Individual was unloading first aid boxes from a van and his finger was smashed between two boxes. Individual was treated at Raleigh General Hospital and was diagnosed with a fracture to the tip of the left middle finger and also received thirteen (13) sutures. Individual is not an employee at this location (works at Maxxim Shared Services).

As Employee was walking from 27 to 28 seal he placed his left foot on edge of a hole and twisted his foot causing his ankle to roll. He was transported by EMS to CAMC General Division where he was diagnosed with multiple fractures of the ankle and had surgery to repair the damage.

Employee was pressing down on ratchet the filter housing broke loose smashing his finger between the ratchet and frame of the machine. Employee was treated at MedExpress and diagnosed with a fractured left middle finger in three (3) places.

Member was setting bits on the left continuous miner and smashed his ring finger with a hammer which cut his finger and cause his fingernail to fall off. Member immediately ceased work and was transported by paramedics to the hospital. At Raleigh General Hospital he was told to remain off work in order to avoid the risk of infection.

Member was in the deck operating the bolter. He went to reposition himself and stuck his hand on top of the canopy to pull himself up and hit the tram. The bolter moved, smashing his right hand into the top. Member immediately ceased to work and was taken by ambulance to CAMC General where he was told his right hand/middle finger had to be amputated.

Member was lifting come alongs above head to put into monkey face in order to bring a wrecked flat car over and onto track rail. While lifting he experience a "pop" sensation in back, he immediately ceased to work. He came outside and reported injury.

Employee was atempting to clean the dumping point with a scoop. As he passed underneath a low hanging cable he attempted to raise it up causing his left little finger to be mashed between the scoop canopy and the mine roof. This injury required stitches to be installed.

While welding on the dipstick tube, a piece of slag popped off, hitting his right eye lid. He was wearing glasses but the slag came in from the top of the frame and stuck to the eye lid. He was also using a welding helmet. EE did not stop work but later he did go to the clinic to make sure the burn did not hurt his eye.

member attached the fire hose to a value and turned the water on. The value was leaking so he was going to turn off the water and use the next value down the hill. value off it exploded into several pieces. Part of the value, still attached to the hose, struck him in the mouth. He was also struck in the left wrist and inside the right arm.

Due to experiencing pain in his neck he went to the hospital and was referred to a specialist for an MRI. On 03-05-2009 EE informed the Safety Department that he had been diagnosed with a bulging disc in his neck and not to work and attend therapy.

All approaches have been timbered off. Ventilation and passage of people not affected. Area was supported with 42" fully grouted bolts with 16' J-channel.
