INJURED WAS GOING THROUGH A SET OF AIR LOCK DOORS ON A BATTERY RIDE. HE CLOSED THE FIRST SET OF DOORS, OPENED THE SECOND SET. GOT ON HIS RIDE TO GO THROUGH THE SECOND SET. WHEN HE PUSHED THE TRAM PEDAL DOWN, THE PEDAL STUCK, HE HAD IMPACT WITH THE TOP OF DOOR FRAME. HE CUT HIS NECK, BROKE INDEX FINGER ON LEFT HAND, AND HAD A FRACTURE TO HIS UPPER JAW.

INJURED WAS LOADING CINDER BLOCKS INTO A SCOOP BUCKET. WHEN HE THREW THE LAST BLOCK, HE CAUGHT HIS HAND BETWEEN THE BLOCK AND THE SIDE OF THE SCOOP BUCKET. WHICH MASHED AND CUT THE MIDDLE AND RING FINGERS ON HIS RIGHT HAND. STITCHES WERE USED ON THE TOP AND BOTTOM OF THE FINGERS. X-RAYS WERE NEGATIVE ON ANY BROKEN BONES.

An unplanned roof fall above the anchorage zone of installed roof bolts has occurred in the No. 5 entry two crosscuts outby survey spad no. 2222. The roof fall measures 20' wide, 40' long and 5' thick. The fall does not affect ventilation or impede travel. The fall has been dangered off and access into the fall have been timbered off.

Injured man broke cut off valve for deluge fire spray. He turned the water off on the 6 inch line, when he bled the pressure off the line, it blew his hand into a pizza pan which was part of the bearing surface for roof support and recieved a cut to his finger.
