EE was taking 4 waterline apart when it busted and projectiles from waterline stuck EE on his right hand. EE finished shift and reported to work on 1/6/09 but did not finish shift. EE is scheduled by doctor not to return to work until 1/16/2009. X-rays were negative.

While doing battery maintenance an acid water solution erupted out of the cell and came in contact with the employees eyes and face. Safety glasses were being used. The employee was immediately doused with water.

The employee arrived at the mine site not feeling very well. He was leaning over the handrail at the mine office, prior to work. He was convinced to seek medical attention.The employee went home, showered and went to the hospital. He is diagnosed with blood flow restrictions, on medication with rest. No incident occured at the site.

An unintentional roof fall was found approximately 11:15 am on 1/17/08. The fall was located at the intersection of spad 793 on the old # 3 Belt Line. The #3 Belt Line is no longer in service. The fall is dangered off and all approaches have been cribbed. The fall will not be cleaned up. The dimensions are 20x20x5. The fall occurred due to weathering in the mine roof.

EE was riding personnel carrier when he went to adjust seat cushion,right hand slipped,he grabbed the side of the personnel carrier,the personnel carrier hit the mine floor, it smashed right ring finger and pinky finger against piece of rock. Cause of injury was awareness.Compliance with rules/regs not a factor,mining equip/systems not a factor, miner proficiency a factor.

An Un-Intentional roof fall was found in the number 5 entry outby spad # 651. The size of the fall measured 60 feet long by 20 feet wide by 5 feet high. All approaches to the fall were cribbed and dangered off. The fall occurred due to weathering in the mine roof. Type of roof support installed was 4 foot double-lock resin assisted bolts.

Employee was riding on mantrip when his CSE SR-100 he was wearing on his belt flipped over off the side of the mantrip and was caught between mantrip and portal canopy. His right arm struck the canopy cutting his arm. Miner proficiency a factor, mining equiment could be a factor, procedures and policies not a factor.

During a weekly airway examination a roof fall was discovered in # 1 enty 6 cross-cuts outby the face at spad # 998. The fall has been cribbed off with danger signs and will not requir clean-up. The fall measured 20 feet X 20 feet and 6 to 7 feet high. The intersection showed signs of failure during development and 6 foot double-lock bolts were used in the intersection.

During a pre-shift mine examination after a holiday week-end a roof fall was discovered in #3 entry, LOB, on 001 section, that measured 60 feet long, 20 feet wide and 6 feet to 7 feet high. The fall was cribbed off and plans are to clean up and re-support the fall.

During a weekly airway examination a roof fall was discovered approximately 40 feet inby spad no. 967. The fall measured 40 ft X 20 ft and above the anchor zone using 48 inch double lock roof bolts. The fall is 12 cross cuts outby the face. The fall area was supported and dangered off on all approaches. The fall will not be cleaned up.

During a weekly airway examination a roof fall was discovered in #4 entry 30 feet inby spad # 937 and extended 30 feet inby spad # 951. The fall is 20 feet wide approximately 40 feet long and 5 feet to 6 feet high. It is located approximately 20 cross-cuts outby the face and will not be cleaned up. Approaches are timbered and dangered off.

Employee was assisting setting conveyor belt tail piece. Employees tried to pry loose the stuck belt, the come-a-longs holding the tail piece loosened and the tail piece fell down on the employee's right foot. Compliance with rules/regs not a factor. A roof fall was found in the crosscut from the number 2 entry to the number 3 entry between survey spads 830 and 831. The fall measured 6' high by 20' wide by 40' long. Cause of the fall was the deterioration of the main roof due to weathering. The roof fall did not obstruct ventilation or escapeways.

EE was operating rubber tire Mack 8 personnel carrier when the front wheel hit a hole in mine floor causing the steering wheel to spin catching his right pointer finger, causing a dislocation.
