EE was walking between #2 and #3 entry, EE's head encountered/bumped a pizza pan that was slightly lower than the mine roof. EE was diagnosed with a neck injury, cervical disk disorder. EE was told to follow up with EE's prior treating Dr for EE's neck as that is the same diagnosis as last time that occurred in 2018.

employee was taking coal height measurements in #8 heading when a piece of drawrock fell striking employee in the lower back, employee was down on knees getting the measurements when the rock hit them, height in the area was 77". Employee was treated and released to return back to work and worked until 7/13/20 when employee was taking off work

Employee was tramming a shuttle car from the miner to the feeder. At spad #13980 there were three hangers stranded together spanning from one bolt to another. One end of the hanger became dislodged swinging down catching the left shoulder of jacket and pulled them from the shuttle car causing injury to left shoulder and right foot.

The employee dropped a bottom roller on the offside of the belt and while coming back across the belt they grabbed a belt chain and put their left foot on the belt rail to step down to the mine floor when their foot slipped and their leg went between the belt and the belt rail hyperextending their knee.

Employee was reaching for drill steel in the tray of the roof bolter when upper back & Rt arm started to hurt. EE was taken to hospital & diagnosed with cervical disk disorder with radiculopathy. EE continued to work until 6/8/18 EE was taken off by a specialist & told EE needed an MRI. Attached are medical documents showing no mechanism of injury & diagnosis.

Employee was retrieving a grease gun off the 4-wheeler and as ee was leaning over foot slipped. EE sought medical attention two shifts later and was diagnosed with moderate degenerative changes. Because experience in low coal seams and the repeated trauma to knees we are reporting this as an occupational illness.

A non-injury roof fall occurred at approximately 05:30 am in the number 6 entry (primary escapeway), break # 55 along the #2 belt flight at or near SS spad # 1179. The area has been timbered and dangered off and the life line has been rerouted. MSHA and the State agency have completed their investigations, this fall will not be cleaned up.

Employee was cutting the bands on a pallet of roof bolts when EE cut the 2nd of 3 bands the bolts shifted, hitting the arm that was holding the knife, knocking it into left arm causing a puncture wound that required 2 stitches.

Employee stated that after dumping load of coal on the feeder ee started back toward the miner. About 15 feet from the intersection inby the feeder the shuttle car breaker knocked causing the park brake to engage. The employee was thrown forward and in an attempt to brace self stiffened causing pain in lower back and down legs.

Each time the employee was released to return to work. On a follow up visit on 7/8 the employee was taken off work due to continued pain in knee.

EE was taken to hospital and released to return to work, continued to until work until 6-25-14 at which time the DR told them they would need surgery to repair the ACL and was taken off work.

According to the incident description there wasn't an event, we request this be an occupational illness. MRI report shows degenerative disc disease. This report shows they had an MRI in 2008 also.
