Employee was Resetting feeder, while tramming the feeder toward himself, the offside cats slipped off a ledge that was in the in the bottom Approx. 6" high, causing the feeder to drop suddenly. Employee's leg, from the knee down in up right position was under the tram controls, this compressed his leg causing a fracture to his left Ankle.

EE was bolting from the operator side in #5 face one the section. Seam height is 50" so he was bending the 48" bolt enough to install. Bent one bolt and was straightening it back after placing in the hole with his left hand. He bumped the drill rotation with his right hand and the bolt whipped around, striking his left wrist. Received a fracture and was placed on light duty.

During a power move, Employee was cutting nylon hangers from highline. Using his left hand he cut hanger while his right hand was supporting the cable. The knife cut his right forearm. He drove to Raleigh General and received four stitches. He returned to work on his next shift.

EE, Electrician and his helper were replacing a feeder tailshaft. EE had the end of the tail shaft raised with a slate bar. He reached under the shaft to make an adjustment when the shaft fell on his middle finger. He drove to ARH hospital and received several stitches to repair damage to the end of his finger. He returned to work on his next scheduled shift.

A unplanned roof fall occurred in the #3 3ntry. 1 break right from spad# 3556.This fall occurred 11,000' from the active face. Ventilation and passage of persons was not affected. Area will be re-hab'd. Fall meas. approx. 12' L. X 20' W. X 4' H. Area was supported with 4' resin bolts. Federal and State agencies were notified immediately.

Employee was helping unload a belt structure sled from a scoop. As he attempted to unhook a chain from the back of the sled the operator pulled the ram back and caught injured finger between the ram and bucket.

Maintenance Forman was using a chain hoist to lift replacement center drum on continuous miner head. The hoist slipped and fell onto his left hand. He finished his shift and then sought medical care. He was diagnosed with multiple fractures to the left hand. He has continued to work.

EE knelt down by the pressure pump to fill out a report. He removed his gloves and laid his left hand on the pressure pump cage as he knelt. His head bumped a pizza pan and dislodged a rock measuring 14"X12"X1. It fell and cut three outside fingers. He finished the shift. Clinic did not treat the cut except for antiseptic until an orthopedic could on 9/16/10. Found small fracture.

EE was spot bolting in #3 entry. He had his hand on the joystick & his right hand on the bolt. He said he was straightening the bolt to go in the chuck. His hand was between the plate & roof bolt when he engaged the joystick. His metacarpal glove caught the plate & twisted his arm with it. He was taken to CAMC where surgery was performed on 8/18/10. Bolt was a 4' resin.

EE was hanging curtain. He turned and ran into scoop canopy. He said it knocked him back and he twisted his shoulder. He did not report this until late on 8/16/2010. There were no witnesses.

While draagging 4" water line with a pull rope, member slipped in mud and twisted left knee. X-rays were negative and he continued to work until July 12, 2010 when he made an appointment to see an orthopedic surgeon. The doctor told him to take 4 weeks off and have tests to determine damage.

EE was welding bit lugs on right continuous miner between shifts and got a piece of slag in his right eye. He drove himself to a clinic after work and had it removed. He returned to his regular scheduled shift today (5/26/10) and to his regular job as section electrician.

Scoop operator leaned forward over controls in deck to check contents of bucket. The scoop was energized & began moving, mashing torso against mine roof. Canopy is not required in area where seam height is between 40-48". Employee received fractures to his eighth and ninth thoracic vertebrae.

EE HAD JUST SET ATRS ON ROOF BOLTER AND SAW IMMEDIATE ROOF CRACK. EE STARTED RAISING AND LOWERING ATRS IN ATTEMPT TO BREAK THE ROCK. AFTER SEVERAL ATTEMPTS THE ROCK BROKE FREE AND RODE BACK TO THE OPERATOR CONTROLS, STRIKING EE IN HIS LEFT SIDE LEG AND HIP AREA. ROCK SIZE WAS 30" LONG X 20" WIDE X 4" THICK.

THE HOSE WAS COILED IN A TIGHT PLACE DURING INSTALLATION, CAUSING O NE END OF THE HOSE TO COME OFF OF THE FITTING STRIKING THE INJURED IN THE RIGHT EYE, CAUSING SCRATCHES ON CORNEA.
