Employee was using a wrench hand tool to loosen up a nut threaded on a bolt. Employee stated when the nut broke free the employee's momentum took their hand forward resulting in left pinky finger being caught between the wrench and guarding. Employee's left pinky finger was cut and broken.

Employee was cutting pieces of rubber off a screen using downward force with a razor knife. Employee had a rubber piece to tear and the downward momentum resulted with the employee striking right leg with the razor knife. Employee had a laceration and was treated at a local facility that administered treatment with 7 sutures.

Due to the size of the material being loaded there was minimal space to stand on the flat bed. While trying to strap the material down, our employee fell from the truck to the ground.

Tractor trailer was dumping a load when a strong wind gust turned trailer and tractor over. There was about 3 tons of coal frozen in the left front of the trailer due to sub zero wind chill. Truck was on a slight grade. All these factors combined is what caused the accident.

Injured employee was lowering a motor using a 1 1/2 ton chain ratchet and unlocked the ratchet to use the wheel to lower the motor. When the motor began to fall, the employee grabbed the chain pulling EE's hand into the ratchet resulting in a laceration that required 7 sutures and fractured the tip of the left index finger. No Lost or Restricted Workdays.

Employee was using a 10lb hammer to hit a steel beam and the beam shifted, causing the employee to miss and hit the left middle finger on a piece of old metal.

Employee and co-worker were pulling a piece of conveyor belt that was 6 feet wide by 8 feet long and the employee lost his footing and then felt pain in his back. Employee was seen by a physician and removed from work.

Employee was per-shifting end loader and as he was walking around the machine and lost his footing and fell. As he fell, he tried to put his hands down and catch himself. In doing so he fractured and dislocated his wrist.

Employee received an abrasion to the leg on 4/1/2013, while transporting an electric motor on a carte. Employee was transported to CAMC, where he was treated and release at this time. On July 10, 2013 received medical documentation that ee was treated and diagnosed for cellulites with chronic ulceration at CAMC. EE was give antibiotics and taken off of work.

Employee was a passenger in a pickup truck being driven by a fellow employee. They were returning from the loadout to the plant after loading a train. The pickup struck a berm and rolled on its right side. Employee received lacerations of the right arm from the accident.

Employee was walking from feed hopper to his end loader. There was a windrow of coal about knee high and he attempted to step over other than walk around it. As he stepped over, he tripped on a lump of coal with his right lower leg striking a sharp piece of coal causing a puncture wound. He received two sutures in treatment of this injury. Missed no work.

EMPLOYEE WAS EXITING THE REAR OF THE FORD F450 SERVICE TRUCK. THE TRUCK WAS EQUIPPED WITH A CABLE STEP MOUNTED TO THE REAR BUMPER. AS HE STEPPED ON THE CABLE STEP, THE WELD CONNECTING THE CABL E AND RETAINING NUT BROKE. AS THE STEP GAVE WAY THIS CAUSED A TWISTING MOTION TO HIS BACK RESULTING IN A BACK MUSCLE STRAIN. HE WAS DIAGNOSED WITH MUSCLE SPASMS AND RESTRICTED FROM WORK.

EE WAS REMOVING A GUARD FROM THE COLLECTING BELT ON THE BASEMENT FLOOR. OTHER EE'S WERE WASHING DOWN THE FLOOR ABOVE. AS HE WAS REMOVING THE GUARD, A RUSH OF WATER CAME FROM ABOVE. HE TRIED TO GET OUT OF THE WAY AND AS HE STOOD UP, HIS HEADHIT A PIECE OF CHANNEL STEEL SUPPORTING THE REFUSE BELT. HE SUFFERED A LACERATION ACROSS THE BRIDGE OF HIS NOSE AND A BLACK EYE.

EE CLAIMS HE PICKED UP A TURNDOWN TO PUT ON A LINE TO A SUMP. WHEN HE PICKED UP THE TURNDOWN TO PUT IT IN PLACE HE HAD TO TWIST WITH IT TO GET AROUND A 4" LINE THAT WAS IN HIS WAY. WHEN HE DID THIS HE FELT A SHARP PAIN IN HIS RIGHT LOWER BACK.

EMPLOYEE WAS REPLACING A SCREEN IN THE REFUSE SIEVE BOX. INJURED WAS LIFTING UP ON ONE END OF THE SCREEN IN AN ATTEMPT TO GET THE SCREEN PROPERLY ALLIGNED. AS HE WAS LIFTING AND PULLING, HE FE LT A SHARP PAIN BETWEEN HIS HIP AND GROIN AREA. ON 9-28, HE FELT A KNOT IN HIS GROIN AND SAW PHYSICIAN. PHYSICIAN DIAGNOSED HIM WITH A HERNIA IN THE GROIN AND SCHEDULED A REPAIR OF HERNIA.

EMPLOYEE WAS WORKING ON ADJUSTMENT IN THE HEIGHT OF THE SEAT IN THE KOMATSU LOADER. AS HE WAS MAKING THE ADJUSTMENT HIS RIGHT HAND SLIPPED OFF THE ADJUSTMENT KNOB. HE WAS BOUNCING IN THE SEAT ATTEMPTING TO GET THE RIGHT ADJUSTMENT. THE END OF HIS RIGHT MIDDLE FINGER ENTERED THE AREA OF THE SCISSOR MECHANISM AND WAS MASHED AS HE PLACED WEIGHT ON THE SEAT.

EE WAS WALKING FROM THE CHANGE TRAILER ACROSS THE ROAD TO START THE REFUSE DOZER. HE CHOSE TO WALK DOWN A SMALL EMBANKMENT ALONG THE ROADWAY. THERE WAS A LIGHT ACCUMULATION OF SNOW ON THE GROU ND AND THE GROUND WAS FROZEN. THE EMBANKMENT HADBEEN RUN OVER BY A LARGE END LOADER MAKING IT HARD & SMOOTH. WHEN EE STEPPED ON EMBANKMENT HIS FEET SLID & HE FELL LANFDING ON HIS BACK & NECK.
