Victim was removing a machine bottom guard (belly pan) on a Caterpillar D-10 N dozer. Apparently the chain he was using to hold the belly pan while he was removing the bolts that held it in place slipped when the bolts were removed. The belly pan swung down and struck the victim inflicting fatal injuries

VICTIM WAS ASSISTING WITH INSTALLATION OF NEW BEARINGS ON CRUSHER ROLLS. ONE BEARING HAD BEEN INSTALLED, MAKING PREPARATIONS FOR SECOND. REACHED UNDER THE SHAFT TO REMOVE SOME DEBRIS, WELD ON THE EYE HOLDING THE ROLL BROKE, FELL ON HIS RIGHT HAND. SURGERY PERFORMED ON HIS RIGHT HAND. THE 1ST AND 2ND FINGERS AMPUTATED TO THE KNUCKLE, 3RD AMPUTATED TO THE SECOND JOINT.

WHILE UNLOADING A TRANSMISSION PUMP FROM THE BACK OF SERVICE TRUCK, HIS FOOT SLIPPED ON THE BUMPER AND HE FELL TO THE GROUND WITH PUMP FALLING ON ARM.

HAULING SHOT ROCK TO FORMER SLIP AREA, WHILE DUMPING A ROCK GOT CAUGHT IN TAILGATE OF TRUCK MAKING LOAD UNBALANCED AND THE TRUCK TURNED OVER ON IT'S SIDE. NO ONE WAS INJURED AND DRIVER OF TRUCK RETURNED TO WORK. NO WORK TIME MISSED. DRIVER TO WATCH AREA OF DUMPING AND LOADER NOT TO OVER LOAD AND TO WATCH LOADS.

EMPLOYEE WAS WORKING IN A CONFINED AREA AND HIS HARD HAT CAME OFF. WHEN HE REPLACED HIS HAT. MATERIAL CAME OUT OF THE HAT AND WENT IN BEHIND HIS SAFETY GLASSES LODGING IN HIS EYE. HE CONTINUED TO WORK BUT HAD TO GO TO THE DOCTOR.
