Member was helping to change pick breaker in feeder, on end dropped mashing his left index finger resulting in a fracture. Member did not report this to mine management until 7-7-2008

Member was helping to install timbers to recover a MRS support, As he was lifting timbers he felt a pain in his back, MRI was preformed on 5-27-08 and he was placed on restricted duty on 6-2-08

Walking along belt line, his utility knife hanging on side of his belt with blade open, caught him in the palm causing a laceration requiring 3 stiches. He returned his next shift to work.

EE had finished moving miner into # 5 face for next cut, after pulling into face and backing up 4' to 5' he shut off the miner and walked along side of miner, after that he restarted miner and was taking pull ropes off cable, as he bent over he must have hit the boom swing lever causing boom the mash him against rib resulting in fracture to 4 ribs on right side

After putting oil into miner member was throwing empty oil cans into scoop, the handle on the can can loose caughting his right middle finger causing a laceration requiring stiches

A accident reportt #544343 was sumitted on employement. Laceration on right middle finger, After putting oil into a miner, he throw the can into a scoop bucket. Handle can loose on can, cutting his right midle finger. On 1-14-2008 he was place on restricted duty due to staff infection to his finger.

EE was tramming scoop through a mud hole. A scraper was in the mud hole. EE did not see it, scraper came up and hit him in the right shoulder.

Member was pushing roof bolts off of the scoop onto the roof bolter when the bundle of roof bolts fell pinning members left pinky finger under the roof bolts.

Employee was walking to the end of the track when he stepped in a hole causing his ankle to twist. X-rays revealed a fracture to his left ankle. He was unable to perform his normal job, so he was placed on restricted duty.

Member was drilling first hole in the #1 entry. A rock fell above canopy and broke, a piece of the rock slid down the drill steel, stricking his left forearm. He was transported to Boone Memorial Hospital, where he received 17 stitches.

EE was assinged to walk belts into section and see what work needed done, at the 1-a drive he helped to unload their tools out of the mantrip, He then walked over to belt and shoveled a couple of shovel's onto the belt, He stated to other EE that his chest was hurting. He sat down for a minute started to shovel again and said he had severe pain. He had 2 blood clots

While making a belt splice using a flexco shop hammer to drive the nails in with a plunger - as he swung to drive in the nails, he missed mashing his index finger between the hammer and river causing a fracture to his index finger.

A roof fall occurred on the #2 section 1 break outby spad 1028 in the belt entry, 1200ft from the working section. Fall was 18'X 18'X 5'. 6ft torque tension bolts were being used in area. The roof fall was called in 0n 5/5/2006 to MSHA.

EE WAS INSTALLING A PIN IN THE OUTBY RIB TAIL HOLE.HE WAS HOLDING THE PIN WITH HIS RIGHT HAND & STRIKING THE PIN WITH A #3 FLEXCO HAMMER. HIS RIGHT LITTLE FINGER WAS BETWEEN THE PIN AND FRAME OF THE CONT. MINER WHEN HE HIT THE PIN WITH THE HAMMER CAUSING IT TO STRIKE HIS FINGER FRACTURING IT.

A roof fall occurred at the Spirit Mine on 01/18/2006. The size of the fall was 20'x 12'x 8'. 6' Torque Tension support was being used. All entrances have been breakered off and the fall will not be rehabilitated.

Employee was moving the right miner to the #9 Entry. When he moved the miner cable out of the roadway he pulled a groin muscle. He completed his shift on 1/16. He called on 1/17 stating he was going to the doctor. He called on Wednesday and stated he was scheduled for surgery on 1/20/2006.

EE WAS CARRYING CRIB BLOCKS TO FLAT CAR AND SLIPPED IN THE MUD. HE FELL DOWN AND THE CRIB BLOCK LANDED AND JAMMED HIS LEFT HAND INTO THE GROUND. HE WAS WEARING GLOVES AT THE TIME OF THE ACCIDENT.

CREW WAS LEAVING SECTION TO GO OUTSIDE, A PIECE OF 4 INCH WATER LINE WAS LYING ALONG SIDE OD TRACK. IT CAUGHT ON MANTRIP AND CAME INTO DECK STRIKING HIM ON THE RIGHT ARM AND SHOULDER. HE WORKED UNTIL 10-18-2005 AND CALL IN STATING THAT HE WAS GOING TO SEE A DOCTOR. AT THIS TIME HE WAS TAKEN OFF WORK.

INJURED WAS MAKING BELT SPLICE. HE WAS DRIVING NAILS IN SPLICE USING MASONRY HAMMER WHEN HEAD OF HAMMER CHIPPED METAL OFF HAMMER STRIKING HIM IN THE UPPER ARM APPROX 2" ABOVE ELBOW. SUTURES WERE REQUIRED.

Track crew was taking rail off cars, co-worker was moving rail with motor while injured was using slate bar to keep rail out of ties. slate bar became hung up, ee tried to free it, rail slid up bar causing him to strain his wrist.

Ee was installing a curtain board to the roof in an intersection between the #3 #4 entries on the operators side of the roofbolter. He drilled the hole with c piece of 3' starter drill steel and was bringing the steel back down out of the hole when it fowed and bowed striking his arm causing a contusion.

Employee was installing a 5' torque tension super-twist bolt. 4.5' of the bolt was up into the hole when it started to bend. He tried to push the bolt on up into the hole when the head broke off. When dropping the wrench to install a new bolt, the plate lying on top of the wrench fell off & hit him on the bridge of the nose causing a laceration. Sutures were required.

MEMBER WAS OPERATING THE RT. SIDE SHUTTLE CAR WHEN THE ACCIDENT OCCURRED. HE WAS LEAVING THE MINER W/A LOAD OF COAL FROM THE #8 HEADING WHEN HE TURNED INTO THE #7 RT. CROSSCUT & CAME IN CONTACT W/THE MIDDLE SHUTTLE CAR THAT WAS WAITING TO CHANGE OUT. THE OTHER SHUTTLE CAR WAS NOT MOVING AT THIS TIME. EE'S SHUTTLE CAR GLANCED OFF THE MIDDLE CAR CAUSING HIM TO STRIKE THE COAL RIB.

EE WAS APPLYING BLOCK BOND TO 2 AIRLOCK BELT STOPPINGS. HE WAS WEARING SHORT RUBBER GLOVES AND THE BLOCK BOND MADE CONTACT WITH HIS WRISTS. EE WORKED THROUGH HIS SHIFT. HE TOLD HIS SUPERVISOR THAT HIS WRISTS WERE BURNING AT THE END OF HIS SHIFT.
