EE was taking belt structure apart. As EE was attempting to pull a belt rail loose from the spreader bar by moving it up and down, the chain came out of the eye of the spreader bar allowing the belt rail to fall down striking the top of the right foot.

Employee was preparing for a belt move, when the belt slipped off the pile employee went to straighten up the belt with channelocks and they slipped off the belt causing employee to lose balance, falling to the ground twisting right ankle and resulting in a fractured right ankle.

The employee was operating a 4-wheeler (personnel carrier) when it stopped tramming. The employee went to the back of the machine and was pecking on the outside of the control box with a sledge hammer. The 4-wheeler jumped into tram backwards towards the employee, pushing EE approximately 10' before the machine stopped.

Employee was making a cable splice on the left continuous miner when a piece of rock measuring 65" Long x 8" thick x 8" thick and a piece 28" wide x 43" long x 2" thick became dislodged from the roof and rib striking employee in the upper back and neck.

On 1/10/19 at 6:00 pm a shuttle car operator started experiencing pain in left side back/kidney area. The pain started as EE was descending a grade in the mine floor. EE finished shift & completed a report. On the way home the pain increased to the point EE called spouse to come meet EE. Spouse took EE to hospital where 1st diag. was pneumonia, 2nd was contusion.

An unplanned roof fall occurred on the #7 belt track entry in the #37 intersection 40 back toward the #36 intersection in the #2 entry. The fall was danger and timber off. The track and life line was run to the #1 entry. The fall will be cleaned up to the rehabilitation cleanup plan.

In a dip on old #6 Belt Track, the employee was a passenger in the first of two man buses. As the first man bus went down the dip and was topping over, it flashed its lights for the second bus to proceed, when the first bus stalled out and rolled back down causing the second bus to hit it. This employee began missing work on 06/04/2019.

Employee was tightening down a load of water line with a chain ratchet, and the pin that connects the hook of the chain ratchet to the chain broke allowing the chain to snap back, striking the employee in mouth and chin, causing a laceration that required sutures to close.

Employee was changing out supply cars at the rock dust switch, and put one in the straight and two in the switch and brought one back out. As the employee was attaching the safety chain, the car in the straight rolled back catching the employee's foot between the supply cars. The incident resulted in a laceration to left ankle requiring sutures to repair.

Employee observed a large rock going through the feeder that was going to contact the "dead-man" cable going across the feeder. EE attempted to hold the lever that is pulled into the e-stop to avoid the feeder from stopping, but the rock hit the cable & pulled the lever mashing EE's finger between the lever & the e-stop causing it to fracture the end of EE's little "pinky" finger.

The employee was taken to the Logan Med Center ER and given a full release to return to work, EE went to Dr on 05-31-2016 and was taken off work for a possible pinched nerve, the employee started missing work on 06-01-2016.

Employee was standing beside the electrician at the feeder, when the number 1 shuttle car pulled onto the dump end of the feeder catching his right leg, causing 5 fractures to his foot and ankle, he was treated onsite and taken to the hospital then transferred to CAMC were surgery was performed on his foot and ankle. Msha has conducted an investigation into this incident.

Two employees was picking up a drawhead to install on a flatcar they were moving, as the other employee picked up his side of the drawhead it allowed it to roll over onto the injured employees right hand, causing a fracture. He was wearing a mechanic's glove.

Employee was tearing down flypads in the #7 entry 50'outby spad number 10757 to move to a new location as he was pulling on the flypads, the flyboard broke allowing a loose piece of drawrock that was above the flyboard to fall onto the ground hitting him on the left ankle resulting in a chipped bone.

While leaving the bathhouse ee told me he hurt his knee, I asked him if it was ok, and if he wanted to fill out a accident report and he said no that he was ok , they wasn't never a clear event or incident to how this employee was injured if he was injured at this mine. Note this was his second day at the mine, his first day was finishing his training.

Diagnosis still unknown.

It is appx. 7,900 ft from the section in the #1 entry at spad # 3108. The fall has been breakered off and will be rehabilitated. The area was bolted with 6 ft torque tension bolts. No rider seam is present.

He was taken off work after another scope was done.
