A highwall failure occurred in the Stockton pit #24 area of Tower Mt Surface believed to be caused by subsidence from previous underground mine works in the coal burg seam below. No injuries resulted from the event.

Employee had been cutting and welding and a small piece of metal got into his eye. He was wearing safety glasses and face shield. He was prescribed eye drops and ointment for his eye.

Employee was cutting off the right side of the truck bed. Had already removed 2/3 of right side. Cut 3 of the 4 wearbars/gussetts. Employee then moved back to the outside of the bed to continue cutting when the port he was working on broke loose and fell striking employee and pinning him on the right side leg and arm along with his left foot.

Using a one inch impact wrench to remove R.O.P.S. bolts on 992G. When bolt broke loose, EE threaded upwards and smashed left hand between wrench & angle above it.

EE was attempting to mount a rock truck. Upon climbing the steps onto the bumper, EE lost his footing and fell from the truck, landing on the ground. He continued to work until he was seen by a specialist on 12/15/08 that referred him for an MRI for diagnostic care. On 1/5/09, EE was taken off work by the specialist and prescribed therapy for his condition.

HE BACKED OVER A ROCK IN A CAT D8R DOZER WHICH IRRITATED HIS PRE-EXISTING INJURY. HE IS CURRENTLY UNDER A PHYSICIAN'S CARE. NO DEFINITIVE DIAGNOSIS HAS BEEN MADE AT THIS TIME.

EE WAS OPERATING A 1995 BURGUNDY COAL TRUCK OFF THE HILL OF BANDMILL COAL HAULAGE ROAD. WE BELIEVE THAT THE EMPLOYEE WAS SHIFTING GEARS DUE TO THE FACT THAT THE TRUCK WAS FOUND TO BE IN NEUTRA L ON THE LOWSIDE. THE EE WENT 7/10 TO 9/10 OF A MILE WITH THE TRUCK IN NEUTRAL AND THE EE WAS RIDING THE BRAKES AT WHICH TIME THE EE JUMPED FROM THE TRUCK.
