EMPLOYEE STATED DROVE A WAYS AWAY FROM THE DUMPSTER TO GET OUT AND USE THE BATHROOM AND TRIPPED OVER SOMETHING. EE STATED NOT AWARE OF WHAT TRIPPED OVER. STATED EE HIT EE'S HEAD AND EE THOUGHT EE WAS OUT A MIN. OR TWO AND ARM FELT NUMB AND LIGHT HEADED.
INJURED HAD PUT UP EE'S SECOND BOLT IN THAT ROW AND NOTICED THAT IT WAS WIDE TO THE RIB. EE SWUNG THE BOOM OVER TO INSTALL AN EXTRA BOLT. EE STARTED DRILLING AND A SECTION OF ROOF FELL ON EE. STRIKING EE ON HEAD AND SHOULDERS.
INJURED WAS TRAVELING TO SECTION ON A LIL MAC THREE WHEELER WHEN A PIECE OF DRAW ROCK FELL STRIKING EE ON THE UPPER PART OF EE'S BODY. EE SUFFERED A LACERATION BESIDE EE'S LEFT EYE, REQUIRING 8 STITCHES. A CT\SCAN WAS DONE AND FOUND NO OTHER INJURIES.
INJURED WAS PUTTING HYDRAULIC OIL IN ROOF BOLTER. WHEN THE OIL CAN WAS EMPTY HE PULLED THE FILL HOSE OUT OF CAN, GRABBED THE OIL CAN WITH HIS RIGHT HAND AND THREW IT TO THE RIB. A SHARP EDGE ON THE CAN CAUGHT HIS GLOVED FINGER AND SHAVED THE SKIN FROM IT. IT WAS THE MIDDLE FINGER ON HIS RIGHT HAND.
Employee was backing up on rubber tired ride when he ran over rock / debris which caused him to hit ledge in roof and drag him off of ride.
Injured was operating a roof bolting machine in very low conditions. He was drilling a hole and was having trouble with the drill steel. While trying to get the steel in the pot, he caught his left thumb between the steel and chuck. Resulted in a cut/mashed and broken left thumb.
Injured was bolting top when a piece of rock fell from the top, striking him in his upper body, pushing him to the ground. He was setting up to install the outside bolt. After emergency room visit, was reported that nothing was broken, but he would be sore.
Injured was installing roof bolts. he put the resin up in the drilled hole, stuck the roof bolt in the hole, then slid his hand down the bolt in order to put the bolt in the chuck. His hand came into contact with a sliver of steel, from where the bolt was punched in manufacturing. the sliver of steel went through the side of his glove and into the small finger on his right hand.
INJURED WAS CHANGING A WHEEL BEARING ON A DAMASCUS LIL MAC BATTERY RIDE. WHILE KNOCKING THE WHEEL BEARING RACE INTO THE REAR-END THE RACE BROKE AND SENT A PIECE OF THE RACE INTO HIS LEFT ARM. THIS LEFT A PIECE OF STEEL UNDER THE SKIN ON HIS LEFT ARM.
INJURED WAS TRAMMING A SCOOP, HIT A HOLE, AND GRABBED TO CATCH HIMSELF FROM HITTING THE BACK OF THE SEAT AFTER THE BOUNCE. HIS LEFT HAND WENT BETWEEN THE DECK AND REAR SECTION OF THE SCOOP WHEN IT WAS CLOSING TOGETHER. THE TIPS OF HIS RING FINGER AND MIDDLE FINGER ON HIS LEFT HAND GOT CAUGHT BETWEEN THE TWO SECTIONS. STITCHES WERE USED TO CLOSE THE ENDS OF HIS FINGER TIPS.
INJURED WAS TRAMMING A BATTERY POWERED SCOOP HAULING SUPPLIES. AROUND BREAK 94 HE HIT A POTHOLE THAT JARRED HIM AND SLAMMED HIM INTO THE DECK OF THE SCOOP. INJURY WAS TO THE BACK AND NECK.
Employee was getting out of a tractor trailer. He slipped and fell hitting his head on the step of the truck.
Employee was unloading belt structure rails. Rail bounced off structure hitting employee on right leg and right rib causing injury.
Employee was moving a roll of belt lost footing twisting left knee.
Employee was changing a tire on a forklift at Rockhouse Creek #2 Mine. He stated he was pulling on the tire when he slipped (lost footing) and he stated he twisted his lower back. Safety dept administered first aid treatment until ambulance arrived
Bilateral carpal tunnel.
EE picked up jack and felt something pull in his groin area.
EE was traveling under bolter cable hanging from mine roof. The bolter cable dislodged striking EE.
Crew was traveling to section, pulled over to get out of the way of another ride. Crew pulled under cable the EE put his hand up to keep the cable away from his face, a rope hanger came loose and snagged EE pointer finger causing a laceration.
EE was bolting in No.1 heading, he had already put up his first bolt. He swung over to put up his second bolt, when he placed his steel against the top, a piece of coal broke loose hitting the him on the back.
Riding in a battery powered mantrip,at a belt drive boom hole, the mantrip driver decided to turn the mantrip around,the victim did not keep his head down in the low seam height and he hit his head on the mine roof.
EE was going to clean No.2 Entry when he turned the corner he hit an oncoming production scoop bucket to bucket. The impact of the collision caused the EE to be thrown into the top.
EE was cleaning up cut, when a piece of rock came off of conveyor chain striking the EE on the knee.
The EE was on a Mac 12 traveling to the outside. The EE had his hand on the side of the ride and his hand was caught between the ride and the rib.
The injured employee was crawling by the scoop. The scoop operator thought he was clear and already by him. The scoop bucket was steered toward the offside of the scoop catching the injured's leg, pinching it.
The injured person was hitting miner bit with a hammer to loosen bit up to change bit. A piece of the miner bit broke off hitting his left index finger.
employee was changing the coupling on the fresh water supply line, when the pump was started, the line broke, hitting him in the face.
Employee was riding three wheeler, struck head against low place in mine roof, causing injury.
Employee was shoveling tailpiece, tripped over 2" waterline, falling and injuring back.
Injured employee was making a cable splice. When striking the connector crimper with a hammer, a piece of metal came off the crimper and struck the employee's right hand.
Injured employee was trying to remove drill steel from the mine roof when it came loose, fell and hit his right eye resulting in a burn injury to the eye.
Injured employee was in a scoop being pushed out of the roadway when his bucket slipped off the other scoop bumper resulting in a back injury from the impact of the scoop falling.
Injured employee was swinging roof bolter boom to install outside roof bolt and caught his legs causing bruising to both legs.
He was helping hang miner cable and twisted lower back.
He was backing down #4 and was turning into #4 & #3 crosscut and ran over a crib block and it threw his knee and head into the top.
Miner was hung and he raised the stabjack and put a crib under it; when he let it down it busted and hit him.
Trying to get steel out of drill hole and caught his finger between drill steel and pot.
Had hand on drill pot, when cap coal came down hitting hand.
Spinning drill steel & pulling down on it to remove stuck steel out of top when glove got caught.
Lifting a rock up off of miner, rock broke off causing him to fall backward onto the ground.
Due to flooding conditions due to excess rain the seven foot drain pipe under the mine yard stopped up and water entered the portals. Resulted in water roofing at the #2 belt drive blocking all entries for travel.
EE was getting on a loader to load himself and his hand slipped and he pulled a muscle in his shoulder and elbow.
Loading scoop with top roller. EE threw roller into scoop and twisted his back.
Was setting steel on pot when rock fell hitting thumb. Rock measured 4" thick x 18" round.
Another ee was going to tram 3-wheels when he went in wrong direction pinning EE against bolter.
Bolting the top & started to drill a hole, loose rock came down and hit the employee.

Person was putting roof bolts up and got glue in right eye at #4 heading.
Running the loader to work, the road ran into a railroad track that was under the road. The operator's head hit the windshield of the loader and injured his head, neck and back. He was not wearing his seat belt.
He was riding down belt line (#4 entry) with co-worker and had his head up too high and caught it in the top.
Rock fell between rib and bolt being installed. Struck on right forearm causing right elbow to hit boom.
While drilling anchor hole, the fitting connected to the hose busted, splashing hyd. oil in eyes and mouth.
Holding cable for electrician to cut, the knife slipped, cutting his left hand.
The #1 & #3 MBCs were tramming. #2 had stopped - this caused the MBC to be pulled making it tilt up. This caused the injured to hit the mine roof.
WHILE INSTALLING ROOF BOLTS, A PIECE OF DRAW ROCK APPROX 12 X 16 X 3/4 THICK, FELL BETWEEN RIB AND BOLT BEING INSTALLED STRIKING VICTIM.
EE WAS SHOVELING ON THE OFF SIDE OF THE BRIDGES AT THE TAILPIECE WHEN THE BELTS WENT OFF. EE WAS CROSSING OVER THE BRIDGE CONVEYOR WHEN IT CAME ON, PULLING HIM DOWN THE CONVEYOR.
PERSON WAS TRAMMING BRIDGE. THE DECK OF THE BRIDGE FLOATED UP CAUSING INJURED TO TWIST HIS RT. KNEE AGAINST THE FRAME.

TRYING TO PULL OUT AN ANCHOR PIN ON THE FEEDER AND IT PINCHED MY FINGER UP BETWEEN THE FEEDER AND PIN.

Ee was operating #2 bridge. Miner backed up, bumped #1 bridge and shoved #2 into ee.
EMPLOYEE WAS CRAWLING ALONG THE BRIDGE LINE WHEN HE HURT HIS LEFT KNEE.
EE SAID LEFT KNEE WAS INJURED WHILE CRAWLING.
UNKNOWN- EE DID NOT REPORT AN ACCIDENT. HE REPORTED TO HIS SUPERVISOR HE WOULD BE OFF ON A PRIOR INJURY FROM ANOTHER COMPANY. OUR FIRST NOTIFICATION OF AN INJURY WAS A COMP. FORM RECEIVED ON 9/30/03 INDICATING A BACK INJURY.
