Employee was hooking up a chain to a battery charger with a track mounted locomotive. As the locomotive operator tightened up the chain to remove the slack, employee's right index finger became pinched. Resulted in a laceration requiring 6 sutures.

While walking down the belt entry, the employee tripped on a cable and fell injuring EE's knee. **This was deemed compensable by workers compensation on July 11th, 2019.**

While changing batteries on scoop, the battery forks on the scoop got caught on a battery tray as operator was bowing them down. The forks slipped off the battery tray causing the operator to be bounced around in the operator deck. Lacerated operator's ear resulted in stitches.

Miner was cleaning entry for track installation utilizing a Fairchild scoop. While cleaning, the scoop ran over the track rail causing the operator's deck to make contact with the mine roof. The miner's head was slightly pinched between the mine roof and scoop frame. Resulted in lost work days and sutures.

Miner was working to clear an obstruction on a wash down hose. The line was pressurized causing the end of the hose to fly up striking the miner in left eye. Eye drops were prescribed.

While backing the roof bolting machine down the no. 3 entry the cable reel was taking up cable causing the trailing cable that was hung to be jerked down striking the miner on the head.

Miner was assisting in changing conveyor boom on a Joy 14/15 continuous miner when the boom unexpectedly shifted. The miner was caught between the boom and the mine roof injuring miners back. Injury resulted in paralysis below miners waist.

An unintentional roof fall occurred approximately 2 breaks inby spad 16356 on the Low Gap Mains at 15 break along the old 8A belt conveyor in an adjacent entry. The area was supported with 48 inch fully grouted roof bolts. The fall measured 40ftL X 20FtW X 4ftH. All approaches to the fall have been supported. The fall will not be cleaned up.

Employee was helping co-worker replace a 6 inch plastic "T" waterline in the conveyor belt entry. After replacing the old "T" they turned the water back on and was examining the "T" for leaks when the 6 inch "T" burst causing pieces of the pipe to strike EE in the face.

As the cats were being raised Employees right ring finger was pinched between the crib and the lifting jack. Employee received treatment at CAMC General Division where he received 13 sutures.

An unplanned roof fall occurred on the #3 Section spad 13020 at 6A Belt Head tailpiece. The fall was approximately 30' x 20' x 4' to 6'. The 6A Belt Head, Tailpiece was covered by the fall. No injuries were reported.

Contractor later went to Raleigh General seeking medical attention, where he was told he had sustained first and second degree chemical burns to his hands and arms.

He felt a pop in his neck on the left side. The area was 48" high and 18 1/2 feet wide. Member was taken by ambulance to the hospital where he was diagnosed with a fractured vertebrae.

A roof fall occurred on # 2 section in # 3 entry at appromixately 4:50 PM. The roof fall measured 80' long 20' wide and 4' high. The area was supported with 48" fully grouted resin bolts. There were no injuries. Fall was immediately timbered or cribbed off at all entrances.

EE WAS DROPPING A PIN THROUGH A TONGUE IN THE ACT OF HOOKING UP A RAIL CAR TO A MOTOR. THE MOTORMAN PULLED FORWARD RESULTING IN THE PIN KICKING FORWARD, MASHING / PINCHING HIS FINGER.
