Driving a rock truck hit a hole in the road felt pain in lower back. Started missing work on 6/18/2013 and returned on 6/22/2013.

While installing a clutch on a truck, the clutch started to fall, tried to hold it up and injured his back. Continued to work started missing work on 2/25/13.

While operating a dozer, a hose busted, spread fluid on engine causing smoke to get in the cab.

EE was hauling coal from surface mine in tractor trailer turned the truck over on it's side rounding Gator Bowl Curve.

EE stated that while dismounting the rock truck, he lost his grip on the hand rail and fell approx 14' from the top step.

EE stated stated while lifting a part from the floor, he injured his back.

Installing a front idler on a drill, had boom controller in right hand and slate bar in left hand. He was reaching the slate bar to another mechanic who was under the drill. Felt a burning sensation in his upper back.

While swinging a hammer to break down a tire, the hammer hit the rim, causing him to jam his wrist.

Employee stated that while swinging a sledge hammer, he lost his balance, slipped and injured his back. (Started missing work on 3/28/10).

Employee stated that while going up grade in loaded coal truck, he missed a gear, the truck rolled backward into a ditch, turned over on its side.

Employee was working on an engine installation for a Highwall Miner Generator Set. We suspect he inadvertently hit the door release button with his leg or foot and fell to the ground. He possibly has a broken left wrist and right shoulder.

After putting antifreeze in the coal truck, he slipped while dismounting and fell approx 3' to the ground.

EE stated that while hauling a load of coal to the prep plant, he hit a hole in the roadway, injuring his back.

Employee stated that while comming off the drill bench, the powder truck went in a ditch & came out and hit a dozer.

While traveling around sap tank curve, EE lost control of the truck and ran the truck into a ditch.

EE stated, that while operating the rock truck he hit a rough place in the road, jarring him.

Was using a pry bar to scoot an end bit off truck.

While driving an 18 wheeler hauling coal to the tipple the driver turned the truck over on the passenger side of the truck. Coming out of the Amos Run curve of Copley Trace haul road.

Employee stated that while operating the loader he felt a sharp pain in his lower back.

Employee stated that while cutting mud from the haul road, the blade caught a rock jarring the operator's back.

Driver was dumping coal truck. The hoist assembly at the top busted metal out and the trailer bed fell back down onto the frame of the trailer.

Employee stated that while operating the dozer the segments slipped in the track causing the dozer to jar him.

On his way to work, was going around a curve and Argus Energy's grader was backing around the same curve on his side of the road. The grader backed up on the front of his car, then pushed the car to the other side of the road.

While wiring up pig tale on truck, stepping down off the truck he slipped and fell and hit head on fender.

EE stated that while mounting the load grader, he twisted his knee.

Driver stated while going down belt line steep the engine brake acted like it working & sounded like it.But it would not hold the coal trk back and it started to speed away so he drove it to the escape ramp and entered the escape ramp.

Employee stated that while taking the radiator cap off the generator, steam came from the radiator causing burns to the right arm.

EE stated, that while mounting the loader, he felt a sharp pain in his left shoulder.

While changing filters on a 992 loader, he had to pound on the filter to loosen it, injuring his right arm.

Changing track pads on drill tracks. While lifting pads & installing on new rails, pulled muscle in stomach.

While driving along haul road a piece of gravel flew from tire, hit the mirror and busted the glass. A piece of glass hit the driver in the eye.

Driver stated while going thru the roller coaster dip the transmission started making a clicking noise. So he down shifted and the truck rolled backward into the ditch and caused the truck to turn over on the passenger side.

Employee using air sander to clean blade on grader. Sanding disk came apart, piece struck employee in the stomach.

Driver stated that as he came into curve truck started to turn and slide coming into and while trying, to go around the curve like the Jake Brake was shoving him as it kicked in.

Employee stated that while rounding the Gator Bowl curve the truck slid and turned over on its side.

EMPLOYEE STATED THAT HE INJURED HIS BACK WHILE LIFTING MATERIAL AROUND THE SHOP AREA.

EMPLOYEE STATED WHILE GOING THROUGH THE OLIVER LUCK DIP HE FELT THE REAR END OF THE TRUCK SLIDE TO THE LEFT AND THE TRUCK TURNED OVER ON IT'S DRIVER SIDE.

EMPLOYEE WAS STANDING IN FRONT OF PICKUP TRUCK WHEN THE FOREMAN STARTED THE TRUCK. IT LUNGED FORWARD STRIKING HIM ON THE RIGHT KNEE

Was raising hood on truck, and hand slipped off and he fell backwards.

The ee was working on a tailgate of a truck and slipped.

EE stated that while operating the endloader he twisted around inthe cab and felt pain in his lower back.

Employee stated that he stepped off of a battery box, twisted around and injured his lower back.

WHILE TRAVELING DOWN A STAIRWAY, EMPLOYEE SLIPPED & FELL INJURING THE LOWER PART OF HIS BACK.

EMPLOYEE STATED THAT WHILE WORKING ON A D9 DOZER, THE BATTERY LID FELL ON HIS LEFT HAND.

ANOTHER CO DRIVER HIT THE RANKAY TRUCK IN THE BACK END.

TRUCK STALLED ON HILL ROLLINGBACKWARDS AND OVERTURNING, RESULTING IN BRUISING TO LEFT KNEE AND LEFT RIBS.

WAS GOING AROUND CURVE AND THE OTHER TRUCK, GOING THE OTHER DIRECTION, SLID INTO TRUCK.

WAS GOING AROUND CURVE, ROAD WAS WET, AND SLID INTO ANOTHER TRUCK COMING THE OTHER DIRECTION.

EMPLOYEE STATED THAT WHILE GETTING A WATER SAMPLE FROM THE POND HE SLIPPED ON A ROCK AND INJURED HIS LEFT LOWER LEG.

WORKING ON TRUCK AND A PIECE OF METAL HIT EYE.

DRIVER WAS ON HIS WAY TO WORK IN HIS PERSONEL VEHICLE, WHEN HE SLID INTO A DITCH.

EE INJURED HIS LEFT HAND AND WRIST WHILE FUELING EQUIPMENT.

HE WAS REMOVING SPIDER DRUM ASSEMBLY AND PULLED SHOULDER OUT OF PLACE. WHILE WORKING ON A TRAILER.

VICTIM AND CO-WORKERS WERE USING A 3/4" LINK CHAIN TO PULL THE CONVEYER CHAIN OF THE HIGHWALL MINING MACHINE, THEY WERE UTILIZING THE HIGHWALL MINER POWER HEAD TO APPLY PULLING POWER. THE CHAI N BROKEUNDER TENSION AND STRUCK THE VICTIM IN THE MID AND LOWER TORSO RESULTING IN A BRUISED KIDNEY. MACHINERY TYPE ACCIDENT. LOST WORKDAYS.

GETTING OUT OF TRUCK AND FELL ON GROUND AND HURT RIGHT KNEE AND RIGHT ANKLE.

EE WAS CLIMBING ROCK TRUCK WITH TOOLS IN ONE HAND, SLIPPED ON LADDER AND FELL.

WALKING FROM DOZER TO MANBUS TRIPPED ON DIRT WINROW. FIRST LOST WORK DAY 4/17/01.

OPERATING LOADED 18-WHEELER. TRAVELED LEFT OF CENTER INTO SUMP HOLE ON LEFT SIDE OF ROAD. TRACTOR AND TRAILER OVERTURNED.

LOADER OPERATOR LOADING ROCK BEGAN EXP BACK ON.

INJURED WAS REPOSITIONING CRIB BLOCKS. CO-WORKERPASSED ONE CRIB BLOCK WHICH HIT INJURED IN LEFT KNEE.

OPERATING DOZER ON ROCKS; JARRED DOZER AND BEGAN EXPERIENCING LOWER BACK PAIN.
