Roof fall occurred in No. 4 entry 150 ft. inby spad No. 1511, 400ft. from active face. The roof fall is 60 ft. long, 20 ft. wide and 6 to 8 ft. thick. All entries are breakered off. The roof is laminated and was bolted using No. 5 x by 48 in. resin grouted re-bar.

The roof fall occured in No. 2 entry of South Mains near spad No. 87, 1900 feet from active face. This area was developed in March 2001. The fall measures 50 feet long by 20 feet wide and 6 foot thick. All entrances to the fall are breakered off.

Roof fall occur in crosscut between No. 3 and No. 4 entries near spad No. 1802. The rock is 35' long, 20' wide and 6' to 8' thick. The area is near the outcrop with low cover, water is present in roof strata along with roof cracks.

Roof fall occured in a 3-way intersection 3000 feet from active face near spad 1033 in return air course. The rock was 40 feet long by 20 feet wide and 5 feet thick. The air course will be rerouted.

A roof fall occured on working section 001, Entry No. 4 near spad 1677. The area is near outcrop, low cover, some water present in roof strata and some cracks present. Miner was turning a right hand crosscut. The fall was 40 feet long by 20 feet wide and 5 to 6 feet thick.

NO DAMAGE TO EQUIP/PERSON
