car and the rib.

While gathering tools from tool box, the lid fell to the closed position pinching the left index finger between the lid and frame of box. Resulted in a laceration requiring seven stitches.

While bending a roof bolt back out after installing in the drill hole, employee's hand slipped causing the roof bolt plate to lacerate the forearm. Stitches were required.

While installing roof bolts in the No. 8 Entry, miner was attempting to bend a 6 foot bolt when miner felt a pop in lower back.

While walking outby in the No. 6 Entry, miner hit a metal roof strap with left shoulder causing a laceration resulting in 10 sutures.

An unplanned roof fall occurred in the #48 Break along Horse Creek #9 Belt. The fall was approximately 20ft long x 20ft wide x 4/5ft in height.

While employee was walking back to the shuttle car, EE struck EE's head on a roof strap.

Employee was diagnosed with pneumoconiosis.

Miner was attempting to remove rock from the boom of the shuttle car when the rock tilted upward smashing right hand between rock and mine roof. Resulted in laceration requiring five stitches.

While attempting to line bolt head up with drill chuck, miner bumped rotation on drill chuck while holding the roof bolt. EE's glove was snagged and EE's fingers became entangled around the bolt.

Employee is diagnosed with cervical myelopathy.

Miner was getting supplies from the off side of the roof bolting machine when a brow from the right rib fell striking miner on back of lower leg.

While unloading supply car underground, miner was standing near underground forklift. While working with the forklift one of the forks became fouled causing the fork to bounce up when it became free striking miner in the head.

Miner was getting out of scoop, slipped and fell under scoop wedging EE's leg against a rock.

While swinging out drill station a rock fell hitting employee in the back of the head. Received stitches

Miner was walking around the forklift when EE slipped and fell injuring EE's left shoulder.

Miner was building stopping when a section of the rib rolled off striking EE's left arm and side. The impact caused the miner to stumble and bump EE's head on a scoop insert. Medical examination revealed a sprained wrist.

Miner was adjusting ventilation curtain when EE slipped and fell onto the mine floor causing injury to EE's shoulder.

Employee was cutting a piece of metal plate when the plate shifted and caught left arm between the plate and some chute work.

While changing a gear case on the continuous miner the pin cap became hung. A belt hanger was welded to the cap and a chain ratchet was being utilized in an effort to remove the pin cap. After applying tension the belt hanger separated from the pin cap striking the injured in the face.

Non Injury roof fall on 2c Track, No.4 Entry, No. 29 Crosscut (2 breaks inby SS 12703)

Employee was using hammer while conducting repair work on a scoop wheel, pinched pinky finger between hammer handle and wheel rim resulting in a laceration and five stitches.

Roof Fall on No. 1 Section, 3 Left Face, 2 Breaks inby survey station 14174

Employee was walking down the entry when ee rolled right ankle in a hole, causing ee to fall.

Miner was pulling a welding box with a scoop and a 3/8" chain. The chain hook broke and struck the miner on the left check. Resulted in a laceration with multiple sutures.

Employee struck head on the mine roof while walking across section. Began missing work on 09/11/2017.

While handling miner cable employee stepped backward striking the back of EE's head on a roof mat that was hanging down on the corner. Resulted in six sutures.

Roof fall was discovered on 07/18/2017 in the #6 Entry near spad 13344. Breaker posts were installed and all approaches to the fall were dangered off.

Employee was diagnosed with pneumoconiosis.

While making a belt splice, the injured miner was using fingers to feel the nail heads to ensure they were adequately seated in the splice while the other miner was still hammering. The injured miner's finger was struck by the hammer resulting in a fracture.

Crew was pulling a water supply line using a rope hanger. Water line pulled tight causing the rope hanger to break striking employee in the face just below the right eye. Resulted in a laceration and fracture.

Employee was handling shuttle car cable when employee's knee popped. Employee complained of pain and was unable to straighten leg out.

Employee was blocking up track switch with crib blocks. While maneuvering around the switch EE stepped on a crib block and turned ankle. After turning ankle, EE stumbled and struck knee on the crib block as EE fell.

Employee was unloading 5 gallon cans of gear oil from the bed of a pick up truck when employee felt pain in employee's shoulder and abdomen. Employee was diagnosed with a hernia and is currently scheduled to have the hernia repaired.

A roof fall was discovered, in the intake on 2C Belt at SS#12759. Length of fall-54 feet, Width of fall 50 feet, height of fall 10 feet.

A 1990 Ford F-150 owned by the driver/mine employee and carrying one additional mine employee crashed when driver lost control after the truck's brakes failed. The employees' shift was over, they were leaving mine property, they were not performing any mine related work activities and the accident was not caused by mining conditions, activities, or equipment.

Employee was bolting top in the #6 heading. As EE was removing drill steel out of the hole, a golf ball sized rock came down with the steel striking EE near upper lip. The area being bolted was approximately 10' high. Received 4 sutures.

Injured was tramming the #2 Scoop off the #1 Section when injured trammed machine over a rock on the operator's side forcing employee's head into the canopy and causing pain in mid to lower back.

While extracting 3 a tail, the water line was hidden under rock box. Sides of rock box and tail piece caught water line and kicked it around at a high rate of speed, catching employee on right foot

Employee was diagnosed with Occupational Pneumoconiosis, Employee ceased work on 12/19/2015 due to family reasons

The employee was using a jack raise a flat car up that had derailed. After raising the flat car the employee used a set of chain ratchets to pull the flat car back onto the rail when the jack came loose and struck the employee in the hand resulting in a fracture to right hand.

The employee was in the process of securing a tow bar onto the end of a Brookville 15 ton motor when the bar shifted and pinched the employee's right hand. This resulted in a laceration to the right hand requiring 7 sutures.

Employee was outby knocking a coupling off of a 6" waterline when a piece sheared off striking Employee in the head. Employee was treated at Raleigh General Hospital where employee received six (6) sutures.

Employee has been diagnosed with Occupational Pneumoconiosis. The Safety Department was notified of employee's diagnosis on Feb 1, 2016.

Employee was walking to the miner when EE struck EE's head on a belt hanger and injured EE's neck. (Employee worked the following day, had a personal day in for the 21st. EE went for medical attention and was put off work. There was no work scheduled for the 22nd, 23rd and 24th.) Began missing scheduled work on the 25th.

As Employee was carrying rock dust employee felt pain in lower back. Employee was treated at Raleigh General Hospital.

While fire bossing Employee began to feel pain in employee's legs. Safety Department was notified on 1/19/2016 of employee's diagnosis.

Employee has been diagnosed with Occupational Pneumoconiosis. The Safety Department received notification of his diagnosis on 11/11/15.

Employee was bolting top in #2 face when a piece of draw rock, approximately 60" X 21" X 1" - 1 1/2" thick dislodged, striking him on the head knocking him to the ground. It was reported to the Safety Department on 12-16-15 Employee was having physical therapy for his injury.

Employee was dropping curtain in the face of #2 entry and when he turned to go back to the scoop, he felt and heard his right knee pop. Employee was treated at CAMC General Division.

Employee slipped in a water hole while walking from section to mantrip and felt pain in his back and right leg. Employee began missing work on November 30, 2015.

Employee was pulling cables by a parked shuttle car when a coal rib fell hitting him in his chest causing him to fall against the rub rail on the shuttle car resulting in a deep bruise on his left thigh bone. Employee was transported to Appalachian Regional Hospital where he was admitted for observation.

Employee was installing new drive belts on fan when his left hand pinky finger was caught between belt and pulley.

Employee was jacking the motor back on track with cable attached to flat car. The jack kicked causing the cable to draw tight striking him in the left leg. He was treated at St. Mary's Hospital in Huntington.

Employee was straddling the take-up when the tailpiece was pulled up, the slack tightened and the take-up dolly struck him in the right leg above ankle resulting in a compound fracture. Employee was treated at CAMC General Division where he had surgery to repair bone.

Contractor was replacing a battery in his strobe light using a pocket knife to remove the screw from the battery cover. The knife slipped causing a laceration to the left index finger. Contractor was treated at Boone Memorial Hospital where he received seven (7) sutures.

Employee was a passenger in supply motor which started to slide down the track. When the motor went through two (2) sets of airlock doors, employee jumped off of motor striking his left hand on an unknown area or object resulting in a compound fracture/laceration. Employee was treated at CAMC General where he was admitted and had surgery.

Employee was handling a 4' x 8' piece of metal sheeting in preparation of constructing a rock box for a new transfer set up when he stumbled over top roller lying in walkway twisting his right ankle.

Employee was attempting to change scoop batteries when he stumbled into a hole losing his balance causing his knee to twist.

Employee was opening air lock doors when the pressure caused the door to slam shut striking his right hand. Employee was treated at Raleigh General Hospital.

Employee was scaling top before bolting. A rock hit the canopy of the bolter then fell striking Employee on the right leg. Employee was treated at MedExpress where he received ten (10) sutures.

Employee pulled his lower back while lifting bottom roller. Employee was treated at Raleigh General Hospital.

Employee a continuous miner operator on the #1 Section was injured when a roof rock fell and struck him. Employee was standing in #6 Entry approximately two breaks inby spad 9050 when the accident occurred. The roof support was properly installed and intact. Employee sustained injuries to his pelvic area as a result of the accident.

Employee has been diagnosed with Occupational Pneumoconiosis. Employee's employment ended May 8, 2014.

While busting up concrete, a piece of rock or metal came up striking Employee in the right jaw causing a laceration. Employee was treated at Raleigh General but was referred to another physician to remove foreign object. Employee had foreign object removed on May 16, 2014.

While straightening structure on the Horse Creek #2 belt, 44 break, employee was trying to pull a leg out from behind a 6 inch waterline when the leg dislodged and came back striking him in the right ear resulting in a cut that required six (6) sutures. Employee was treated at Raleigh General Hospital.

While walking across section, employee slipped and fell injuring his lower back. Employee did not seek medical attention at the time of the incident. Due to the continued discomfort Employee went to the doctor on April 23, 2014 and was placed off work for two (2) days.

Employee was struck in the neck by a fly board hanging from the roof. Employee was treated at Boone Memorial Hospital.

An unintentional Roof Fall has occurred on the #1 Section 2nd Left plan on Chase SubMains spad 9420. The fall was approximately 60ft x 19ft x 4 1/2ft. There were no injuries reported and no equipment was involved.

Employee was walking up the #3 Entry from the mantrip to the section. As he stepped through the back up check he slipped on a piece of discharge line causing him to fall injuring his lower back.

An unintentional roof fall has occurred on the #1 Section #2 Entry at Sycamore 2 head spad 5108. The roof fall was approximately 25ft x 20ft x 3ft. There were no injuries reported and no equipment was involved.

Employee was skiving belt when the utility knife he was using slipped and cut his left hand. He was treated at Raleigh General Hospital where he received six (6) sutures.

Employee was removing a slide skid from a flat car that was hooked to the motor. When removed the flat car moved backwards from the slack in the draw head causing it to flip downward which smashed Employees index finger on his right hand.

Employee was tightening lug nuts on the #1 DBT scoop when his ratchet slipped striking Employee on the chin and broke his front tooth.

Employee has been diagnosed with Coal Workers Pneumoconiosis. Marfork Safety Department was notified of Employee's diagnosis on August 18, 2013.

As Employee was crossing over belt he stepped down placing his foot on a rock causing his ankle to twist. Employee was transported by EMS to Raleigh General Hospital for evaluation.

As Contractor was loading belt structure when a bottom roller fell off and hit his foot.

Employee was hanging continuous miner cable when he felt a slight pain in his shoulder. The Safety Department was notified April 1, 2013 Employee was receiving physical therapy for his injury.

An unplanned roof fall has occurred at Sycamore #1 track spad #336 crosscut between #2 and #3 Entry. The roof fall was approximately 30' x 18' x 5'. There were no injuries reported and no equipment involved.

Employee was removing rock from continuous miner using a slate bar. The rock slid off the miner striking Employee on the left ankle and foot. Employee was treated a Boone Memorial Hospital for his injury.

Employee was replacing cutter head on continuous miner and was attempting to replace top pin of the cutter jack. Coworker raised jack with slate bar in upward motion. Employee was using his hands to pull jack in place to replace pin. The slate bar slipped allowing cutter jack to smash Employee's left ring finger against frame of pan.

Employee was walking #5 entry intake which is on a down hill grade. Employee slipped and fell landing on his rescuer and striking his head on a rock causing a tooth to break off. Employee was transported by EMS to Raleigh General Hospital for evaluation.

Employee was carrying a vulcanizer utility knife on belt when he tripped landing on knees. The knife was not fully closed which caused a small laceration above the right knee. Employee was treated at Raleigh General Hospital where he received three (3) sutures.

A roof fall at #9 break slip ridge 6 belt spad 2795 between #3 and #4 entries. length of fall 40' wide 20' in length 4.5' in height no injuries and fall will not be cleaned up

An unplanned roof fall has occurred on the #1 Section outby #4 spad 3426. The roof fall was approximately 20' x 20' x 4'. This area is 12,800ft from section and is inby stopping at EP#13B. There was no equipment involved and no injuries reported.

EE finished the cut in 8 and was moving the miner to 7. the ee backed the miner to the last open line and while gathering his cables using 2 ropes 1 rope had been removed from the boom the 2nd was still connected to the boom. the boom swung to the operators side and the employee began tramming the miner to the face of 7 the boom pinched his lower left leg into the rib

An unplanned roof fall occurred at the face of #1 Section #6 Entry. The roof fall was approximately 56ft x 16ft x 3ft 10in. The right continuous miner was involved and a clean up/retrieval plan posted. The continuous miner operator was struck by a piece of the rock and had a small abrasion.

Employee was attempting to get belt spindle out of splice by utilizing a claw hammer to hold spindle while using a mason hammer to strike head of claw hammer. A piece of metal broke off and punctured right above left eye. He had surgery on March 29th to remove metal from the orbital socket of the left eye.

Employee's shuttle car was stuck on the dump due to losing power. Employee was attempting to hook a chain to shuttle car and scoop when the scoop struck Employee's left leg. Employee had a small puncture wound and bruising. Employee was transported by EMS to CAMC General for evaluation. Xrays were negative.

While shoveling #2 belt at the 77 break the employee hurt his left knee. The employee's knee started to swell. The employee had stated he had hurt this knee previously.

As Employee was helping align tailpiece - the tailpiece slid off of crib blocks landing on Employees left foot. Employee was treated at the scene by EMTs and then transported to CAMC General. Xrays taken and were negative.

An unplanned roof fall has occurred on #1 Section Sycamore #2 Belt 26 break - 2 breaks inby #5279 spad. The roof fall was approximately 20' x 20' x 4'. No injuries were report and no equipment was involved.

While Employee was putting supplies on the Roof Bolter he slipped and fell on a bundle of roof bolt plates causing a laceration to his right forearm. Employee was treated at Raleigh General where he received nine (9) sutures.

Injured was unloading supplies onto a roof bolter and hit his right elbow and forearm against a bolt plate causing a laceration.

As Employee set bits in the continuous miner he felt a pain in his lower abdomen area. Employee ceased work and was treated at MedExpress.

As Employee was adjusting forks on an underground forklift the fork slid catching his right hand and pinching it between the forks. Employee received five (5) sutures at Raleigh General Hospital.

Employee was attempting to remove a rock that was caught between an impact roller and flow thru. Employee was using a slate bar to remove the rock when the slate bar struck the belt pulling his hand against flow thru causing a laceration to his left finger. He was treated at Raleigh General Hospital where he received five (5) sutures.

Employee was bolting in #1 when a piece of rock approximately 6"x4"x1.5" broke around steel. As Employee spun the steel back out the rock broke striking Employee on the face causing a laceration. Employee was transported to Raleigh General Hospital by EMS where he received 8 sutures.

As Employee was unplugging mantrip a piece of metal was sticking up causing a laceration on his right index finger. Employee drove to Raleigh General Hospital for treatment and received four sutures.

While employee was carrying dust filters bent over his hard hat struck the corner of a metal strap which was hanging approximately 6" from the top causing him to fall on his backside. Employee was complaining of pain in his neck and back. He was transported by EMS to Raleigh General Hospital for treatment. A CT scan was taken and the results were negative.

Employee was bolting in 6 right when a piece of rock approximately 24" x 3" x 2" fell from the rib striking employee on the lower back. Employee was treated by EMT's at the scene and transported by Whitesville Ambulance to CAMC General Division. Employee had xrays taken which was negative but was referred to a spine specialist for further treatment.

As employee was walking around mantrip he slipped and fell. Employee landed on the ground causing his shoulder to dislocate. He was transported to Raleigh General Hospital and was diagnosed with a shoulder dislocation and a small fracture. Medical staff set his shoulder back in place and referred employee to a specialist for further evaluation.

Employee was conducting a pre shift examination on Horse Creek 9 Belt at 76 break. Employee crossed under the belt at the cross under. As employee bent down he felt a sharp pain in the right knee which started to swell. Employee drove to Raleigh General Hospital for treatment. X-rays were taken, he was diagnosed with a sprain knee and to follow up with an orthopedist.

Employee was hanging cables at the right dump on the Feeder line. Employee felt a sharp pain in his back. Employee refused ambulance and drove to Raleigh General Hospital. Employee was diagnosed with a strain and was instructed to remain off work for five (5) days. Employee had assistance hanging cables.

An unplanned roof fall has occurred on the #1 Section #4 Right Return panel spad 4553. The roof fall was 20' x 19' x 4'. No injuries were report or no equipment was involved.

While Employee was moving a roof jack it fell striking employee on the right foot and leg.

While conducting fireboss run on Sycamore #1 belt this morning, member walked through a crosscut at 188 break and stepped in a rut in the bottom. Member heard a loud pop in his left foot and could not put much weight on it. He managed to drive the mantrip outside and transport himself to the Raleigh General.

All members plastered stoppings during the move and EE complained after the move of his hands being irritated by the b-bond. It had worn a hole in his gloves and gotten on his bare hands.

An unplanned roof fall has occurred at #4A Belt line break 23 on #2 Section inby spad #6011. The roof fall was 20' x 20' x 10'. No injuries or equipment was involved.

EE was repairing some cable. While doing so, he attempted to use his knife to make a cut in the cable and accidentally cut his hand. He then proceeded to seek first aid until his shift ended where he went to the doctor and received 12 stitches.

Employee was walking across the feeder and slipped and twisted his ankle, causing a sprain.

Employee was cleaning off the bolter. His hand and forearm grazed the strap cutting him on the top of his forearm. He was bandaged then transported outside. From there he went to the hospital where he received 13 stitches and was told he could possibly have nerve damage.

EE was fixing the parking brake on the #1 mantrip. While working, the shield fell and struck him in the back of the head, causing him to hit his face on the mantrip. The accident caused a minor laceration across the bridge of his nose.

Member was welding and looked at the arc and accidentally burned his eyes. He was unaware of this until he went home and tried to sleep. His eyes began to swell after a few hours.

Employee was in the process of replacing shield over the pump compartment on a shuttle car. He placed the shield on top of the frame of the shuttle car. The shield began to slide off, as he attempted to catch it he lost his balance and fell backwards on the ground holding the shield. He experienced soreness and tenderness in his groin, knees, and back.

Surveyor was walking down a hill toward the mantrip when he slipped on slick mine floor and fell striking his head on the mine floor.

Employee was replacing the center section of a 35C Fairchild Scoop. While grinding on the center section the grinder kicked back hitting his upper lip causing a laceration. He completed his shift and that evening went to Raleigh General Hospital for further treatment and he received four stitches.

Member was tightening up a face ventilation curtain when hand slipped off of curtain and struck a roof pan lacerating members finger requiring 9 sutures

Member was rock dusting offside of belt line, when his knee gave out and he fell onto the beltline that was around 33" off of the mine floor. Member then rolled off of belt line.

Employee was moving a miner cable to the side of the rib and ran into a bent roof bolt plate on the corner of a ledge.

Member was operating continuous miner in 4 left crosscut when he was struck by a falling rock that fell from inbetween bolts. Rock measured 2.5 wide, 5' long and 0"-3" thick. Member received an abrasion that was not properly cleaned at the hospital and became infected.

EE was walking down the #2 entry at spad #6429 when a piece of rock fell out and hit him on the head and back. He was sent to the Raleigh General Hospital and dianosed with a concussion and bruises and then released.

Member was installing the last row of roof bolts in the #6 Entry. He slipped on a piece of rock and reached out to grab something to regain his balance. He accidentally grabbed hold of the rotating drill steel causing tissue damage in his right shoulder. EE continued to work until going to Raleigh General Hospital on 06-22-10 and was told the damage would require surgery.

While lifting miner cable and water lines to hang, felt pain in left shoulder, it gave out.

On 3-05-2010 EE stated that as he exited a Brookville Personnel Carrier he twisted his right knee. He continued to work until 04-30-2010. On 04-30-2010 due to continued problems with his knee a knee arthroscopy was preformed. This was not reported to Marfork Safety by the EE or the operation until 05-20-2010.

Several roof falls in the 2nd left panel off of Sycamore Mains, ranging from 30' to 150' in length and 18' wide, 4' to 5' in heigth. This is a worked out panel, requiring weekly exams approximatly 4000' from an active section. Location of the falls are at spad numbers 3650, 3653, 3688, 3831, and 3662. No injuries or equipment involved.

Roof fall in the last open cross cut of the #5 entry, two cross cuts in by spad # 5962. No injuries or equipment involved.

Roof Fall in the Intersection between #5 and #6 Entries. One cross cut inby spad # 5886. No injuriesor equipment involved.

Transferring 48" top belt structure from a flat car to a scoop insert.

EE was removing a belt pin with a claw hammer and a pick hammer. A piece of metal off of the claw hammer broke off and punctured his right wrist on top of his arm. The wound required 3 stitches.

Rock had fallen out of the rib. Employee was busting it with a hammer when a corner fell and struck him on the right ankle.

On 8/9/09 at about 1925 hours Security Officer was discovered unconscious in his vehicle by the roving security guard. He was awakened, complained of illness and was transported by ambulance to Raleigh General Hospital at 2030 hours. Died at hospital at 0237 hours.

EE was backing a 35C Fairchild Scoop down the #3 entry along the #2A beltline. At the #7 crosscut there were five fly boards lying to the side of the entry. The rear tire of the scoop ran over one of them, causing one end of it to flip up and enter the operator's deck, hitting EE in the ribs and back pinning him in the deck.

While roof bolting EE attempted to shove a 48" resin bolt up he was holding on to. While spinning the bolt, he pushed it up. His glove became tangled, causing a fracture to four fingers. He refused emergency transportation and transported himself to Raleigh General Hospital Emergency Room where diagnosed with the fractures and referred to a surgeon.

When injury attempted to take a shuttle car anchor down it fell and hit him in the mouth breaking his front tooth

Employee was attempying to hang a continuous miner cable. As he bent over to pick it up his back popped, causing a severe pain.

A roof fall occured in a neutral entry crosscut approximately 3500' outby section near horse creek #9 belt at break #71 between #3 & #4 entries. Fall measured 50' long 18' wide & 5' thick. No injuries no passages of men affected. exact time of fall unknown, found by preshift examiner.

Roof Fall in the #7 Entry of the #4 Panel off of Blue Tick Mains at spad # 1632. The Fall is 4000' from an active Section . No airways or passages is blocked and no injuries. Roof Fall is 40'x20'x5'.

EE was walking in the #2 entry on #1 section, when he stepped on a rock, causing his left knee to twist. EE continued to work until 2/13/09. After EE had an MRI he was instructed to be off work.

Injury was changing out the stationary roller on the #4 Hornet Mains take up . As he attempted to place the shims underneath the bearing as it was being held up by another person with a slate bar, the bar slipped out causing it to fall on his left thumb. The injury required surgery to repair the damage.

A roof fall occured in a neutral entry intersection approx. 2500' outby #1 section 8 breaks inby sycamore #1 belthead fall measured 25' long, 18' wide and 6' thick. no injuries, no passages of men affected. exact time of fall unknown.

A roof fall occurred #2 section return 2000' outby section inby spad #4926. Fall measured 20' long, 18' wide & 6' thick. No injuries, no passage of men affected.

While walking through the mine office, member tripped causing his left hand to hit an open drawer of a filing cabinet, causing a laceration to his left hand that required 9 stiches.

EE was putting up T-S channel straps in the belt entry, while handling the straps, he pinched his finger causing a laceration that required 7 stitches to repair.

EE worked untill 7/26/08 at that point he saw a DR who told him to remain off work untill 8/1/08 due to an infection in his knee from falling onto a rock.

Rooffall in #3 entry in the cross cut to the right while mining. Fall was 12" wide x 15' long x 5' high. No injuries results from this fall.

EE was walking across the track and stepped on a rail which caused him to twist his knee, he continued to work untill 7/31/08 at that point he was told not to work by DR.

Member was setting timbers. While setting a timber, one of the previously set timbers became dislodged and struck him on the top of the head causing pain to the head, neck, & back. No accident report was filed with Marfork Safety Dept until 5/15/08.

A fall of the mine roof occurred near spad #2669 the size of the fall was 50'Lx20'Wx0-6'H. There was no injuries and no airways were blocked.

Ee was assisting in the process of removing a set of airlock doors a piece of block became lodged and he was going to use his hand to remove it the other person helping him didn't see his hand and struck it with a sledge hammer causing a laceration that required 9 stitches and a broken middle finger.

Employee got off of mantrip to remove a piece of rock from track. He bent down to move rock and stated he felt a sharp pain in his lower back.

A fall of the mine roof occured in a room off of section #2. The roof bolter machine was parked in this room. There were no injuries and no airways were blocked. Fall measured 15'L x 20' W x 4' H.

EE stated that while moving the miner cable over, he felt a pain in his back on 10/15/07. On 10/17/07, when he did not report to work and attempts were made to contact him and were not successful. On 10/22/07 EE called and said he was off due to injury.

While operating a scoop, member had his hand on outside of canopy causing his hand to get mashed between the rib & canopy, causing a laceration that required stitches.

Roof fall in #5 entry - last open crosscut. 20' x 10' x 8'. No injuries. A roof bolting machine was blocked.

Roof fall, 20' x 20' x 5' in the intake. No injuries, production was stopped, miners withdrawn and escapeway was rerouted.

Roof fall in the #2 Entry at spad #2700. Fall was 40' X 20' X 5'. No injuries occurred.

Roof fall in the #2 entry at spad #3188. 30' X 12' X 4'. Fal was discovered during a weekly exam. Fall was 8800' from an active section. No injuries.

Employee was making a flexco belt splice with a co-worker. As he reached for the nail plunger, the co-worker accidentally hit his hand with the hammer.

While roof bolting the drill steel became stopped. Harper towered the drill boom to check the selction, when he raised and back up. He placed his hand on top of the drill pot causing it to be pushed into the drill steel

A piece of rock measuring 15" x 16" x 1" fell from the roof striking him on the shoulder. He was transported to the hospital; not given a diagnosis. On 6/21/07 after having a MRI he was diagnosed with a fractured clavicle.

While tramming a continuous miner employee attempted to place a loop of cable slack onto the conveyor boom without deactivating the remote control. He accidently hit the conveyor swing pinning himself against the coal rib.

Miner was diagnosed with pneumoconiosis and excercised his right to be moved to an atmosphere with not more than 1.0 concentration of dust.
