A highwall failure occurred in the Stockton pit #24 area of Tower Mt Surface believed to be caused by subsidence from previous underground mine works in the coal burg seam below. No injuries resulted from the event.
Employee had been cutting and welding and a small piece of metal got into his eye. He was wearing safety glasses and face shield. He was prescribed eye drops and ointment for his eye.
Employee was cutting off the right side of the truck bed. Had already removed 2/3 of right side. Cut 3 of the 4 wearbars/gussetts. Employee then moved back to the outside of the bed to continue cutting when the port he was working on broke loose and fell striking employee and pinning him on the right side leg and arm along with his left foot.
Employee was welding throughout the course of his shift and developed flash burn to his eyes.
Using a one inch impact wrench to remove R.O.P.S. bolts on 992G. When bolt broke loose, EE threaded upwards and smashed left hand between wrench & angle above it.
Member was assisting in unloading a tool box and the handle unexpectedly came loose, causing the member to twist his ankle resulting in a fracture.
EE was attempting to mount a rock truck. Upon climbing the steps onto the bumper, EE lost his footing and fell from the truck, landing on the ground. He continued to work until he was seen by a specialist on 12/15/08 that referred him for an MRI for diagnostic care. On 1/5/09, EE was taken off work by the specialist and prescribed therapy for his condition.
Member was operating d10 dozer in 15' shot material and backed into a large rock with right track causing him to jar his lower back.
Employee was carrying an oil can through the shop when he tripped and hit his left hand on the oil can cutting his left middle and ring fingers.
Member was pressing a bearing on the push arm trunnion, when a piece of metal flaked off embedding in his right forearm.
Employee was mounting a cat DIIR dozer when he slipped & fell to the ground.
While pulling out dipstick to check oil level on a 785 truck, he cut his right thumb.
EE was helping to lift a piece of metal off his welding truck, when the metal fell, mashing his left ring finger.
Member was changing a tire on a coal truck when the wheel spun and caught his right hand between the tire and spoke.
Employee was removing a hydraulic line from a truck when wrench slipped allowing his hand to strike sharp surface resulting in cut to right hand.
EE WAS TRAMMING A 16G GRADER TOWARD THE PARKING LOT WHEN THE MEMBER RAN INTO A SUMP AND EJECTED FROM THE GRADER
Member picked up a 55-gallon barrel weighing approximately 150 pounds.
While employee was being loaded a rock slid across the load hitting the right rear side of the bed causing employee to be jarred in the truck.
Member was dismounting hoe when he missed the last step on the ladder and landed on his right foot.
Member was leaving property after shift and overturned his personal vehicle on property access road.
MEMBER ALLEGES HE HIT A HOLE IN THE HAUL ROAD CAUSING THE SEAT BACK TO FALL BACK IN THE ROCK TRUCK HE WAS OPERATING.
EMPLOYEE WAS USING A KNIFE TO CUT HOLES IN A MUD FLAP TO ALLOW HIM TO MOUNT THE MUD FLAP & HIS KNIFE SLIPPED CAUSING A LACERATION TO HIS LEFT FOREARM.
ON 9/20/04, EMPLOYEE SOUGHT MEDICAL TREATMENT FOR LOWER BACK PAIN. HE DOES HAVE A PRE-EXISTING BACK INJURY. HE WAS LAST OFF WORK ON 9/14/00 TO 11/06 DUE TO HIS CONDITION. HE BACKED OVER A ROCK IN A CAT D8R DOZER WHICH IRRITATED HIS PRE-EXISTING INJURY. HE IS CURRENTLY UNDER A PHYSICIAN'S CARE. NO DEFINITIVE DIAGNOSIS HAS BEEN MADE AT THIS TIME.
MEMBER WAS CLIMBING ON A 988F LOADER WHEN HIS FOOT SLIPPED CAUSING HIM TO FALL APPROX. 4' TO THE GROUND. RESULTING IN 3 CRACKED RIBS.
CLIMBING ON A 992G LOADER, FOOT SLIPPED, TWISTED LEFT KNEE.
EE WAS GREASING THE RIPPER ON THE REAR OF THE D-11R DOZER BETWEEN SHIFTS. AS HE WAS STANDING ON THE REAR OF THE DOZER HIS LEFT FOOT SLIPPED & WENT THRU AN OPENING, CAUSING HIM TO FALL WITH 1 L EG ABOUT 2'.
BASED ON EE WORK HISTORY IT APPEARS HE IS ENTITLED TO THE PRESUMPTION THAT ANY CHRONIC RESPIRATORY DISABILITY RESULTED FROM EMPLOYMENT.
MEMBER WAS CUTTING A PIECE OF RUBBER HOSE WITH A UTILITY KNIFE. HE CUT THROUGH THE HOSE INTO HIS LEFT THIGH.
EMPLOYEE WAS OPERATING LOADER. HE STUCK HIS HAND INTO FAN. HIS FINGER WAS CUT BY FAN REQUIRING 7 STITCHES.
EMPLOYEE WAS CONDUCTING POST BLAST INSPECTION. HE WAS STUNG TWICE BY YELLOW JACKETS (RIGHT ARM, LEFT EYE) CAUSING ALLERGIC REACTION. EMPLOYEE WAS TREATED AT EMERGENCY ROOM BY INJECTION AND PRE SCRIPTION.
MEMEBER WAS USING A SLEDGE HAMMER TO DRIVE A SHAFT OUT OF SPEED REDUCER. HE STATED THAT HE WAS USING A HAMMER AND FELT PAIN IN THE SHOULDER.
MEMBER WAS WALKING DOWN THE STEPS OF A 785 ROCK TRUCK WHEN HE SLIPPED ON ICE ON THE BOTTOM STEP CAUSING HIM TO SLIP DOWN TO THE BUMPER. THIS CAUSED A STRAIN TO THE LOWER PART OF HIS BACK.
EMPLOYEE WAS CUTTING WITH TORCH OVERHEAD WHEN A HOT PIECE FELL INTO SHIRT SLEEVE AND BURNED HIM ABOVE THE RIGHT WRIST. THE BURN GOT INFECTED AND HE HAD TO GO FOR TREATMENT 12/4/01.
MEMBER BEGAN TO CLIMB IN HIS COAL TRUCK AT START OF SHIFT, WHILE PRE-SHIFTING. AS HE STEPPED ON THE BOTTOM STEP OF THE TRUCK, IT BROKE CAUSING HIM TO TWIST HIS RIGHT ANKLE.
EE WAS IN THE PROCESS OF CHOPPING COAL WITH A 992C LOADER WHEN HE BEGAN TO FEEL PAIN IN HIS LOWER BACK. EE HAS A HISTORY OF MILE DEGENRATIVE CHANGES IN HIS PRE-EMPLOYMENT PHYSICAL THEREFOR THI S HAS BEEN CLASSIFIED AS AN OCCUPATIONAL ILLNESS.
EE WAS OPERATING A 1995 BURGUNDY COAL TRUCK OFF THE HILL OF BANDMILL COAL HAULAGE ROAD. WE BELIEVE THAT THE EMPLOYEE WAS SHIFTING GEARS DUE TO THE FACT THAT THE TRUCK WAS FOUND TO BE IN NEUTRA L ON THE LOWSIDE. THE EE WENT 7/10 TO 9/10 OF A MILE WITH THE TRUCK IN NEUTRAL AND THE EE WAS RIDING THE BRAKES AT WHICH TIME THE EE JUMPED FROM THE TRUCK.
EE SOUGHT MEDICAL TREATMENT FOR LOWER BACK PAIN FROM A PRE-EXISTING BACK INJURY. EE IS CURRENTLY UNDER A PHYSICIAN'S CAR AND NO DEFINITE DIAGNOSIS HAS BEEN MADE AT THIS TIME. THIS INJURY HAD B EEN CLASSIFIED AS AN OCCUPATIONAL ILLNESS.
