While cleaning #1 heading with a scoop, employee's elbow was caught between scoop and rib. When they tried to free themselves they steered the wrong way mashing it.

Employee was struck in head and knocked to ground by a piece of draw rock and struck in legs by more draw rock.

Employee was plastering a brattice. As ee made way across the brattice moving the bucket of plaster with ee, moisture on the surface of the plaster inadvertently splashed and found its way underneath ee safety glasses and into ee left eye.

INJURED WAS PULLING CABLE UP A BANK AND TWISTED EMPLOYEE'S KNEE.

Injured was tramming a scoop underground from surface. At break 38 a piece of rock fell from the top striking injured on left shoulder. X-rays revealed no broken bones or dislocations.

Injured was attempting to open a door on number one fan. The hinges broke allowing the door to fall on injured. injured received a broken left scapula.

A FALL OF THE ROOF WAS FOUND ON OCTOBER 19, 2015. THE FALL WAS IN AN INTERSECTION ONLY AND WAS REPORTED TO BE 3 TO 4 FEET THICK. ROOF BOLTS WERE SEEN STICKING UP THROUGH SOME OF THE FALLEN ROCK. THE FALL IS ONE BREAK INBY SPAD NUMBER 1965 IN A NEUTRAL/NONE ESCAPEWAY ENTRY. THE AREA HAS BEEN MADE SAFE BY CRIBBING AND DANGERING OFF.

A fall of roof was found at or near spad number 2306. This was not a regularly traveled airway or travelway.

Blocked travel to left side return. Airflow was still able to cross fall area. Fall was at or near spad number 129.

fall of roof has occurred from cross-cut 16 to cross-cut 18 in the number one belt entry. All activity is prohibited from cc 16 to cc18.the fall is at or above anchorage.

INJURED WAS GOING THROUGH A SET OF AIR LOCK DOORS ON A BATTERY RIDE. HE CLOSED THE FIRST SET OF DOORS, OPENED THE SECOND SET. GOT ON HIS RIDE TO GO THROUGH THE SECOND SET. WHEN HE PUSHED THE TRAM PEDAL DOWN, THE PEDAL STUCK, HE HAD IMPACT WITH THE TOP OF DOOR FRAME. HE CUT HIS NECK, BROKE INDEX FINGER ON LEFT HAND, AND HAD A FRACTURE TO HIS UPPER JAW.

Injured was helping to put a wrecked rail-bus back on track. He slipped and fell hurting back, groin, and knees. Injured finished shift and returned to work on his next scheduled shift. Injured stopped work on march 11, 2015.

INJURED WAS LOADING CINDER BLOCKS INTO A SCOOP BUCKET. WHEN HE THREW THE LAST BLOCK, HE CAUGHT HIS HAND BETWEEN THE BLOCK AND THE SIDE OF THE SCOOP BUCKET. WHICH MASHED AND CUT THE MIDDLE AND RING FINGERS ON HIS RIGHT HAND. STITCHES WERE USED ON THE TOP AND BOTTOM OF THE FINGERS. X-RAYS WERE NEGATIVE ON ANY BROKEN BONES.

EE was running a shuttle car. When the s/c was loaded he was going to switch seats, and as he was doing so, he twisted his shoulder.

EE was standing in a crosscut. EE stepped out of crosscut onto track. EE was struck by moving man-trip.

An unplanned roof fall above the anchorage zone of installed roof bolts has occurred in the No. 5 entry two crosscuts outby survey spad no. 2222. The roof fall measures 20' wide, 40' long and 5' thick. The fall does not affect ventilation or impede travel. The fall has been dangered off and access into the fall have been timbered off.

injured was helping the miner operator clear rock from miner. he was getting a piece of rock off of the miner when he said he hurt his back.

INJURED WAS MOVING HIS CABLE ANCHOR FOR SHUTTLE CAR UP. THE CABLE REEL TURNED PULLING ANCHOR INTO THE BATWING WHICH CAUSED THE CHAIN ON THE ANCHOR TO STRIKE INJURED ACROSS FACE.

EE FOUND THE FALL AT APPROXIMATELY 10:00 am ON 07/14/14. THE FALL IS ABOUT 80 FEET LONG AND ABOUT 30 FEET INBY SPAD #153

Employee was working on roof bolter leveling arm chain, chain broke striking employee on bridge of nose and right eye causing injury. Employee was transported by ambulance to hospital.

EE was on his knees picking up TC plaster into motor. EE felt pain in the center and lower part of his back and neck.

AN UNPLANNED FALL OF ROOF HAS OCCURRED 1 BRK INBY SPAD NO.7027 IN RIGHT RETURN OF NO. 6 WEST MAINS. THE FALL MEASURES 100 FEET LONG, 20 FEET WIDE, AND 10 FEET THICK. ALL ENTRANCES TO THE FALL HAVE BEEN CRIBBED AND DANGERED OFF. THE FALL DOES NOT AFFECT VENTILATION OR IMPEDE TRAVEL IN THIS AREA.

EE was trying to unloosen a packing nut from the starter box. EE was using an 18" pipe wrench and 3' bar to wedge box. While pushing down EE felt pain in his stomach. "THIS DID NOT BECOME REPORTABLE UNTIL 02/17/2014 WHEN EE BEGAN TO MISS WORK DUE TO THIS ACCIDENT"

An unplanned roof fall has occurred in the No. 5 entry intake escapeway the fall measures 22'long, 24' wide and 48" high the fall does not impede travel or affect ventilation. all entries around the fall has been cribbed and dangered off.

EE was bending 6ft bolt and EE began to put bolt into hole he said he felt burning in his back.

EE tripped going up a set of steps leading into the changing room. EE fell and landed on his dinner bucket hitting his lower back.

EE was putting straps together on conveyor chain. EE put a chisel between press and the conveyer chain. The chisel came out hitting EE in the left wrist.

EE was carrying bits to the miner and stepped around the boom of the miner twisting his back.

EE was bolting top and the pin steel was caught/fouled in the drill pot. EE was removing the pin steel, the pin steel came lose and struck EE underneath the right eye causing a laceration.

A piece of rock fell out between ATRS and hit the operators canopy. A piece of the rock broke off and hit EE on the right wrist and arm.

EE was operating the continuous miner. A piece of rock fell out between bolts striking the EE on the left hand.

EE was getting out of scoop to unload a set of airlock doors. EE tripped while getting out of the scoop. EE twisted his back while falling out of scoop. EE proceeded to get up and go lift the doors. When he picked up the doors he felt a pain in his back.

Occupational Pneumoconiosis

An unplanned roof fall has occurred in the No.7 entry return airway one crosscut inby survey spad No.1765 the fall measures 20'x 20'x 4'. The fall does not impede travel or affect ventilation in this area. All access to the fall have been cribbed off to prevent travel into the affected area.

Occupational Pneumoconiosis

EE was helping replace drive belts on belt head. The EE pinched his finger causing a laceration.

EE was hauling ripper motor to section and a piece of structure flipped up and hit finger causing a laceration.

EE was throwing roof bolt plates to the front of the roof bolter and twisted his back.

EE pulled upward on charging cable ground clamp came up under safety glasses and scraped right eye.

There has been a fall found in the primary escapeway. All approaches to the fall have been cribbed and dangered off to keep workers from entering the area of the fall. The primary escapeway has been re-routed. We have had safety meetings with all three shifts discussing and going over on the map the re-routing of this entry.

EE was setting a timber, the ee did not have it wedged completely and the timber fell hitting ee in the mouth causing a broken tooth.

EE was putting up cable bolts. The head on the cable bolt broke off. The ee took a wrench and hit the plate, the plate came off hitting the ee in the face and right eye.

EE was pulling cable with a rope hanger, the hanger broke striking the EE above his right eye.

The EE was scooping up a roll of belt when the deck of the scoop hit the rib causing the rib to roll out hitting the operator.

The EE was putting up a roof bolt and a piece of rock fell from the top smashing his left ring finger between the rock and the drill head.

Occupational Pneumoconiosis

The EE was going under mining cable, lifting sagging cable up to cross under, his left elbow caught top and pulled his shoulder down.

The injured employee was recovering belt structure. He was carrying a piece of top structure to load it into the scoop bucket, the injured employee tripped catching his left wrist between the scoop bucket and structure, cutting his wrist on the structure.

On 05/30/2011 at approximately 10:00 p.m. there was a roof fall found in the No.4 entry. The fall was found by EE while fire bossing. This fall is located at survey station No.1708 and measured 25'x25'x5'. The fall will be cleaned up in accordance with the clean up plan.

An unplanned roof fall was found on 04/11/2011. This fall was found one break inby survey station No. 3361. This fall measured 20' wide x 20' long x 8' high. This fall does not impede travel. This fall has been dangered off.

This fall was found 1 break inby survey station 162. The fall measured 4' thick x 20' wide x 20' long. This fall was found by an EE and was reported by telephone on 04/02/2011. The fall has been cribbed and dangered off. This fall does not impede travel.

A fall was found 1 break inby 6069 in the No.6 entry the fall measured 70' long x 18' wide and was 12" thick. The fall has been dangered off and will not be cleaned up. The fall was found by an EE and was reported by telephone on Sat 03/26/2011. This fall does not impede travel.

The injured employee and another person were removing a guard from the continuous miner. They lost grip of the guard, resulting in the injured employee's hand getting caught between the guard and the mine floor.

A fall has been found inby survey station No. 3391. This fall does not impede travel or affect the course of air flow. All access entries to this fall will be cribbed and dangered off. This fall measured 20' long x 19' wide x 3' high.

A fall has been found 2 breaks inby survey station No. 3387. This fall does not impede travel or affect the course of air flow. All access entries to this fall will be cribbed and dangered off. This fall measured 25' long x 19' wide x 3.5' high.

There has been an unplanned roof fall in the no.2 entry 2 breaks outby spad no. 1303. The fall measured 35' long x 5' high x 19' wide. This fall is located approx 14,000' from the working section. All access entrys to this fall has been timbered and dangered off.

There has been a fall found one break outby spad no. 1304 in the no.1 entry. The fall measured 22'long x 5' high x 20 wide. This fall has been cleaned up and the area rehabilitated.

A roof fall occured 2 breaks inby spad 1360. The fall was aprox. 10 feet high X 20 feet wide and 20 feet long. The fall area has been cribbed and dangered off. The fall does not affect any travelways and airways. Fed. inspector has been to the fall area and inspected the area on 12/15/10.

The injured person was traveling outside on a 3 wheeled personal carrier and hit a sink hole, which resulted in the injured person hitting his head against the mine roof, injuring his neck and back. We were made aware that he was going to see a doctor on 12/02/2010, resulting in this filing date.

A roof fall was found on 11/16/2010. The fall measured 20'x 20'x 3'. The fall was one break inby spad No.3460 No.7 Entry. After finding the roof fall, all accessble areas was timbered off and visible danger signs where posted.

A roof fall has occurred in the No 5 entry, just inby survey station spad No 3185 of the No 2 section area of the mine, in the right return, the area measured approximately 25 feet long,19 feet wide and 5 foot high. The fall did not entrap men or impede travel. The area will be cribbed and dangered off and shown on the mine map.

An unplanned roof fall has occurred in the No. 3 netural entry two crosscuts inby survey spad No. 3127, the fall measures 20 feet long, 20 feet wide, and 5 feet thick. The fall was above installed roof bolts. The fall does not impede travel or affect ventilation. All areas of the fall will be timbered off and danger signs posted. The fall was reported to MSHA.

Injured man broke cut off valve for deluge fire spray. He turned the water off on the 6 inch line, when he bled the pressure off the line, it blew his hand into a pizza pan which was part of the bearing surface for roof support and recieved a cut to his finger.

PUTTTING LIDS ON SCOOP BATTERIES, DROPPED LID AND MASHED LITTLE FINGER

a roof fall occurred above the anchorage,outby the No 2 section, in the No 6 entry just inby survey spad 3223, just outby survey spad 3215. the fall measured 20 ft wide and 60 ft long and 5 to 6 ft high. There was no ventilation interruption nor did this fall affect the passage of men. The area will be cribbed and dangered off.

Riding 3 wheeler vehicle through a check curtain and got left ear ring caught in the curtain and cut ear.

An unplanned roof fall has occurred alone the No.2 conveyor belt entry at break No. 56, survey spad No. 504 the fall measures 20' in lenght, 20' wide and 6' high, the fall does not impede travel or ventilation in this area. The fall will be cleaned up and resupported.

Employee was checking centerline for continuous miner when struck by mobile equipment.

An unplanned roof fall has occured in the Main No. 3 entry at crosscut No. 94 survey spad No. 1293 the fall does not impede travel or ventilation in the area. the fall was above the anchorage zone of installed roof bolts.

An unplanned roof fall has occured in the No.5 intake entry at crosscut No. 37 which is 3 crosscuts outby survey spad No.185, the fall does not impede travel or affect ventilation in this area. The fall was above the anchorage zone of installed roof bolts.

Employee was cleaning track entry, complained of material in eyes.

Injured employee was pulling the 6" water line together with a chain rachet. The chain rachet slipped and hit the employee over the right eye.

Injured employee stated that he had been pulling the miner cable and upon arriving at home, he experienced back pain.

Employee was replacing the conveyor motor guard, when the guard fell catching his right hand between the guard and rub rail on the contiuous miner, resulting in a fracture to his right ring finger.

an unplanned roof fall has occurred in the 2nd left panel off the No 1 Mains at survey spad No 503. Ventilation or travel is not affected by the fall.

an unplanned roof fall occurred at survey station spad no 681 which measured approximately 20 feet by 20 feet by six feet.

An unplanned roof fall has occurred in the first left off NO.1 Mains, the fall is located two crosscuts inby survey spad no. 541 the fall measures 40' long, 20' wide, and 6' thick ventilation and travel are not affected in this area.

An unplanned roof fall has occurred in the No.1 unit, No.2 entry one crosscut inby spad No. 2329 the fall measures 90' in length, 19' wide , and 5/6' thick.

Employee was pulling cable with a battery powered personnel carrier, ran over something and his head contacted the roof resulting in a neck injury.

Employee was moving a piece of rock with slate bar felt pain in lower back.

Employee was installing roof bolts accidently hit rotation lever causing roof bolt to spin stricking employee on left hand.

Employee was removing stuck drill from drill hole pulled stomach or groin muscle.

Employee was helping move miner cable out of roadway when coal rib rolled off hitting right hand.

An unplanned roof fall has occurred in the No. 1b conveyor belt entry at survey spad No.246, the fall measures 30' in length, 20' wide, and 3/4' thick, the fall does not impede travel or restrict ventilation in this area.

An unintentional roof fall has occurred in the No. 3 out by entry at No. 79 crosscut in the secondary escapeway. The fall does not impede travel of miners or interrupt ventilation.

Miner was shoveling track entry. 6" waterline busted getting dust and dirt into eyes.

Miner was making splice in cable tripped over cable injured hands when trying to brace for fall.

Employee was freeing the V belt on a rockduster and got four fingers on his right hand caught in the pulley.

A roof fall occurred in the No. 2 entry at crosscut No. 26. No ventilation or travel of miners was blocked.

A roof fall occurred in the #2 entry return in the Mains section at Survey Station #62 approximately 2500" outby the face, measuring 20' wide, 20' long and 8' high.

Injured employee was finishing a stopping and a cinder block fell on his right hand.

Employee was drilling a roofbolt hole when a piece of roof fell striking him in his right arm and right side.

Employee was postioning the roofbolter into place, when a piece of roof fell on the ATRS, causing the rear of the machine to raise upward. The employee struck the mine roof, injuring his neck.

Breaking rock with a hammer, a piece of metal struck employee in left arm resulting in a small cut that did not require stitches.

Employee was traveling on a battery powered railrunner, drop his cap light, raised up and struck two guarded high voltage cables that crossed the track with his head.

Employee was tramming a Mac-8 rubbered tired mantrip to the charger and struck the rib dislodging a piece of rock that struck his right hand.

Injured employee was installing a flyboard with the roofbolting machine. When he raised the drill head to install the board, his right index finger was caught between the board and the roof, causing a laceration that required 14 stitches.

Operator was installing bolt into hole while attempting to straighten bolt, it spun around twisting his upper back.

While turning around in shuttle car to change directions, employee twisted his left knee.

Crawling while moving miner and twisted knee in the mud.

Employee was helping load track rail on scoop and pulled back.

Trying to get drill steel out of top, the drill steel fell out striking him on the right hand.

Employee was building stopping and a piece of draw rock fell hitting him on the right side.

While bolting, a piece of draw rock fell hitting him on face and neck area.
