Injured was moving a 6 inch discharge line from one ditch to another ditch when EE fell. The injury resulted in a ruptured disc requiring surgery to lower back. Injury occurred on 4/3/16, but injured continued to work until 12/2/16 when EE had surgery and began missing work on 12/5/16 at which time this became a reportable injury.

Employee states that as he was attempting to dump a load of coal from trailer, the coal stuck in trailer causing the front wheels of tractor to raise off the ground. When the coal dislodged, the trailer came down suddenly, jarring employee.

A contract employee, and a co-worker were in process of splicing a 6-inch water line with a metal band and banding tool. During the process, EE was pulling one end of the stuck band out of the banding tool with a pair of pliers. When the band sprang out, the sharp edge struck the side of his left hand, resulting in a laceration.

Injured stated he was standing outside the shop area talking to employees when he noticed something felt like it went in his eye. He immediately washed out his eye. However, two (2) days after the event, he sought medical attention where an abrasion was observed and was given a prescription for eye ointment. Note: This incident was not reported to managment until 11/5/2010.

The injured, a contract employee, was in process of servicing a Caterpillar de-watering pump generator. While performing the task, his foot slipped on the trailer step causing his foot to slide in the space between the trailer and the house of the generator. As a result, he twisted and bumped his knee.

Employee stated while working from a 2-foot step ladder and placing boxes of filters in the appropriate bins, she lost her balance and fell to the floor. In doing so, she twisted her left knee resulting in a sprain. Note: Employee started missing work due to injury on May 31, 2009.

The injured was helping install a seat in a bulldozer. During the process, while his hand was underneath, the handle that latches the seat was inadvertently bumped which caused his ring and middle finger to get caught between the seat and base. As a result, his middle finger sustained a fracture at the finger nail area.

After making electrical repairs at the dragline, the injured re-set the three (3) 480 volt breakers and while examining the circuit panel, one of the breakers suddenly 'blew,' causing the injured to sustain burns to his face and neck area. He was taken to the local hospital where he was treated and admitted over night for observation.

The injured employee stated he was in process of checking an air leak at the main water stand. While walking toward the air valve location, he stepped on a loose rock. When this happened, his foot rolled off the side causing him to twist his knee. The injured continued to work until his physician took him off work on 6/27/2008 due to the MRI showing a torn cartilage.

EE was checking machine for proper working condition after returning to staging area to obtain pressure wand that was missing from truck. His foot became tangled in hose or hose reel section causing him to fall from back bumper/step of truck & accidently squeeze gun handle. He then said gun stuck in the open position. He let go of wand to catch himself. Water struck him in chest.

The EE was operating the dozer, dug a trench then drove into the trench. He was not wearing his seatbelt and struck his head on the dash board, He cut his head and had to have stiches. The EE was given three days without pay for falling to wear his seatbelt.

On July 25, 2007, the injured was walking on the crossover of a drill when his foot slipped, injuring his knee. As the months went by, his knee pain continued to get worse. Please note, on May 1, 2008, surgery was performed to repair a torn cartilage, thus making the injury a reportable event at that time.

EE was in process of removing the transmission from truck at the truck bay in the main shop. While removing bolts from the cross member it suddenly fell catching a portion of his left hand between the cross member and frame. As a result, he sustained a laceration on the palm of his hand, at the base of the little finger.

3/21/07, EE reported pain in left shoulder while tossing a truck headrest to coworker. EE was seen by a dr who ret'd EE to work on next shift. 5/21 EE off work due to pain. EE had rec'd cortis one treatment in Feb. EE has pre-exist condition & treated for pain prior to 5/21 report. Determined likely to accumlated trauma disorder.

Employee states on evening of the injury, he lost a corner bit off his dulldozer. While he and a co-worker were scooting the corner bit back toward the bulldozer, he placed his fingers in the bolt hole resulting in his finger striking a burr resulting in a laceration. Note; management was not aware the ee went to a medical facility and received stitches until 3/13/07. see letter

This caused a puncture wound to the back portion of his head. He was taken to Charleston area medical ctr where he was kept over night for observation. He was released the next day.

Injured stated he was in process of cutting weeds using a weed eater. While doing so, he encountered some bees and was stung 6 or 7 times. Shortly thereafter, he had difficulty breathing and was taken to he hospital for an apparent allergic reaction.

Employee apparently pulled a muscle in his lower back when he slipped on ice at the fuel farm. He had exited the cargo area of the fuel truck and when he stepped on the concrete pad, his foot slipped on the ice jerking his lower back.

The injured stated after performing his preshift inspection, he started to mount his rock truck. Upon reaching the third boarding step, the left side of the step broke, causing him to loose his balance and fall to the next rung. It is believed this incident aggravated his groin/hip area that was injured at the end of the previous shift.

Injured and co-worker were working on a 785 rock truck engine. They had to work underneath the engine and were having problems trying to remove a piston. During the process, he overexerted or twisted his back while pushing up on a connecting rod. The pain grew worse as the shift proceeded. He worked the next shift but had to seek medical attention on Monday, 10/31/2005.

Employee was helping install a large pin in the bucket door for the 2800 P&H electric shovel. During the process, he was using a 20-pound sledgehammer & while striking the pin, a piece of metal flew off (pin), striking & penetrating his inside upper left leg. The piece of metal was successfully removed at the hospital the same evening; laceration required stitches.

The crew were removing `anfo mixture? from a disabled powder truck and placing it in the bin of an adjacent truck. The employee was standing on top of the powder truck and after placing the mixture in the bin, he lost his balance and fell (3 feet) down on the walkway. When he attempted to catch himself, he sustained a fracture to his left arm, right above the wrist.

EE and co-workers were in process of working on a 900 Komatsu loader bucket, installing a lip protector. AS he was striking the lip protector with a 16-pound sledge hammer, a piece of metal chipped off from the lip protector, striking and penetrating his left forearm. The piece of metal was successfully removed at the hospital the next morning.

EE stated that on the morning of the injury, a tire had to be removed from his grease truck. As he was pulling on tire, he felt pain in his lower back area. No report was filled out at the time of injury; safety department was informed on July 11, 2005. Note: Employee did not start missing work until August 25, 2005.

Employee was working on a gasoline engine for a mechanics truck. As he was picking up the intake manifold, his little finger on left hand apparently struck a burr or sharp edge on the gasket. As a result, he sustained a laceration that required six (6) stitches to repair. He was able to return to work on the same day.

Employee stated that on 05/28/2005, he was on the ground helping with the 2800 Shovel move. During this time, he thought he had gotten an eyelash in his eye. He continued to work with no problem until after shift and later in the night, his eye became irritated. The next day, he was taken to hospital where a rust particle was removed. He was given a prescription for eye drops.

WHEN THIS OCC URRED, HE SOMEWHAT SLIPPED CAUSING HIS WEIGHT TOSHIFT AND WHILE REGAINING HIS BALANCE HIS ONE ARM WAS MAINTAINING HIS WEIGHT AND THEREFORE STRAINED HIS NECK AND SHOULDER.

EMPLOYEE WAS IN PROCESS OD STARTING EQUIPMENT AND SINCE THE ROCK TRUCK HE WAS WORKING ON WOULD NOT START HE CLIMBED THE BOARDING LADDER AND REMOVED THE OUTER AIR FILTER IN ORDER TO SPRAY STARTING FLUID. WHILE DOING THIS, HE LOST HIS BALANCE AND FELL APPROX 7 FT AND LANDED ON HIS FEET. HE SUBSEQUENTLY SPRAINED HIS LOWER LEFT LEG.

HE SPIT MOST OF THE MIXTURE OUT EXCEPT F OR ABOUT 1/2 TEASPOON. SHORTLY THEREAFTER, HE GOT NAUSEOUS & COMPLAINED OF A HEADACHE. HE & THE MSDS SHEET WAS SENT TO CLINIC WHERE HE WAS GIVEN A SHOT FOR HIS HEADACHE.
